Mechanistic aspects of blood-contacting properties of polypropylene surfaces--from the viewpoint of macromolecular entanglement and hydrophobic interaction via water molecules.

Polypropylene surfaces with a particular crystalline-amorphous microstructure have been demonstrated to reduce protein adsorption and platelet activation. Such blood-contacting properties may be affected by the crystalline-amorphous microstructure of the surfaces, although wettability such as dynamic contact angles and surface free energy components were almost constant, being independent from the variation in the microstructure. In order to clarify the mechanistic aspects on their blood-contacting properties, the physicochemical properties of the surfaces were evaluated for a series of compression-molded polypropylene sheets in terms of the work of adhesion and the structure of sorbed water. The work of adhesion of the compression-molded sheets increased with decreasing surface layer crystallinity, presumably due to macromolecular entanglement with a polymeric glue used. The work of adhesion involving macromolecular entanglement may occur between proteins and the surfaces. Thus, a decrease in the surface layer crystallinity is considered to cause an increase in the protein adsorption. The structure of water sorbed into the sheets changed--it was more gaseous (isolated) at the surfaces with a higher crystallinity. This suggests that the hydrophobic interaction via water molecules increased with surface layer crystallinity, resulting in increasing protein adsorption and denaturation. Thus, it is considered that both macromolecular entanglement and hydrophobic interaction are important on the mechanistic aspects of blood-contacting properties of polypropylene surfaces. In order to confirm this hypothesis, the evaluation of the physicochemical properties and blood-contacting properties was also performed on a series of uniaxially drawn polypropylene films. A decrease in the work of adhesion and the hydrophobic interaction at the surfaces was observed with increasing draw ratio, and the protein adsorption and platelet activation were effectively prevented with increasing draw ratio. This result supports our hypothesis. Therefore, it is concluded that the excellent blood-contacting properties of polypropylene surfaces can be achieved by reducing the macromolecular entanglement and the hydrophobic interaction with proteins.

Venous response after lidocaine administration in arm veins.

We previously reported that cubital venous pressure (Pv) tended to increase initially, but this was followed by a drop in a dose-dependent response after intravenous lidocaine administration in subjects with various diseases. In this study we examined whether Pv responses after small-dose intravenous lidocaine administrations are related to the stimulating effect of lidocaine on vascular smooth muscle (VSM). In 5 subjects free of cardiovascular disease, Pv increased slightly with decreased pulsations after a 10 mg dose (p<0.01) with no change in central venous pressure. In the cinephlebographic test performed on 2 healthy volunteers, Pv increased during recovery from proximal venoconstriction caused by an injection of contrast medium mixed with 10 mg lidocaine. In 9 subjects with cardiovascular disease, deltaPv spread in the same directions (+ or -) after 5 and 10 mg drug administrations. In 6 of those tested with both drug doses, deltaPv had positive means and no significant difference was observed. Thus, Pv responses after small-dose lidocaine administrations are consistent with neither the stimulating effect of lidocaine nor with a dose-dependent response. They could be attributed to the spasmolytic effect of lidocaine on the basal tone of VSM, which could be modulated by disease conditions.

[Frameless registration for chest SPECT and X-ray CT image by volume matching].

Image registration of functional (SPECT) and morphological (X-ray CT/MRI) images has been studied in order to improve the accuracy and the quality of the image diagnosis. This paper describes a new registration method for chest SPECT and X-ray CT images. Presented method is a frameless and automatic registration method which calculates a transformation matrix between two coordinate systems of image data by an optimization method. This registration method uses similar physical characteristics of X-ray CT and transmission CT image. The three-dimensional overlap of the body region is used for image matching. Precision evaluation and visual image evaluation were conducted. The result of the precision evaluation with a phantom and clinical data suggested the clinically acceptable robustness in registration procedure. Visual evaluation of registered images confirmed the usefulness of this method in clinical applications.

Reduction of surface-induced platelet activation on phospholipid polymer.

omega-Methacryloyloxyalkyl phosphorylcholine (MA-PC) polymers which have been synthesized with attention to the surface structure of a biomembrane show excellent blood compatibility, i.e., resistance to protein adsorption and blood cell adhesion. To clarify the stability of platelets in contact with the MAPC polymer surfaces, cytoplasmic free calcium concentration ([Ca2+],) in the platelets was measured. A platelet suspension was passed through a column packed with various polymer beads after treatment with plasma, and the [Ca2+]i in the platelets eluted from the column was measured. The [Ca2+]i in contact with the MAPC polymers, i.e., poly[2-methacryloyloxyethyl phosphorylcholine-co-nbutyl methacrylate (BMA)] (PMEB) and poly(6-methacryloyloxyhexyl phosphorylcholine-co-BMA) (PMHB), was less than that in contact with poly(BMA). However, poly(10-methacryloyloxydecyl phosphorylcholine-co-BMA) (PMDB) was not effective in suppressing the increase in [Ca2+]i, and thus was at the same level as in the poly(BMA). This result indicated that platelets in contact with PMEB or PMHB were less activated compared with those in contact with PMDB and poly(BMA). Moreover, the state of the platelets adhered to these polymer surfaces, both morphologically and immunologically, was examined. Scanning electron microscopic observation of the polymer surface after contact with a platelet suspension revealed that many platelets adhered and changed their shape on the poly(BMA). The numbers of adhetent platelets were reduced on all MAPC polymer surface. The relative amount of alpha-granule membrane glycoprotein (GMP-140) which appears on the cell membrane by activation of platelets on the PMEB surfaces was less than that on poly(BMA) and poly(2-hydroxyethyl methacrylate). These results suggest that PMEB and PMHB suppressed not only platelet adhesion but also activation of the platelets in contact with these surface.

Thallium-201 single-photon emission tomography in the treatment follow-up of nasopharyngeal carcinoma.

In order to assess the usefulness of thallium-201 single-photon emission tomography (SPET) in the treatment follow-up of nasopharyngeal carcinoma (NPC), a total of 75 201T1 SPET studies were performed in 18 patients with histologically proven NPC. The findings were compared with those of magnetic resonance imaging (MRI) before and after therapy. Four patients received radiotherapy alone while the other 14 received concurrent chemo-radiotherapy. Treatment response was classified as complete (CR) or partial (PR) based on the findings of MRI and 201T1 SPET. Intense 201T1 uptake by the tumour was seen in all 18 patients before treatment. After treatment, MRI showed seven CRs and 11 PRs, whereas 201T1 SPET showed 13 CRs and five PRs. In 12 patients, the results of 201T1 SPET were in agreement with those of MRI. In six patients MRI showed PR but 201T1 showed CR. Follow-up (mean 10.6 months) MRI and 201T1 SPET studies of these six patients revealed that tumour gradually decreased and finally vanished in three patients. This preliminary study indicates that 201T1 SPET has potential in the assessment of early response to treatment of patients with NPC when compared with MRI.

Quantitative evaluation in tumor SPECT and the effect of tumor size: fundamental study with phantom.

An experimental study with phantoms was performed in order to evaluate the effect of the tumor volume on the quantitative estimation in tumor SPECT. The ratio of mean count/pixel in the phantom to that of the background (T/N ratio) was well correlated with the size of the phantom: even when the concentration of the Tc-99m O-4 solution of globular phantoms with diameters of 29, 37 and 46 mm was constant, the greater the size of the phantom, the higher was the T/N ratio. This study showed that we should understand that the T/N ratio was certainly affected by the reduction of the tumor size itself whenever we evaluate treatment-response or assess tumor viability after treatment by reference to the T/N ratio.

Abnormal cerebral perfusion in chronic methamphetamine abusers: a study using 99MTc-HMPAO and SPECT.

1. Cerebral blood flow of nine methamphetamine abusers with technetium-99m-hexamethylpropyleneamine oxime (HMPAO) and single photon emission computed tomography (SPECT) as well as morphological examination with magnetic resonance imaging (MRI) were investigated. 2. Six of these subjects exhibited multiple focal perfusion deficits in cerebral cortices without abnormalities in MRI including cerebral atrophy and/or infarctions. 3. Cerebral perfusion deficits were detected in methamphetamine abusers even after a long abstinence period, suggesting that vascular changes were irreversible to some degree. 4. HMPAO SPECT study appeared to be sensitive to the detection of cerebral perfusion abnormalities in drug abusers.

Blood compatibility of polypropylene surfaces in relation to the crystalline-amorphous microstructure.

Blood-contacting properties of polypropylene surfaces with different crystalline states at the surface layer were examined in terms of plasma protein adsorption and changes in cytoplasmic free Ca2+ levels in platelets. Though the wettability of polypropylene surfaces was almost constantly independent from the surface layer crystallinity and interlamellar spacing, an increase in adhesiveness was observed with decreasing surface layer crystallinity and interlamellar spacing. It is suggested that the surface properties of the sheets varied in relation to the crystalline-amorphous microstructure. Minimum magnitudes in albumin and fibrinogen adsorption were observed on the polypropylene surface with a particular surface layer crystallinity (c. 55 wt%). A decrease in interlamellar spacing resulted in enhancing albumin adsorption and diminishing fibrinogen adsorption. Transient phenomena in plasma protein adsorption were observed on their surfaces with a plasma concentration. It is considered that the polypropylene surface with a particular crystalline-amorphous microstructure reduces the denaturation of adsorbed proteins. An increase in cytoplasmic free Ca2+ levels in platelets was prevented at the polypropylene surface with a surface layer crystallinity of 55 wt%: the particular crystalline-amorphous microstructure of such apolar surfaces as polypropylenes acts to reduce platelet activation. Thus, it is concluded that the blood compatibility of polypropylene surfaces is greatly improved by controlling a crystalline-amorphous microstructure at the surface layer.

Synthesis and characterization of biodegradable polyrotaxane as a novel supramolecular-structured drug carrier.

Polyrotaxanes were synthesized as novel biodegradable polymers with supramolecular assembly and their properties evaluated in vitro. The synthesis of biodegradable polyrotaxanes consists of three steps: preparation of an inclusion complex consisting of alpha-cyclodextrins (alpha-CDs) and amino-terminated poly(ethylene glycol) (PEG); introduction of L-phenylalanine (L-Phc) at each complex terminal via peptide linkages: and hydroxypropylation of alpha-CDs in the polyrotaxanes. Succinimide ester of benzyloxycarbonyl-L-Phe was condensed with the terminal amino groups of the inclusion complex. 1H-NMR and GPC results showed that alpha-CDs were threaded onto a PEG chain and L-Phe moieties were introduced at each terminal of the PEG chain. Further, the amount of threaded alpha-CDs was found to be governed by the molecular weight of PEG. The hydroxypropylation of alpha-CDs improved the solubility of the polyrotaxanes in PBS (pH 7.4). The hydroxypropylated (HP-) polyrotaxanes were characterized by terminal peptide cleavage using papain. In vitro degradation of HP-polyrotaxanes revealed that HP-alpha-CDs threaded onto a PEG chain were released only when terminal peptide linkages were cleaved. Moreover, threaded HP-alpha-CDs onto a PEG chain was found to be completely released. Kinetics of terminal peptide cleavage were also evaluated by catalytic efficiency (kcat/K(m)). The kcat/K(m) values were found to be independent of the molecular weight of HP-polyrotaxanes but to be affected by terminal hydrophobic moieties. It is proposed that our designed polyrotaxanes are feasible as novel drug carriers.

Double-stimuli-responsive degradation of hydrogels consisting of oligopeptide-terminated poly(ethylene glycol) and dextran with an interpenetrating polymer network.

Biodegradable hydrogels consisting of oligopeptide-terminated poly(ethylene glycol) (PEG) and dextran (Dex) with an interpenetrating polymer network (IPN) structure were prepared as models of novel biomaterials exhibiting a double-stimuli-response function. The IPN-structured hydrogels were synthesized by sequential cross-linking reaction of N-methacryloyl-glycylglycylglycyl-terminated PEG and Dex. In vitro degradation of the IPN-structured hydrogels was examined using papain and dextranase as model enzymes of hydrolyzing oligopeptide and Dex, respectively. Specific degradation in the presence of papain and dextranase was observed in the IPN-structured hydrogel with a particular composition of oligopeptide-PEG and Dex. This same hydrogel was not degraded by one of the two enzymes. The IPN-structured hydrogels were characterized by water content, thermal mechanical analysis, and wide-angle X-ray diffraction, and the results were compared with those of co-cross-linked hydrogels consisting of N-methacryloyl-glycylglycylglycyl-terminated PEG and methacryloyl Dex. The results suggest that the IPN-structured hydrogels contain physical chain entanglements between networks as well as chemical cross-linked networks. It is concluded that the double-stimuli-responsive degradation observed in the IPN-structured hydrogel is achieved by controlling the chain entanglements between the two biodegradable polymers. Such degradation property of the IPN-structured hydrogel can be useful as a fail-safe system for guaranteed drug delivery and/or medical micromachines.

[Quantitative measurement of cerebral blood flow by the microsphere model with super-early 123I-IMP brain SPECT].

Cerebral blood flow (CBF) was quantitatively measured in 6 healthy young volunteers based on "super-early" acquisition of N-isopropyl-p-[123I]iodoamphetamine (IMP) brain SPECT obtained 4-6 min after IMP injection with a three-head rotating gamma camera and the microsphere (MS) model. The ratio of radioactivity (count/pixel/min) in the conventional early SPECT image (taken 25-55 min after IMP injection) to that in the "super-early" image for each brain region negatively correlated with regional CBF value obtained with the "super-early" MS method. This indicates that wash-out of IMP from the regions with higher CBF is faster than that from the regions with lower CBF and that CBF values are underestimated with the conventional MS method in regions with higher CBF. Regional CBF was quantitatively measured with the "super-early" MS method and the ARG method, a recently developed method based on two-compartment model. The mean cortical CBF was 52.5 +/- 7.0 (ml/100 g/min, mean +/- SD) with the "super-early" MS method and 47.5 +/- 3.3 with the ARG method. The CBF values obtained with the "super-early" MS method agreed with those previously reported with positron emission tomography. Since the MS method is theoretically the simplest model, the "super-early" MS method can be applied various disorders of the central nervous system where the behavior of IMP is not fully understood.

The effect of steroid on thallium-201 uptake by malignant gliomas.

In order to assess the effect of steroid on thallium-201 uptake by glioma, 201Tl single-photon emission tomography was performed before and after steroid administration in four patients with recurrent malignant glioma. After steroid administration the 201Tl index, expressed as the ratio of 201Tl uptake in the tumour to that in the contralateral cerebral hemisphere, was 0.77+/-0.11 of the value before steroid (mean+/-SD: P<0.05 by paired t test). The 201Tl index has been used as a possible indicator for the differentiation of malignant gliomas from relatively benign tumours or radiation necrosis. The present results indicate that the effect of steroid has to be taken into account when semi-quantitative analysis, e.g. by means of the 201Tl index, is used in patients with brain tumours.

Quantifying brain tumor blood flow by the microsphere model with N-isopropyl-p-[123I]iodoamphetamine super-early SPECT.

Regional cerebral blood flow was quantitatively measured in 6 patients with brain tumor by the microsphere model with N-isopropyl-p-[123I]iodoamphetamine (IMP) "super-early" single photon emission computed tomography (SPECT) images obtained 4-6 min after IMP injection with a three-head rotating gamma camera. The ratio of radioactivities (counts/pixel/min) in the "early" SPECT images (taken 25-55 min after IMP injection) to the "super-early" images of the brain tumors was 1.47 +/- 0.13 (mean +/- SD, n = 6), which was significantly lower than the ratio in the normal cerebral cortices (1.93 +/- 0.25) (p < 0.01). This indicates faster clearance of IMP from the tumor tissue than that from the normal brain tissue. Blood flow values for the brain tumors obtained by the microsphere model based on the "super-early" SPECT images were 39.3 +/- 12.4 ml/100 g/ min, which was similar to the blood flow values for normal gray matter and in agreement with previous studies with positron emission tomography.

Cytoplasmic calcium levels and membrane fluidity of platelets in contact with polyether-polyamide multiblock-copolymer surfaces.

Cytoplasmic calcium levels and the membrane fluidity of rabbit platelets stored in mini blood bags of crystalline-amorphous microstructured polymers (polyether-polyamide multiblock-copolymers) were studied. Fluorescent dye (Fura 2 or 1,6-diphenyl-1,3,5-hexatriene)-loaded platelet suspensions were stored at 37 degrees C for 1 h in the blood bags, and metabolic changes in the platelets during storage were evaluated by the fluorescent spectroscopic technique. The surfaces of poly(vinyl chloride) and polyolefin elastomers, which are used for commercially available blood bags, enhanced the progress of platelet metabolism; i.e., there was a dramatic decrease in membrane fluidity and an increase in [Ca2+]i. Furthermore, the decrease in membrane fluidity was observed prior to the increase in [Ca2+]i. These results suggest that the decrease in membrane fluidity of platelets in contact with polymer surfaces can be the dominant stage in the activation of these platelets. In contrast, the surfaces of polyether-polyamide multiblock-copolymers exhibited few changes in either membrane fluidity or [Ca2+]i levels. These results suggest that the platelets in contact with the crystalline-amorphous microstructured copolymer surfaces can be inert and inactivated in terms of the prevention of a decrease in membrane fluidity.

Improved blood compatibility of drawn polyamide sheets.

A series of aliphatic polyamide (nylon) sheets with different draw ratios were prepared and their blood-contacting properties examined in relation to the bulk and surface characteristics. Increases in both the molecular orientation and crystallinity were observed with increasing draw ratio. Their surface wettability slightly decreased with draw ratio, indicating less amide linkages at their surfaces by the drawing process. An increase in contact angle hysteresis by the drawing process is thought to be due to the existence of more amorphous chains at the surfaces. The adsorption of albumin and fibrinogen onto nylon sheets was significantly reduced with draw ratio, suggesting the formation of well-established crystalline-amorphous microdomain structures at the surfaces. Blood compatibility of these surfaces was greatly improved with draw ratio in terms of a change in cytoplasmic calcium levels in platelets contacting these surfaces. Thus, it is suggested that the control of crystalline-amorphous microdomain structures by the drawing process is a feasible approach to improve blood compatibility of commercially available semicrystalline polyamides.

[Recent trends in drug delivery systems using biomaterials].

Since the concept of sustained release of biologically active agents was established in 1970's, the sustained release has been examined by controlling the diffusion of drugs through polymeric matrices and/or the degradation of these polymers. Recently, drug release in proportion to internal or external stimuli has been getting important, which can be achieved by using stimuli-responsive polymeric materials. Majority of these polymers have been designed as to achieve their functions by changes in temperature, pH, glucose concentration, and the release of ribosomal enzymes. Especially, among those materials, biodegradable polymers have much potential for applications as implantable carriers for drug delivery system (DDS). In an auto feed-back drug delivery, several physiological changes in a living body can be utilized as the signal inducing polymer degradation and subsequent drug release. A hydrogel composed of hydrophilic biomaterials has been focused because of their high responsibility to the stimulus. In this paper, currently investigated DDS using hydrogels are reviewed with their strategies.

[Diagnostic accuracy of 201T1 SPECT.MRI in brain diseases and inter-reader variance of diagnostic performance].

Fifteen-four studies of 201T1 brain tumor SPECT were independently interpreted by 9 nuclear medicine physicians with and without reference magnetic resonance images in 2 separate sessions to define an effect of referring images, and inter-observer variations. The physicians were requested to detect foci of abnormal deposits, and to discriminate whether they were malignant or not according to 5-grade scaling of subjective diagnostic confidence. Receiver-operating characteristics (ROC) analysis was performed. Mean sensitivity for presence of lesions (SEP), and sensitivity and specificity for malignancy of 201T1 SPECT were 84, and 53 and 55%, which were changed to 94, and 74 and 55% after referring to the MR images. The SFP was significantly improved (p < 0.05), but sensitivity and specificity for malignancy, assessed by areas under the ROC curves, were not significantly improved. Inter-observer variation of the SFP was significantly reduced with addition of the referring MR images. Cerebral lobar localization of lesions by SPECT showed great inter-observer variations (true localization ranged from 30 to 68.4%). It is concluded that 201T1 brain tumor SPECT has moderate sensitivity and specificity for malignancy, which is not improved by addition of anatomical reference images, that additional MR images reduce inter-observer variation of confidence on lesion presence, and that SPECT localization of lesions has great inter-observer variations.

[Effectiveness of the radioactive strontium (89Sr) chloride agent, SMS.2P for pain palliation in patients with metastatic bone tumor in phase III multicenter clinical trial].

The phase III clinical trial of strontium-89 chloride agent (SMS.2P) was performed in 90 patients with painful bone metastases secondary to prostate (53), breast (18) and other types of cancer (19). Some patients experienced a transient increase in pain or nausea and vomiting. However both symptoms subsided and serious side effects were not observed in any of the patients. As reported, we confirmed some abnormal changes in peripheral blood picture. A decrease in the number of white blood cells and platelets was considered to be partly a result of bone marrow suppression due to 89Sr irradiation. Pain was substantially improved after 89Sr therapy in 58% of the patients and there was some alleviation in 12%. The release from pain was accompanied by an improved quality of life for these patients including sleep patterns and morbidity. Some patients were able to resume their former life styles. Most of the improved patients experienced pain relief from days to one week following 89Sr therapy and in half cases, this remained effective for 2 or 3 months. There were even cases in which the pain relief continued over an observation period of time of clinical study.
