Simultaneous double contrast cystography and modified polycystography in the evaluation of bladder carcinoma.

Accurate knowledge about the morphology and degree of infiltration of a bladder tumor is an essential prerequisite to correctly plan, and forecast the results of, the treatment of these tumors. Among the radiological investigations double contrast cystography is superior to excretory cystogram or conventional cystogram in delineating the morphology of bladder tumors. Polycystography demonstrates, radiologically, the degree of infiltration of a bladder tumor. By simultaneously performing these procedures, the information necessary for planning the treatment is obtained in a single sitting, with one catheterization. This procedure was performed in 15 patients, in good correlation with other parameters such as cystoscopy, bimanual examination under general anesthesia and biopsy.

Double contrast polycystography: new radiologic procedure.

To plan the treatment of carcinoma of the bladder correctly accurate knowledge of the morphology and degree of infiltration of the tumor is essential. Double contrast cystography and polycystography are radiologic techniques that can provide such information. Hitherto, these two procedures have been performed separately. Herein is described a new method combining these two investigations. Thus maximum information necessary to plan the treatment of bladder tumors is obtained from a single radiograph. The advantages of this procedure are discussed.

Anuria following silver nitrate irrigation for intractable bladder hemorrhage.

Intravesical silver nitrate has been used for intractable bladder hemorrhage. A case of anuria resulting from this technique is reported. The etiology of anuria in this patient was probably owing to obstruction from deposition of silver salts. The level of obstruction was the ureterovesical junction on 1 side and the collecting ducts at the renal papillae on the other side. Therapy consisted of vigorous endoscopic evacuation of deposited silver salts from the bladder and hemodialysis with good results.

Malignant penile horn.

The fifth case of carcinoma within a penile horn is reported in a patient who also had erythroplasia of Queyrat of the glans penis. Penile horns should be considered as pre-malignant lesions, since a third of them undergo malignant change and they may be associated with a pre-malignant condition of the glans penis.

Extra-adrenal pheochromocytoma producing renal artery stenosis.

A case of extra-adrenal pheochromocytoma on the left side in relation to the left renal pedicle, producing renal artery stenosis, is described because of its rarity, and diagnostic and therapeutic problems. The pheochromocytoma was excised and the kidney was preserved.

Adenocarcinoma of the bladder in a boy.

The first case of adenocarcinoma of the bladder in a boy is described. Clinically, there were red friable granular areas over an exstrophic bladder. Multiple bladder biopsies revealed adenocarcinoma, which was treated with total cystoprostatectomy, ureterosigmoidostomy and reconstruction of the epispadias with a good result. In view of the early malignant change in an exstrophic bladder it is advantageous to perform multiple bladder biopsies, especially in older children before undertaking surgical treatment. These biopsies would help to detect the early malignant change and determine the plan of treatment.

Polycystography in the evaluation of carcinoma of the urinary bladder.

Polycystography is a safe, simple, useful, radiological procedure to assess the degree of infiltration of bladder tumors. It broadly classifies bladder tumors into two groups: infiltrating and noninfiltrating. This information is important to plan the treatment. In 96% of the cases, the radiological findings correlated well with other methods of assessing the degree of infiltration. The standard technic, modifications, advantages and disadvantages of this procedure are discussed.

Noonan's syndrome associated with cake kidney.

Noonan's syndrome is an established clinical entity. These cases are often associated with correctable cardiac abnormalities, cryptorchism, and abnormal urinary tract. Herein is described the first case of Noonan's syndrome associated with cake kidney. The very high incidence of renal tract abnormalities in association with this syndrome warrants an excretory urogram in these cases for proper evaluation and management.
