Vaginal absorption of insulin in the rat: effect of penetration enhancers on insulin uptake and mucosal histology.

The absorption of insulin across the vaginal mucosa into the systemic circulation was studied in ovariectomized rats given subsequent estrogen treatment. Blood glucose levels were determined as an indirect measure of insulin absorption, and the effect of various enhancers on the hypoglycemic response was investigated. In the absence of any enhancer, no decrease in blood glucose levels was observed after vaginal administration of insulin. However, the coadministration of sodium taurodihydrofusidate, polyoxyethylene-9-lauryl ether, lysophosphatidylcholine, palmitoylcarnitine chloride, and lysophosphatidylglycerol significantly increased hypoglycemia, whereas citric acid had little effect. The histological changes in the vaginal epithelium after treatment with the enhancer systems were variable and often severe. While the efficacy of these compounds in promoting the vaginal absorption of insulin is encouraging, their mechanisms of action and long-term histological effects are yet to be defined.

Early repair of iatrogenic injury to the ureter or bladder after gynecological surgery.

Contrary to the traditional doctrine of delayed intervention in post-hysterectomy injuries of the ureter or bladder, the policy at our department has been to operate as soon as possible after the diagnosis is made. Of 68 patients (25 with vesicovaginal fistulas and 43 with ureteral injuries) early intervention was possible in 40 (59%). Primary healing was obtained in all patients. These results suggest that there is no disadvantage in early repair.

Epithelial restitution in the large intestine of the rat following insult with bile salts.

In situ loops of large bowel of anaesthetised rats were used to observe epithelial restitution following surface desquamation using solutions of bile salts. The treatment induced complete surface desquamation but no disruption of the basal lamina. There was evidence of, cell migration at 30 min, and a complete surface epithelium two h post treatment. Neither a continuous contact between migrating cells and the basal lamina, nor a complete covering of secreted mucus, appeared necessary for epithelial restitution to occur.

Effect of a selected suppression of the reticuloendothelial system on the distribution of model carrier particles.

The distribution of small labeled latex test particles has been studied after intravenous administration to the rabbit using the noninvasive technique of gamma scintigraphy. The reticuloendothelial system was suppressed using polystyrene latex particles and dextran sulfate as blocking agents. Both materials gave rise to significant uptake of particles in the lungs of the order of 60% of the administered dose. The effect seen with suppression with latex particles remained unaltered over an 8-d period, while for suppression with dextran sulfate the effect was transient, and particles that were initially deposited in the lungs were subsequently redistributed to the liver. The results are discussed in terms of previous studies on reticuloendothelial suppression using different agents, test colloids, and animal models.

A comparison of the effects of hyperosmotic salicylate solutions in closed and perfused rectal in-situ loops of rats.

The effects of presentation of hyperosmotic solutions of sodium salicylate to the closed and perfused rectal loop preparations commonly used in assessing drug absorption have been compared. Epithelial cell loss was quantified in control and treated loops. Tissue damage was significantly greater (P less than 0.01) in treated perfused loops. Fluid efflux, which was associated with a small but significant change in the haematocrit value, was noted in both systems.

The blood supply of the islets of Langerhans in the Mongolian gerbil.

Ink injection studies and point counting have been used to follow and quantify the development of vascularity in the islets of Langerhans of the Mongolian gerbil. Vascularization is essentially a postnatal event in this species. Initially, islet capillaries form part of the general capillary bed of the gland, but subsequently they develop a direct arteriolar supply and venous drainage. The capillary connections with the exocrine bed are retained to form the basis of an insulo-acinar portal system. The volume fraction of islet vascularity remains constant at 19% from birth to adulthood, while the volume fraction of the intra-insular capillaries increases gradually in the first week; it accounts for half the total islet vascularity by day 14 and two thirds of total islet vascularity in the adult.

The regeneration of rectal epithelium in the rat following wounding with suppositories of polyoxyethylene (23) lauryl ether.

The regeneration of the epithelial compartments of the rectal mucosa in rats has been quantified at time intervals up to one week following wounding with suppositories of the surfactant polyoxyethylene-(23)-lauryl ether. Regeneration of glandular tissue was complete within one week of the wounding, with new glands arising from residual gland bases and from surface invagination of empty crypt skeletons and underlying granulation tissue. This method of wounding appears to be particularly useful for the study of epithelial regeneration since there was a minimal connective tissue response to the insult.

Histology and cytology of the islets of Langerhans in the Mongolian gerbil (Meriones unguiculatus).

The histology and cell types of the islets of Langerhans were studies in the Mongolian gerbil during post-natal development using granule stains, fluorescence methods and electron microscopy. Relative islet volume, islet size distribution, islet-duct association, islet-fat association, alpha- and D cell populations and the size distribution of secretion granules in islet cells were quantified. In adult gerbils islets have an elliptical profile enclosed by a thin collagenous capsule, a central core of beta-cells, a peripheral shell of alpha-cells and an incomplete layer of D cells between. In animals of 1 week or less, islets lack a capsule, are poorly vascularized and not innervated, though alpha, beta- and D cells are present. alpha-Cells exhibit green fluorescence during these early stages that disappears when adrenergic nerve fibres grow into the islets. Electron microscopy shows a fourth (unidentified) granular islet cell type, and small granular cells, that probably correspond to intensely fluorescent cells, were found within islets and neuroinsular complexes.

Fixation of Celestin tube to the anterior abdominal wall. A new technique.

Palliative intubation in the management of an unresectable carcinoma of the esophagus can be accomplished by "push through" or "pull through" techniques. The most frequently encountered complication is tube dislodgment-proximal or distal migration. Dislodgment occurs more freqeuntly with the "push through" tubes such as the Souttar tube than with the "pull through" tubes such as the Celestin tube. The incidence of migration reported for the "pull through" tubes varies from 3 to 14 per cent. In order to prevent migration, we sutured the tube to the anterior abdominal wall in 6 patients. The sixth patient developed a gastrocutaneous fistula and fasciitis at the point of anterior abdominal wall fixation. In the next 4 cases, we used a two-suture technique, in which no single suture passed from the gastric lumen to the anterior abdominal wall fascia. The Celestin tube was fixed to the gastric wall over a Dacron felt pledget and this pledget was then anchored to the rectus fascia. This technique has prevented both migration of the tube and complications related to gastric perforation.

Hemodynamic consequences of total removal of the tricuspid valve without prosthetic replacement.

Tricuspid valvulectomy without prosthetic replacement has been advocated as a life-saving measure in the treatment of Pseudomonas endocarditis of the tricuspid valve. This report describes the hemodynamic data obtained in 10 patients before and after removal of the tricuspid valve. Seven patients remained free of carciac decompensation, but right heart failure developed in three. Analysis of the preoperative data did not permit differentiation of these two groups of patients.
