A patient with repeated syncopal attacks after using isosorbide dinitrate.

The case of a patient with repeated attacks of collapse induced by sublingual isosorbide dinitrate is reported. The patient was an 81 year-old female who was admitted to Yura Hospital because of attacks of precordial pain. Several minutes after the sublingual administration of isosorbide dinitrate (10 mg) for an anginal attack, she developed a sensation of general weakness, and thereafter because unconscious. Arterial blood pressure fell and became unmeasurable. Electrocardiograms recorded during the syncopal attack showed sinus tachycardia and significant elevation of ST-segment in right precordial leads. In response to a drip infusion of noradrenaline, arterial blood pressure returned to normal with recovery of consciousness. Two similar syncopal attacks induced by sublingual isosorbide dinitrate occurred in the next three days. These attacks were not due to augmentation of the vagal reflex. Decrease of venous return probably was the primary etiological factor.

Pregnancy-associated globulin in body fluids of pregnant women.

Pregnancy-associated globulin (PAG) in saliva, urine and amniotic fluid was investigated just before and after delivery by Ouchterlony's method and immunoelectrosyneresis. In saliva, the incidence was more than 60 percent; 12 out of 18 samples by immunoelectrosyneresis and 11 out of 18 by Ouchterlony's method. In urine, 8 (33 percent) and 6 (25 percent) out of 24 samples were positive by immunoelectrosyneresis and Ouchterlony's method, but 19 out of 21 samples (91 percent) were positive by immunoelectrosyneresis. Of 12 pregnant women in whom serum, saliva, urine and amniotic fluid were tested by immunoelectrosyneresis, four showed positive reaction in the above four samples and 11 in three samples except urine.

Incidence of pregnancy-specific beta-1-glycoprotein in sera of malignant tumor patients.

Pregnancy-specific beta-1-glycoprotein (SP1) is said to be found only in sera of pregnant females. In this paper, SP1 in the sera of malignant tumor patients was investigated by means of Ouchterlony's method and counter immunoelectrophoresis (CIEP). On the examination by Ouchterlony's method, all samples were negative, but by CIEP, 34 out of 90 samples (38 percent) showed positive reaction; 25 out of 61 (41 percent) in males and 9 out of 29 (31 percent) in females. There was no indication that the patients with a certain type of cancer tend to have a high incidence of SP1, but the examination of SP1 in the sera of patients with cancer or suspicious cancer had some clinical significance.

Further studies on the blue and near ultraviolet reversible photoreaction with an intracellular particulate fraction of the fungus, Alternaria tomato.

Further studies have been made on the blue and near ultraviolet-reversible photoreaction in a cell-free system of the fungus Alternaria tomato, prepared by two-layer sucrose density gradient centrifugation. A light-minus-dark difference spectrum of the supernatant fraction showed a dip in the near ultraviolet region, mainly at 280 nm, when irradiated with near ultraviolet light which could not be reversed by following the initial irradiation with blue light. The difference spectrum of the dialyzed particulate fraction, located in the 68% sucrose band, showed a peak absorbance in the blue region, mainly at 400 nm, when irradiated with near ultraviolet light, and also could not be reversed by blue light. When the particulate fraction plus the supernatant fraction was irradiated with near ultraviolet light, the increment rate of peak absorbance in the blue region increased compared to that of the particulate fraction lacking the supernatant, and both peak absorbance in the blue region and the dip in the near ultraviolet region were reversed by irradiation with blue light following the application of near ultraviolet light. Thus, it was concluded that two photoresponsive systems are involved in blue and near ultraviolet reversible photoreaction on an intracellular particulate fraction.

Involvement of reactive oxygen species in post-ischaemic flap necrosis and its prevention by antioxidants.

In attempts to substantiate the possible participation of reactive oxygen species, and the significance of the xanthine oxidase system in both post-ischaemic reperfusion necrosis of the island flap and distal necrosis of the pedicle flap, and to develop new pharmacological measures for salvaging flap necrosis, a series of experiments were made using an island flap model and a random-pattern flap model in rats. The results were as follows: (1) Epoxysuccinyl derivative (E-64c), allopurinol and L-SOD salvaged post-ischaemic reperfusion necrosis of the island flaps; (2) E-64c and allopurinol did not salvage anticipated necrosis of the distal region of random flaps but L-SOD did; (3) tissue SOD activity did not reflect the fate of the island flap, but did of the distal region of the random flap. These results demonstrated a possible involvement of ROS in both post-ischaemic necrosis of island flaps and distal necrosis of random flaps. However, xanthine oxidase was significant in producing ROS only in the former.

Advanced glycation endproducts and diabetic nephropathy.

Diabetic nephropathy is currently the single largest cause of endstage renal disease (ESRD) in the United States and many European countries. The primary cause for the development of diabetic complications (including diabetic nephropathy) is persistent exposure to hyperglycemia, although genetic and other incompletely understood factors also play an important role. Although much consideration has been given to the pathogenesis and genetics of the disease itself, the mechanisms by which persistent exposure to hyperglycemia cause biochemical and metabolic alterations have been very sketchily understood. Recently, a growing body of evidence has linked the accumulation of the late products of glucose-protein interaction to a variety of chronic complications, including diabetic nephropathy. The formation of irreversible advanced glycosylation endproducts (AGEs) resulting from the spontaneous reaction between glucose and proteins occur most noticeably on long-lived structural proteins. Recent studies demonstrate that the pathogenesis of diabetic nephropathy is caused by the hyperglycemia-accelerated formation of AGEs. Also, reactive AGE peptides in the circulation are thought to play a role as a new version of so called middle molecule toxic substances. This evidence is opening a new window for our understanding of the pathogenesis of diabetic nephropathy.

DNA binding of iron(II) mixed-ligand complexes containing 1,10-phenanthroline and 4,7-diphenyl-1,10-phenanthroline.

Absorption spectroscopy and circular dichroism (CD) have been used to characterize the DNA binding of [Fe(phen)3]2+, [Fe(phen)2(DIP)]2+ and [Fe(phen)(DIP)2]2+ where phen and DIP stand for 1,10-phenanthroline and 4,7-diphenyl-1,10-phenanthroline, respectively. Both [Fe(phen)3]2+ and [Fe(phen)2(DIP)]2+ bind weakly to calf thymus DNA (CT-DNA) in an electrostatic mode, while [Fe(phen)(DIP)2]2+ binds more strongly to CT-DNA, possibly in an intercalation mode. The hypochromicity, red shift and Kb increase in the order [Fe(phen)3]2+ < [Fe(phen)2(DIP)]2+ < [Fe(phen)(DIP)2]2+ in accordance with the increase in size and hydrophobicity of the iron(II) complexes. The thermodynamic parameters obtained suggest that the DNA binding of both [Fe(phen)3]2+ and [Fe(phen)2(DIP)]2+ is entropically driven, while that of [Fe(phen)(DIP)2]2+ is enthalpically driven. A strong CD spectrum in the UV and visible region develops upon addition of CT-DNA into the racemate solution of each iron(II) complex (Pfeiffer effect). This has revealed that a shift in diastereomeric inversion equilibrium takes place in the solution to yield an excess of one of the DNA-complex diastereomers. The striking resemblance of the CD spectral profiles to those of the pure delta-enantiomer indicates that the delta-enantiomer of the iron(II) complexes is preferentially bound to CT-DNA. The mechanism of the development of Pfeiffer CD is proposed on the basis of kinetic studies on the DNA binding of the racemic iron(II) complexes.

Pedunculated bronchogenic cyst mimicking pleural lesion.

We report a rare case of a bronchogenic cyst presenting as a pleural nodule. CT and MRI demonstrated a well circumscribed, homogeneous lesion located on the pleural surface apart from the mediastinum. It was proven to be a bronchogenic cyst localized between the parietal and visceral pleurae and connected with the mediastinum only through fibrous tissue. Radiologists should be aware that a mediastinal bronchogenic cyst may be located apart from the mediastinum.
