Comparison between steroid hormones and cortisol in serum and follicular fluid in stimulated and unstimulated cycles of in vitro fertilization patients.

The present study was undertaken to evaluate the effects of the concentrations of serum and follicular fluid steroids and cortisol levels on the establishment of pregnancies in in vitro fertilization (IVF). Our study group consisted of 42 women (group A) who received gonadotropins for induction of ovulation for IVF. The control group included 23 women (group B) who underwent in vitro fertilization without stimulation. Serum estradiol and progesterone levels were significantly higher in group A than in group B. Serum and follicular fluid cortisol levels were similar in both groups A and B. There was no significant difference in the fertilization rates of the stimulated or unstimulated cycles. However, there were no pregnancies in group B whereas there was a 28.5% pregnancy rate in group A. There were no correlations between the estradiol, progesterone and cortisol levels when compared to the oocyte maturity and the fertilization rates in both groups of patients.

Non-stress test: a fifteen-year clinical appraisal.

During a 15-year period (Jan. 1981 to Dec. 1995) a total of 14,950 patients were delivered in our hospital. Throughout this period fetal heart rate monitoring during labor was increased from 10% up to 85%. The overall antepartum testing was also increased from 8 to 15%. In patients with significant complications in pregnancy the mean number of non-stress tests (NST's) per patient was 1.8 tests in 1981 to 4.8 tests in 1995. The average perinatal mortality was 15.2/1000, with a gradual decline. In patients who were subjected to antepartum testing the previous rate was only 3.7%/1000. Our conclusion is that the use of antepartum and intrapartum cardiotocography has increased during the last 15 years in our clinic. As a consequence a considerable decrease was noted in the overall perinatal mortality. The non-stress test is still today the first line of antepartum fetal assessment.

Transvaginal Doppler ultrasound with color flow imaging in the diagnosis of luteal phase defect (LPD).

OBJECTIVE: Our purpose was to determine whether color flow pulsed Doppler could predict a luteal phase defect (LDP). METHOD: Twenty-one women with regular menstrual cycles and at risk for luteal phase defect were examined by transvaginal color Doppler during the follicular and luteal phase of the menstrual cycle. Progesterone was measured on the day of the Doppler exam. Ovulation was determined from the lutenizing hormone (LH) surge. Endometrial biopsy during the late luteal phase was performed on each patient. RESULT: Six patients (28.5%) were diagnosed with luteal phase defect. Mean resistance index in patients with luteal phase defect was significantly higher only throughout the luteal phases (p = 0.02). Mean progesterone levels were significantly lower for patients with LPD than for normal women (p < 0.001). Mean pulsatility index in luteal phase deficient cycles was significantly higher throughout the follicular and luteal phases (p = 0.03). CONCLUSION: Color Doppler may aid in assessing luteal phase adequacy. Doppler indices of corpus luteum blood flow in combination to plasma progesterone may be a useful index of luteal function.

Is color Doppler necessary in the evaluation of tubal patency by hysterosalpingo-contrast sonography.

BACKGROUND: To evaluate the diagnostic efficacy of hysterosalpingo-contrast sonography (HyCoSy) and to establish whether, in doubtful cases, the additional use of transvaginal Color Doppler can improve the tubal diagnosis reached with gray scale imaging alone. STUDY DESIGN: Ninety-six cases of unknown tubal function with infertility complaints were included. Within four weeks after sonographic hydrotubation, hysterosalpingography was performed. The diagnostic efficacy of gray scale and Color Doppler were compared with each other and against HSG. RESULTS: We calculated a sensitivity value of 82.9% and a specificity value of 86.1% for both tubes. The concordance value was of 83.5%. The total number of Color Doppler examinations compared with those of HSG showed a sensitivity of 91% for the right tube and 93% for the left tube and a specificity of 100% for both tubes. CONCLUSIONS: HyCoSy provides reproducible findings for the evaluation of uterine abnormalities and tubal patency. The additional use of Color Doppler is recommended as a supplement to gray scale imagin in cases of suspected tubal occlusion.

Which is the threshold glycose value for further investigation in pregnancy?

The aim of this study was to investigate the lower threshold glycose value in a 50 gr-1 hour screening test for further testing in diagnosing gestational diabetes mellitus (GDM). Our study consisted of 602 pregnant women between 24-28 weeks of gestation, who received a 50 gr glycose load followed by glycose determination one hour later. After screening 273 out of 602 women showed test serum glycose values > 125 mg/dl. All 273 women received a 100 gr 3-hour glycose tolerance test (GTT). Thirty out of 273 women were found to have carbohydrate intolerance. The overall incidence of GDM was 4.98%. When the threshold glycose value was > or = 130 mg/dl the incidence of GDM was 17.8%, but lowering the threshold to 126 mg/dl a further 1.3% of the women showed carbohydrate intolerance. In the above threshold value the sensitivity was 100% but the specificity was only 54%. However, we recommend 126 mg/dl threshold for GTT since this threshold allows for the diagnosis of all women with carbohydrate intolerance during pregnancy.

Smoking and preterm labor.

The aim of the present study was to investigate the role of maternal smoking during pregnancy in the occurrence of the premature rupture of the membranes (PROM) and premature labor . Our study consisted of 1,133 women of which 283 (group A) had premature labor (gestation < or = 37 weeks), while 850 (group B) had term labor (gestation > 37 weeks). The two groups did not differ in their socioeconomic status and did not include women with serious complications during pregnancy. There were no apparent effects of smoking on the length of gestation. However, our results showed that smoking had a marked effect on preterm labor of less than 32 weeks; we also found a statistically significant correlation between PROM in premature deliveries and smoking during pregnancy, but no gradient was observed between the number of cigarettes smoked per day and the risk for PROM, in cases of premature labor. We conclude that smoking during pregnancy raises the risk of delivery before the 32nd week, as well as the PROM in premature deliveries, independently of the number of cigarettes smoked per day.

Pregnancy rates in gonadotrophin stimulated cycles with timed intercourse or intrauterine insemination for the treatment of male subfertility.

OBJECTIVE: To compare the pregnancy rates achieved by intrauterine insemination or timed intercourse in gonadotrophin stimulated cycles in couples whose only detectable abnormality was poor sperm quality. DESIGN: Sixty-two couples with primary or secondary infertility due to male factor entered the study. The 62 couples were randomly equally divided into two groups. Each group began one of the two treatment modalities (controlled ovarian hyperstimulation in conjunction with timed intercourse or intrauterine insemination) for three consecutive cycles and then switched to the alternative treatment after one rest cycle, if pregnancy was not achieved. RESULTS: Five pregnancies (3.9%) were achieved after 128 cycles with timed intercourse and 15 pregnancies (11.5%) after 130 cycles with intrauterine insemination. The difference was found to be statistically significant (P < 0.05). CONCLUSION: We suggest that intrauterine insemination during hMG stimulated cycles improves the pregnancy rates of couples whose only detectable abnormality is poor sperm quality.

Transvaginal sonography in postmenopausal women treated with low-dose estrogens locally administered.

OBJECTIVE: The objective of this study was to determine the efficacy of low-dose estrogens, administered locally, in postmenopausal women with symptoms and signs of atrophic vaginitis. Transvaginal ultrasonography was performed for the evaluation of endometrial or ovarian abnormalities. MATERIALS AND METHODS: Fifty-six healthy postmenopausal women with symptoms of atrophic vaginitis due to estrogen deficiency were examined with transvaginal ultrasound. The endometrial thickness, the uterus and the ovaries were measured before and after 6 months of treatment with low-dose estrogens. RESULTS: The mean endometrial thickness, before and after treatment was 3.1 +/- 0.8 mm and 3.1 +/- 1.2 mm respectively. The mean ovarian volume before treatment was 4.5 ml and there was no difference after treatment. There were no changes in uterine thickness during the treatment period. CONCLUSIONS: Our study, using transvaginal ultrasonography, has shown that low-dose estrogens, administered locally, give no sign of endometrial proliferation, measured as endometrial thickness, and do not alter the ovarian volume in postmenopausal volume.

Effect of smoking on ceruloplasmin and its ferroxidase activity in pregnant women.

This study deals with the effects of smoking on serum ceruloplasmin and its ferroxidase activity in pregnant women. The mean value (+/- SD) of serum ceruloplasmin in women smokers during pregnancy is significantly higher (p < 0.01) in comparison to that of nonsmokers, whereas the ferroxidase activity of ceruloplasmin in smokers is not significantly different from that of nonsmokers. These findings suggest that ceruloplasmin in women smokers loses part of its ferroxidase activity.

Prostaglandin F1a and prostaglandin E2 plasma levels after transvaginal cervical cerclage.

Plasma prostaglandin metabolites, prostaglandin F1a (PGF1a) and prostaglandin E2 (PGE2) were measured in a serial set of maternal serum samples by radioimmunoassay after elective transvaginal cervical cerclage (Shirodkar) in 18 patients early in the 2nd trimester (14-15 weeks of gestation) for a history of cervical incompetence. Eight patients received progesterone preoperatively as a myometrial suppressant. The basal PGF1a and PGE2 were 134.0 +/- 25.9 pg/ml and 14.9 +/- 1.8 pg/ml, respectively. A gradual rise in both metabolites was observed within 1 hour after the operation (206.81 +/- 48.3 pg/ml and 16.7 +/- 1.6 pg/ml, respectively, p > .05), peaking at 6 hours (265.4 +/- 51.8 pg/ml, p < .01 and 25.9 +/- 4.9 pg/ml, p < .05), and falling to basal levels within 24 hours (136.7 +/- 26.5 pg/ml and 14.0 +/- 1.2 pg/ml, respectively, p > .05). The increase in PGF1a was proportionately greater than PGE2 metabolite (r = 0.838, p < .001). No differences were found in prostaglandin levels amongst patients who received progesterone as compared to the non-recipients for all the time intervals studied (p < .05). Our findings, further suggest that a temporary increase in prostaglandin production occurs following cervical cerclage, but its role remains unclear.

Controlled ovarian hyperstimulation with or without intrauterine insemination for the treatment of unexplained infertility.

OBJECTIVE: To evaluate whether there are any benefits from intrauterine insemination (IUI) as opposed to timed intercourse (TI) in stimulated ovarian cycles in couples with longstanding, unexplained infertility. METHODS: Forty-six couples with diagnoses of unexplained infertility were evaluated in a crossover study after a total of 141 cycles. Sixty-seven cycles were with IUI after controlled ovarian hyperstimulation (COH) while 74 cycles were after COH and TI. RESULTS: The pregnancy rate after COH/TI was 16.7% and after COH/IUI 45.2%. Cycle fecundity however was 8.9% after COH/TI and 25.7% after COH/IUI, which is a statistically significant difference (P < 0.05). CONCLUSIONS: A trial of human menopausal gonadotropin and IUI is justified in couples with prolonged infertility of unknown cause.

The predictive value of a nonstress test taken 24 h before delivery in high-risk pregnancies.

OBJECTIVES: A series of 180 cases of high risk pregnancies were studied in order to assess if a nonstress test taken 24 h before delivery is of any prognostic significance. METHODS: To assess the predictability of the NST (reactive or nonreactive) in terms of fetal outcome, the following variables were taken into consideration: fetal distress during labor, low Apgar score (< 7) and perinatal mortality rate. RESULTS: A reactive test was found to be a good predictor of the healthy fetus (negative predictive value = 91.2%). Also, specificity of the test was found to be 85.4%. CONCLUSIONS: The nonreactive test could identify a population at risk but it was not helpful as a 'stand alone' modality in decision making, because of the low sensitivity and positive predictive value rates (40.9% and 28.1%, respectively).

Changes of serum transferrin and ceruloplasmin after radiation therapy in women with cervical or uterine carcinoma.

Serum transferrin and ceruloplasmin were measured in 27 healthy women as well as in 21 women who received fractionated radiation therapy (RT) after undergoing surgery for cervical or uterine carcinoma. Although no significant difference in the mean values of transferrin was found between the healthy women and the patients before the initiation of RT, a significantly lower concentration of transferrin after the end of RT (p < 0.001) was observed in comparison to that of controls and of patients before RT. On the contrary, a significantly higher serum concentration of ceruloplasmin was found in patients before and after RT compared to that in healthy women (p < 0.001). The observed increase of ceruloplasmin after RT was not found to be significantly different from the mean value before RT. In conclusion serum transferrin is reduced by RT, while ceruloplasmin which is higher in patients before RT shows a tendency for increase following RT.

Serum changes of Ferroxidases and iron-binding capacity in pregnancy.

Serum changes of Ferroxidase I and II well as the total iron binding capacity (TIBC) and unsaturated iron binding capacity (UIBC) were measured throughout their gestation in 32 normal pregnant women. Significantly higher concentrations for all the above mentioned parameters were found as pregnancy advanced. Moreover, a significant and positive correlation between the weeks of gestation and a) serum Ferroxidase I (r:0.568 p < 0.001), d) serum Ferroxidase II (r:0.619 p < 0.001), c) serum TIBC (r:0.549 p < 0.01), and d) serum UIBC (r:0.424 p < 0.05) was found. The parallel serum changes of both ferroxidase with those of TIBC and UIBC are also shown in this study. The correlation of Ferroxidase I with TIBC (r:0.734 p < 0.001) and UIBC (r:0.536 p < 0.01) as that of Ferroxidase II with TIBC(r:0.634 p < 0.001) and UIBC (r:0.513 p < 0.01) was significant and positive. In conclusion, serum Ferroxidase I and II are progressively increased with serum TIBC and UIBC as pregnancy advances.

Blighted ovum: ultrasonic, histopathologic and hormonal considerations.

Thirty women experiencing early pregnancy failure (blighted ovum) were studied at frequent intervals with serial measurements of serum human chorionic gonadotropin (beta-hCG), progesterone (P4) and estradiol (E2). The diagnosis of blighted ovum had previously been made by careful ultrasonic examination (s). Histopathologic findings in the placenta were studied in all cases. Hormonal levels were variable and in most of the cases indistinguishable from normal pregnancies. In only 50% of the cases the histopathologic examination revealed findings compatible for the blighted ovum criteria.

The role of androgens in the late-premenopausal woman with adenomatous hyperplasia of the endometrium.

To examine the possible role of androgens in hyperplasia of the endometrium, the concentrations of testosterone (T), androstenedione (A), dehydroepiandrosterone sulfate (DHEA-S), sex hormone-binding globulin (SHBG), estradiol (E2), follicle stimulating hormone (FSH) and luteinizing hormone (LH), were measured in peripheral venous serum obtained from 42 premenopausal women with adenomatous hyperplasia of the endometrium (group A) and from 18 women without hyperplasia (group B). In patients with hyperplasia of the endometrium, significantly higher peripheral venous concentrations were found for all androgens (P less than 0.05) and estradiol (P less than 0.001) than in control patients. In contrast, there was no statistically significant difference in the mean serum values of SHBG as well as in gonadotropins among the population of compared groups. We conclude that apart from the increased estrogenic activity in patients with adenomatous hyperplasia of the endometrium, androgens may play a significant role.

Intrauterine insemination as a treatment of infertility in women with antisperm antibodies.

Twenty-four women with infertility caused by antisperm antibodies were treated by homologous intrauterine insemination. Initially, all the women had timed intrauterine insemination by washed spermatozoa for three cycles. The pregnancy rate per couple was 4.20%. The remaining 23 patients received a combined treatment of chlomiphene citrate and intrauterine insemination for three cycles, which did not increase the pregnancy rate per couple and per cycle (4.3% and 1.4% respectively). Thereafter, the remaining 22 patients received a combined treatment of hMG and intrauterine insemination for another three cycles which resulted in a pregnancy rate per cycle (6.1%) and per couple (18.20%) that was significantly greater (P less than 0.01). We conclude that infertile women with antisperm antibodies can benefit after a trial of induction of multiple follicular development with hMG in combination with intrauterine insemination.
