trans- and cis-linalool 3,6-oxide 6-O-beta-D-xylopyranosyl-beta-D-glucopyranosides isolated as aroma precursors from leaves for oolong tea.

New glycosidic aroma precursors (1 and 2) of the main volatile constituents, trans- and cis-linalool 3,6-oxides (linalool oxides I and II), were isolated from oolong tea leaves (Camellia sinensis var. sinensis cv. Maoxie). The isolation was guided by an enzymatic hydrolysis with acetone powder prepared from fresh tea leaves (cv. Yabukita) followed by GC or GC-MS analyses. Chromatographic purification of hot water extracts of the tea leaves on active charcoal, Amberlite XAD-2, and Sephadex LH-20 columns as well as HPLC gave two new glycosides, trans- and cis-linalool 3,6-oxide 6-O-beta-D-xylopyranosyl-beta-D-glucopyranosides (1 and 2).

A new glucuronide saponin from tea leaves (Camellia sinensis var. sinensis.

A new glucuronide saponin (1) was isolated as its methyl ester (2) from the leaves of Camellia sinensis var. sinensis. On the basis of its spectral data and the results of chemical degradation, the structure was elucidated to be 3-O-[beta-D-galactopyranosyl(1-->2)-[beta-D- xylopyranosyl(1-->2)-alpha-L-arabinopyranosyl(1-->3)]- beta-D-glucuronopyranosyl]-21-O-cinnamoyl-16,22-di-O-acetylbarr ingtogenol C.

Good's syndrome with a block in the early stage of B cell differentiation and complicated by Campylobacter fetus sepsis.

A 63-year-old man was admitted for Campylobacter fetus sepsis and immunodeficiency syndrome with thymoma (Good's syndrome). Serological examination demonstrated hypoimmunoglobulinemia. Analysis of lymphocyte subsets in the peripheral blood and bone marrow showed marked decreases in the proportion of cells bearing B cell markers. However, there were no abnormalities of cellular immunity. This is a rare case of Good's syndrome in Japan in which the pathogenic mechanism involved a block in the early stage of B cell differentiation. Moreover, this is the first case ever reported of Campylobacter fetus sepsis associated with Good's syndrome.

A scanning and transmission electron microscopic study of fiber arrangement in the hepatic capsule.

The arrangement of fibrous elements in the rat hepatic capsule was examined under a scanning electron microscope (SEM) after alkaline or acid maceration of the serous coat, in conjunction with examination of the thin sections using a transmission electron microscope (TEM). The elastic fibers appeared as thin threads in a densely meshed network, lying just beneath the serous coat. Their surface was granular with short rods in the materials fixed with paraformaldehyde. In contrast, the collagen fibers were observed as relatively thick threads, with fascicles of collagen fibrils that were uniform in size. These fascicles extended in various directions to form rough meshes that were traversed by small fascicles and anastomosed with each other. The fibrous branches of the elastic fibers extended on or into the underlying collagen fibers to be anchored, while the collagen fibers converged on many areas of the liver surface, and were transferred into the interlobular connective tissues. The findings of the present study thus suggest that the fiber arrangement plays an effective role in the mechanical protection of the fragile liver cells and delicate serous cells from pressure and friction damage by the neighboring abdominal organs and walls of the abdominal cavity due to the elastic mobility of the subserosal elastic network in addition to the possible slippery cushion of a serous layer on the serous cells.

Surfactant-producing epithelium in the dorsal part of the cartilaginous eustachian tube of mice. Light, transmission, and scanning electron microscopic observations.

The nonciliated area in cartilaginous roof tubothelium of 14 conventional mice was examined histologically and classified as a modified transitional respiratory epithelium. On the free cell surface numerous short microvilli were found. On the lower roof surface in the midcartilaginous portion and especially near the pharyngeal orifice, a convoluted pattern of ridges and pits was observed. These structural peculiarities are interpreted as adaptive features, ensuring survival of the cells in a dynamic tubal environment. On the basis of systematic observations of the multilamellar bodies in the cytoplasm resembling the phospholipid lamellar bodies of pulmonary surfactant, which are discharged in the tubal lumen and to be found in the pits, it is suggested that the nonciliated cells are "specialized" surfactant-producing tubocytes. The synthesis of surfactant-precursors starts in the basal layer. Different phases of the secretory process were observed in the neighboring cells. This finding is related to the cell-cooperation constantly releasing the secretory product, and natural cell-turnover. Unlike the previously reported surfactant-producing cells in the lower tubothelium of other species, roof tubothelial cells of mice are morphologically similar to type II pneumocytes.

Serum c-erbB-2 in breast cancer patients.

The c-erbB-2 oncogene product in serum (serum c-erbB-2) was measured by an enzyme-immunoassay kit. The 12 U/ml cut-off level was estimated as the mean plus two standard deviations for 250 healthy women. With this cut-off level increased serum c-erbB-2 was found in 12.0% of primary breast cancer cases (n = 25), in 4.9% of non-recurrent breast cancer patients (n = 82), and in 31.4% of patients with recurrent breast cancer (n = 35). In patients with primary and recurrent breast cancer, whose sera were assayed concurrently for serum c-erbB-2, CEA and CA15-3, the positive rates of these markers were fairly similar. However, their combined use significantly increased the sensitivity as compared to the use of any one marker alone.

Expression of multidrug resistance P-glycoprotein on peripheral blood mononuclear cells of patients with granular lymphocyte-proliferative disorders.

The immunological findings and clinical course of 33 patients with granular lymphocyte-proliferative disorders (GLPD) are presented. Based on the surface phenotypes of peripheral blood granular lymphocytes (GL), the GLPD were divided into two groups, namely CD3+ T cell-lineage GLPD (T-GLPD) and CD3- CD16+ NK cell-lineage GLPD (NK-GLPD). Twenty-one patients had T-GLPD, and 12 had NK-GLPD. One patient with T-GLPD and two patients with NK-GLPD had progressive clinical courses and died of the disease despite receiving combination chemotherapy. Among eleven patients analysed for the expression of multidrug resistance P-glycoprotein, six of eight patients with T-GLPD and all three patients with NK-GLPD clearly expressed P-glycoprotein. Since patients with immature NK-GLPD appear to undergo a progressive clinical course, we suggest that therapeutic trials using P-glycoprotein blockers in addition to chemotherapy might be beneficial for such patients.

[Clinical evaluation of biapenem (L-627), a new carbapenem antibiotic in the pediatric field].

We studied the clinical efficacy of biapenem (L-627), a new parenteral carbapenem beta-lactam antibiotic in the pediatric field. L-627 was administered intravenously to 11 patients with ages ranging 2 months to 10 years and 5 months with acute infectious diseases. Doses ranged 28.1 to 72.6 mg/kg/day. The diagnosed diseases included 7 respiratory tract infections, 1 purulent meningitis, 1 sepsis, 1 cervical lymphadenitis and 1 urinary tract infection. Two of these cases one with Mycoplasma infection and the other which had been administered with other antimicrobial agents were not evaluated. The clinical efficacy rate was 77.8% (7/9) and the bacteriological eradication rate was 66.7% (4/6). Laboratory examinations revealed that there was one case with elevated liver enzyme levels with showing elevation of GOT, GPT and LDH. No other side effects attributable to this drug were observed. Thus, it appears that L-627 is a useful antibiotic in treating moderate to severe acute bacterial infections in children.

Reversible functional asplenia. Autoimmune hemolytic anemia with thrombocytopenia in chronic lymphocytic leukemia.

An enlarged spleen failed to accumulate Tc-99m-labeled phytate, but it showed normal perfusion and uptake of labeled and heat-denatured red blood cells. A blood cell count demonstrated moderately low hemoglobin and hematocrit levels and a markedly low platelet count, and results of direct Coombs' and antiplatelet-antibody (IgG) tests were positive. Steroid therapy resulted in normal splenic function. This case demonstrates reversible functional asplenia caused by reticuloendothelial dysfunction secondary to pure splenic chronic lymphocytic leukemia.

[Hemodynamic effects of Amrinone in patients following open heart surgery].

The hemodynamic effects of amrinone, a phosphodiesterase III inhibitor, were studied early after cardiac surgery in 10 patients. CI, SVI and SWI increased significantly. SAP, mPCWP, TSVR and PVR decreased significantly. DO2 and VO2 increased significantly. HR and DP were not affected. These data demonstrate that amrinone is a valuable inotropic agent with vasodilating effect for the early post-operative management of cardiac surgical patients, since it augments left ventricular performance without increase of myocardial oxygen consumption and induction of arrhythmia.

[A case of bronchiectasis accompanied by ulcerative colitis (UC) and HTLV-1 associated myelopathy (HAM)].

We report a case of bronchiectasis with marked thickening of the respiratory tract wall occurring in a 37-year-old man with UC and HAM. He was diagnosed as UC at age 20. HTLV-1 was presumably transmitted to this patient by means of a blood transfusion he received at around age 30. On admission, chest X-ray films, tomography and CT-scan revealed dilated lumens and thickened airway walls extending from the trachea to subsegmental bronchi. Pulmonary function tests showed hypoxemia and mixed ventilatory disturbance with a predominantly obstructive component. HAM was diagnosed on the basis of neurological examination and cerebrospinal fluid analysis. A biopsy specimen from the carinal mucosa showed marked T cell infiltration. In these T cells, we detected polyclonal integration of HTLV-1 proviral DNA. Some of the infiltrating T cells showed atypia. In recent times, respiratory diseases other than infiltration of adult T-cell leukemia cells or opportunistic infection have been reported in HTLV-1 carriers and new clinical entities designated as HABA (HTLV-1 associated bronchiolo-alveolar disorder) and HBA (HTLV-1 associated bronchopneumopathy) have been proposed. This case is classified among these new entities, in a broad sense, and is a rare case in that the respiratory disorder is apparently related to UC.

Intracranial vertebral artery dissection in Wallenberg syndrome.

PURPOSE: To assess the prevalence of vertebral artery dissection in Wallenberg syndrome. METHODS: Sixteen patients (12 men, 4 women; mean age at ictus, 51.6 years) with symptoms of Wallenberg syndrome and an infarction demonstrated in the lateral medulla on MR were reviewed retrospectively. The study items were as follows: (a) headache as clinical signs, in particular, occipitalgia and/or posterior neck pain at ictus; (b) MR findings, such as intramural hematoma on T1-weighted images, intimal flap on T2-weighted images, and double lumen on three-dimensional spoiled gradient-recalled acquisition in a steady state with gadopentetate dimeglumine; (c) direct angiographic findings of dissection, such as double lumen, intimal flap, and resolution of stenosis on follow-up angiography; and (d) indirect angiographic findings of dissection (such as string sign, pearl and string sign, tapered narrowing, etc). Patients were classified as definite dissection if they had reliable MR findings (ie, intramural hematoma, intimal flap, and enhancement of wall and septum) and/or direct angiographic findings; as probable dissection if they showed both headache and suspected findings (ie, double lumen on 3-D spoiled gradient-recalled acquisition in a steady state or indirect angiographic findings); and as suspected dissection in those with only headache or suspected findings. RESULTS: Seven of 16 patients were classified as definite dissection, 3 as probable dissection, and 3 as suspected dissection. Four patients were considered to have bilateral vertebral artery dissection on the basis of MR findings. CONCLUSIONS: Vertebral artery dissection is an important cause of Wallenberg syndrome.

Pressor effect of recombinant human erythropoietin: results of home blood pressure measurements in hemodialysis patients.

We investigated whether the treatment of anemic hemodialysis patients with a low dose of recombinant human erythropoietin (rHEpo) for a short period would increase the blood pressure (BP). Home BP measurements were used to detect minute increases in BP. Fifty-one anemic patients on maintenance hemodialysis with a hematocrit of 25% or less received rHEpo at the dose of 4500 IU/week by the intravenous route for 8 weeks. Overall, rHEpo did not increase the BP whether measured at home or in the clinic (causal BP). Hemoglobin concentration increased significantly from 7.1 +/- 0.7 to 8.8 +/- 0.7 g/dl. Patients were classified into two groups according to the change in mean (M) home BP induced by rHEpo: a pressor group (delta MBP > or = 5 mmHg, n = 17) and a non-pressor group (delta MBP < 5 mmHg, n = 34). The hemoglobin concentration rose significantly in both groups, but there was no change in casual BP. Home blood pressure measurements showed a gradual and continuous rise in BP in the pressor group, but not in the non-pressor group. Patients administered antihypertensive medications before rHEpo treatment accounted for 88% of the former and 50% of the latter groups. Two patients with malignant nephrosclerosis were included in the pressor group. The findings indicate that rHEpo, even given at a low dose for a short period, elevates the BP, as determined by home BP measurement, but not by casual measurements obtained in the clinic.
