The investigation of 26 acromegalic patients long after irradiation of the pituitary body with a proton beam showed clinical and hormonal improvement in 38% of the patients, clinical improvement without complete growth hormone normalization in 46%, and the absence of any effect in 15%. The latter was mainly a result of partial irradiation of pituitary adenoma. Some patients after proton irradiation demonstrated signs of adenohypophyseal functional insufficiency, more marked in patients with a decreased level of Gy (over 5 ng/ml). A conclusion is that proton beam therapy is effective (provided all criteria for patients' eligibility are observed) and can be used as an independent method for the treatment of acromegalic patients.
Suprasellar cysts containing cerebrospinal fluid are considered a rare pathological condition. Characteristic symptom complexes in children are discussed on the basis of 12 cases. It is assumed that signs of affection of the chiasma and optic nerves in slowly progressive hydrocephalus, diencephalic disorders, and hyperkinesis suggest the presence of a suprasellar cyst. The authors present data on the use of diagnostic methods of examination and give their opinion on the informative value of these methods: craniography, gamma-topography and radionuclide cisterno- and ventriculography, computerized tomography, and computerized-tomographic cisterno- and ventriculography.
As compared to ordinary computer tomography, computer-aided tomographic cisternography (CTC) demonstrates better the anatomy of the brain cisterns and at the same time gives an idea of the processes of c.s.f. circulation in dynamic examination. The article discusses the results of examination of 112 patients with various neurosurgical diseases by means of CTC. A detailed classification of the brain cisterns is given. The authors show the advantages of the method in examination of patients with small three-dimentional processes in the chiasma-sellar region, cerebellopontine angle, and posterior cranial fossa, with the syndrome of an "empty" sella turcica, cystic structures, and disorders of c.s.f. circulation.
