Staged repair using omentum for posttraumatic aortoesophageal fistula.

Aortoesophageal fistulas from traumatic thoracic aortic pseudoaneurysms are almost uniformly fatal. We report a case of a young woman who nearly exsanguinated soon after diagnosis. Immediate operative intervention consisted of prosthetic graft replacement of the pseudoaneurysm and pleural patch coverage. Definitive treatment of the esophageal perforation was necessary later for mediastinal sepsis. Primary repair of the esophagus accompanied by mobilization of the omentum into the space between the esophageal closure and the prosthetic graft led to a successful outcome.

Capping of bcr-abl antisense oligonucleotides enhances antiproliferative activity against chronic myeloid leukemia cell lines.

Leukemia due to a chromosomal translocation offers an attractive model for the design and testing of antisense agents because the sequence at the translocation junction is unique to the leukemic cell population. Chronic myeloid leukemia is the most common such translocation-induced leukemia. We have found antisense phosphodiester oligonucleotides directed at the bcr-abl junction to be ineffective in inhibiting the growth of CML cell lines. Therefore, we have investigated the effects produced by certain structural modifications of bcr-abl antisense oligonucleotides. For assay purposes we used K562 cells which have the bcr exon 3/abl exon 2 junction and BV173 cells which have the bcr exon 2/abl exon 2 junction. We have found that 5'-capping with a dimethoxytrityl group and 3'-capping with an amino-2-hydroxypropyl group confer antiproliferative activity. The enhancement of activity by capping appears at least partly attributable to exonuclease resistance since stability in serum-containing medium is increased.

Identification of chromosomal structural alterations in human ovarian carcinoma cells using combined GTG-banding and repetitive fluorescence in situ hybridization (FISH).

In order to identify chromosomal structural alterations in the ovarian carcinoma cell line MLS/P, fluorescence in situ hybridization with centromeric probes for chromosomes 1, 8, 9, 13/21, 14/22, 15, 17, and X and whole chromosome painting probes for chromosomes 1, 3, 4, 5, 7, 8, 9, 10, 12, 13, 14, 17, 19, 22, and X were performed subsequent to GTG-banding. This combined approach identified 14 of the 18 clonal structurally rearranged chromosomes, with the X chromosome involved in three aberrations. In contrast, only eight of the 14 rearrangements were identifiable by G-banding alone. These results indicate that the combined G-banding and FISH approach can significantly improve the cytogenetic analysis of human neoplasia.

B-cell acute lymphoblastic leukemia with L1 morphology and coexistence of t(1;19) and t(14;18) chromosome translocations.

We report a case of adult de novo acute lymphoblastic leukemia (ALL) with unique cytogenetic abnormalities and discrepant morphologic and immunologic features. Morphology was L1 by the French-American-British classification, but flow cytometry was consistent with a mature B-cell phenotype. Cytogenetic analysis showed numerous chromosome abnormalities nonspecific for lymphoid neoplasm except for t(1;19) and t(14;18). The former is characteristic of pre-B-ALL and the latter is characteristic of follicular lymphoma. This is the first report of these two translocations occurring concurrently in ALL.

Control of cytoskeletal mechanics by extracellular matrix, cell shape, and mechanical tension.

We have investigated how extracellular matrix (ECM) alters the mechanical properties of the cytoskeleton (CSK). Mechanical stresses were applied to integrin receptors on the apical surfaces of adherent endothelial cells using RGD-coated ferromagnetic microbeads (5.5-microns diameter) in conjunction with a magnetic twisting device. Increasing the number of basal cell-ECM contacts by raising the fibronectin (FN) coating density from 10 to 500 ng/cm2 promoted cell spreading by fivefold and increased CSK stiffness, apparent viscosity, and permanent deformation all by more than twofold, as measured in response to maximal stress (40 dyne/cm2). When the applied stress was increased from 7 to 40 dyne/cm2, the stiffness and apparent viscosity of the CSK increased in parallel, although cell shape, ECM contacts, nor permanent deformation was altered. Application of the same stresses over a lower number ECM contacts using smaller beads (1.4-microns diameter) resulted in decreased CSK stiffness and apparent viscosity, confirming that this technique probes into the depth of the CSK and not just the cortical membrane. When magnetic measurements were carried out using cells whose membranes were disrupted and ATP stores depleted using saponin, CSK stiffness and apparent viscosity were found to rise by approximately 20%, whereas permanent deformation decreased by more than half. Addition of ATP (250 microM) under conditions that promote CSK tension generation in membrane-permeabilized cells resulted in decreases in CSK stiffness and apparent viscosity that could be detected within 2 min after ATP addition, before any measurable change in cell size. Permanent deformation only decreased after 20 min, once the CSK lattice had physically contracted. Importantly, regardless of cell shape or membrane continuity, CSK stiffness increased in direct proportion to the applied stress, as predicted by tensegrity (tensional integrity) cell models. These results suggest that the effects of ECM on CSK mechanics are not due to changes in osmotic or hydrostatic pressures. Rather, ECM alters CSKstiffness and apparent viscosity by binding integrins, promoting formation of molecular links with the CSK, transmitting mechanical stresses across these linkages, and inducing structural rearrangements within a continuous, tensionally integrated CSK lattice. In contrast, permanent deformation in the CSK appears to be more tightly coupled to cell extension and depends on both passive plasticity and dynamic remodeling events.

Long-wave ultraviolet radiation induces protein kinase C in normal human keratinocytes.

Skin tumor promotion by phorbol ester is believed to be mediated by the phospholipid-dependent ser/thr kinase, protein kinase C (PKC). Long-wave ultraviolet radiation (320-400 nm, UVA), which has also been shown to promote skin tumors, induces elevated levels of PKC in murine fibroblasts, suggesting that UVA may promote the development of basal and squamous cell skin cancers by a mechanism involving PKC. To examine UVA effects on PKC in a model relevant to skin, we maintained normal human epidermal keratinocytes (NHEK) in serum-free medium and exposed the cultured cells to various doses of UVA or to the phorbol ester, 12-O-tetradecanoylphorbol-13-acetate (TPA). Fifty minutes after exposure to UVA (5-20 J/cm2), PKC activity was elevated up to three-fold in NHEK cytosolic fractions, and membrane-associated PKC activity was elevated up to two-fold by UVA. The TPA treatment induced a 10-fold increase in membrane-associate PKC activity only. Immunoblot analysis suggested that a UVA-induced increase in PKC protein occurred. Both UVA and TPA reduced the cell number by 50-75% in the first 24-48 h; however, irradiated cultures began to recover at 72 h post-UVA due to an increased proliferative rate beginning after 48 h. Treatment with TPA induced a high level of differentiation as measured by cornified envelope formation. Ultraviolet A irradiation exposure was not followed by increased differentiation. These findings suggest that acute UVA exposure elevates PKC activity in human keratinocytes and may act through PKC to promote actinic skin cancer. The molecular mechanism is like to differ from that of the phorbol esters, however.

[A comparison between "moist ointment" and 0.25% iodophor, silver sulfadiazine paste and 0.1% rivanol in the treatment of deep II degree burn wounds].

One hundred and fifteen patients suffering from deep II degree burn were randomly divided into four groups, and "Moist ointment," 0.25% iodophor, silver sulfadiazine paste and 0.1% rivanol were respectively used as topical agents. Their effects were observed and compared. The results showed that "Moist ointment" group was significantly inferior to other groups in respects of healing of wound surface, bacteriostatic property, cost of treatment and formation of hyperplastic scar. Therefore, we suggested that the use of moist ointment in the treatment of deep II degree burn wound should be prohibited.

Study on the mechanism and effects of Gly-Tyr-NH2 and Gly-Tyr-Lys on rat luteal cells in vitro.

The action of Gly-Tyr-NH2, (GY-NH2) and Gly-Tyr-LYS(GYK) on 125I-LH binding, cAMP accumulation and progesterone production was investigated. Incubation of rat luteal cells for 2.5 h with GY-NH2 and GYK at dosage of 0.2 mmol/L caused a significant inhibition of basal and gonadotropin-stimulated steroidogenesis. GY-NH2 and GYK were also found to reduce cAMP formation in response to hCG. The activity of adenylate cyclase of luteal cells was inhibited by 0.2 mmol/LGY-NH2 and GYK. GY-NH2 and GYK at a concentration of 0.2 mmol/L were not found to have an inhibitory effect on 8Br-cAMP-stimulated progesterone production. GY-NH2 and GYK did not affect 125I-LH binding to LH receptors on the luteal cell surface. These results suggest that GY-NH2 and GYK inhibit steroidogenesis at the step of gonadotropin-stimulated cAMP formation in luteal cells. Adenylate cyclase in luteal cells was also inhibited.

Mechanism and etiology of primary chronic angle closure glaucoma.

The ocular anatomic features, pupil-blocking force, status of angle synechiae closure and positivity of provocative tests were compared between the primary chronic angle closure glaucoma (PCACG) and primary acute angle closure glaucoma (PAACG) by using ultrasonic biometry, computerized anterior ocular segment image processing technique, gonioscopy and provocative tests. The studies showed that the anterior chamber depth of PAACG were shallower than that of PCACG; the pupil-blocking force of PAACG was stronger than that of PCACG; the status of angle synechiae closure of PCACG were mainly creeping synechiac closure; the positivity of mydriasis test was much higher (46.01%) in PCACG than that (6.8%) in PAACG after iridectomy. These facts suggest approximately half of the PCACG cases may be mult-mechanism to contribute to the angle closure besides the mechanism of pupillary block. Therefore, the authors emphasis that the pupillary block must be eliminated as well as the other mechanism for the treatment of PCACG and finally, a criteria of diagnosis and principles of management of PCACG were proposed.

[Effects of (+/-) and (+) 15-methyl PGF2 alpha methyl ester on early pregnancy].

Treatment with 15-methyl PGF2 alpha methyl ester (PG05) resulted in termination of pregnancy and peristatic effects in mice. (+) PG05 is the active stereoisomer of racemic PG05. The therapeutic index of (+) PG05 was found to be closely similar to that of (+/-) PG05. Pregnancy of ovariectomized rats given progesterone and estrone was maintained normally. PG05 was shown to have no effect on the gestation of ovariectomized rats receiving progesterone and estrone injection. PG05 at a concentration of 20 micrograms/ml reduced cAMP secretion of rat luteal cells in response to hCG in vitro. PG05 at concentration of 10 and 20 micrograms/ml also exerted an inhibitory effect on 8 Br-cAMP-stimulated progesterone production. These results suggest that in luteal cells PG05 inhibits steroidogenesis at the steps of gonadotropin-stimulated cAMP formation and cAMP-induced progesterone production.

[Effect of tyrosine on progesterone production by perifused rat large and small corpus luteal cells].

The effect of tyrosine on progesterone production by rat large and small luteal cells was studied by perifusion method. The large and small luteal cells were separated by sucrose density gradient centrifugation. After pre-perifusion for 1 h the effluent from the perifused large or small cells was separately collected at 10 min intervals by a fraction collector. Progesterone content of each sample was measured by RIA. Results showed that the basal progesterone production by large luteal cells was 2-fold more than that of the small ones. However, the suppressive effect of tyrosine on hCG- induced progesterone production was more pronounced in small luteal cells. These differences may be due to the fact that small luteal cells uptake more 3H- tyrosine.

[Epidemiological study on the infections of Mycoplasma and Chlamydia in immoral persons and healthy controls in seven areas of China].

1,284 immoral persons and 1,161 healthy controls, from seven areas of China, were detected for the antibodies to Ureaplasma urealyticum (Uu), Mycoplasma hominis (Mh), and Chlamydia trachomatis (Ct) by indirect hemoagglutination (IHA). The results showed that: (1) in the immoral persons, the positive rates of antibody was the highest in Uu (25.47%), higher in Ct (18.22%), and lower in Mh (8.80%); (2) the levels of antibody to Mh, Uu, and Ct were significantly higher in the immoral persons than in the healthy controls; and (3) the immoral persons were commonly complicated with infections caused by Uu and Ct. It was indicated that the immoral persons were risk population of Mycoplasma and/or Chlamydial veneral diseases, and it must be noted for us controlling veneral diseases.

Suspension culture of Catharanthus roseus crown gall cell induced by Agrobacterium C58.

In comparison with calli from leaf or stem, Catharanthus roseus crown gall cell cultured on MS basic medium was superior in growth, total indole alkaloids and ajmalicine contents. The effects of illumination, cultural temperature, sucrose level of the medium and exogenous L-Trp on the growth, total indole alkaloids and ajmalicine contents of C. roseus crown gall cell cultures were studied. The results will provide a theoretical basis for the attempt of using suspension cultures of C. roseus crown gall cells to produce indole alkaloids.

[Synthesis of tyrosine-related peptide and their effect on progesterone production].

Twenty peptides containing hydroxy-amino acids have been synthesized manually by stepwise solid-phase procedure. The chloromethyl resin and MBHA resin were used as solid supports. A new reagent of 0.5 mol.L-1 DDSi/1.5 mol.L-1 phenol/DCM was applied for the removal of N alpha-Boc group. TFMSA was the cleaving reagent. After purification by C-18 column; all products were assayed according to amino acid analysis. The bioactivity of synthetic peptides was tested for the effect on progesterone production in vitro. Eight peptides, GlyTyrAlaLys, (SarSer)2Lys and its methyl ester, TyrLys, HisTyr-NH2, ThrProTyrLys-NH2 TyrThrProArgLys and AspHisProThrPheLys showed significant effect on inhibiting hCG-induced progesterone production, and first three of them could also inhibit basal progesterone secretion. However, peptide GlySerTyr exhibited stimulative activity on basal progesterone secretion. So far, no reasonable relationship between structure and bioactivity was found.

The prediction of fitness levels of United States Air Force officers: validation of cycle ergometry.

The purpose of this investigation was to evaluate the accuracy of the United States Air Force cycle ergometer test (CET) to predict maximal oxygen uptake (VO2max) of Air Force officers. The United States Air Force is currently evaluating cardiovascular fitness by cycle ergometry. In order to validate the accuracy of the CET, 50 male Air Force officers who were attending professional military education at Maxwell AFB were evaluated by a maximal treadmill Bruce test and open circuit spirometry. Characteristics of the subjects included the following mean values +/- SD: VO2max = 47.9 +/- 10.3 ml.kg-1.minute-1; age = 39.3 +/- 3.9 years; weight = 76.9 +/- 9.5 kg; height = 176.4 +/- 8.8 cm; percent fat = 20 +/- 9%; heart rate max = 174 +/- 10 beats.minute-1; and respiratory exchange ratio = 1.09 +/- 0.09. The predicted VO2max with the cycle ergometer test was 39.9 +/- 9.8 ml.kg-1.minute-1. The following product-moment correlation was derived between criterion VO2max (maximal treadmill test and the CET: r = 0.74 and standard error of estimate = 6.9 ml.kg-1.minute-1). A repeated measures analysis of variance between the CET and maximal treadmill test found a significant mean difference of 8.0 ml.kg-1.minute-1 (p < 0.0001). The CET significantly underpredicted fitness levels of Air Force officers. Based upon Air Force standards, the CET underpredicted oxygen uptake values by approximately one fitness category or 17%.

Synthesis of docosahexaenoic acid by retina and retinal pigment epithelium.

Rod outer segments of vertebrate retinas are highly enriched in docosahexaenoic acid (22:n-3), a fatty acid that is essential for optimal retinal function. The high levels of retinal 22:6n-3 are maintained through conservation (recycling) within the eye and incorporation from the circulation. The liver is thought to be a major source of 22:6n-3 through synthesis from appropriate n-3 precursors and delivery to target tissues via plasma lipoproteins. The contribution of other tissues to the total body pool of 22:6n-3 is not known. We investigated the synthesis of 22:6n-3 from [1-14C]18:3n-3 or [3-14C]22:5n-3 in frog retina and retinal pigment epithelium (RPE). RPE cells rapidly converted each precursor to 22:6n-3, which contained about 23 and 35%, respectively, of the label after 8 h. Significant labeling of 24:6n-3 and 24:5n-3 occurred when [3-14C]22:5n-3 was the substrate. In contrast, the major end products of retinas incubated with [1-14C]18:3n-3 and [3-14C]22:5n-3 were 18:4n-3 and 20:5n-3, respectively, neither of which is found in retinal lipids. Less than 5% of the radioactivity from either precursor was in 22:6n-3 after an 8-h incubation. Our results demonstrate an active in vitro synthesis of 22:6n-3 in frog RPE, but not in the retina. The labeling of 24:5n-3 and 24:6n-3 is consistent with the proposal of Voss et al. [Voss, A., Reinhart, M., Sankarappa, S., & Sprecher, H. (1991) J. Biol. Chem. 266, 19995-20000] that they are intermediates in the conversion of 22:5n-3 to 22:6n-3. Since frog RPE contains measurable amounts of 18:3n-3, 20:5n-3, and 22:5n-3, which are readily converted to 22:6n-3 in these cells, we suggest that the RPE is a source of 22:6n-3 for the retina.

A human mitochondrial ATP-dependent protease that is highly homologous to bacterial Lon protease.

We have cloned a human ATP-dependent protease that is highly homologous to members of the bacterial Lon protease family. The cloned gene encodes a protein of 963 amino acids with a calculated molecular mass of 106 kDa, slightly higher than that observed by Western blotting the protein from human tissues and cell lines (100 kDa). A single species of mRNA was found for this Lon protease in all human tissues examined. The protease is encoded in the nucleus, and the amino-terminal portion of the protein sequence contains a potential mitochondrial targeting presequence. Immunofluorescence microscopy suggested a predominantly mitochondrial localization for the Lon protease in cultured human cells. A truncated LON gene, in which translation was initiated at Met118 of the coding sequence, was expressed in Escherichia coli and produced a protease that degraded alpha-casein in vitro in an ATP-dependent manner and had other properties similar to E. coli Lon protease.
