Caffeine consumption among medical students.

Recently, caffeine consumption in Japan is thought to have increased. Although caffeine had been considered to be harmless, there have been studies which suggests an association between caffeine and health and give rise to vigorous discussion. However, in Japan, there have been few epidemiological studies on caffeine consumption among a general population. A questionnaire survey was conducted among medical students and the results were as follows: 1) High dose users (estimated daily caffeine use is 250 mg or more) were observed in 15.2% and the proportion was higher in males than in females. 2) The respondents gave sleepiness, dry mouth and so on, as reasons for taking caffeine beverages, and gave, as the effects of caffeine, becoming clear-headed, shaking off sleepiness, and epigastric discomfort or pain. 3) A third of respondents have experienced taking caffeine tablets and ampules to shake off sleepiness and, in males, the more caffeine they had daily, the more who reported the experience. 4) Caffeine consumption has an association with alcohol use and smoking habit among males.

Caffeine consumption and anxiety and depressive symptomatology among medical students.

Caffeine is one of the most widely consumed psychoactive substances in the world and is ingested in a variety of favorites, such as coffee, tea, cola and so on. Although it has been suggested that high dose caffeine users have more anxiety and depressive symptoms than low users, this relationship is not clear in Japan, where caffeine consumption is considered to be less than in Western countries. A questionnaire survey was conducted among medical students and 291 out of 423 initial subjects completed it. Among males, caffeine consumption was significantly and positively correlated with anxiety symptoms, when alcohol use and smoking habit were adjusted. However, there was no relationship between caffeine consumption and depressive symptoms. Among females, although there was no association between caffeine consumption and anxiety symptoms, high dose caffeine users showed less depressive symptoms than moderate and low users, when alcohol use was adjusted. It is suggested that caffeine use is one of the important factors, in researching psychological health among the general population. We need further epidemiological studies to determine whether there is a causal relationship between caffeine and psychological ill health or not.

[Isolation of children with attention deficit disorder in their classrooms].

The isolation of children with attention deficit disorder (ADD) among peers was examined by conducting sociometric tests in 28 classrooms to which ADD children belonged. The conclusions are as follows: 1. Among ADD children, 39.3% were classified as isolated children who received no Social Choice votes, a proportion that was significantly higher than normal controls. 2. Results suggests that isolation was correlated with such characteristics of ADD children as higher grade in school, later first consultation and lower IQ levels. 3. Classroom characteristics which correlated with isolation of ADD children were larger number of children per class and stronger class cohesion. It is thought that reducing the number of children in each class will be effective in preventing isolation of ADD children. 4. The possibility of an association between assessment of parents for social relationships of ADD children and their isolation exists and needs to be assessed. The authors emphasize the need for ADD children to have support in their interpersonal relationships, and discuss ways of intervention to prevent their being isolated in their classrooms.

[Is MAC effected by liver dysfunction?--Bilirubin and MAC].

If MAC is decreased in patients with liver injury, the amount of inhaled anesthetic administered to these patients could be reduced, and thereby reducing the impact of the anesthetic in such patients. The effect of jaundice in liver injury on halothane MAC was investigated in this study. After MAC for control was determined, 12 mongrel dogs received ligation of the common bile duct (CBD). MAC determination followed 1, 2, and 3 weeks after CBD ligation in 6 dogs in experiment 1. The other 6 dogs received bilirubin i.v. 1 week after CBD ligation, and MAC was determined before and after bilirubin, i.v. in experiment 2. Halothane was the only anesthetic used throughout the studies. In experiment 1, serum total bilirubin (TBil) increased to 7mg.d1-1; bilirubin did not enter the cerebro-spinal fluid (CSF); and MAC did not decrease significantly. In experiment 2, TBil increased to 21mg.dl-1; bilirubin entered the CSF; MAC significantly decreased 15% from control; and the correlation between MAC decrease and TBil increase was statistically significant (P less than 0.05). The results suggest that increase in serum bilirubin in liver injury will decrease MAC.

Trisomy of chromosome 9q: specific chromosome change associated with tumorigenicity during the process of X-ray-induced neoplastic transformation in golden hamster embryo cells.

We have reported that trisomy of chromosome 7 is commonly observed in anchorage-independent clones isolated from X-irradiated golden hamster embryo cells. All 10 clones derived from different irradiated populations showed tumorigenicity when 1 x 10(7) cells were injected s.c. into nude mice (BALB/c, nu/nu). From karyotypic analysis, we found that 8 of 10 cells showed trisomy of chromosome 9. One cell line had a translocation between chromosomes 9q and 19q and trisomy of chromosome 7. The other cell line contained trisomy of chromosome 9 and a translocation between chromosomes 7q and 8q. Using Southern blot analysis, we observed no amplification of v-myc, v-Ha-ras, v-Ki-ras or N-ras-related oncogenes. Furthermore, we could not detect either an increase in expression of v-myc- and v-Ha-ras-related genes or the activation of any oncogene, by the NIH 3T3 transfection assay. Our results suggest that trisomy of chromosome 7 is insufficient for the expression of tumorigenicity and that increased dosage of chromosome 9q may play an important role in the malignant progression of X-ray-induced neoplastic transformation.

Internal-surface reversed-phase silica support for direct injection determination of drugs in biological fluids by liquid chromatography.

A new internal-surface reversed-phase (ISRP) silica support has been designed for direct injection analysis of drugs in biological fluids by liquid chromatography. The support, prepared by using a new enzyme, polymyxin acylase, has N-octanoylaminopropyl phases, bound only to the internal surfaces of the porous silica, and N-(2,3-dihydroxypropyl)-aminopropyl phases on the external surfaces in order to be nonadsorptive to proteins. The average pore diameter of the prepared ISRP silica support was 50 A, which is small enough to exclude macromolecules such as serum proteins from the pores. The new ISRP support can be used for the direct injection analysis of hydrophilic and hydrophobic drugs in serum or plasma without destructive accumulation of proteins over the eluent pH range of 3 to 7. The recovery of drugs from serum was almost 100%, regardless of the difference in their protein bindings.

Effect of halothane and enflurane on hepatic blood flow and oxygen consumption in dogs.

We investigated the relative effects of 0.5, 1.0, 1.5, 2.0 MAC halothane and enflurane, and concurrent noxious stimulus on hepatic blood flow and oxygen consumption in 14 mongrel dogs randomly divided into groups of seven each. Hepatic arterial and portal venous blood flow (HABF and PVBF, respectively) were measured continuously using ultrasonic transit time flow meter. Mean arterial blood pressure (MAP), cardiac index (CI), hepatic oxygen supply, and hepatic oxygen consumption (HVO2) were measured. Halothane significantly deceased HABF, but not PVBF in a dose dependent manner. Enflurane did not affect HABF and PVBF significantly. MAP and CI decreased in both groups, with halothane producing more marked decreases than enflurane. HVO2 did not change with enflurane, but did with halothane, producing significant differences, with halothane being greater at 1.5, 2.0 MAC. A noxious stimulus only caused minor change in blood flow. The results suggest that liver blood flow and oxygen consumption are affected differently by halothane and enflurane and that halothane has a stronger tendency to cause an imbalance between liver oxygen supply and consumption than dose enflurane.

Primary small intestinal adenocarcinoma diagnosed by endoscopic examination prior to operation.

Progress in diagnostic procedures has made it possible to diagnose neoplasms of the gastrointestinal tract preoperatively. However, preoperative diagnosis of tumors of the small intestine remains difficult. We report here detection of an adenocarcinoma of the jejunum by endoscopic examination prior to operation. A 73-year-old woman was admitted with complaints of intermittent periumbilical pain, nausea, and vomiting. An upper gastrointestinal series showed an abnormal segmet 10 cm distal to the duodenojejunal flexure. Small intestinal endoscopic examination revealed a tumor with a crater and an irregular surface of mucosa near the duodenojejunal flexure, and annular constriction due to tumor extension, and endoscopic biopsy specimens contained tissue from a poorly differentiated adenocarcinoma. Wide resection, including the duodenum, proximal jejunum, and adjacent mesentery was performed. The resected tumor was confirmed histologically to be a poorly differentiated adenocarcinoma of the jejunum.

Epinephrine-induced premature ventricular contractions and changes in arterial blood pressure and heart rate during I-653, isoflurane, and halothane anesthesia in swine.

I653 is a new inhalation anesthetic having especially desirable recovery characteristics because of its very low blood and tissue solubility. Investigations of its cardiovascular and electroencephalographic effects have revealed actions similar to those of isoflurane. However, these studies did not evaluate the potential of I653 to predispose the heart to epinephrine-induced arrhythmias. In this investigation, we studied eight domestic swine to compare the effects of I653 with those of other anesthetics on the cardiac arrhythmogenic actions of intravenously infused epinephrine. I653, isoflurane, and halothane each were given, on separate days, at 0.7-0.8 and at 1.1-1.2 MAC. The rate of infusion of epinephrine needed to produce premature ventricular contractions (PVCs) when the animals were anesthetized with I653 (6.9 +/- 0.7 and 6.6 +/- 0.9 micrograms.kg-1.min-1 at 0.8 and 1.2 MAC) did not differ from that during isoflurane anesthesia (5.7 +/- 1.1 and 6.0 +/- 1.0 micrograms.kg-1.min-1 at 0.7 and 1.1 MAC), but was greater than that required during halothane anesthesia (1.3 +/- 0.2 and 1.1 +/- 0.3 micrograms.kg-1.min-1 at 0.7 and 1.1 MAC). Similar mean arterial blood pressures and heart rates resulted from like infusions of epinephrine during I653 and isoflurane anesthesia. PVCs occurred at lesser infusion rates of epinephrine and at lower mean arterial blood pressures and heart rates with halothane than with I653 or isoflurane. Anesthetic concentration, over the range studied, did not alter the infusion rate of epinephrine required to produce arrhythmias with any anesthetic. The authors conclude that I-653 and isoflurane have similar properties with respect to epinephrine-induced arrhythmias and increases in heart rate and arterial blood pressure.

Cardiovascular safety and actions of high concentrations of I-653 and isoflurane in swine.

The ratio of lethal-to-anesthetic concentration can be used to define the margin of safety of an inhaled anesthetic. In mechanically ventilated swine the fatal concentration of I-653, a new inhaled anesthetic, was 23.9 +/- 0.06% (mean +/- SE), and of isoflurane, 6.22 +/- 0.23%. The ratio of fatal anesthetic concentration-to-MAC for I-653 (2.45 +/- 0.11) was less than that determined for isoflurane (3.02 +/- 0.13; P less than 0.01) but relatively greater than that reported previously for other inhaled anesthetics. As with other inhaled anesthetics, the concentration of I-653 causing cardiovascular collapse exceeds that producing apnea, making cardiovascular collapse during spontaneous ventilation unlikely. Mean aortic blood pressure and cardiac output decreased as linear functions of anesthetic concentration. Values for these variables for isoflurane were greater than those for I-653 at concentrations exceeding 1.5 MAC. Heart rate, blood lactate concentration, and base-deficit did not change with anesthetic depth. Mixed venous PO2, mixed venous oxyhemoglobin saturation, and the ratio of oxygen transport to oxygen consumption remained at or above values in conscious swine but decreased similarly with both anesthetics when anesthetic concentration increased to within 0.5 MAC of the fatal concentration. Thus, the latter three variables, reflecting the fraction of delivered oxygen that is consumed, and "mean" tissue PO2 appear to be useful indices of anesthetic concentrations approaching those producing cardiovascular collapse.(ABSTRACT TRUNCATED AT 250 WORDS)

A large Japanese family with Machado-Joseph disease: clinical and genetic studies.

A large Japanese family with probable Machado-Joseph disease (MJD) is described. Detailed neurological examination in 12 patients from 3 generations revealed variable combinations of cerebellar ataxia, ocular paresis, difficulty in eye-lid opening, bulging eyes, facial "myokymia", facial dystonia, pyramidal signs, extrapyramidal signs, and peripheral neuropathy. Mode of inheritance was in all likelihood autosomal dominant. Blood components were typed for 19 conventional chromosome markers. Although association of the affected members with the PGM1 system was high, linkage analysis failed to reveal any markers studied with a lod score higher than 3. The unique constellation of symptoms appeared sufficient to rule out other types of spinocerebellar degeneration. When there is a typical case in a given family, MJD appears to be a clinically recognizable entity.

Production of 2',3'-dideoxyadenosine and 2',3'-dideoxyinosine from 2',3'-dideoxyuridine and the corresponding purine bases by resting cells of Escherichia coli AJ 2595.

A novel microbial method for the production of 2',3'-dideoxynucleosides by transdideoxyribosylation has been developed. By screening microorganisms producing 2',3'-dideoxyadenosine (DDA) from 2',3'-dideoxyuridine (DDU) and adenine, Escherichia coli AJ 2595 was selected as the best producer. Optimal pH and temperature for the DDA-producing reaction were ca. 6.5 and 50 degrees C, respectively. Pi seemed to be an essential factor for the reaction, and its optimal concentration was ca. 25 mM. Moreover, polyethylene glycol had a notable effect on DDA production. Under the best conditions established, 52 mM DDA was obtained from 100 mM DDU and 100 mM adenine after 48 h of incubation from resting cells of E. coli AJ 2595. This strain could also produce 2',3'-dideoxynucleosides, such as 2',3'-dideoxyinosine (DDI), 2',3'-dideoxyguanosine, and 2',3'-dideoxythymidine, from DDU and the corresponding bases. In particular, this strain could produce DDI in high yield (ca. 32 mM from 100 mM DDU and 100 mM hypoxanthine) after 24 h of incubation. However, 2',3'-dideoxycytidine was not produced from DDU and cytosine by resting cells of E. coli AJ 2595.

Vacuolated plasma cell: ultrastructural distribution of acid phosphatase and intracellular immunoglobulin.

In two cases of vacuolated plasma cells, one of which was associated with primary macroglobulinemia and the other was with kappa-chain Bence Jones multiple myeloma, we examined the immunopathological features of the vacuoles in order to know whether the Ig-secreting system or the lysosomal system is of importance in the process of vacuole formation. Immunofluorescent studies detected no Ig in the vacuoles. Detection of intracellular Ig by immunoelectron microscopic technique revealed that Ig was localized only in a small portion of the vacuoles but not in most vacuoles. Even when Ig was included in the vacuoles, only the contents of the vacuoles were positive for Ig but their demarcating membrane was negative for Ig. In contrast, electron microscopic studies of acid phosphatase activity revealed the presence of its activity in all vacuoles. These findings suggest that the lysosomal system but not the Ig-secreting system may play a major role in vacuolation of these myeloma cells.

Studies on pharmacological activation of human serum immunoglobulin G by chemical modification and active subfragments. VIII. Effect of carboxamide-methylated light chain (Fr.I-L) on leukocyte functions.

Human serum immunoglobulin G light chain (Fr.I-L), which was reduced and carboxamide-methylated, showed no effect on formyl-methionyl-leucyl-phenylalanine (FMLP)-induced chemotaxis nor on phagocytosis of yeasts when directly added to guinea pig polymorphonuclear leukocytes (PMNs). However, intravenously administered Fr.I-L inhibited emigration of leukocytes into the peritoneal cavity and promoted phagocytosis of yeasts in a yeast-induced peritonitis model in mice. Moreover, Fr.I-L reduced FMLP-induced chemiluminescence (CL) from PMNs. These facts indicated that the anti-inflammatory action of Fr.I-L was caused by inhibiting emigration of leukocytes into the injured site and scavenging superoxide radicals from the cells.

The inhibitory effect of bis[2-[(E)-2-octenoylamino]ethyl] disulfide on the cyclooxygenase pathway.

Bis[2-[(E)-2-octenoylamino]ethyl] disulfide (compd. I-3) inhibited collagen- and arachidonic acid-induced rat platelet aggregation, although not adenosine 5'-diphosphate (ADP)-induced rat platelet aggregation. Based on these results, we then investigated the inhibitory effect of compd. I-3 on thromboxane B2 formation from arachidonic acid in rat platelets, and prostaglandin I2 formation in rat aortae. Compound I-3 inhibited both thromboxane B2 and prostaglandin I2 formation, suggesting that it has an inhibitory effect on cyclooxygenase. The inhibitory effect of compd. I-3 was confirmed in experiments using a crude preparation of sheep seminal vesicle microsomal prostaglandin synthetase. These findings suggested that compd. I-3 has an inhibitory effect on cyclooxygenase activity, like nonsteroidal anti-inflammatory drugs.
