Calcific bursitis in a three-year-old boy: a case report.

This is a case report of calcific bursitis adjacent to the first metatarsophalangeal (MP) joint in a 3-year-old boy. The patient had two attacks of pain on the medial side of the first MP joint, resembling an acute gouty attack. Roentgenograms showed calcified opacity adjacent to the medial side of the first MP joint. Surgical removal of the calcified bursa was performed. X-ray analyses revealed that this calcified deposit was hydroxyapatite.

Identification of fusion regulatory protein (FRP)-1/4F2 related molecules: cytoskeletal proteins are associated with FRP-1 molecules that regulate multinucleated giant cell formation of monocytes and HIV-induced cell fusion.

Fusion regulatory proteins (FRPs) regulate virus-mediated cell fusion and multinucleated giant cell formation of monocytes. Anti-FRP-1 mAbs immunoprecipitated 80 kDa and 38 kDa proteins from HeLa cells. After long exposure other bands were detected, suggesting the presence of molecule(s) associated with FRP-1. To identify the molecule(s), we prepared monoclonal antibodies against immunoaffinity-purified FRP-1 complex derived from membrane fractions of HeLa cells. Immunofluorescence microscopy revealed that these monoclonal antibodies recognized the intracytoplasmic molecules in HeLa cells. Using immunoblotting, the antibodies reacted with 200 kDa, 70 kDa, 55 kDa and 35 kDa molecules, so we designated these molecules as FRP-related molecules (FRMs). Subsequently, we performed gene cloning from a HeLa lambda gt11 cDNA library using anti-FRM mAbs and immunoblotting analysis with either purified cytoskeletal proteins or specific antibodies against various cytoskeletal proteins. Three kinds of positive clone were obtained, which encoded partial sequences of vimentin, tropomyosin, and heat shock cognate protein 70 (hsc70). The 200 kDa molecule was expected to be a myosin heavy chain, judging from the immunoblotting pattern. Immunoblotting confirmed that these purified proteins were readily recognized by anti-FRM mAbs. Furthermore, anti-vimentin and anti-myosin mAbs reacted with the precipitates by anti-FRP-1 mAb, indicating a physical association between FRP-1 molecules and these cytoskeletal proteins. When anti-FRP-1 mAb was added to culture fluids of HeLa cells, the cell-shape and immunofluorescence-pattern stained with anti-FRM mAbs changed. Taken together, the fusion regulatory molecular complex is suggested to consist of at least FRP-1, hsc70, actomyosin and vimentin systems.

Study of the de novo synthesis of cholesterol in the rat liver: a newly developed radiotracer technique, "TLC-autoradioluminography".

A possible evaluation technique was devised for examing timely cholesterol synthetic activity in the rat liver. The principle of the technique is to measure the incorporation rate of 14C atoms into a few target metabolites excreted in bile with time after an intraportal injection of [1,2-14C] acetate to the conscious rats. The measuring technique, TLC-autoradioluminography (TLC-ARLG), was devised for much more accurate, quantitative, simple and quick ultramicroanalysis of radioactive metabolites than ever before. Practically, a few microliters of the bile samples were analyzed by TLC before recording of a 2-dimensional radioactive image on the TLC plate. The incorporation ratio of 14C into cholesterol to succinic acid (C/S ratio) varied such that the value found in a 24-h fasting group was less than one-tenth of that in a sufficiently fed control group. For use as an antihyperglycemic drug, the bile C/S ratio in the nicotinic-acid-dosed group was approximately one-fifth of that in the control group.

1'-Acetoxychavicol acetate as an inhibitor of phagocytosis of macrophages.

We screened extracts of edible plants for inhibitors of phagocytosis by peritoneal exudate macrophages. 1'-Acetoxychavicol acetate was isolated from the ethyl acetate extract of Languas galanga, and this compound strongly inhibited phagocytosis at an IC50 value of 1.2 microM with negligible effects on pinocytosis and cell viability. Target(s) of 1'-acetoxychavicol acetate was suggested to be downstream of the signal transduction pathway that is mediated by protein kinase C.

Purification and properties of two chitinases from Streptomyces sp. J-13-3.

Two chitinases (Chi-A and Chi-B) purified from Streptomyces sp. J-13-3 had the same molecular weights (31,000) and enzymatic properties (optimum pH and temperature of pH 6.0 and 45 degrees C) but had significantly different isoelectric points (3.9 for Chi-A, 3.5 for Chi-B). Chi-A and -B had identical N-terminal amino acid sequences (ADXAAAWNASSVYTGGGSASYNGHN), similar amino acid compositions, and immunological cross-reactivities. A concomitant decrease of Chi-A and increase of Chi-B was observed in their productions during cultivation.

Pressor effect of recombinant human erythropoietin: results of ambulatory blood pressure monitoring and home blood pressure measurements.

We investigated whether treatment of anemic hemodialysis patients with a low dose of recombinant human erythropoietin (erythropoietin) for a short period would increase their blood pressure. Ambulatory blood pressure monitoring and home blood pressure measurements were used to detect minute increase in blood pressure. Thirty-two patients with a hematocrit of 25% or less received erythropoietin at the dose of 4500 IU/week, by the intravenous route for 8 weeks. Erythropoietin increased the hematocrit from 20.9 +/- 2.1 to 26.2 +/- 2.1%. Erythropoietin elevated mean ambulatory blood pressure by 5 mmHg or more in two-thirds of patients (n = 20; pressor group), while it elevated home mean blood pressure by 5 mmHg or more in one-third of patients (n = 11). An increase in clinic mean blood pressure by more than 5 mmHg was observed only in one-fourth of patients (n = 7). Circadian variation of blood pressure (nocturnal fall and diurnal rise) had been attenuated in the patients of the pressor group before erythropoietin treatment and erythropoietin decreased the nocturnal fall of blood pressure further more. Erythropoietin elevated nocturnal blood pressure more than diurnal blood pressure. Therefore, the increase in blood pressure induced by erythropoietin was detected more reliably by ambulatory blood pressure monitoring. There was no relation between the change in hemoglobin concentration and the increase in ambulatory blood pressure induced by erythropoietin. Erythropoietin tended to decrease cardiac output and plasma volume while it increased total peripheral resistance. It also decreased plasma norepinephrine and vasopressin levels but did not affect other humoral factors. Although the pressor effect of erythropoietin treatment for 8 weeks at the dose of 4500 IU/week was not evident on clinic blood pressure measurements, any increase in blood pressure determined by ambulatory blood pressure should be treated carefully to reduce the risk of a cardiovascular complication in patients receiving hemodialysis.

Expression of C-terminal src kinase in human colorectal cancer cell lines.

C-terminal src kinase (CSK) is a cytoplasmic protein which decreases activities of the Src family protein-tyrosine kinases. We produced a polyclonal antibody specific for CSK and analyzed the expression of CSK by immunoblotting in two human colorectal normal cell lines and eighteen cancer cell lines. CSK was detected in both the two normal and all the eighteen cancer cell lines. The expression of CSK obtained from human colorectal cancer cell lines was greater than that from human colorectal normal cell lines in most cases. The elevated expression of CSK in human colorectal cancer cell lines appeared to correspond to the elevated p60c-src (c-Src) and p62c-yes (c-Yes) protein-tyrosine kinase activities found in other studies. Thus, CSK may not have an anti-oncogenic role to play through the negative regulation of Src family kinases in colorectal carcinogenesis.

[A benign clear cell tumor of the lung that grew gradually over five years].

A 38-year-old man was found to have an abnormal shadow in his chest X-ray film during a general medical examination. A nodule with a homogeneous density and a clear margin was seen in the upper lobe of the right lung. Since this shadow gradually enlarged during a five-year follow-up period, he was admitted to our hospital. Chest contrast and dynamic CT scans and aortography showed characteristics of a benign tumor with somewhat rich vascularity. The doubling time of this tumor was about 430 days. Transbronchial brushing for cytologic study and a transbronchial biopsy showed no malignancy. However, the tumor was surgically resected because of its gradual growth over the five years. The resected tumor was histologically diagnosed as a benign clear cell tumor of the lung. This disease was first described by Leibow and Castleman in 1963, as a benign pulmonary tumor similar to metastatic renal cell carcinoma. This disease is extremely rare and only 61 cases have been reported: twenty cases, including the present case, in Japan and 41 in other countries.

[Free radical generation in electron transport system of mitochondria after cold preservation in UW solution].

UNLABELLED: In the liver transplantation one of the causes of primary graft nonfunction is likely associated with lipid peroxidation at reperfusion after cold preservation. In this study, we investigated whether free radicals produced in mitochondrial electron transport system would result in lipid peroxidation after cold preservation in UW solution. MATERIALS AND METHODS: Under anesthesia, the liver of male Wistar rat was flushed via the portal vein with cold Ringer lactate, and then placed in 4 degrees C UW solution for 24 hr. After cold preservation, we examined mitochondrial respiratory control ratio (RCR), free radicals (O2-) from mitochondrial electron transport system by MCLA induced chemiluminescence, and lipid peroxidation by chemiluminescence (CL) after reoxygenation. RESULTS: RCR gradually decreased up to 24 hr preservation. O2- generation from mitochondrial electron transport system was not recognized at any length of the time after cold preservation up to 24 hr. At 6 hr, CL values were slightly higher than those of the control, but significantly decreased at 24 hr. CONCLUSION: O2(-)-generated in electron transport system of mitochondria did not increase with cold preservation up to 24 hr, therefore, the radical from mitochondria did not cause a lipid peroxidation at reperfusion injury.

[A prospective randomized clinical trial comparing intra-arterial chemotherapy alone and when combined with hyperthermia for metastatic liver cancer].

To evaluate the efficacy of intra-arterial chemotherapy combined with hyperthermia for metastatic liver cancer, our cooperative study group carried out a randomized clinical trial comparing intra-arterial chemotherapy alone and intra-arterial chemotherapy combined with hyperthermia. Patients were treated with combined chemotherapy of epirubicin (EPIR), mitomycin C (MMC), 5-fluorouracil (5-FU), by hepatic infusion using a subcutaneously implanted reservoir. Hyperthermia (8MHz radiofrequency) was usually performed for 40-60 min every week, and intra-arterial chemotherapy was performed immediately before hyperthermia. Twenty-six patients were registered by telephone contact and allocated at random to groups treated with either intra-arterial chemotherapy alone (14 patients) or combination therapy (12 patients). The response rate was 7% in the chemotherapy alone group (1 PR among 14 evaluable patients), and 40% in the combination therapy group (4 PR among 10 evaluable patients). Our results suggest that intra-arterial chemotherapy combined with hyperthermia is a useful modality for the treatment of metastatic liver cancer.

[Dysfunction of E-cadherin due to mutation of beta-catenin in a scirrhous gastric cancer cell line].

E-cadherin is a calcium dependent cell-cell adhesion molecule. In cancer tissue, detachment of the adhesion system is indispensable for invasion and metastasis of cancer cells. We have investigated mechanism of the dysfunction of E-cadherin-dependent cell-cell adhesion system in gastric carcinoma cells. Although the high expression of E-cadherin in a scirrhous gastric cancer cell line HSC-39, the function of E-cadherin was completely abolished. Western blotting of beta-catenin in HSC-39 cells demonstrated that a truncated beta-catenin was detected. The truncation was due to partial deletion of beta-catenin DNA. It was concluded that in HSC-39 loss of E-cadherin dependent cell-cell adhesion was due to mutation of beta-catenin gene.

[Seroprevalence of HTLV-I in various ocular diseases in central Japan].

Recently human T-cell lymphotropic virus type I (HTLV-I) has been shown to be responsible for a kind of uveitis in southwestern Japan, where HTLV-I infection is highly endemic. To investigate further the association of HTLV-I with various ocular diseases, we studied HTLV-I seroprevalence in 1,350 patients in central Japan, where HTLV-I infection is not endemic. Of 1,350 patients with various ocular diseases, 40 (2.96%) were seropositive for the retrovirus. There was no statistically significant difference in HTLV-I seroprevalence among various ocular diseases. Four of 9 (44.4%) seropositive cases from southwestern Japan developed HTLV-I associated uveitis (HAU), whereas only 2 of 26 (7.7%) seropositive cases from central Japan developed HAU. The difference was statistically significant (p < 0.05). This suggests that the incidence of HAU could be influenced by environmental or hereditary factors. On the other hand, 3 of 7 (42.9%) patients with episcleritis showed HTLV-I infection.

Semiquantitative SPECT tumor uptake of technetium-99m-labeled anti-CEA monoclonal antibody in colorectal tumor.

UNLABELLED: Technetium-99m-BW431/26 images were compared to images of resected tumor specimens to evaluate the efficacy and accuracy of SPECT imaging of colorectal carcinoma. METHODS: Immunoscintigraphy with 99mTc-BW431/26 was performed on seven patients with colorectal carcinoma and one patient with a benign colorectal tumor. Accumulation of 99mTc-BW431/26 in the tumor was expressed as the tumor-to-normal tissue count ratio (SPECT T/N ratio) calculated by a region of interest method on the SPECT images obtained 24 hr after administration of 99mTc-BW431/26. All patients underwent research of the tumor immediately after 24-hr imaging, and the radioactivity in tumor specimen and normal tissue was measured to calculate the tissue T/N ratio. RESULTS: SPECT demonstrated definite increased tracer uptake by the tumor in all colorectal cancer patients. The benign lesion showed tracer uptake to a lesser extent. SPECT, however, failed to visualize a 10-mm lesion in a patient with familial adenomatous polyposis despite a tissue T/N ratio of 4.8, while autoradiography showed radioactivity uptake in the polyps. CONCLUSION: Although SPECT has limitations in detecting small lesions because of its limited spatial resolution, T/N ratios could be measured exactly by SPECT if the lesion is of a certain volume. SPECT imaging with 99mTc-BW431/26 can precisely evaluate tracer uptake in tumors and predict the efficacy of radioimmunotherapy in patients with colorectal cancer.

Effect of inhaled furosemide on lung clearance of technetium-99m-DTPA.

UNLABELLED: The diuretic furosemide has been reported to have a protective effect on allergic asthmatic reactions. This study was performed to investigate the effect of aerosolized furosemide on the lung clearance of 99mTc-diethylene triamine pentaacetic acid (99mTc-DTPA). METHODS: Pulmonary clearance rates of 99mTc-DTPA were measured by a computerized gamma camera with and without the inhalation of aerosol furosemide in 6 nonsmoking normal volunteers (Group 1), 7 smokers without pulmonary disease (Group 2) and 11 patients with asthma (Group 3). RESULTS: None of the six normal volunteers showed significant effects of inhaled furosemide on the 99mTc-DTPA clearance rates. Three of seven smokers presented an accelerated 99mTc-DTPA clearance by inhaled furosemide and the other four showed no significant change of 99mTc-DTPA clearance by furosemide inhalation. However, in 10 of 11 patients with asthma, there was significant suppression of 99mTc-DTPA clearance by furosemide inhalation. CONCLUSION: Asthmatics possess a furosemide-sensitive mechanism. Pulmonary aerosol scintigraphy with 99mTc-DTPA will be useful in predicting the effect of inhaled furosemide therapy.
