[Delay of constitutional development--studies on therapy with growth hormone].

Human growth hormone was given to 6 boys with constitutional delay of growth and development. Three patients had normal growth hormone responses following exercise or insulin-induced hypoglycemia. Subnormal growth rate (less than 2 SD below normal for bone age) was found in 4 patients. The mean growth rate of the patients treated with 12 U hGH/m2 per week increased from 4.6 to 5.1 cm/year which is not significant. Only 2 boys responded with an increment of the growth rate of more than 1 cm/year. In an age-matched control group with the same growth variation the growth rate was 4.9 and 5.0 cm/year during the same time periods. It can not be recommended to treat patients with constitutional delay of growth and development with hGH except those with severely depressed growth rate and subnormal spontaneous growth hormone secretion during sleep.

[Growth and growth hormone in children with congenital lip and palatal clefts].

In order to investigate the correlation supposed between cleft lip and palate of children and growth hormone deficiency we determined the amount of growth hormone in the blood serum of 199 children with cleft lip and palate and partly with other additional diseases. In contrast to a previous result we could not find a significant relation between the cleft-syndrome and growth hormone deficiency. All patients had a normal and sufficient concentration of the hormone. As result of our work we propose to determine the hormone concentration only in those cases in which less growth is observed too.

[Magnetic resonance tomography studies of the hypophysis in children with growth hormone deficiency, born with umbilical cord entanglement].

Eight children with either panhypopituarism, severe growth hormone deficiency or neurosecretory dysfunction for growth hormone with a common history of umbilical cord encirclement around the neck underwent magnetic resonance imaging of the pituitary gland and the hypothalamus. Panhypopituitarism was associated with small to absent adenohypophysis, narrow or absent stalk and ectopic neurohypophysis whereas patients with partial GHD showed normal anatomical structures. Etiology of the endocrinopathy could be a primary malformation of the pituitary gland, an apoplexy of the gland or disturbation within the hypothalamus. We believe that the latter cause is predominant in umbilical cord encirclement.
