Tuberculous pleural effusions: ultrasonic diagnosis.

Twenty patients with tuberculous pleural effusions were studied with ultrasonography. In 18 patients, ultrasonography demonstrated regular pleural thickening which was less than 1 cm except in 1 case. In 4 cases there were a few pleural nodules, whereas in 2 cases the pleural surface showed small nodularity. The latter finding may be diagnostic for a tuberculous etiology. Eighteen patients had multiple, delicate, mobile septations in the effusions, and a lattice-like appearance had formed in 6 cases. Computed tomography was obtained in 7 cases, and pleural thickening was demonstrated in 6 of them. Ultrasonography is a useful imaging modality in the diagnosis of tuberculous pleurisy.

Internal mammarial lymphoscintigraphy with SPECT after ultrasound-guided injection of 99mTc-dextran.

Twenty-two women with stage II or III breast cancer were evaluated by SPECT to determine the number, size, three-dimensional localization and depth from skin surface of the internal mammary lymph nodes for accurate radiotherapy portal planning. The results were also compared with those of planar imaging (PI). Two-step injections of 99mTc-dextran were made under ultrasound guidance into the anterior sheath of the M. rectus abdominis first at the ablation side. 1.5 h thereafter an anterior planar view was obtained to evaluate cross drainage (13.6% was observed). Then, the second injection was done at the opposite side in the same manner. Planar and tomographic images were obtained 1.5 h after the second injection. Similar values were obtained for the depth from skin surface, distance from the midline and diameter of the lymph nodes with both PI and SPECT. The total number of nodes in 22 patients detected by SPECT was higher (138) than that from PI (129), the difference being statistically significant (0.005 greater than p greater than 0.0005).

Percutaneous transcatheter ethanol sclerotherapy of postoperative pelvic lymphoceles.

Although percutaneous procedures have been used for the treatment of lymphoceles, transcatheter sclerosing therapy has not been widely applied. We present the results of transcatheter sclerotherapy of lymphoceles with 96% absolute ethanol in 7 patients who had developed lymphocele after pelvic lymphadenectomy for uterine cancer. Seven of the eight lymphoceles (88%) completely disappeared after treatment. The duration of catheter drainage ranged from 4 to 21 days. Although one lymphocele did not resolve completely, it did not require surgery as the patient's symptoms resolved.

Meckel Gruber syndrome: a case diagnosed in utero.

A case of Meckel Gruber syndrome is presented, diagnosed prenatally from the medical history of the mother which revealed a previous malformed stillborn with anencephaly, meningomyelocele, polydactyly and ambiguous genitalia. This was the first prenatally diagnosed case ever reported in Turkey. The clinical, computed tomography and postmortem findings and the related literature are reviewed.

Tuberculous peritonitis: ultrasonic diagnosis.

Tuberculous peritonitis is an uncommon manifestation of tuberculosis and specific radiologic features of it have not been defined. The diagnosis of tuberculous peritonitis has been made prospectively in 4 patients with sonography. The findings included free ascites; multiple, fine, delicate septations and incomplete mobile strands of fibrin; peritoneal thickening and nodularity. In the differential diagnosis of tuberculous peritonitis, peritoneal carsinomatosis, peritoneal mesothelioma, pyogenic peritonitis, and hemoperitoneum should be considered.

Nephrocalcinosis due to vitamin D intoxication.

Two cases of vitamin D intoxication are presented. The patients, one aged three months, and the other aged four months, were given Vit D in doses ranging from 30,000 IU/day to 60,000 IU/day for over a period of from one to three months. Laboratory data showed serum calcium levels of 17.6-19.5 mg/dl and phosphorus levels of 2.6-3.2 mg/dl. Renal ultrasonographic examination demonstrated medullary nephrocalcinosis in both patients. In this study the clinical, biochemical and ultrasonographic findings and the therapy of Vit D intoxication are reviewed, and preventive measures are suggested.

Renal lymphoma. An unusual presentation in a child.

An unusual case of non-Hodgkin's lymphoma with primary renal involvement in a four-year-old boy is presented. The diagnosis was established by renal biopsy. The findings of an exploratory laparotomy and an extensive staging procedure showed no other primary site. The patient has been followed up for 14 months and no evidence of recurrence has been observed.

Biopsy
