[Gender differences in migraine and wake-sleep migraine attacks].

A follow-up clinical study was made of 320 patients examined in Autonomic Pathology and Headache Center, Nervous disease clinic, Sechenov Moscow Medical Academy, since 1989 to 1999. The peculiarities of migraine attack course and remission period, psychological characteristics, concomitant diseases and quality of life in men and women were described. In women, migraine attacks were more typical, prolonged and severe, being often stipulated by provoking factors, especially by emotional ones, as well as accompanied by autonomic disturbances, anxiety and depression. Despite a more favorable migraine course, men proved to score lower on quality of life, revealing more maladaptation. Patients with sleep migraine attacks were distinguished by more severe migraine course, multiple disturbances in remission period, higher depression and anxiety levels, as well as lower quality of life. Common serotoninergic mechanisms were suggested to underlie sleep migraine and depression. The leading role of depression in the origin of night migraine attacks is discussed.

[Clinical and psychophysiological characteristics of headache in childhood].

Complex clinical psychophysiological as well as neurophysiological examinations of 210 children with migraine, chronic tension headache (CTH) and chronic posttraumatic headache (CPH) were carried out. Detailed comparative description of cephalalgia types is presented. In a CPH structure, predominates CTH (77.5%), combined type (14.1%); migraine (5.7%) and cervicogenic cephalalgia (2.7%) occur less often. CPH development mechanisms do not principally differ from non-traumatic cephalalgia pathogenesis. Trauma plays a role of a non-specific trigger in realization of these mechanisms. Chronic cephalalgia is caused mainly by poor social and psychological factors and patient's personality peculiarities. Psychovegetative syndrome in children with CTH, migraine or CPH reflects non-specifically diencephal structure disintegration; its clinical signs depending more on child's age than on cephalalgia type.

[Clinical and psychological analysis of migraine population].

The study presents a clinical analysis of a large population of migraine patients (n = 320). Computed database used in the study included over 100 clinical parameters and was based on analysis of primary medical records. As a result of statistic processing of the migraine archive, a general review of clinical picture and migraine course in patients from Moscow and Moscow region community and analysis of the peculiarities of migraine heredity, concomitant symptoms, provoking factors, co-morbid diseases, state in attack-free period, psychological characteristics and quality of life are provided. Factors determining course severity and quality of life in migraine are discussed.

[Chronic posttraumatic headache in juveniles].

The paper presents a comparative analysis of biologic, social-culturological and psychological factors of the premorbid, peculiarities of personality as well as the conditions of getting a slight craniocerebral trauma (CCT) as well as the therapy in the acute period and a psychosocial situation during 1 year after the trauma in juveniles with chronic posttraumatic headache (PTH) (74 individuals) and without it (24 cases). As the main clinical variation of a chronic PTH was chronic PTH of tension, we studied comparative anamnestic data and peculiarities of personalities in 22 juveniles with a chronic headache of tension without PTH in the anamnesis. It was found common pathogenetic mechanisms of the development of both traumatic and non-traumatic cephalgias; the risk factors of PTH chronicity were identified, that was quite necessary for the elaboration of effective rehabilitation programs.

[Clinical and psychological characteristics and therapy of patients with cluster (bunch) headache].

The paper presents a discussion on clinical psychological peculiarities of patients with cluster headache (CHA), an analysis of these data in dependence on CHA form (episodically or chronically), on the sex of the patients, on the side of the headache localization (right-sided, left-sided). Taking into consideration a correlation of the headache attacks and the quality of the night sleep, the therapy with imovan (zopiclone) preparation was proposed. Such therapy resulted in positive changes including disappearance of the night attacks and the decrease of the intensitivity of the algesic attacks during daily time.

Effects of calcium antagonists on serotonin-dependent aggregation and serotonin transport in platelets of patients with migraine.

Flunarizine and cinnarizine (IC50 6.8x10(-6) and 2.8x10(-5) M, respectively) inhibited 3H-serotonin uptake by platelets. In higher doses, they blocked serotonin-induced platelet aggregation and stimulated 3H-serotonin release from these cells. Imipramine did not affect serotonin-releasing effects of preparations. In all patients cinnarizine was more potent in inhibiting serotonin uptake, and in half of the patients cinnarizine displayed higher activity as an inductor of serotonin release.

[Migraine: dynamics of psychophysiologic pattern].

85 patients with migraine without aura and 20 healthy individuals were examined between the attacks, before the attack, during and after it (1-2 days). The examination included clinical-neurological analysis; psychological Spilberger's and Beck's tests, scale of alexithymia and complex algesic questionnaire; recording of the conditional negative wave; determination of nociceptive flexor reflex; recording of trigeminal evoked potentials; spectral analysis of the heart's rhythms; polysomnographic study. Psychophysiologic pattern characteristic for all the patients was established. Psychological and neurophysiological parameters changed synchronously in the periods before the attack, during and after it. Before the migraineous attack a specific pathologic integration increased and presented maximally (the activity of antinociceptive system weakens anxiety increased, tension of catecholaminergic systems grew, hyperactivity in trigeninal sphere elevated, tolerance to the pain decreased). A clear tendency to the reversibility of these changes was observed after an attack. A dynamics revealed in the psychophysiological pattern could serve as a predictor of a migraineous attack and open some perspectives for prophylaxis of the algesic attack.

Migraine associated with panic attacks.

Migraine headache and panic disorder are two conditions that have a number of underlying physiological and psychological abnormalities in common. The temporal relationship between the occurrence of migraine headache and panic attacks could be different, however. According to our observations, some migraine subjects develop panic attacks with the typical symptoms (palpitation, dyspnea, anxiety/fear, shiver, sweating, polyuria) on the "peak" of their attacks. This variant of migraine without aura was conditionally defined as "panic migraine". Here we describe two patients suffering from migraine without aura in whom migraine was associated with the typical panic attack. It is suggested that a pronounced autonomic dysregulation along with marked psychological abnormalities could be responsible for the constellation of migraine and panic symptoms during one episode. Taking into account the previously obtained results, it is concluded that compared to "pure" migraine, "migraine associated with panic attacks" is characterized by a severe course, marked autonomic and emotional disturbances during pain-free intervals, seriously impaired quality of life, and requires a specific therapeutic approach.

[Treatment of various forms of tense headaches by biofeedback].

Efficiency of electromyographic (EMG) biofeedback (BFB) in the treatment of tension type headache (TTH) has been analyzed. 50 patients with episodic and chronic TTH have been treated by a standard course of BFB-therapy (10 sessions). The results have been compared with the data in comparison group (12 patients who received lerivon therapy) and in controls (15 healthy individuals). The severity index has been calculated, different psychologic data have been tested, the amplitude of EMG of frontal, temporal, trapezius, orbicularis oris muscles has been examined. The best results have been obtained in the episodic TTH group--87%. In chronic TTH group the efficiency was lower (full effect--26%, partial--35%, no effect--39%). The 6 month period after the treatment showed that BFB effect is more protracted than that of the pharmacological therapy.

[Trigeminal evoked potentials in migraine].

Trigeminal evoked potentials (TEP) were studied in 19 patients with migraine without aura. Significant shortening of latent periods of early and late TEP components was observed during stimulation on the side of headache as compared with the same indices during stimulation on nonalgesic side. Hyperreactivity of trigeminal vascular system in patients with migraine without aura during the period between the attacks is discussed. Asymmetry of laten TEP periods may determine the preferential side of headache, on the one hand, and may reflect the changes in trigeminal system following the repeated lateralized fits of migraine, on the other hand. Administration of 1 g of aspirin resulted in increase (normalization) of the latentions of TEP components. After efferalgan administration TEP changes were not registered. The suggestion is made that inhibitory influence of aspirin on the truncal trigeminal system is one of the central mechanisms of its action.

[Acetylsalicylic acid in therapy of migraine].

The paper reports the results of preventive therapy of 105 patients with migraine without aura by acetylsalicylic acid in doses 1000, 500 and 100 mg (in 3 groups of patients). The data were presented about the efficiency of these schemes of treatment as well as about the predictors and contra-indications for their prescription. Central and peripheral mechanisms of drug's action were considered.

[The comparative results of pharmacotherapy in migraine].

The questions concerning both control of migraine attacks and prophylaxis of migraine with standard drugs are considered. The results of own observations are presented. There are predictors of either elevation or a decrease of efficiency of the preparations used.

[The tension headache].

Clinical-psychophysiological characteristics of tension-type headache are analyzed. Comparison of some revealed peculiarities with both the form of tension-type headache (episodic, chronic) and manifestation of anxious or depressive disorders as well as with the duration of ache syndrome was performed. The problems concerning diagnosis of tension-type headache and factors contributing to its chronization are discussed.

[Clinico-neurophysiological correlations in migraine].

The more accurate definition of the role of cerebral mechanism in migraine development by means of contingent negative deviation (CND) method was the aim of this investigation. The significant alterations of some parameters were revealed in patients with migraine between the fits as compared with controls. The same indices were normal during the very fits. The CND indices were similar in patients with chronic Headache of strain and healthy individuals. The clinical neurophysiological analysis didn't reveal any significant dependence of CND changes clinical peculiarities and anxious and depressive disorders. The significant elevation of CND parameters observed between the attacks suggests presence of superfluous activity in cerebral structures (noradrenergic mainly) in patients with migraine without aura. Such alterations in their action lead, apparently, to headache attack development.
