A clinical investigation on massage for prevention and treatment of recurrent respiratory tract infection in children.

The TCM therapeutic principle of "eliminating the pathogenic factors by supporting the healthy energy" was adopted in massage to treat and prevent recurrent respiratory tract infection of children. Susceptible and healthy children of the same age were used as controls. As a result, the therapeutic effect of the treatment group is significantly better than that of the controls, with all of the immunologic indexes being approximately normal when the patients were reexamined 3 and 6 months after the massage. Statistically, the difference was significant (P < 0.01). Massage has been proved very helpful in improving the general constitution, enhancing the immune functions, preventing and treating the condition, and in health care as well.

[Investigation of intrauterine microbes after intrauterine operation].

OBJECTIVE: To explore the postoperative changes in the cultures of ureaplasma urealyticum (UU), mycoplasma hominis (Mh), L-form bacteria (L-form), anaerobic bacteria (Ana) and chlamydia trochomatis (CT) after intrauterine operation. METHODS: Four groups of patients were set up: group 1, induced abortion; group 2, intrauterine device (IUD) insertion; group 3, penicillin i.m. after IUD insertion; group 4, oral lincomycin after IUD insertion. Intrauterine secretion were aspirated to identify the above microbes before operation and within 1 week of ending of menstrual bleeding for 4 consecutive postoperative cycles. Bacteria-carrier was defined as at least one of the 5 microbes detected. RESULTS: No difference was shown in the incidence of bacteria-carrier (IBC) among the 4 groups preoperation. The IBC tended to be the highest in the first menstrual cycle postsurgery in all the 4 groups, then decreased. Compared with preoperation, there were significantly higher IBC in the 3 IUD groups (P < 0.05) except group 1. CONCLUSION: IUD is a major factor for intrauterine microbes existing after operation, and the natural body defense system can help to get rid of the organism by time. Small doses and short period of penicillin or lincomycin administration proved not effective in clearing the intrauterine microbes after IUD insertion.

[Determination of telomerase activity in BALF cells from lung cancer patients].

OBJECTIVE: To investigate the telomerase activity in BALF cells from patients with lung cancer. METHOD: The silver staining and PCR technique were used to detect the telomerase activity in BALF cells from 30 patients with lung cancer and 30 patients with chronic bronchitis or benign pulmonary diseases. RESULT: The telomerase activity in BALF cells was positive in all the patients with lung cancer (25/30, 83%), and in none of the patients with chronic bronchitis or benign pulmonary diseases (0/30, 0%). The difference was highly significant (P < 0.001). In the same patient with lung cancer, the positive rate of telomerase activity was 83% in BALF cells from the diseased lung, but only 27% in BALF cells from the normal lung (P < 0.001). CONCLUSION: The telomerase activity may be used as a diagnostic marker for lung cancer. There is a correlation between telomerase activity and the development of lung cancer.

[Six year follow-up of suspects of primary angle-closure glaucoma].

OBJECTIVE: To understand the natural history and the risk factors of primary angleclosure glaucoma and obtain experiences that might help screening for the disease. METHODS: Four hundred and eighty-five suspects were screened from 6 548 population aged over 40 years. The suspects were then followed up within six years. The following examinations were performed: visual acuity, axial anterior chamber depth, peripheral anterior chamber depth, intraocular pressure and cup/disc ratio of optic nerve papilla. The suspects with high risk were further examined. RESULTS: During the follow-up period, twenty patients (4.1%) in the suspects developed angle-closure glaucoma, 14 cases had natural attack (Acute onset developed in 6 cases and 8 cases were in chronic stage) and 6 cases were found at early stage. The anterior chamber depth shallowed progressively in 28% of suspects. CONCLUSION: Anterior chamber depth is an important index for screening of primary angle-closure glaucoma, and the regular follow-up is of significance for the suspects.

[Ultrasound biomicroscopic dark room provocative test].

OBJECTIVE: To compare the clinical application of ultrasound biomicroscopic (UBM) dark room provocative test and traditional one in screening primary angle-closure glaucoma (PACG). METHOD: 22 eyes (22 cases) with PACG in prodromal stage and 30 eyes (15 cases) with deep anterior chamber and wide angle of 15 normal persons were observed in this trial. Traditional dark room provocative test and UBM dark room provocative test were performed on all the 52 eyes at the same day time on separate days. With different positive diagnostic criteria, the sensitivity of the two methods was compared by using chi-square analysis. RESULTS: After staying in dark room for 2 hours, in PACG group, the intraocular pressure (IOP) in 10 of 22 eyes raised more than 1.07 kPa (1 kPa = 7.5 mmHg), and in 12 eyes less than 1.07 kPa or had no changes; appositional angle closure was found by Goldmann gonioscopy in 8 eyes; 3 eyes in two quadrants, 5 eyes in more than two quadrants; the appositional angle closure was found by ultrasound biomicroscopy in 15 eyes; 3 eyes in one quadrant, 5 eyes in two quadrants, 7 eyes in more than two quadrants. In the control group, IOP raised more than 1.07 kPa in 3 eyes. There was no appositional angle closure found in the control group. With the appositional angle closure as a positive diagnostic criterion, the sensitivity of the traditional test and UBM dark room test was 31.8% and 68.2%, respectively. With statistic chi-square test, there is a statistically significant difference between the two methods (P < 0.05). CONCLUSIONS: The sensitivity of UBM dark room provocative test is higher than that of the traditional one. The specificity of both methods is 100%. UBM dark room provocative test elevates the sensitivity and specificity of traditional dark room test, and reduces the false negative or positive rate in screening PACG, that is helpful to its accurate diagnosis.

[Observation of anterior chamber angle with ultrasonic biomicroscopy after posterior chamber implantation of intraocular lens].

OBJECTIVE: To observe the changes of anterior chamber angle after extracapsular cataract extraction (ECCE) with posterior chamber implantation of intraocular lens (IOL) and discuss the associated factors. METHODS: The changes of anterior chamber angle in 31 cases (41 eyes) with senile cataract were observed with ultrasonic biomicroscopy after ECCE and posterior chamber implantation of IOL. RESULTS: (1) There were 6 eyes with peripheral anterior synechia of the iris, 12 eyes with abnormal pupillary configuration, which were related to the position of IOL haptics. (2) The IOL haptics fixed out of the capsular bag were not all fixed at the ciliary grooves. (3) After postoperative 3 months, the residual cortex of the lens still existed. Too much residual cortex in the eye caused postoperative peripheral iris bombe and narrowing of chamber angle. CONCLUSION: The changes of anterior chamber angle after posterior chamber implantation of IOL are related to the position of IOL haptics and much residual cortex must be avoided.

[Quantitative real time measurement of iris configuration in living human eyes].

OBJECTIVE: To evaluate the intraobserver and interobserver reproducibility of real time measurement of iris morphology in living human eyes. METHODS: Based on the platform of software (Autocad, version 12), we developed an ultrasound biomicroscopy (UBM) image assistant measuring system. Using the system, we can perform the iris configuration quantitative measurement in living eyes. The measuring parameters include: iris rest length, radius of iris curvature and the thickness of different part of iris. Ten anterior segment images of one normal individual were obtained by a single operator to evaluate the intraobserver reproducibility of image capture, and ten times of measurement of one image were performed by a single operator to assess the reproducibility of image measurement. The measurements of three independent observers were compared to investigate interobserver reproducibility in quantitative measurement. Intraobserver and interobserver reproducibility of measurement were assessed by calculating the coefficient of variation for each individual observer and by using F test to detect a difference among the observers. The iris configurations of 96 subjects (192 eyes) were measured. RESULTS: Intraobserver reproducibility ranged from 0.9 - 4.9% in all measured parameters. Interobserver reproducibility for some of the measured parameters varied considerably and was affect by subjective interpretation of visualized anatomic landmarks. The preliminary measured parameters in Chinese show that iris rest length is 3.699 +/- 0.397 mm, radius of iris curvature is 9.101 plus minus 1.408 mm, the average thickness of iris is 0.406 +/- 0.042 mm. CONCLUSIONS: The intraobserver reproducibility and the measured accuracy can fit the requisition of the ocular biometry, physiology and pharmacology studies. The method supplies a new assistance for UBM image measurement.

[The clinical application of functional endoscopic sinus surgery and conventional technique in sinus surgery].

OBJECTIVE: To properly understand the basic theory and the clinical application of the functional endoscopic sinus surgery and the traditional technique. METHODS: The indications, extend of excision, operative approach, complication and curative effect of 1,740 cases(3,140 sides) done in the China-Japan Friendship Hospital and Jining Jiaotong Hospital were analysed. Among them, 522 cases(1,032 sides, 30%) received the traditional technique for sinusitis; 1,218 cases(2,108 sides, 70%) received functional endoscopic sinus surgery. RESULTS: Three hundred and ninety-eight cases of traditional ethmoid sinus operation and 352 of functional sinus surgeries were followed-up and their cure rates were 80% and 93% respectively. Both groups had no serious complications. CONCLUSIONS: The main indications of functional sinus surgery are infectious nasal sinus diseases or meatus nasi lesion that may interfere with the drainage. Such kinds of diseases have high morbidity than the diffused polyposis and deserves attention. Traditional operations for frontal and maxillary sinusitis should be controlled appropriately, but the traditional sphenoid sinus operation is still an effective treatment for the diffused polyposis.

[Effects of methotrexate on early pregnancy in mice].

Single s.c. injection of MTX at dosages of 8, 24 and 72 mg.kg-1 on day 7 of gestation was found to have a feticide effect in mice. The mortality of fetuses on day 13 of pregnancy was 18.0%, 55.5% and 60.3%, respectively. Treatment with MTX caused a decrease of serum progesterone level on day 13 of pregnancy. Treatment with MTX at a dosage of 24 mg.kg-1 on day 7-9 of gestation terminated total early pregnancy. MTX combined with hCG 10 U/mouse or progesterone 0.2 mg/mouse was shown to have antagonistic effect to terminate early pregnancy. Synergism of RU486 combined with MTX on termination of early pregnancy in mice was observed.

[Study on the mechanism of basic fibroblast growth factor (bFGF) release induced by phorbol ester PMA].

Basic fibroblast growth factor (bFGF) is a strong mitogenic factor and inducer of angiogenesis. It may play an important role in the growth of solid tumors. Whereas bFGF is known to act extracellularly, the protein lacks a transient signal peptide. No defined mechanism for bFGF secretion has been characterized besides release from dead or injured cells. To explore molecular mechanism that modulates bFGF release, we treated CNE-2 cells with PMA for two days and found that the treatment increased bFGF gene expression in the cytoplasm and bFGF release significantly after 48 hours. This result suggests that protein kinase C is very likely to be involved in the bFGF release regulation. Our results have also shown that PKC-alpha activated by PMA in CNE-2 cells can phosphorylate bFGF (18 KDa) in CNE-2 cells. The result suggests that PKC-alpha translocation and activation can phosphorylate bFGF in CNE-2 cells and increase bFGF release.

[Structural elucidation of the anticoagulation and anticancer constituents from Allium chinense].

Six compounds were isolated from the anticoagulation and anticancer fractions of the bulbs of Allium chinense G. Don. On the basis of chemical evidence and spectral analysis (IR, EI-MS, 1HNMR, 13CNMR, 1H-1H COSY, HMBC, HMQC and NOESY), their structures were established as (25R, S)-5 alpha-spirostane-3 beta-ol 3-O-(beta-D-glucopyranosyl-(1-->2)-[beta-D-glucopyranosyl-(1-->3)]-beta-D- glucopyranosyl-(1-->4)-beta-D-galactopyranoside) (1), (25R, S)-5 alpha-spirostane-3 beta-ol 3-O-(beta-D-glucopyranosyl (1-->2)-[beta-D-glucopyranosyl-(1-->3)](6-acetyl-beta-D-glucopyranosyl)- (1-->4)-beta-D-galactopyranoside) (2), (25R, S)-5 alpha-spirostane-2 alpha, 3 beta-diol 3-O-(beta-D-glucopyranosyl-(1-->2)-O-beta-D-glucopyranosyl-(1-->4)-beta-D- galactopyranoside) (3), (25S)-24-O-beta-D-glucopyranosyl-3 beta, 24 beta-dihydroxy-5 alpha-spirost-3-O-alpha-arabinopyranosyl-(1-->6)-beta-D-glucopyranoside (4), chinenoside II (5) and 2,3,4,9-tetrahydro-1-methyl-1H-pyrido [3,4-b] indole-3-carboxylic acid (6). 4 is a new steroidal saponin, named chinenoside VI. Compounds 1 to 3 are three pairs of steroidal saponin epimers. Among them, the 25S epimer of 2 is first reported, the 25R epimer of 2 and compound 6 were isolated from this title plant for the first time. The relative configuration of compound 6 was firstly determined by NOESY spectrum, and signals of C, H were assigned definitely.

[The real-time observation of anterior segment of primary congenital glaucoma].

PURPOSE: To study the characteristics of primary congenital glaucoma in ultrasound biomicroscopy and discuss the pathogenic mechanism of primary congenital glaucoma. METHODS: Ten eyes of 6 congenital glaucoma patients and 7 eyes of 7 normal persons were examined using ultrasound biomicroscope (UBM). RESULTS: UBM can show the dysplasia of anterior angle, iris, ciliary body and scleral spur of primary congenital glaucoma. The base of iris is thin, the ciliary body is small, and the anatomic characteristic is not clear. The thickness of iris and size of ciliary body of primary congenital glaucoma are significantly smaller than those of normal control group. CONCLUSION: The results suggest UBM can inspect some part of eye where the normal examinations can not reach, e.g., ciliary body, the posterior surface of iris, etc. The characteristics of congenital glaucoma in UBM include thin iris, small ciliary body and unclear scleral spur. That suggests congenital glaucoma not only has dysplasia of anterior angle, but also has dysplasia of iris, ciliary body and scleral spur. The dysplasia of iris and ciliary body may play a relative important role in the pathogenic mechanism of congenital glaucoma.

Rieger syndrome.

PURPOSE: To discuss the clinic features of Rieger syndrome, the reasons of making wrong diagnosis, the way of treatment, and the research progress of its molecular characterization and gene mapping of this syndrome. METHODS: Two cases of Rieger syndrome which affected a patient and his daughter were studied. Multiple clinical examinations including photography of anterior segment, gonioscopy and fundus, Humphrey perimetry, A-scan ultrasonography, multiple tonometry in a day and chromosome examination were performed. Most importantly, ultrasonic biomicroscope (UBM) was first used to show the abnormalities of anterior segment in this syndrome. RESULTS: Gonioscopic examination revealed many mesoderm tissues remained and some parts of the iris adhered to cornea. In addition to cornea, iris and chamber angle, UBM showed that there was also hypoplasia of ciliary body. The result of the chromosome examination indicated normal. CONCLUSIONS: Rieger syndrome is an autosomal-dominated disorder with mesoderm dysgenesis. Recent researches have confirmed that a locus for this syndrome maps to 4q25. Besides hypoplasia of cornea, iris and chamber angle, its ocular phenotype maybe include dysgenesis of ciliary body which is probably one of the reasons of secondary glaucoma.

[Determination of 15 trace rare earth elements in rice by microwave digestion ICP-MS].

An ICP-MS method for direct determination of 15 trace rare earth elements in rice was developed. Rapid and complete dissolution of rice was achieved by a microwave digestion procedure with concentrated nitric acid and hydrogen peroxide. The detection limits are 7.0-25.8 pg/mL, the recoveries for spiked samples are 95-105%, RSDs are 2.3-4.2%. This method is rapid, sensitive and accurate.

Mitochondrial myopathy, encephalopathy, lactic acidosis, and strokelike episodes (MELAS) syndrome: an autopsy report.

Autopsy reports of patients with mitochondrial encephalopathy with lactic acidosis and strokelike episode (MELAS) are rare. This report documents the clinical and autopsy findings of a 47-year-old woman with MELAS syndrome. The diagnosis was corroborated by documenting a mitochondrial DNA mutation tRNA-Leu (UUR) at position 3243. The patient's clinical history was marked by schizophrenia, peptic ulcer disease, constipation requiring hemicolectomy, migraine headaches, deafness, and a left temporal lobe infarct. At autopsy, a muscle biopsy demonstrated numerous ragged red fibers and a partial cytochrome C oxidase deficiency. By electron microscopy, increased numbers of slightly hypertrophic mitochondria were observed focally within myocytes and vessel walls; paracrystalline mitochondrial inclusions were not seen. The brain at autopsy showed mild cerebral atrophy and diffuse cortical gliosis. Prominent bilateral basal ganglia calcifications and vascular sclerosis were present, and a small remote left temporal lobe infarct was seen.

Characterization of delayed rectifier Kv channels in oligodendrocytes and progenitor cells.

We examined the molecular identity of K+ channel genes underlying the delayed rectifier (IK) in differentiated cultured oligodendrocytes (OLGs) and oligodendrocyte progenitor (OP) cells. Using reverse transcription-PCR cloning, we found that OP cells and OLGs expressed multiple Kv transcripts, namely Kv1.2, Kv1.4, Kv.1.5, and Kv1.6. Immunocytochemical and Western blot analyses revealed that Kv1.5 and Kv1.6 as well as Kv1.2 and Kv1.4 channel proteins could be detected in these cells, but definitive evidence for functional K+ channel expression was obtained only for the Kv1.5 channel. In addition, mRNA and immunoreactive protein levels of both Kv1.5 and Kv1.6 channels were significantly lower in differentiated OLGs when compared with levels in OP cells. Proliferation of OP cells was inhibited by K+ channel blockers, but not by incubation with either Kv1.5 or Kv1.6 antisense oligonucleotides. We conclude that (1) IK in OP cells and OLGs is encoded partly by Kv1.5 subunits, possibly forming heteromultimeric channels with Kv1.6 or other Kv subunits; and (2) inhibition of Kv1.5 or Kv1.6 channel expression alone does not prevent mitogenesis. Concomitant inhibition of other Kv channels underlying IK may be necessary for OP cells to exit from cell cycle.
