Etiological diagnosis in sinusitis: ultrasonography as clinical complement.

Accumulation of purulent sinus secretion reflects an inflammation caused by pyogenic bacteria, whereas the accumulation of nonpurulent effusion may have origins other than a bacterial infection. Ultrasonography differentiates the disordered sinus from the healthy one, but not purulent from nonpurulent secretion. In 90 patients, supposed to suffer from sinusitis, a correct diagnosis, sinus empyema vs. not sinus empyema, was established in a majority of cases by means of clinical evaluation alone. By introducing ultrasonography as a complement to the clinical evaluation, the diagnostic reliability became lower. It is concluded that ultrasonography seems to have little or no implication in the therapeutic decision unless diagnostic puncture is also performed.

Immunonephelometric determination of proteins in cerebrospinal fluid in various neurological disorders.

The purpose of this investigation was to examine the diagnostic value of nine specific proteins in differentiating multiple sclerosis from other neurological disorders. The quantitative determination of albumin, transferrin, haptoglobin, IgG, IgA, IgM, orosomucoid, complement-C3 and prealbumin was carried out using an automated immunonephelometric method. Using a combination of IgG-index ((Sp-IgG/S-IgG)/(Sp-albumin/S-albumin)) and haptoglobin-index ((Sp-haptoglobin/S-haptoglobin)/(Sp-albumin/S-albumin)) it is possible to increase the diagnostic value. Using a modified index it is possible to obtain the same diagnostic value by combining modified IgG-index ((Sp-IgG/S-IgG)/Sp-albumin) and modified haptoglobin-index ((Sp-haptoglobin/S-haptoglobin)/Sp-albumin). The diagnostic specificity and sensitivity for the modified combined index was 0.60 and 1.00, respectively.

Fluoroimmunoassay of pregnancy zone protein.

A specific and sensitive fluoroimmunoassay for determination of pregnancy zone protein in serum is described. One hundred and seventy-three apparently healthy subjects were investigated. The median concentration of pregnancy zone protein for apparently healthy males is 7 mg/l and for females 43 mg/l. The fluoroimmunoassay is compared with the electroimmunoassay.

Contraceptive habits in women between thirty and fifty years of age. A comparison of two periods, 1967--69 and 1972--74.

The use of contraception by women 30--50 years of age is presented. The material was composed of participants in the prophylactic investigations for cancer colli uteri in the southern part of Storstøms County, which was first performed in the period 1967--69 and secondly in 1972--74. A total of 17 028 women were examined, of whom 8 234 participated in both surveys. A comparision of the two periods of investigation revealed a significant increase (p<0.01) in the utilization of contraception and in shifting to more effective methods. This improvement was attributed to the increased amount of information about contraceptive methods which was made available in the country. It was observed that a remarkably large percentage of the women did not use contraception, 24.9 per cent among those 30--34 years of age, 26.9 per cent among those 35--39 years of age, 40.7 per cent among those 40--44 years of age and 57.2 per cent among those 45--49 years of age. Analysis of the influence of socioeconomic conditions showed that women of the lowest social stratum, with slight education and low income, made up a large proportion of those who abstained from contraception. The number of pregnancies was found to play a role in the use of contraception. An expansion of the distribution of information about contraception to the public is recommended, as well as a possible economic subsidy to the socially disadvantaged for procurement of contraceptives.

Determination of pregnancy zone protein in serum using an automated immunoprecipitin reaction method.

Determination of pregnancy zone protein in serum by means of an automated immunoprecipitin reaction method and pretreatment with polyethylene glycol for reducing the serum blanks is described. Using this procedure the sensitivity of the method was greatly improved, from originally 100 mg/l to 1 mg/l. The precision "between days" was 9.9%. A positive and significant correlation to electroimmuno assay is demonstrated. In apparently healthy controls the median serum pregnancy zone protein in females was 38 mg/l (2--91 mg/l) and in males 2 mg/l (0--20 mg/l). No correlation between serum pregnancy zone protein and age could be demonstrated. In malignant diseases our results seem to confirm a relationship between increased pregnancy zone protein and spreading of the tumours. Serum pregnancy zone protein in disseminated malignancies are increased significantly compared to controls. In males 75% with disseminated tumours have elevated serum pregnancy zone protein.

Effect of encephalitogenic protein on the migration in agarose of leucocytes from patients with multiple sclerosis. Correlations with clinical parameters, localization of antigenic determinants, and influence of corticotrophin treatment.

We have extended the results from our previous investigation of the reactivity of leucocytes from patients with multiple sclerosis and some other neurological diseases to bovine encephalitogenic protein, using the migration in agarose technique. 1. Reactivity (i.e. behaviour in the test deviating from that expected with cells from healthy subjects) is manifested either as an enhancement or an inhibition of leucocyte migration. 2. The incidence of subjects with cells showing reactivity to encephalitogenic protein is higher in the group of patients with multiple sclerosis than in the group of patients with other neurological diseases. 3. Reactivity to encephalitogenic protein is correlated to disease course score; patients with a chronic progression of the disease show the strongest reactivity in the test. 4. The tryptophan-containing region of the encephalitogenic protein appears to be a major determinant of significance for this test. 5. Patients tested during corticotrophin treatment tend to show a higher degree of reactivity in the test than expected-the increased reactivity seems to be primarily directed to the tryptophan-containing region.
