[Urodynamic miction studies--physiological aspects].

In this work, definition and interpretation of different functional parameters, which were obtained through urodynamic investigations, are given. Normal patterns in child age are presented. They were the results of personal studies in patients with normal urinary tract. The pressure curve must be seen as an entity. The so-called after contraction is no detrusor contraction. But possibly it is the result of a determined lapse of contracting and relaxing occurrences in the different parts of the bladder and its outlet at the end of micturition.

[Neurogenic disorders of bladder emptying in children].

Disturbed micturition of the neurogenic type is found in children suffering from spinal tumors and anomalies of the vertebral column. The most frequent cause is the so-called myelomeningocele. Myelodysplasia can occur in 1-2 out of every 1,000 new-born babies. Operative treatment is available today (closing the celes immediately after birth and drainage of the associated hydrocephalus) so that these children survive and their fate essentially depends on the early diagnosis and therapy of the neurogenic micturition disturbance. Urodynamic examinations and the determination of the urethra pressure profile are vital for diagnosis. Using the results four different groups can be established each requiring a specific therapy. Individual therapies must also be selected for each individual case. Apart from operative treatment, drugs and electrostimulation can be used on a large scale. The authors present their own experience and results with regard to diagnosis and therapy.

[Neurogenic bladder dysfunction in childhood. I. Diagnosis].

The diagnostic demands of the neurogenic disturbances of the voiding of the bladder in patients with myelodysplasia are summarized in tabulated form. It is reported in detail on own experiences of the authors with urodynamic measuring methods and with the electromyography of the striated musculature of the floor of the pelvis. At present the two methods are not yet used everywhere and they are a considerable enrichment of the diagnostics.

[Urodynamic data].

This paper gives a survey of urodynamic data that may be used for evaluating functional aspects of the bladder and its excretory tract. The different parameters are physically defined. Units of the International System are used in the dimensional formulae, which facilitates the establishment of relations between these parameters. The so-called urethral drag can be derived from the pressure law of Blasius. The calculation is as follows: urethral drag = vesical pressure/(maximum urethral discharge). This value is proportional to the true vesical resistance in a physical sense.

[Method of measuring pressure using perfusion manometry].

Based on the principle of perfusion manometry, a device for the assessment of intraluminal pressures in urethra and anal canal is presented. The technical parameters are reported. The method may be utilized for clinical diagnosis in cases of disturbed micturition and defecation in children as well as for the function of esophagus.

Urethral pressure profile measurements in children.

After a brief representation of the physiological processes during micturition in the region of the bladder-urethra junction, the pathological changes in neurogenic micturition dysfunctions leading to an increase of the infravesical flow resistance are considered. A suitable procedure for determining exactly the dysfunction is the registration of a urethral pressure profile. The construction is based on pressure measurement by means of a catheter with a lateral eye, with constant slow perfusion. Measuring errors had to be determined in preliminary trials. Examples of investigations in children with various subvesical urinary dysfunctions demonstrates different points concerning this method.
