Do biophysical properties of the airway smooth muscle in culture predict airway hyperresponsiveness?

Airway hyperresponsiveness is a cardinal feature of asthma but remains largely unexplained. In asthma, the key end-effector of acute airway narrowing is the airway smooth muscle (ASM) cell. Here we report novel biophysical properties of the ASM cell isolated from the relatively hyporesponsive Lewis rat versus the relatively hyperresponsive Fisher rat. We focused upon the ability of the cytoskeleton (CSK) of the ASM cell to stiffen, to generate contractile forces, and to remodel. We used optical magnetic twisting cytometry to measure cell stiffness and traction microscopy to measure contractile forces. To measure remodeling dynamics, we quantified spontaneous nanoscale motions of a microbead tightly anchored to the CSK. In response to a panel of contractile and relaxing agonists, Fisher ASM cells showed greater stiffening, bigger contractile forces, and faster CSK remodeling; they also exhibited higher effective temperature of the CSK matrix. These physical differences measured at the level of the single cell in vitro were consistent with strain-related differences in airway responsiveness in vivo. As such, comprehensive biophysical characterizations of CSK dynamics at the level of the cell in culture may provide novel perspectives on the ASM and its contributions to the excessive airway narrowing in asthma.

Mutation analysis of 218 Chinese patients with Wilson disease revealed no correlation between the canine copper toxicosis gene MURR1 and Wilson disease.

Wilson disease (WD) is the most common disorder resulting in hepatic copper overload. A similar form of copper-associated cirrhosis caused by mutations of the canine copper toxicosis MURR1 gene is also observed in Bedlington terriers. Recent studies indicate that MURR1 might influence human copper metabolism and the clinical presentations of WD. However, the correlation between the MURR1 gene and the Chinese patients with WD has not been reported. In the present study, all three exons of the MURR1 gene including the intron-exon boundaries were directly sequenced in 120 unrelated healthy Chinese and 218 unrelated Chinese patients with WD. No mutations were detected in coding and splice site sequence in the human MURR1 gene. A novel polymorphism 3'+119T-->A in the 3' untranslated region (UTR) was identified in three healthy individuals and four patients with two disease-causing mutations in the ATP7B gene and a great diversity of clinical presentations. Of the ATP7B mutations reported here, Gly1268Arg is a novel one. Also, the previously described nucleotide change IVS2+63C-->G was detected in 31.66% of normal chromosomes and 26.15% of WD chromosomes. The results have indicated that there is no correlation between MURR1 and WD in the Chinese population.

Association of sonographically detected calcification with thyroid carcinoma.

BACKGROUND: Calcification can be detected in both benign and malignant nodules and is often neglected by clinical physicians. The purpose of this study was to investigate the association of thyroid nodule calcification detected on ultrasound with thyroid carcinoma. METHODS: One hundred seven cases of thyroid carcinoma and 215 cases of benign thyroid nodules were selected from the records of inpatients of our hospital who had high-resolution ultrasonography preoperatively and pathologic diagnosis postoperatively between 2001 and 2004. The case numbers and percentage of calcification, fine stippled psammomatous (FSP), and non-FSP calcification in benign nodules and thyroid carcinoma, sex, and age groups (<45 years and >or=45 years) were retrospectively reviewed. Statistical analysis was performed using chi-square test and odds ratio. Sensitivity and specificity of calcification and FSP calcification on ultrasonography were also calculated. RESULTS: The incidence of calcification, non-FSP calcification, and FSP calcification were significantly higher in thyroid carcinoma than in the benign group (p < .001, p = .03, and p < .001, respectively). However, FSP calcification was more significant than the non-FSP calcification (p = .001) for predicting thyroid cancer. The incidence of non-FSP calcification and FSP calcification did not differ significantly between the sexes (p = .50 and p = .83, respectively). The relative risk of malignancy incidence was significantly higher in those younger than 45 years old with FSP calcification (p < .001). The incidence of non-FSP calcification was significantly higher in the older group (p = .03). The sensitivity of calcification and FSP calcification for the detection of malignancy was 63.55% and 24.30%, respectively; the specificity was 69.77% and 96.77%, respectively. CONCLUSIONS: The detection of calcification on ultrasonography should increase the clinical index of suspicion for thyroid carcinoma and alert the physician. FSP calcification is valuable and has a very high specificity for predicting thyroid carcinoma, particularly for those younger than 45 years old or with calcified regional lymph nodes. To increase the sensitivity for the diagnosis of thyroid carcinoma, tests such as fine-needle aspiration cytology should also be performed. The use of these modalities could result in earlier detection of thyroid carcinoma. The use of ultrasound to detect calcification and FSP calcification is as efficient as thyroid papillary macrocarcinoma in predicting microcarcinoma.

Acute myelogenous leukemia-derived SMAD4 mutations target the protein to ubiquitin-proteasome degradation.

Disruption of transforming growth factor-beta (TGFB1/TGF-beta) signaling contributes to the formation of human hematological malignancies. Smad4, a tumor suppressor, functions as an essential intracellular signal transducer of the TGF-beta signaling pathway. Recent studies have demonstrated that some tumor-derived mutations of Smad4 are associated with protein instability; however, the precise mechanism by which mutated Smad4 proteins undergo rapid degradation remains to be elucidated. A missense mutation of the SMAD4 gene in the Mad homology 1 (MH1) domain (c.305C>T, Pro102Leu) and one frameshift mutation resulting in termination in the Mad homology 2 (MH2) domain (c.1447_1448insAATA, Delta483-552) have been identified in acute myelogenous leukemia. It is not known whether protein instability of these SMAD4 mutants is one of the contributors to TGF-beta signaling disruption in acute myelogenous leukemia. Here we report that these two acute myelogenous leukemia-derived SMAD4 mutants are degraded rapidly when compared to their wild-type counterpart. We have demonstrated that both mutated proteins exhibit enhanced polyubiquitination (or polyubiquitylation) and proteasomal degradation. Importantly, we found that beta-transducin-repeat-containing protein 1 (beta-TrCP1), an F-box protein in the ubiquitin E3 ligase Skp1-Cullin-F-box protein (SCF) complex, directly interacts with and acts as a critical determinant for degradation of both mutated SMAD4 proteins. In addition, small interference RNA (siRNA)-triggered endogenous beta-TrCP1 suppression increased the protein expression level of both overexpressed SMAD4 mutants and endogenous mutated SMAD4 protein in acute myelogenous leukemia cells. These data suggest that mutated SMAD4 proteins undergo rapid degradation in acute myelogenous leukemia cells via SCF(beta-TrCP1) E3 ligase-mediated protein ubiquitination (or ubiquitylation) and subsequent proteasomal degradation.

Diisocyanate asthma and gene-environment interactions with IL4RA, CD-14, and IL-13 genes.

BACKGROUND: Diisocyanate asthma (DA) affects 2% to 10% of exposed workers, yet the pathogenetic mechanisms underlying this disorder remain ill defined. OBJECTIVE: To determine if specific single nucleotide polymorphisms (SNPs) of interleukin 4 receptor alpha (IL4RA), IL-13, and CD14 promoter genes are associated with DA. METHODS: Sixty-two workers with DA confirmed by specific inhalation challenge (SIC) and 75 diisocyanate-exposed, SIC-negative workers were analyzed for SNPs associated with IL4RA, IL-13, and CD14 promoter genes. RESULTS: No associations were found with individual SNPs and DA. When stratified according to specific diisocyanate exposure, a significant association was found between IL4RA (I50V) II and DA among individuals exposed to hexamethylene diisocyanate (HDI) (odds ratio [OR], 3.29; 95% confidence interval [CI], 1.33-8.14; P = .01) only. Similarly, the IL4RA (I50V) II and IL-13 (R110Q) RR combination was significantly associated with DA in HDI-exposed workers (OR, 4.13; 95% CI, 1.35-12.68; P = .01), as was the IL4RA (I50V) II and CD14 (C159T) CT genotype combination (OR, 5.2; 95% CI, 1.82-14.88; P = .002) and the triple genotype combination IL4RA (I50V) II, IL-13 (R110Q) RR, and CD14 (C159T) CT (OR, 6.4; 95% CI, 1.57-26.12; P = .01). CONCLUSIONS: Gene-environmental interactions may contribute to the pathogenesis of DA, and gene-gene interactions may modulate this relationship.

Men who have sex with men and human immunodeficiency virus/sexually transmitted disease control in China.

OBJECTIVES: To address the role of men who have sex with men (MSM) in the human immunodeficiency virus (HIV)/sexually transmitted disease (STD) epidemic in China. GOAL: To explore the prevalence of risky sexual behaviors and the existing prevention efforts among men who have sex with men (MSM) in China. STUDY DESIGN: Review of behavioral and STD/HIV prevention studies addressing MSM in China. RESULTS: Sexual risk behaviors including unprotected group sex, anal sex, casual sex, and commercial sex were prevalent among Chinese MSM. Many Chinese MSM also engaged in unprotected sex with both men and women. Most MSM either did not perceive that they were at risk of HIV/AIDS or underestimated their risk of infection. Surveillance and intervention research among these men are still in the preliminary stages. CONCLUSIONS: Chinese MSM are at risk for HIV/STD infection and potential transmission of HIV to the general population. In addition to sexual risk reduction among MSM, reduction of homosexuality- related stigma should be part of effective intervention efforts. Volunteers from the MSM community and health care workers in primary health care system may serve as valuable resources for HIV/STD prevention and control among MSM.

Syphilis among female sex workers in southwestern China: potential for HIV transmission.

OBJECTIVE: The objective of this study was to investigate the prevalence and risk factors of syphilis infection among female sex workers in a southwestern Chinese city along a drug trafficking route. METHODS: From December 2004 to January 2005, 343 female sex workers (FSWs) were recruited through community outreach and peer referring. Confidential questionnaire interviews were administered to collect information on sexual and other sexually transmitted diseases/HIV risk behaviors. Cervical and blood samples were collected to test for syphilis and HIV antibodies and Neisseria gonorrhoeae infection; Condyloma acuminatum infection was diagnosed clinically. RESULTS: Of 343 FSWs, 15.7% were infected with syphilis. The prevalence of HIV, gonorrhea, and C. acuminatum was 0.6%, 2.0%, and 1.5%, respectively. The subtype of 2 HIV-1 infections was both CRF 07BC. Nearly 10% of FSWs reported using illicit drugs. Consistent use, inconsistent use, and never use of condoms with commercial sex clients in the last month were reported by 53.9%, 38.5%, and 7.6% FSWs, respectively. Longer duration of sex work (odds ratio [OR] = 1.98; 95% confidence interval [CI] = 1.08-3.62), working at low-end establishments (OR = 2.03; 95% CI = 1.10-3.76), and if primary sex partners had sex with other women in the past 6 months (OR = 2.06; 95% CI = 1.08-3.91) were independently associated with syphilis infection. CONCLUSIONS: High prevalence of syphilis and overlapped unprotected commercial sex and drug using behaviors among FSWs along a drug-trafficking route may suggest a potential for rapid spread of HIV from injection drug users to FSWs and then to the general population and underscore the urgency of preventive interventions to break the bridge of FSWs for HIV/sexually transmitted disease spread.

Tyrosinase maturation through the mammalian secretory pathway: bringing color to life.

Tyrosinase has been extensively utilized as a model substrate to study the maturation of glycoproteins in the mammalian secretory pathway. The visual nature of its enzymatic activity (melanin production) has facilitated the identification and characterization of the proteins that assist it becoming a functional enzyme, localized to its proper cellular location. Here, we review the steps involved in the maturation of tyrosinase from when it is first synthesized by cytosolic ribosomes until the mature protein reaches its post-Golgi residence in the melanosomes. These steps include protein processing, covalent modifications, chaperone binding, oligomerization, and trafficking. The disruption of any of these steps can lead to a wide range of pigmentation disorders.

Eotaxin-3 and a uniquely conserved gene-expression profile in eosinophilic esophagitis.

Eosinophilic esophagitis (EE) is an emerging disorder with a poorly understood pathogenesis. In order to define disease mechanisms, we took an empirical approach analyzing esophageal tissue by a genome-wide microarray expression analysis. EE patients had a striking transcript signature involving 1% of the human genome that was remarkably conserved across sex, age, and allergic status and was distinct from that associated with non-EE chronic esophagitis. Notably, the gene encoding the eosinophil-specific chemoattractant eotaxin-3 (also known as CCL26) was the most highly induced gene in EE patients compared with its expression level in healthy individuals. Esophageal eotaxin-3 mRNA and protein levels strongly correlated with tissue eosinophilia and mastocytosis. Furthermore, a single-nucleotide polymorphism in the human eotaxin-3 gene was associated with disease susceptibility. Finally, mice deficient in the eotaxin receptor (also known as CCR3) were protected from experimental EE. These results implicate eotaxin-3 as a critical effector molecule for EE and provide insight into disease pathogenesis.

Linkage disequilibria and haplotype structure of four SNPs of the interleukin 1 gene cluster in seven Asian Indian populations.

Variation at four single nucleotide polymorphism (SNP) sites of the interleukin 1 (IL1) gene cluster was investigated among 280 unrelated individuals, representing 7 caste groups from the state of Karnataka, India, and one European American community of Boston, Massachusetts. Allele and haplotype frequencies, strength of linkage disequilibrium, and signatures of recombination varied considerably among populations. Variable community sizes and traditions of consanguinity may account for the observed variation.

[The changes of advanced glycation end products in a rat liver fibrosis model and the interventional effect of aminoguanidin].

OBJECTIVE: To study the changes of advanced glycation end products (AGEs) in different phases of a rat liver fibrosis model induced by CCl4, and the interventional effect of aminoguanidin (AG). METHODS: Fifty-four SD rats were divided into three groups: a control group, a CCl4 model group and an intervention group. Their blood serum AGEs and hyaluronic acid (HA) and AGEs in their liver homogenates were measured. These measurements were correlatively assessed to the degrees of liver fibrosis at different phases of the rat model before and after the intervention with aminoguanidin. RESULTS: The content of AGEs in their blood sera and liver homogenates, and the level of blood serum HA, and the score of liver fibrosis degree at week 12 in our rat liver fibrosis mode groups were significantly higher than those in the control group (P < 0.01). In the intervention group with aminoguanidin, these figures were lower than those in the liver fibrosis model group (P < 0.05). The content of AGEs in their blood sera and liver homogenates had a linear correlation with the level of HA in their blood sera. CONCLUSION: The contents of AGEs in their blood sera and liver homogenates were increased in the late phase of our rat liver fibrosis model. To some extent, the level of AGEs may reflect the fibrosis degree of the rat livers. Aminoguanidin has an interventional effect in our CCl4 induced rat liver fibrosis model.

[Study on sexual behavior and HIV/STIs among miners in Yunnan province].

OBJECTIVE: This paper aims to describe human immunodeficiency virus/sexual transmitted infections (HIV/STIs) related knowledge, attitudes, practice and the prevalence of HIV/STIs amongst miners. METHODS: Two focus-group related discussions with a total number of 13 members including Community Advisory Boards (CAB) and 12 miners were conducted in a mining township in Yunnan province. Questionnaire surveys and HIV/STIs tests were conducted among 233 miners recruited by cluster sampling in two towns where the mines were located. RESULTS: The average age of respondents was 28 year old with 82.8% of them younger than 35 year old. 95.3% of the respondents attended the education level of junior middle school. AIDS related knowledge among miners was low. The percentage of right answers to the routes of transmission was only 54.4%. The ratio of self-reported prostitutes visits was 9.0%. The prevalence rates of Neisseria gonrrhoeae, HIV and Chlamydia trachomatis were 0.4%, 0.4%and 8.2% respectively. The correlation between Chlamydia trachomatis infection and education (P = 0.0347) was significant, and so was that between Chlamydia trachomatis infection and marriage status (P = 0.032). CONCLUSION: This study showed that the awareness of HIV/STIs prevention was limited and the rate of condom use was low, suggesting that miners needed to be viewed as a key population in HIV/STIs prevention and control.

[Recent advances in the studies of genetic polymorphisms in diabetic retinopathy].

Diabetic retinopathy is one of the most serious complications of diabetes mellitus, which affects the health and life quality seriously. The development of procedures for prevention and treatment of diabetic retinopathy is one of the most important problems that should be solved. Epidemiological studies indicate that the development of diabetic retinopathy is not only determined by duration of diabetes and blood glucose control; individual diversities also play an important role. Some patients have a long course of disease and poor control of blood glucose, but they do not have diabetic retinopathy or only show slight pathological changes after a long term follow-up. Other patients only have a short term disease and well control of blood glucose, but they develop serious proliferative diabetic retinopathy. These patients usually have a positive family history. This phenomenon may relate to various genetic liabilities. This review introduce the recent advances in the studies of genetic polymorphisms in diabetic retinopathy.

Control of stress propagation in the cytoplasm by prestress and loading frequency.

One fundamental question in cell biology is how mechanical stresses are distributed inside the cytoplasm. Recently we have developed a synchronous detection approach to map cytoplasmic displacements and stresses using yellow fluorescent protein tagged mitochondria as fiducial markers of the cytoskeleton (CSK) in response to a localized load applied via an RGD-coated magnetic bead (7). We have shown that stresses are propagated to remote sites in the cytoplasm, a finding that contradicts continuum model predictions. Here we show that long distance force propagation in the cytoplasm was abolished when the contractile prestress in the CSK was lowered by relaxing agents and enhanced when the prestress was increased by contractile agonists. Surprisingly, when the loading frequency was varied from 0.03 Hz to 30 Hz, the total area of induced displacements (an index of the extent of stress propagation) first increased with loading frequency and then decreased with loading frequency. These results demonstrate that the long distance force propagation in living adherent cells might be controlled by the level of contractile prestress in the CSK and by the loading frequency.

[Effect of vascular endothelial growth factor antibody Avastin on angiogenesis of human gastric cancer growing orthotopically in nude mice].

BACKGROUND & OBJECTIVE: The failure to treat gastric cancer is often due to the recurrence or dismal metastasis of cancer and the poor response to traditional chemotherapeutic or radiotherapeutic regimens. Vascular endothelial growth factor (VEGF) is related closely to the growth and metastasis of gastric cancer. This study was to examine the effects of Avastin, a monoclonal antibody developed against VEGF, with or without 5-fluorouracil (5-FU) on the growth, metastasis, and angiogenesis of a gastric cancer model. METHODS: A metastasis model of human gastric cancer was established by orthotopic transplantation of histologically intact human tumor tissue blocks in the gastric walls of nude mice. All mice were randomly divided into 4 groups: control group, 5-FU group, Avastin group, and combination treatment group. Forty-two days after treatment, tumor weight, inhibition rates, intratumoral microvessel density, apoptosis index, and presence of metastasis were evaluated. RESULTS: Combination therapy with 5-FU and Avastin augmented the reduction of primary tumor growth and inhibited metastasis compared with treatment with either agent alone. After treatment with Avastin plus 5-FU, a significant increase in apoptotic tumor cells and a decrease in microvessel density were observed. CONCLUSIONS: Avastin plus 5-FU can induce apoptosis in gastric cancer cells and has strong inhibitory effects on tumor growth and the metastasis to the liver. Anti-VEGF therapy in combination with traditional chemotherapy may be a novel therapeutic approach for advanced gastric cancer.

[Clinical analysis of dopa-responsive dystonia and mutation analysis of the GCH I gene].

OBJECTIVE: To investigate the clinical characteristics and GCH I gene mutations in patients with dopa-responsive dystonia (DRD). METHODS: The clinical features of 3 families with 6 affected members and 8 sporadic cases were analyzed to determine the clinical characteristics, and 2 families with 4 affected members and 2 sporadic cases were screened for mutations of the GCH I gene. RESULTS: Age at onset was (10 +/- 3) years. Onset occurred earlier in female (9 +/- 4) years than in male (12 +/- 1) years. The initial symptom was a gait disorder, dystonia or tremor in most patients and nine patients (64%) presented with diurnal fluctuation. Thirteen patients (93%) were cured and one was improved after administration of low doses of levodopa for 3 months and no long-term side effects of levodopa had occurred. Two independent mutations were found in three patients. Gln161Pro, a new missense mutation, was found in a sporadic case, leading to a relatively severe phenotype. The two patients with mild phenotype in one family were found to have Lys224Arg mutation, as previously described. CONCLUSIONS: DRD patients have diverse phenotypes and diurnal fluctuation is an important feature. They have dramatic and sustained response to levodopa. There may be a correlation between genotype and phenotype. The detection of GCH I mutations is helpful in early diagnosis of non-typical cases.

[Epidemiology of pediatric HIV infection in six provinces of China].

OBJECTIVE: To investigate the epidemiology of pediatric human immunodeficiency virus (HIV) infection in six provinces of China. METHODS: A cross-sectional study was conducted in six provinces with the highest HIV prevalence. Surveys on demographics and HIV-related questions (transmission modes, time of diagnosis, clinical stage, laboratory test) were distributed to clinicians in these provinces. Descriptive and bivariate analyses were performed on the completed surveys. RESULTS: Survey results of 650 children [405 males and 245 females; average age: (7.9 +/- 3.2) years] were eligible for analysis. The interval between possible transmission and diagnosis was (7.1 +/- 3.2) years. The location distribution was as follows: 570 cases (87.7%) in Henan Province, 23 cases (3.5%) in Guangxi Province, 21 cases (3.2%) in Yunnan Province, 19 cases (2.9%) in Hubei Province, 10 cases (1.5%) in Anhui Province, and 7 cases (1.1%) in Shanxi Province. Transmission routes included mother-to-child transmission (75.1%), blood transfusion/ plasma donation (15.7 %), and injecting drug use (IDU, 0.5%). Former plasma donation (FPD) was the main transmission route in some provinces (Henan, Shanxi, Hubei, and Anhui), while IDU was the main transmission route in other provinces (Guangxi and Yunnan). The average age in the FPD provinces was significantly higher than that in IDU provinces [(8.1 +/- 3.2) vs. (5.4 +/- 2.2) years, P <0.001]. Among 178 patients in all six provinces who required treatment (on the basis of CD4 count or WHO staging), 133 (74.7%) did not receive treatment and 45 (25.3%) received antiretroviral therapy. CONCLUSION: Mother-to-child transmission is the main transmission mode in pediatric patients. Efforts should be made to strengthen the diagnosis and treatment of pediatric HIV/AIDS patients.

The adrenal gland is a source of stress-induced circulating IL-18.

The present study compared plasma IL-18 levels between sham-operated and adrenalectomized mice following stress to investigate whether the adrenal gland contributes to the elevation of circulating IL-18 during stress. Two hours of stress provoked a robust, stressor-dependent, elevation of IL-18 mRNA and peptide in the adrenal gland in sham-operated mice. Consistently, levels of circulating mature IL-18 increased during stress and remained elevated for up to 6 h after stress. The stress-induced increase in circulating IL-18 was abolished by adrenalectomy. These findings demonstrate that the adrenal gland is required to achieve elevation of circulating IL-18 during stress.
