Encephalocraniocutaneous lipomatosis: a new case report and review of the literature.

Encephalocraniocutaneous lipomatosis is a rare neurocutaneous syndrome characterized by lipomatous hamartomas ranging in size from a few millimeters to several centimeters and affecting the head. Ocular anomalies and a variable degree of mental retardation with or without convulsions are usually observed. This disorder should be distinguished from other mosaic neurocutaneous phenotypes such as Proteus syndrome, oculocerebrocutaneous syndrome, and nevus sebaceous syndrome. We report the clinicopathologic findings of a 4-year-old Brazilian girl affected by this syndrome and review the literature. To our best knowledge, this is the first documented case of encephalocraniocutaneous lipomatosis occurring sporadically in South America.

Genital angiofibromas in tuberous sclerosis: two cases.

Cutaneous lesions are an important feature of tuberous sclerosis (TS). Facial angiofibromas usually begin to appear at the end of the first decade of life and are considered to occur in about 88% of adults with TS. They are only rarely reported on sites other than the face or front. In this paper, we report two patients with the complete syndrome of TS that had, in addition to classic facial lesions, multiple papules on the genital area. Histopathology confirmed the diagnosis of angiofibroma. One patient's lesions were misdiagnosed as genital warts and were so being treated before the correct diagnosis was made. Although we don't know the incidence of genital angiofibromas in TS patients, we believe that they might be underreported, because dermatologists generally don't look for them when they see these patients.

[Histopathologic findings for the research of acid-fast bacilli in erythema nodosum].

It was possible to prove, by means of retrospective histopathological studies of 51 glass slides of common erythema nodosum (EN) and 39 glass slides of erythema nodosum leprosum (ENL), that the histopathological features of the two groups differed in several ways. It was found that the most prominent divergent findings between these two diseases was the presence of a lepromatous granulomatous infiltrate in the ENL; the predominant location of the inflammatory infiltrate was derma-hypodermal in ENL and hypodermal in common EN; there was a preferential septa attack in common EN and lobular in ENL; foreign giant cells occurred only in common EN; the lymphocyte cell infiltration in common EN and of neutrophils in ENL and the presence of inflammatory infiltrate around nerves in ENL. Our study has proved that the search for M. leprae does not have to be made in all cases of EN as there are histological differences in the routine histopathological exam between common EN and ENL that can alert to the real need for this procedure. This search must be performed when during the routine histopathological exam of sections stained by hematoxilin-eosin the ENL histopathological characteristics delineated herein are observed.

[Importance of Perls stain in histologic diagnosis of Kaposi sarcoma].

The authors studied 27 cases of Kaposi's sarcoma, 23 cases of pyogenic granuloma, 18 cases of hemangioma, 17 cases of stasis dermatitis and 9 cases of dermatofibroma. In all these cases, they used the hematoxylin-eosin stain and Perls' stain', and they showed the presence of hemosiderin in all the cases of Kaposi's sarcoma and stasis dermatitis. The hemangioma and the dermatofibroma presented 50% of positivity, and the pyogenic granuloma, only 26.1%.

Subcutaneous mucormycosis caused by Mucor hiemalis Wehmer f. luteus (Linnemann) Schipper 1973.

The authors report a case of subcutaneous mucormycosis in a diabetic patient with verrucous lesions in the fourth finger of the right hand. Initially diagnosed as a fixed cutaneous sporotrichosis case, success was obtained with the use of potassium iodide with healing of the lesion in a period of nearly 5 months. At present the patient remains under clinical observation with no treatment or relapse.

[Poikiloderma congenitale of the Kindler type associated with multiple seborrheic warts].

A case an eleven year old girl with congenital poikiloderma, generalized xerosis and cutaneous atrophy, recurrent blister in extremities, sclerosis of the fingers and toes diffuse hyperkeratosis of palms and soles and webbing between some toes is reported here. Multiple seborrheic keratosis were also found, an association hitherto not mentioned with congenital poikiloderma.

[Hypereosinophilic syndrome. Cutaneous picture of "erythema annulare centrifugum"--comparison with ultrastructural study].

The authors present the third case in the literature of hypereosinophilic syndrome with "erythema annulare centrifugum" like lesions. Cutaneous manifestations were the first sign of the disease. Laboratory investigation including ultrastructural study of cutaneous lesions. This kind of investigation has been done before in only one instance. Clinical and laboratory features of the case are fully discussed taking into account literature findings.

Cutaneous and ungual phaeohyphomycosis caused by species of Chaetomium Kunze (1817) ex Fresenius, 1829.

Two cases of cutaneous and ungual phaeohyphomycosis caused by species of Chaetomium are reported. The patients showed no clinical signs of immunodeficiency. In Case 1 there was a small, ulcerated, crusted lesion on the right forearm. Direct microscopical examination of material from this lesion showed light-brown hyphae with thick-walled cells. The fungus isolated was identified as Chaetomium globosum. Case 2 had lesions of the fingernails. Direct microscopy showed dematiaceous septate hyphae in the nail. The isolate from the nails was identified as Chaetomium perpulchrum. Identification of the fungi was based on the classification of Ames (1963) as adapted by Cooke (1986). Such infections due to Chaetomium species are rare.

[Large cell acanthoma].

The authors relate a clinical case of large cell acanthoma with multiple lesions that predominate on the inferior limbs. They comment the nosological position, the clinical aspects and, concerning the histopathology, emphasize the enlarged dimensions of the cellular nuclei in acanthoma and the differential diagnosis. Probably, the large cell acanthoma is a much more frequent entity that has not been diagnosed due to the little knowledge of this pathology.

[Treatment of chromomycosis with liquid nitrogen].

Five patients with chromomycosis have been treated by liquid nitrogen application. The evolution of disease has been of one to twenty five years, with 4 patients with verrucous patches and one patient with sarcoid-like lesion. In all cultures grows-up Fonseca pedrosi. The authors believe that this method of treatment has advantage to be more economic, to avoid hospitalization and surgical acts with anesthesia, and also could be realized at ambulatory level. The efficacy of this method has been proved by the follow-up the patients during a period between two to four years, without any recurrency.

[Malignant melanoma (review of 68 cases)].

A clinica pathologic revision was performed about 68 patients with malignant melanoma of the Hospital dos Servidores Publicos do Estado de São Paulo. The authors had checked the data concerning to color, age and sex of the patients, localisation, clinical aspect of the lesions and clinical evolution of the cases. They made comments on the histopathology related to Clark's levels, the depth of tumors (Breslow), solar elastosis and inflammatory infiltrate, in relation with the clinical evolution of the cases.

[Pitted keratolysis. Presentation of 2 cases with culture of lesions positive for bacteria of the genus Corynebacterium].

The authors briefly review the literature about Pitted keratolysis emphasizing the etiopathogenesis. They report two cases of the disease in which the lesions were investigated for the etiologic agent. The search for Fungi using direct exam and culture in Sabouraud's medium was negative. Gram, Neisser and Neisser-Gins' stains, culture in BHI medium and the biopsy were positive for bacteria of the Corynebacterium genus. These results are discussed.

Folliculitis ulerythematosa reticulata (atrophoderma vermiculata): early detection of a case with unilateral lesions.

A 5-year-old girl had a unilateral rash on her right cheek. Histologic investigations showed aberrant pilosebaceous units and elastotic material in the papillary dermis. Unilateral folliculitis ulerythematosa reticulata (a variant of keratosis pilaris atrophicans) is strikingly rare, and its very early clinical and microscopic changes are demonstrated in this case report.

[Primary cutaneous malignant melanoma: retrospective studyfrom 1963 to 1997 at Hospital do Servidor Público Estadual de São Paulo].

OBJECTIVES: A retrospective study on the Primary Cutaneous Malignant Melanoma in the Hospital do Servidor Público Estadual de São Paulo (HSPE-SP) analyzing its distribution according to age, sex, race and cutaneous site. METHODOLOGY: We studied 222 patients with Primary Cutaneous Malignant Melanoma as diagnosed at Hospital do Servidor Público Estadual de São Paulo, Brazil between the period from 1963 to 1997. A retrospective study was performed. Data were expressed as inance of caucasians (98.19%) over afro-americans (1.81) and of the female sex (69.36%) over the male sex (30.63%) was found. The predominant age on the occasion of the diagnosis was between 50 and 60 years for the women (25.32%) and between 60 and 69 years for the men (22.52%). The most frequent site of the cancer in men was the back region (29.41%) and in the lower members in the women (38.31%). The most frequent level of the tumor invasion (Clark) was IV (39.77%), and the average of tumor thickeness (Breslow) was < 0.75 mm (28.4%). A 5 years survival was observed in 73.3% of the patients. CONCLUSIONS: At our Hospital the incidence of Primary Cutaneous Malignant Melanoma has shown an increase in recent years; these results are compatible with the most recent international surveys.

[Subcutaneous phaeohyphomycose due to Exophiala jeanselmir. Report of 3 cases in patients with a kidney transplant].

We report three cases of subcutaneous phaeohyphomycosis due to Exophiala jeanselmei (Langeron) McGinnis et Padhye 1977, in kidney transplant patients. Exophiala jeanselmei is a dematious fungus having also ability to rarely procedure eumycetoma (black grains). According to KWON-CHUNG & BENNETT (1992) such fungus is antigenically very heterogeneous, since so far three serotypes have been identified; each serotype including subgroups. Subcutaneous phaeohyphomycosis is becoming more and more frequent in kidney transplant patients submitted to an immunosuppressive treatment. As Exophiala jeanselmei has already been isolated from the environment it becomes difficult to explain the pathogenicity of these cases by a reactivation of quiescent processes. The authors suggest an occasional fungistatic action of cyclosporine A upon Exophiala jeanselmei.
