[Studies on the mode of progression of alcoholic liver disease].

In order to elucidate the mode of progression of alcoholic liver disease, relationships among the drinking style, laboratory data, anti-HCV antibody and histological changes were investigated on 36 patients in whom the liver biopsy was repeatedly done. Following results were obtained (1) In the group of continuous drinking over 100g ethanol per day, histological progression was found in 11 of 13 patients (85%) regardless of positive anti-HCV. On the other hand, in the group of abstinence or temperance less than 60g daily alcohol intake, histological improvement was found in 6 of 11 patients (55%). (2) Histological improvement was predominantly seen by abstinence or temperance in the cases with lower levels of serum IgA and adenosine deaminase (ADA) on hospitalization and those with rapid decrease in serum gamma-GTP after hospitalization. In conclusion, the amount of ethanol was considered to be the most important factor to affect on a progression of alcoholic liver diseases. Assessment of laboratory data such as IgA and ADA on hospitalization and change in gamma-GTP after hospitalization were also thought to be useful in foreseeing the prognosis of alcoholic liver disease.

[Pharmacokinetic and clinical studies of cefpirome in pediatric field].

We conducted a study on the pharmacokinetics and clinical application of cefpirome (CPR) in children. 1. A single intravenous injection of 20 mg/kg of CPR was given to a two-month-old boy, and the concentration of the drug in the blood was measured. Fifteen minutes after administration, the concentration was 53.3 micrograms/ml, and it gradually decreased thereafter, reaching a level of 5.18 micrograms/ml after 8 hours with a half-life in the plasma of 2.36 hours. 2. A single intravenous injection of 700 mg (50 mg/kg) of CPR and that of cefotaxime (CTX) were given to a girl with suppurative meningitis (3 years old, 14 kg, causative bacteria, Haemophilus influenzae), and concentrations of the drugs in plasma and cerebrospinal fluid after 1 hour were measured. On the second day of illness, the concentration of CTX in the plasma was 39.4 micrograms/ml and the concentration of desacetyl-CTX (D-CTX) was 25.2 micrograms/ml, while concentrations in the cerebrospinal fluid were 6.22 micrograms/ml (15.8%) for CTX and 3.94 micrograms/ml (15.6%) for D-CTX. On the third day of illness, concentration of CPR in the plasma was 59.3 micrograms/ml, while its concentration in the cerebrospinal fluid was 7.44 micrograms/ml (12.5%). 3. CPR was intravenously administered in daily dosages of 37.7-75.0 mg/kg in 2-3 portions for periods of 4-15 days to 2 patients with septicemia (causative bacteria, Klebsiella pneumoniae in 1 case and Escherichia coli in the other), 1 patient with bronchitis (K. pneumoniae), 9 patients with pneumonia (1 case of Staphylococcus aureus, 3 cases of H. influenzae, 2 cases of Haemophilus parainfluenzae, 1 case of K. pneumoniae + Pseudomonas cepacia, 2 cases of H. influenzae + Branhamella catarrhalis), 2 patients with cellulitis (1 case of S. aureus, 1 case, causative agent unknown), 1 patient with suppurative lymphadenitis (causative agent, unknown), 1 patient with staphylococcal scalded skin syndrome, 1 patient with renal abscess (causative agent, unknown), and 1 patient with a urinary tract infection (E. coli), for a total of 18 patients, with excellent results in 9 cases and good results in 9 cases, hence an efficacy rate of 100% was obtained. 4. As an accompanying side-effect, eruption was observed in 1 of the 18 patients, but when administration was discontinued, the symptom gradually receded, and it disappeared by the 4th day.(ABSTRACT TRUNCATED AT 400 WORDS)

Effects of 5-hydroxytryptamine 3 receptor antagonists on gastrointestinal motor activity in conscious dogs.

We studied the effect of 5-hydroxytryptamine 3 (5-HT3) receptor antagonists on gastrointestinal (GI) motor activity in conscious dogs with force transducers implanted chronically. During the digestive state, GR38032F, BRL 43694 and ICS 205-930 did not affect GI motor activity at all at doses up to 1 mg/kg, whereas BRL 24924 at 1.0 mg/kg i.v. significantly stimulated GI motor activity from the stomach to the colon. When GR38032F, BRL 43694 or ICS 205-930 was given i.v. at doses of 0.1 to 1.0 mg/kg during the phase I period, no direct effect was observed on GI motor activity, but the treatment inhibited the occurrence of the subsequent phase III activity in the stomach completely and inhibited it partially in the duodenum without affecting the plasma motilin concentration. GR38032F, BRL 43694 and ICS 205-930 did not influence the caudal migration of phase III activity in the small intestine. In contrast, BRL 24924 at doses of 0.1 to 1.0 mg/kg i.v. significantly stimulated GI motor activity during the phase I period. On the other hand, when GR38032F, BRL 43694 or ICS 205-930 was given during natural or motilin-induced phase III activity, the total contractions in the stomach and partial contractions in the duodenum were inhibited but the caudal propagation of phase III activity in the small intestine was not affected.(ABSTRACT TRUNCATED AT 250 WORDS)

AS-4370, a new gastrokinetic agent, enhances upper gastrointestinal motor activity in conscious dogs.

The effects of AS-4370 [4-amino-5-chloro-2-ethoxy-N-(4-(4-fluorobenzyl)-2 morpholinyl)methyl)benzamide citrate] on gastrointestinal (GI) motor activity were compared with those of cisapride and metoclopramide in conscious dogs with force transducers implanted chronically. In postprandial state, AS-4370 given 0.2 to 1 mg/kg i.v. or 1 mg/kg intraduodenally (i.d.) stimulated the antral and duodenal motor activity without affecting the colonic motor activity. Cisapride at 0.2 to 1 mg/kg i.v. or 1 mg/kg i.d. stimulated the motor activity in all sites of the GI tract from the stomach to the colon simultaneously. Metoclopramide, like AS-4370, stimulated the antral and duodenal motor activity, but its effect was less potent than that of AS-4370. AS-4370 did not stimulate the GI motor activity under treatment with atropine but stimulated it under vagotomy. Furthermore, AS-4370 did not potentiate the methacholine-induced antral contraction, whereas neostigmine at 10 micrograms/kg i.v. significantly enhanced it. AS-4370 at 1 mg/kg i.v. did not antagonize the inhibition of antral motor activity induced by i.v. infusion of dopamine (1 mg/kg/hr), whereas cisapride (0.5 mg/kg i.v.) and metoclopramide (1 mg/kg i.v.) antagonized it. In addition, the enhancement of GI motor activity induced by AS-4370 was not prevented by propranolol, prazosin, yohimbine, methysergide, ketanserin, ICS 205-930 or naloxone. 5- Hydroxytryptamine (5-HT) receptor desensitization induced by 5-HT (300 micrograms/kg/hr) in the antrum reduced the enhancement induced by AS-4370.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of coronary-pulmonary artery fistula with saccular aneurysm].

Recently, due to the wide-spread use of coronary angiography (CAG), reports of coronary-pulmonary artery fistula have become more frequent. However, a case of coronary-pulmonary artery fistula with saccular aneurysm is very rare. We report a case of coronary-pulmonary artery fistula with saccular aneurysm which was discovered as an abnormal chest shadow. A sixty-two-year old female was admitted to our hospital with an abnormal chest shadow. CAG showed fistula from the left anterior descending coronary artery (LAD) proximal to the pulmonary artery (PA), and also from the LAD distal to the PA, with saccular aneurysm. Due to the risk of a rupture of the saccular aneurysm, we performed a resection. After this operation, no abnormal chest shadows appeared on the chest X-rays. CAG showed no saccular aneurysm and she is progressing favorably.

Biological characterization of human brain natriuretic peptide (BNP) and rat BNP: species-specific actions of BNP.

We examined the diuretic-natriuretic activities of rat BNP and human BNP in anesthetized rats in vivo and their vasorelaxant activities for rat thoracic aorta and porcine coronary artery in vitro. Rat BNP was almost equipotent to rat ANP in diuresis and natriuresis with relative potencies of 1.6 and 2.5, respectively, while human BNP exerted no significant activity. Rat ANP, rat BNP and human BNP relaxed PGF2 alpha-contracted rat aortic strips with IC50 values of 0.62, 0.64 and 12.1 nM, respectively, while they relaxed PGF2 alpha-contracted porcine coronary arteries with IC50 values of 0.04, 1.10 and 0.02 nM, respectively. These results strongly suggest that the biological action of BNP is species-specific.

Distribution and cyclic change of aromatase cytochrome P-450 activity in human uteri.

Activities of aromatase cytochrome P-450 in the columnar epithelial region (CE), squamous epithelial region (SE) and connective tissue (CT) of uterine cervix, and endometrium (EM) during the menstrual cycle were determined using [4-14C] and [1 beta-3H]androstenedione. Aromatase activities in the proliferative phase (n = 8) were 15.0 +/- 7.9, 10.9 +/- 10.3, 9.4 +/- 10.6 and 8.0 +/- 7.3 (mean +/- SD) fmol/h/mg protein in CE, SE, CT and EM, respectively, and aromatase activities in the secretory phase (n = 6) were 31.5 +/- 7.6, 19.1 +/- 7.1, 5.6 +/- 4.6 and 6.3 +/- 1.5 fmol/h/mg protein, respectively. The results show that the aromatase activities in these regions in the proliferative phase were not significantly different from each other. On the other hand, the aromatase activity in the secretory phase was significantly higher in CE than in any other regions (P less than 0.05), and significantly higher in SE than in CT or EM (P less than 0.05). There was no significant difference in aromatase activity between CT and EM. By comparison of aromatase activity between these two phases, the activity in CE was significantly higher in the secretory phase than in the proliferative phase (P less than 0.05), but no significant difference was observed in other regions.

Isolation and characterization of human reg protein produced in Saccharomyces cerevisiae.

reg was originally identified as a gene expressed during the regeneration of insulin-producing pancreatic beta-cells of the rat. We built an expression vector containing human reg cDNA to drive Saccharomyces cerevisiae to synthesize the reg protein, and purified it from the culture medium. The 144-amino acid sequence of the recombinant protein was consistent with that deduced from the cDNA and genomic DNA sequence except that the signal sequence of 22 amino acids was eliminated, and the amino-terminal residue of the protein was pyroglutamic acid. The secondary structure of the reg protein was predicted by determination of the intramolecular cystine linkage and of alpha-helix and beta-sheet contents.

Isolation and sequence determination of human brain natriuretic peptide in human atrium.

We isolated human brain natriuretic peptide (human BNP) from the human atrium. Sequence analysis has revealed that it is a 32-amino-acid peptide with the sequence S-P-K-M-V-Q-G-S-G-C-F-G-R-K-M-D-R-I-S-S-S-S-G-L-G-C-K-V-L-R-R-H, which is identical to the C-terminal sequence (77-108) of the human BNP precursor deduced from the cDNA sequence. The sequence of human BNP (77-108) is preceded by Pro75-Arg76 in the human BNP precursor, which is the same processing signal as Pro97-Arg98 of the precursor of atrial natriuretic peptide (ANP). The processing of the BNP precursor occurs in the cardiocyte, although that of the ANP precursor in the cardiocyte is unclear at present.

Plasmodium falciparum: restricted polymorphism of T cell epitopes of the circumsporozoite protein in Brazil.

We examined the extent of variation of the 3' region of the circumsporozoite gene among Plasmodium falciparum isolates through amplification of a selected DNA fragment followed by DNA sequencing. A total of 32 isolates were analyzed, of which 24 were from Amazon endemic areas in Brazil and 8 from widely separated geographical regions in the world. Among Brazilian isolates only 2 variants were detected: 19 displayed the same sequence of strain 7G8 whereas the 4 remaining isolates differed from the 7G8 strain at five nucleotide positions which also led to amino acid changes. Variation was restricted to one of the T-helper epitopes while the sequence identified as a cytotoxic T cell epitope was conserved in all Brazilian isolates. P. falciparum samples from other geographical regions in the world showed sequences distinct from those of Brazilian isolates. However, some constancy could be observed within that variation. For instance, the most frequent nucleotide substitutions, from A and C at nucleotide positions 1015 and 1024, were the same in all isolates.

The stage-specific 90-kilodalton surface antigen of metacyclic trypomastigotes of Trypanosoma cruzi.

The 90-kDa antigen, previously identified by the monoclonal antibody 1G7 to be a stage-specific surface protein of metacyclic trypomastigotes of Trypanosoma cruzi, has been further characterized in this study. Experiments of metabolic labeling with [35S]methionine, [2H]mannose and [3H]galactose revealed that the 90-kDa antigen is the main glycoprotein synthesized by metacyclic forms (G strain). Through pulse-chase experiments with [35S]methionine-labeled metacyclic trypomastigotes, it was found that the antigen is synthesized as a 75-kDa precursor polypeptide that is rapidly processed to the mature 90-kDa molecule. When metacyclic trypomastigotes were treated with tunicamycin, the production of 90-kDa antigen was greatly diminished, and the 75-kDa species, which was also expressed on the cell surface, accumulated. Concanavalin A bound strongly to the 90-kDa antigen, but failed to recognize the 75-kDa polypeptide. Treatment of neuraminidase had no effect on the 90-kDa antigen, whereas digestion by endoglycosidase H generated a polypeptide of 82 kDa. Altogether these data indicate that the 90-kDa antigen is a glycoprotein containing N-linked oligosaccharide side chains of the high-mannose type. The 90-kDa glycoprotein may be involved in the process of host cell invasion, since the internalization of metacyclic forms into Vero cells was partially inhibited by monoclonal antibody 1G7.

Clinical guidelines for the early detection of plantar malignant melanoma.

Of 144 pigmented lesions excised from the soles of Japanese patients, 140 were melanocytic. Apart from congenital melanocytic nevi, only a few benign acquired melanocytic nevi on the sole were more than 7 mm in maximum diameter and none exceeded 9 mm. In contrast, all plantar malignant melanomas, including malignant melanoma in situ, were 9 mm or more in maximum diameter. In addition, the majority of plantar melanocytic lesions excised from patients who were older than 50 years of age were malignant melanoma. On the basis of these data, we propose clinical guidelines for the early detection of plantar malignant melanoma.

Novel benzamides as selective and potent gastric prokinetic agents. 1. Synthesis and structure-activity relationships of N-[(2-morpholinyl)alkyl]benzamides.

With the purpose of obtaining more potent and selective gastric prokinetic than metoclopramide (1), a new series of N-[(2-morpholinyl)alkyl]benzamides (17-52) were synthesized and their gastric prokinetic activity was evaluated by determining effects on the gastric emptying of phenol red semisolid meal and of resin pellets solid meal in rats and mice. The morpholinyl moiety was newly designed after consideration of the side-chain structure of cisapride (2) and produced the desired activity when coupled with the 4-amino-5-chloro-2-methoxybenzoyl group of both metoclopramide and cisapride. Modification of the substituents of the benzoyl group markedly influenced the activity. In particular, 4-amino-N-[(4-benzyl-2-morpholinyl)methyl]-5-chloro-2-methoxybenzamide (17) and the 4-(dimethylamino) and 2-ethoxy analogues (25 and 29) of 17 showed potent and selective gastric prokinetic activity along with a weak dopamine D2 receptor antagonistic activity.
