Ectopic geographic tongue and AIDS.

A 23-year-old man with AIDS developed a lesion with the clinical characteristics of an ectopic geographic tongue in the lower lip, near the right commissure.

[Pruritus in chronic kidney insufficiency].

Itching is the dermatologic symptom more often found in patients with chronic renal failure. We have studied 80 patients with end-stage renal disease; itching was present in 56.3% of the cases. We didn't detected in this study any correlation between itching, long term hemodialysis, high calcium, phosphorus, alkaline phosphatase or phosphocalcic product levels. We have seen: 1. Those patients with itching were older than the rest. 2. Itching was important in those patients with residual diuresis less than 500 ml/day (p greater than 0.01). 3. Those men without itching had higher hematocrit levels (p greater than 0.01). 4. Histologic findings on optic and electronic microscopy were more frequent in patients who presented this symptom.

Chronic benign familial pemphigus.

Ten patients with benign familial chronic pemphigus (BFCP) (Hailey-Hailey disease) were evaluated; semiologic and localization differences were described, and special attention was given to solitary or atypical forms. In all the cases, the diagnosis was confirmed by histopathology; some histopathologic differences and the results achieved by direct immunofluorescence of skin are discussed. Most of the patients responded to the treatment with corticosteroids and antibiotics.

[Cutaneous metastasis of a cloacogenic tumor].

A case of cloacogenic carcinoma with cutaneous metastases in a 74 year old man is reported. The patient presented with a six months history of two polyps of the lowest portion of the anal canal. Physical examination of the inguino-scrotal area revealed a 5 X 7 cm. hard infiltrative plaque with three ulcero-vegetative scars. Histologic examination of both polyps revealed two of the four cytologic patterns described in this neoplasm. One polyp showed undifferentiated cells, type IV and second one, type II differentiated. The biopsy of the cutaneous metastases showed dermal infiltrative clusters of the cytologic type II differentiated epidermal cells. Treatment with radiotherapy and chemotherapy was ineffective. Cloacogenic carcinoma of the canal anal is a transitional cells neoplasm believed to arise from vestigial remnants of cloacal origin. Metastases occurred in 19% of cases but cutaneous colonization develops exceptionally.

[Chronic benign familial pemphigus. Clinical, histological and immunological study of 7 cases].

A clinical, histopathological and immunological study was carried on a series of seven patients of Familial Benign Chronic Pemphigus (FBCP). This condition is characterized by recurrent small blisters, mainly, on intertriginous areas and on the sides of the neck, that become wet and crusted rapidly. They are generally sharply marginated and Nikolsky's sign is often positive. The lesions appear spontaneously and may be precipitated by warm, humid environment, mechanical trauma, radiations, bacterial or mycotic infection. Healing occurs with residual non scarring hyperpigmentation. Histopathologically, the epidermal alteration respond to a primary acantholytic mechanism. Ultramicroscopic studies have suggested an alteration on the desmosome-tonofilament complex. Comparatively with Pemphigus, another acantholytic disease in which immunological pathogenesis is strongly suspected, only few reports are referred to immunological studies in FBCP. In the present paper, a direct immunofluorescent study on spontaneous and provoked blisters was made in order to investigate deposits of immunoglobulins and complement. Indirect IF was performed with sera of teh patients for detection of circulating antibodies. Two cases were also sensitized with erythrocytic antigen. The immunological response to this substance was evaluated. The clinical and histopathological findings of the present series, are similar with previous descriptions. The immunofluorescent studies do not provide evidence of antibodies to epidermal intercellular space, like Pemphigus.(ABSTRACT TRUNCATED AT 250 WORDS)

[Post-traumatic nodular scleroderma].

Scleroderma of rare appearance in children appears in minor scale as to the five per cent on the whole incidence of this collagen disease. The children usually present localized scleroderma and at times associated with other pathologies, traumatisms and injections were referred. Two patients aged 5 and 13 years old are presented, both with nodular lesions on anterolateral thigh area, and in the right buttock respectively. The patches of a side bigger than the palm of hand were only touchable and the skin that covered them only showed a slight hyperpigmentation in the edge in one of the cases. The limits were not precise and the nodulose surface was irregular. The evolution was as of two and three years, right after inoculation of antitetanical vaccination and puncture thorn of Yuca leaves. The histological control showed intensive phenomenons of fibrohyalinosis covering almost all the dermis. The studies of the laboratories didn't produce interesting data. The histological and clinical set of symptoms shows differences with the esclerodermic like states as a consequence of excipient of vitamin K, B 12, norhydroxprogesterone and anti-tetanic serum, in which they settle in the cellular subcutaneous tissue and they involution spontaneously. There are also differences with the paniculitis artefacta and with the lincar morphea associated to bifid spine. At last the authors make special mention of the case described by Desmons of progressive linear scleroderma right after the triple vaccination. The nodular or subcutaneous scleroderma is a clinical form in which the histological alteration decays in deep dermis and superficial hypodermis. The cases shown suggest a conection between a previous traumatism and the nodular scleroderma.

Experimental study on the etiology of congenital multiple arthrogryposis.

The effect of temporary immobility of the muscles in the embryo, induced by continuous infusion of d-tubocurarine into the ovum, was studied in 31 chick embryos. The ankle and foot joints showed flexion contractures with equal frequency, but the knees were less severely affected. Histological examination demonstrated peri-articular and intra-articular fibrosis, and fibrosis of the muscles of the limb. These experimental lesions closely resemble those of arthrogryposis. It is therefore suggested that the aetiology of human arthrogryposis may be based on a common pathogenetic mechanism; lack of joint movement during embryonic life.

Hereditary complement (C2) deficiency with discoid lupus erythematosus and idiopathic atrophoderma.

A family with hereditary deficiency of the second component of complement was studied. Three siblings were homozygous for C2 deficiency and two of them had associated skin diseases. One sister presented with idiopathic atrophoderma and the other had clinical and pathological manifestations of discoid lupus erythematosus. This is the first description of an association between idiopathic atrophoderma and C2 deficient state.

[Carcinoid papillomatosis].

Papilomatosis Carcinoides, presents clinical and histological characteristics that may be authentically described as a transition state between the benign papillomatous proliferations and spindle cell carcinoma. It involves a series of clinical conditions, that are described with different names that correspond to the condyloma accuminatum gigantum. [Buschke and Lowenstein) wartic carcinoma [Ackerman], Papillomatosis oral florid [Rock and Fisher], carcinomatoid [Gilbert] profuse conjuntival papillomatosis [Bazex], etc. These latter only refer to partial localizations or a special clinical form. That is why we believe the denonmination Papillomatosis Carcinoides, is preferable, which is its cutaneous, mucose or semimucose forms involves all of them. The treatments applied to the 18 cases presented have been very diverse: they vary from the elimination of the lesions by the application of topical podophylin to large surgical ablations. But most of them showed a regression or cure of their lesions by citostatics [methotrexate] not prevent the proliferative advance of the lessions. These therapeutic data can also contribute to its nosologic classification. In some cases they have an evolutive character with slow progression for years, still being curable with podophylin and in others that due to their chronic evolution with added supuration, and complications of the general health, make surgery necessary. In most cases, the antimetabolities are the therapy of choice the radiations seem inoperative. It may be noted that tumoral papillomatous lesions produced by virus, exist in veterinatian pathology (fibropapillomatosis in the bovine genitals, equine sarcoid, papilomatosis of goats, monkeys, etc.). Noteworthy are those of the oral mucosa of rabbits which are white greyish sesiles or pedunculated small nodules localized mostly on the inferior surface of tongue with a similar histology to the papilomatosis carcinoides. These observations would suggest a viral ethiology, even though all the efforts to isolate the virus of the lesions gave negative results. Our attention is drawn to case n. degrees 14. That began as a simple plantar wart. There have also been numerous cases in which from the beginning they were simple condylomas accuminatum, caused without any doubt by a virus. The carcinoid papillomatosis could be related to the spindle-cell carcinoma that developed in the areas of chronic infection with herpetic virus n. degrees 2, where the virus can be isolated from the lesions; the virus disappearing later with the development of the epithelial tumoral state [cases described previously by some authors as neophasias on vaccination scars and on recurrent genital herpes].

[Preparation and study of a Paracoccidioides antigen needed for skin tests].

A cellular antigen from Paracoccidioides brasiliensis, obtained from the supernate of the disrupted yeast phase, was studied. It was prepared by a method similar to that for obtaining the spherulin from Coccidioides immitis. This antigen is composed of 55 mug/mg of polysaccharides, determined by the phenol method, and 7-8 mug/mg of protein, determined by Folin-Ciocalteu's technique. In vitro tests of antigenicity were done by immunodiffusion and complement fixation. The optimum dilution of the antigen was 0-5 mg/ml determined by microtiter complement fixation. The in vivo standardization of the cellular paracoccidioidin was performed on three groups of patients. The first was composed of 56 patients with mycologically proved paracoccidioidomycosis. The disease was active in 32 and clinically healed in 24 of this group. The second was composed of 15 cases of histoplasmosis with strong histoplasmin skin tests. The third group were 70 patients with no mycotic pulmonary diseases who had come from different zones in Argentina. Each individual was injected with 0-1 ml cellular paracoccidioidin (3 mg/ml), 0-1 ml of Fava Netto's antigen diluted 1/10, and 0-1 ml histoplasmin L48, diluted 1/100. The cellular paracoccidioidin skin test was positive in 44/56 (78.5%) patients with paracoccidioidomycosis. The Fava Netto's antigen gave 42 (75%) positive results and the histoplasmin skin tests gave 28 (50%) in the same group. Twenty-eight (50%) presented positive histoplasmin skin tests. The paracoccidioidin skin test was positive in 2 of the histoplasmosis group (13-3%) and the Fava Netto's antigen gave 3 positive results in the same group (20%). The histoplasmin skin test was positive in 32% of the individuals with no mycotic disease. Ten per cent of these exhibited positive reaction with both P. brasiliensis antigens. Of those paracoccidioidomycosis cases (Group 1) who had negative skin tests with 3 mg/ml of the cellular paracoccidioidin 7/10 gave positive skin tests when tested at a concentration of 8 mg/ml. Characteristic changes of cell medicated hypersensitivity were observed in the histopathological studies of the cellular paracoccidioidin skin test.

[Folliculoma].

The authors present a new case of a rare tumor, folliculoma, located in the face of a 57 year old woman. Concerning histogenesis, it appears most likely that Folliculoma represents a nevus developed from primary epithelial germ.
