Induction of nucleolar changes in rat luminal cells by single injection or low-dose infusion of estradiol.

The nucleolar area of rat uterine luminal epithelial cells was measured for 41 h after beginning either of two dosage regimens of estradiol (E2): a single injection of 1.0 microgram E2 or a continuous infusion of E2 at a rate of 1.0 microgram/24 h. In response to a single injection of E2, mean nucleolar area was significantly elevated above control levels by 10 h. Nucleolar morphology showed slight changes, but the fibrillar component was always associated with 2-3 fibrillar centers by 10 h. In response to a continuous infusion of E2, mean nucleolar area was elevated by 10 h, reached a maximum by 18 h, and remained relatively constant thereafter. Nucleolar morphology showed significant changes by 10 h; the fibrillar component was commercially denser. Bivariate histograms of nuclear versus nucleolar areas showed a bimodal distribution after both injection and infusion, suggesting that two size classes of nuclei existed after estrogen stimulation. The results suggest that: 1) a continuous infusion of E2 at a rate of 1.0 microgram/24 h was more effective in inducing larger increases in nucleolar area and transcription than a single injection of 1.0 microgram E2 at time 0; 2) using analysis of nuclear and nucleolar areas two subpopulations of cells were identified; and 3) continuous infusion of E2 produced a steady state of ribosome production and cytoplasmic transport by 18 h.

[Personal experience using the Cloward dorsal intercorporal spondylodesis].

We herewith report on our experience with dorsal, intercorporal fusion of the lumbar spine, according to Cloward, that has been performed on twenty-five cases from 1974 to 1980. The most suitable indication for this procedure has proved to be acute herniated disc, and spinal stenosis, whereas less favourable results have been obtained in cases with spondylolisthesis and spondylitis. In ten cases a pseudarthrosis developed. The report discusses whether the period of postoperative immobilisation by means of a plaster bed or plaster cast, or other factors such as a well vascularised graft substrate, or intercorporal compression, are of prime importance in affecting successful fusion. The method is still employed but now only in combination with stabilising metallic implants.

[Treatment of femur head necrosis using a total hip prosthesis following kidney transplantation].

From 1972 to 1978 we implanted 30 total hip prostheses in 20 patients with kidney transplant and femoral head necrosis. All were patients who still had immune suppressing drug therapy. To reduce the infection risk the patients had a special preoperative treatment -- single room -- always the same surgeon, short operative procedure in the greenhouse of 45 minutes on an average and only 4 blood infusions. Thereby, we had no complications peroperative and none were observed during the following checks extending from 4 months to 6 years. Hence, we believe that the implantation of total hip prostheses in high risk patients with kidney transplants and femoral head necrosis is recommendable.

Evaluation of ready-to-use and multi-dose influenza vaccine formats in large clinic settings.

Large immunization clinics are commonly held to deliver influenza vaccine to seniors and others. Vaccine is typically dispensed from multi-dose vials but pre-filled syringes are now available, offering time savings for vaccinators. To determine if the higher purchase price of such syringes is offset by savings in time and injection supplies, we did a controlled comparison of syringe and vial formats in two large, concurrent, community-based influenza vaccination clinics. Vaccine preparation and immunization times were carefully documented along with costs for vaccine purchase, storage and injection supplies. Servicing 1,000 clients required 27 nurse hours using syringes and 36 hours using vials but the savings for personnel ($234) and supplies ($1,190) using syringes were exceeded by higher vaccine cost ($2,090 premium) and extra storage costs ($260) for bulkier packaging. Depending upon product and packaging style, programs using vials are cheaper by $709-$926 per 100 doses delivered compared to using pre-filled syringes.

Performance characteristics of a blood cholesterol measuring instrument used in screening programs.

Four experiments were conducted to assess the precision and accuracy of the Boehringer Mannheim Diagnostics Reflotron, an instrument that is being adopted by many public health groups to conduct blood cholesterol screening programs. Our study is one of the first to evaluate and document the instrument's performance characteristics. Successive generations of reagent tabs supplied by the manufacturer were tested against a reference laboratory method standardized by the Centers for Disease Control. Three of the experiments also compared two or more Reflotrons to assess intermachine reliability. The Reflotron analyzer provided precise blood cholesterol measurements both in repeated tests and among instruments. The accuracy of the method varied across reagent lots, with an average negative bias of 21.05 milligrams per deciliter (mg per dl), decreasing steadily to a negative bias of 4.07 mg per dl. Reliability between and within analyzers was high. The data provide support for the use of the instrument in blood cholesterol screening efforts, yet signal the need for attention to quality control procedures, both by the manufacturer and operators, to ensure the validity and accuracy of the results.
