Radioimmunoassay for vasoactive intestinal polypeptide (VIP) in plasma before and during endoscopic examinations.

A specific and sensitive radioimmunoassay of human vasoactive intestinal polypeptide using synthetic VIP as standard preparation and antiserum to synthetic VIP R 502 (Yanaihara et al. 15) is described. No crossreactions with a number of other gastrointestinal hormones such as glucagon, secretin, GIP, HPP or substance P was detected. Aprotinin (Trasylol, Bayer) or heparin had no influence on the antigen--antibody reaction. High concentrations of sodium or potassium chloride in plasma, or hyperlipoproteinemia, did not interfere with the antiserum in the described system. Basal plasma concentration of VIP in 18 females (means +/- sem = 24.05 +/- 1.79 VIP/l) and 20 males (means +/- sem = 24.11 +/- 1.91 pmol/l VIP) showed no sex specific variations. During gastroscopy plasma VIP levels were significantly higher than basal values, showing a peak secretion when the gastric antrum was inspected. During the entire colonoscopic examination, VIP levels were significantly higher than basal values. Endoscopic examination may possibly liberate VIP from the VIP containing cells which can be found throughout the gastrointestinal tract.

[Surgical or endoscopic papillotomy? (author's transl)].

Endoscopic papillotomy has become a competitive method to the surgical technique. Endoscopy took over many cases formerly treated surgically. The indications to either procedure are defined. The alterations of papilla and peripapillary region of choledochus are treated by endoscopy. The surgical method is indicated in gallbladder diseases, in further accessory operations and in failing of endoscopic proceeding. The complication rate of endoscopic papillotomy is 6.7 p.c. and about 20 p.c. under the rate of the surgical method. The relation of mortality rate is given with 1.5 p.c.: 3.8 p.c. in favour of endoscopy. Indications, advantages, disadvantages and complications of both methods are discussed, using the own case material of 211 surgical and 714 endoscopic papillotomies.

[Relaparotomy or endoscopic surgery in complications after bile duct surgery].

The number of cases in bile duct surgery has continuously increased since second world war. Correcting operation is necessary in 5 to 7% after cholecystectomy. Since 1973 more and more the endoscopic treatment takes place of surgery. Typical complications can be divided into two groups belonging to their manifestation. Early complications are due to an insufficient intraoperative diagnosis or injuries of bile tract. Late complications comprehend successive development of duct stenosis and cholangitis. The therapeutic possibilities of endoscopic papillotomy are directed only to the papilla and near-by sections of the choledochus, while surgical treatment is due to the more difficult performance of iatrogenic lesions. The indications to each method are shown and discussed by 138 surgical and 714 endoscopic treatments.

[Treatment of complications after endoscopic papillotomy (author's transl)].

In 717 endoscopic papillotomies there were 52 complications in 48 patients, requiring 15 emergency operations with 11 deaths. Surgical intervention was undertaken in six patients for cholangitis after obstruction by stone, in four for retroperitoneal perforation, in two each for bleeding or jamming of the dormia basket, and in one for pancreatitis. Cause of death was very poor general condition or age over 80 years in four; retroduodenal abscess in four; haemorrhagic pancreatitis in two, and cholangiogenic sepsis in one.

Ultrastructural morphometric investigations on normal human liver biopsies.

Normal liver biopsies of 14 volunteers were investigated ultrastructural morphometrically. Besides the determination of morphometric baseline data for comparative purposes particular emphasis was placed on the question, if morphometric evaluations of human livers yield reliable results in spite of the higher range of distribution than in experimental liver material. The results showed that the surface of the smooth endoplasmic reticulum of human liver is significantly greater than the surface of rough endoplasmic reticulum. Otherwise the morphometric data of the human liver tissue did not differ essentially to the values of the laboratory rat. The variance analytical consideration of the mean evaluated from the data of the 14 liver biopsies indicated that the standard error of nearly all proved parameters was less than 10% of the mean. The results suggest that ultrastructural morphometric evaluation of human liver biopsy samples yield reliable results provided the samples investigated are sufficiently extensive.

[Therapeutic endoscopic retrograde cholangiography (author's transl)].

Endoscopic papillotomy is a further development of endoscopic cholangiography. This therapeutic procedure was successful in 516 (94.7%) of 545 cases. Choledocholithiasis (405 cases) and papillary stenosis (97) were the main indications for the procedure. Of 405 patients with choledocholithiasis, 376 (93%) are now free of stones. There were 38 (7.4%) complications and 5 (1%) of these had a lethal outcome with (11) (2%) requiring emergency laparotomy. Cholangitis was the most serious complication. It is becoming increasingly evident that the endoscopic papillotomy is replacing reoperation in many cases of choledocholithiasis.

[Mitochondrial morphology and metabolism in extrahepatic obstruction (author's transl)].

Corresponding to the increased activity of mitochondrial Glutamate-Dehydrogenase (GLDH) in serum in extrahepatic cholestasis a swelling of hepatic mitochondria could be shown on ultrastructural morphometric analysis. The morphometric alterations parallel the serochemical findings. A significant correlation could be calculated between the swelling of mitochondria and the activity of GLDH in serum.

[On thyroid phosphatase in human serum (author's transl)].

Patients with hyperthyreoidism often present an increased activity of alkaline phosphatase in serum. Determination of isoenzymes may help to assign to different organs an elevated enzyme-activity in serum. This report deals with a patient suffering from hyperthyreoidism. In his serum alkaline phosphatase of the thyroid gland could be identified by comparison with organ-extracts. Raised activity of this isoenzyme can contribute besides bone, liver and small bowel to the increased serum-activity of total alkaline phosphatase in patients with hyperthyreoidism.
