Prenatal diagnosis of abdominal wall defects and their prognosis.

To determine whether associated anatomic features may be useful in predicting fetal prognosis, 43 fetuses who had ultrasonographic evaluation for abdominal wall defects during a 13-year period were reviewed. Thirty-one fetuses had omphalocele and were classified into three groups according to fetal ultrasonography results: ruptured omphalocele with exposed liver, giant omphalocele with exposed liver, and small omphalocele without liver herniation. Twelve fetuses had gastroschisis. Ten of the 12 fetuses with gastroschisis survived; one died in utero. Nine of the 12 with a small omphalocele survived. Ten of 12 fetuses with giant omphalocele survived, but six of the seven with ruptured omphalocele died of pulmonary hypoplasia and respiratory insufficiency. Ruptured omphalocele recognized in utero was accompanied by intrauterine growth retardation and liver herniation and frequently was associated with deformity of the spine, diaphragmatic defects, vesicointestinal fissure, and meningocele. These results indicated that, together with the deformity of the spine, rupture and absence of the covering membrane with an exposed liver (noted through fetal ultrasonography) may suggest a poor prognosis because of pulmonary hypoplasia.

Alanyl-glutamine dipeptide-supplemented parenteral nutrition improves intestinal metabolism and prevents increased permeability in rats.

OBJECTIVE: The authors determined the effects of alanyl-glutamine-supplemented total parenteral nutrition (TPN) on mucosal metabolism, integrity, and permeability of the small intestine in rats. METHODS: Male Sprague-Dawley rats were randomized to receive TPN supplemented with a conventional amino acids mixture (STD group) or the same solution supplemented with alanyl-glutamine; both solutions were isocaloric and isonitrogenous. On the seventh day of TPN, D-xylose and fluorescein isothiocyanate (FITC)-dextran were administered orally. One hour later, superior mesenteric vein (SMV) D-xylose and plasma FITC-dextran concentration were measured. Intestinal blood flow and calculated intestinal substrates flux were measured with ultrasonic transit time flowmetery. RESULTS: Plasma FITC-dextran increased significantly in the STD group. Intestinal blood flow and SMV D-xylose concentration did not differ between the groups. Mucosa weight, villus height, mucosal wall thickness, mucosal protein, and DNA and RNA content in jejunal mucosa were significantly increased in the alanyl-glutamine group. Jejunal mucosal glutaminase activity and net intestinal uptake of glutamine (glutamine flux) were significantly higher in the alanyl-glutamine group as compared with the STD group. CONCLUSION: Addition of alanyl-glutamine dipeptide to the TPN solution improves intestinal glutamine metabolism and prevents mucosal atrophy and deterioration of permeability.

[Cytomegalovirus retinitis and Pneumocystis carinii choroidopathy in a patient with AIDS].

A case of Pneumocystis carinii (P. carinii) choroidopathy is reported. The patient was a 17-year-old man with acquired immunodeficiency syndrome (AIDS) who developed bilateral, multifocal, yellow-white, round, flat choroidal lesions ranging in size from 1/8 to 1/6 of the disc diameter while undergoing treatment for cytomegalovirus retinitis. He had had P. carinii pneumonia 43 months previously, and received inhaled pentamidine as suppressive therapy. Over a 4-week period of observation, the choroidal lesions appeared to enlarge slowly and increased in number in the posterior pole, with no clinical evidence of intraocular inflammation and retinal involvement. He was diagnosed as having P. carinii choroidopathy and treated with intravenous pentamidine. Three months after systemic pentamidine therapy was begun the choroidal lesions disappeared. Despite the fact that P. carinii choroidopathy is a rare opportunistic ocular infection, ophthalmic manifestations may be an important initial marker of extrapulmonary disseminated infection in some patients. Therefore we recommend ophthalmologic examinations in patients with AIDS who receive long-term aerosolized pentamidine prophylaxis for pneumonia.

Quantification of intestinal blood flow by ultrasonic transit time flowmetry in fed and endotoxaemic rats.

OBJECTIVE: To compare the dye dilution method with ultrasonic transit time flowmetry (UTTF) for quantifying intestinal blood flow in the same experimental animals. DESIGN: Open experimental study. SETTING: University hospital, Osaka, Japan. MATERIAL: 11 (experiment 1) and 25 (experiment 2) adult male Sprague-Dawley rats. INTERVENTIONS: In experiment 1, the rats were fasted overnight. In experiment 2, endotoxin, Escherichia coli lipopolysaccharide (endotoxin) or saline (sham) was injected intraperitoneally. MAIN OUTCOME MEASURES: Superior mesenteric venous and abdominal aortic blood flow. RESULTS: Experiment 1: intestinal blood flow measured by UTTF was significantly decreased by 17% (p < 0.05) and 56% (p < 0.05) after 30 minutes infusion through the tertiary branch of the mesenteric vein and during simultaneous drawing of blood samples from both the carotid artery and the superior mesenteric vein over a 1.5 minute period, respectively. The intestinal blood flow measurements obtained by UTTF at different intervals before simultaneous drawing of blood from the carotid artery and the superior mesenteric vein differed significantly (p < 0.05) from those obtained by the dye dilution method. Experiment 2: intestinal blood flow was also decreased by 21%-34% (p < 0.05) during similar simultaneous drawing of blood. Aortic blood flow in the endotoxin group was reduced by 66% (p < 0.05) compared with fed animals and 63% (p < 0.05) compared with sham animals. Simultaneously, intestinal blood flow in the endotoxin group was also reduced by 57% (p < 0.05) compared with fed or 48% (p < 0.05) compared with sham treated animals. CONCLUSION: Real intestinal blood flow might not be measured by the procedure of simultaneously drawing blood from the carotid artery and the superior mesenteric vein in rats as is usually done in the dye dilution method. Intraperitoneal endotoxin reduced aortic as well as intestinal blood flow. We propose that UTTF is an alternative method for quantifying intestinal blood flow in fed and endotoxaemic animals.

Mitochondrial encephalomyopathies preceded by de-Toni-Debré-Fanconi syndrome or focal segmental glomerulosclerosis.

We report two rare cases of mitochondrial encephalomyopathies which were preceded by renal diseases. One occurred in an 11-year-old girl diagnosed with Kearns-Sayre syndrome, which was preceded by de-Toni-Debré-Fanconi syndrome 8 years previously. The other occurred in a 14-year-old girl diagnosed with mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes, who developed focal segmental glomerulosclerosis 3 years prior to confirmation of the diagnosis. Tissue from both patient demonstrated morphological abnormalities of the mitochondria in the distal renal tubular epithelium and leiomyocytes of the small renal artery. The present cases illustrate various clinical and morphologic evidence of renal diseases which may further our understanding of mitochondrial encephalomyopathies.

Relationship between oral breathing and nasal obstruction in patients with obstructive sleep apnea.

In 30 normal subjects and 20 patients complaining of snoring or sleep apnea nasal percentage of total respiratory air volume (nasal fraction) was measured when nasal resistance was gradually increased. The switching point from nasal to oral breathing was also examined. Nasal respiratory resistance was measured by a rhinograph, and nasal fraction was measured with an inductive body-plethysmograph and with a CPAP nasal mask when the patients were at rest. Several sizes of cotton pledgets were inserted into the nasal vestibules as obturators, and nasal respiratory resistance and nasal fraction were measured repeatedly until subjects had to breathe through the mouth completely. Although the value of nasal respiratory resistance in patients with complaints of snoring or sleep apnea was higher than that of normal subjects, most of the patients did not complain of a sensation of nasal obstruction. However, the switching point from nasal to oro-nasal breathing in patients with complaints of snoring or sleep apnea was statistically higher than that in normal subjects. These results suggest that patients with complaints of snoring or sleep apnea can easily breathe through the mouth during sleep, and that chronic nasal obstruction may induce obstructive sleep apnea. Therefore, measurements of nasal respiratory resistance and nasal fraction are useful to evaluate obstructive sleep apnea.

Changes in nasal and oral respiratory resistance before and after uvulopalatopharyngoplasty.

Twelve patients with complaints of sleep apnea were examined with the all-night sleep test. They underwent uvulopalatopharyngoplasty (UPPP) and were studied with the all-night sleep test again 3 to 6 months after the operation. Nasal and oral respiratory resistance was measured in sitting and supine position before and after surgery using a microrhinograph. The physiological effect of UPPP was also compared with the results of an MRI anatomical study. Nasal and oral respiratory resistance was significantly decreased post-operatively (p <0.05). The decreased value of post-operative nasal respiratory resistance in a supine position was more marked than that of oral resistance, suggesting that UPPP eliminates the obstruction at the soft palate, especially in a supine position. Although our study was performed in awake patients enlargement of the airway in the middle pharyngeal region may improve the nasal and oral respiratory function during sleep.

Bronchial reconstruction for bronchopulmonary foregut malformation: a case report.

In this case of bronchopulmonary foregut malformation (BPFM), the bronchus of the anomalous pulmonary tissue was reconstructed and tracheoplasty for associated congenital tracheal stenosis was performed at 5 days of age. After reconstruction the right lung was ventilated, which was followed by improved blood gas data. However, the persistent fatal circulation developed, and the patient died of respiratory insufficiency on postoperative day 5. The autopsy showed that both lungs were hypoplastic. This is the first case of bronchial reconstruction of BPFM.

Development of the upright postural sway of children.

From 1988 to 1992, 1188 children (576 boys and 612 girls) three to 11 years old were studied using the Stasio-analyzer to clarify developmental change of upright postural sway. Developmental change in the contact surface area of the sole, the coefficient of variation for the contact surface area, total sway area, total sway path and position of the centre of gravity were analysed. The contact surface area increased markedly between the ages of three to five and of seven to nine years, and increased slightly between six and seven years. Age six to seven years is proposed as a turning point of plantar arch formation as regards change in the contact surface area of the sole. Upright postural sway decreased markedly between the ages of three to five years and then slowly after age six. Boys under ten years swayed more than girls. The centre of gravity of the foot in children standing upright shifted towards the toes with increasing age: its distance from the heel was about 36 per cent of the foot length at age three to five years and 42 per cent at 11 years.

[Rapid diagnosis of adenoviral conjunctivitis by enzyme-linked immunosorbent assay (Adenoclone)].

We evaluated an enzyme-linked immunosorbent assay test (Adenoclone) for the rapid diagnosis of adenovirus infection in 589 cases of acute follicular conjunctivitis. Of 255 cases of adenoviral conjunctivitis proven by positive virus isolation. Adenoclone was positive in 51.0% by visual determination and 40.4% by spectrophotometry. Twenty-seven of 130 cases giving visually positive results were interpreted to be negative by spectrophotometry. In 334 adenovirus-negative cases, Adenoclone was negative in 99.1% and 99.4 by visual and spectrophotometric determination, respectively. Adenoclone was less sensitive in cases of adenovirus 3, 4 or 37, infections than in those of adenovirus 8, and also showed lower sensitivity in cases presenting mild conjunctivitis. Adenoclone is a rapid and easy test with high specificity but low sensitivity, and thus the test seems to be helpful in diagnosing adenoviral conjunctivitis.

Recombinant transforming growth factor beta 1 and beta 2 protect mice from acutely lethal doses of 5-fluorouracil and doxorubicin.

Transforming growth factor beta 1 (TGF-beta 1) and TGF-beta 2 can reversibly inhibit the proliferation of hematopoietic progenitor cells in vivo, leading us to hypothesize that such quiescent progenitors might be more resistant to high doses of cell cycle active chemotherapeutic drugs, thereby allowing dose intensification of such agents. Initial studies showed that whereas administration of TGF-beta 1 or TGF-beta 2 did not prevent death in normal mice treated with high doses of 5-fluorouracil (5-FU), those mice that received TGF-beta 2 did exhibit the beginning of a hematologic recovery by day 11 after administration of 5-FU, and were preferentially rescued by a suboptimal number of transplanted bone marrow cells. Subsequently, it was found that the administration of TGF-beta 2 protected recovering progenitor cells from high concentrations of 5-FU in vitro. This protection coincided with the finding that significantly more progenitors for colony-forming unit-culture (CFU-c) and CFU-granulocyte, erythroid, megakaryocyte, macrophage (GEMM) were removed from S-phase by TGF-beta in mice undergoing hematopoietic recovery than in normal mice. Further studies showed that the administration of TGF-beta protected up to 90% of these mice undergoing hematologic recovery from a rechallenge in vivo with high dose 5-FU, while survival in mice not given TGF-beta was < 40%. Pretreatment of mice with TGF-beta 1 or TGF-beta 2 also protected 70-80% of mice from lethal doses of the noncycle active chemotherapeutic drug, doxorubicin hydrochloride (DXR). These results demonstrate that TGF-beta can protect mice from both the lethal hematopoietic toxicity of 5-FU, as well as the nonhematopoietic toxicity of DXR. This report thus shows that a negative regulator of hematopoiesis can be successfully used systemically to mediate chemoprotection in vivo.

Administration of recombinant human interleukin-7 to mice induces the exportation of myeloid progenitor cells from the bone marrow to peripheral sites.

The administration of recombinant human interleukin-7 (rhIL-7) to mice twice a day for 7 days does not appreciably change bone marrow (BM) cellularity, but does result in a threefold to fivefold increase in the total number of leukocytes in the spleen, an eightfold to 10-fold increase in the total number of nonparenchymal cells (NPC) obtained from the liver, and up to a 20-fold increase in the total number of peripheral white blood cells (WBC). This regimen of rhIL-7 administration also causes a profound reduction in the total number of progenitors in the BM for both single-lineage colony-forming units-culture (CFU-c) (> 90%) and multilineage CFU-granulocyte, erythroid, monocyte, megakaryocyte (CFU-GEMM) (> 99%) colonies. In contrast, mice treated with rhIL-7 exhibited increases in both CFU-c (20- to 40-fold, 20-fold, and 15- to 40-fold) and CFU-GEMM (8- to 10-fold, 30-fold, and 6- to 10-fold) cultured from the peripheral blood, spleen, and NPC, respectively. The increase in CFU in the NPC was accompanied by a fivefold increase in the number of MAC-1+ cells and a ninefold increase in the number of 8C5bright+ cells. Splenectomy of mice before the administration of rhIL-7 further increased the total number of WBC, NPC, and myeloid progenitors as compared with the rhIL-7-treated nonsplenectomized mice. Finally, selective depletion of the BM by intraperitoneal administration of 89Sr (98% reduction in BM cellularity and > 99% reduction in BM myeloid progenitors) abrogated the rhIL-7-induced increases in cellularity and myeloid progenitor number in the peripheral blood, spleen, and NPC. These results show that the changes in myelopoiesis observed after in vivo administration of rhIL-7 to mice result largely from the emigration of myeloid progenitors from the BM through the blood to the spleen, liver, and, possibly, other peripheral organs.

[Detection of herpesvirus DNA in intraocular tissues].

To elucidate the pathogenic mechanism of end-ophthalmitis due to viruses of the herpes family, one approach is to utilize the polymerase chain reaction (PCR) method for detection of the viral DNA. Using PCR, we examined 15 human eyes for the presence and distribution of DNA of the herpes simplex virus (HSV), varicella-zoster virus (VZV), Epstein-Barr virus (EBV), human cytomegalovirus (HCMV), and human herpesvirus-6 (HHV-6). HSV DNA was found in the retinal pigment epithelium (RPE) of 3 eyes, VZV DNA was found in the RPE of 2 eyes and the retina of 1 eye, EBV DNA was found in the iris and ciliary body of 2 eyes, the retina of 4 eyes, the RPE of 2 eyes, and the choroid of 1 eye, and HCMV DNA was found in the retina and choroid of 1 eye. These results indicate the possibility that herpes family viruses have an affinity for intraocular tissues, particularly the retina and the RPE.

Epstein-Barr virus genomic sequences and specific antibodies in cerebrospinal fluid in children with neurologic complications of acute and reactivated EBV infections.

Four children with infectious mononucleosis (IM) and one with reactivated Epstein-Barr virus (EBV) infection had concomitant central nervous system disorders. Cerebrospinal fluid (CSF) samples from all five patients contained EBV genomic sequences and EBV-specific antibodies in the neurologic stage, but not during convalescence. Cerebrospinal fluid from two non-neurologic IM patients had neither EBV DNA nor EBV antibodies. The EBV-positive CSF of the five with neurological disorders were aseptic in culture and all negative for other human herpesvirus DNAs and antibodies: herpes simplex virus types 1 and 2, cytomegalovirus, varicella-zoster virus, and human herpesvirus 6. Epstein-Barr virus DNA and EBV antibodies were not detected in the CSF of 17 EBV-seropositive patients with mumps meningitis, rubella encephalitis, unknown febrile convulsion, or partial epilepsy. It is suggested that EBV plays a causal role in neurologic manifestations in patients with acute and reactivated EBV infections, through direct viral invasion and immunopathological reactions.

Behaviour of hamster and mouse round spermatid nuclei incorporated into mature oocytes by electrofusion.

Round spermatids of the hamster and mouse were electrofused with homologous mature oocytes to examine the behaviour of their nuclei within the ooplasm. A single spermatid was inserted in the perivitelline space of a mature oocyte and an electric fusion pulse given. In the hamster, the best spermatid-oocyte fusion took place when the oocytes were pretreated with neuraminidase, subjected to 30 s AC (2 MHz, 20 V/cm) followed by a single fusion DC pulse (3000 V-cm, 10 microseconds) and another 30 s AC current. Inclusion of micromolar Ca2+ and Mg2+ in the fusion medium was essential for oocyte activation. Under these conditions all oocytes were activated and 20-40% fused with spermatids. Of these fused oocytes only 5-10% had fully developed spermatid (male) and oocyte (female) pronuclei. In the rest the spermatid-derived pronuclei remained small throughout the pronuclear stage. However, nucleolus-like structures appeared de novo and DNA synthesis occurred in these small pronuclei. Regardless of the size of male pronuclei, chromosomes from the spermatid and oocyte appeared to mingle and participate in the first cleavage. About 70% of the fused oocytes developed into the 2-cell stage. Electrofusion of mouse oocytes with spermatids was less efficient. Even under the best conditions tested, less than 10% of the oocytes fused with spermatids. Here again, most spermatid nuclei remained small throughout the pronuclear stage.

Morphologic variations of dense deposit disease: light and electron microscopic, immunohistochemical and clinical findings in 10 patients.

Twenty-one renal biopsy specimens obtained from 10 patients with dense deposit disease (DDD) were investigated using light microscopy, electron microscopy and immunohistochemistry. The patients included four females and six males aged 6 to 35 years (mean 16.1 years). A morphological diagnosis of DDD was made following the ultrastructural detection of continuous intramembranous dense deposits (CIMDD) in some capillary loops of at least one of the series of the repeated biopsies from each patient. With light microscopy, six patients showed membranoproliferative glomerulonephritis (MPGN). The other four patients showed diffuse proliferative glomerulonephritis (DPGN) with acute lesions showing intraglomerular neutrophilic infiltration, hump formation and endothelial swelling in three and minor glomerular abnormalities in one. Follow-up biopsies were obtained in six patients. Two patients progressed from DPGN to MPGN within 7 months, whereas three patients with MPGN showed morphologic improvement that featured increased capillary patency and regional disappearance of dense deposits along with the reduction of proteinuria. Dense deposit disease did not always feature typical amorphous and osmiophilic CIMDD spreading across the whole width of the lamina densa. This classical ultrastructural manifestation was mainly found in the patients with histologic non-MPGN and a linear peripheral pattern of complement component (C3) deposition. The MPGN patients with a granular peripheral pattern of C3 deposition also had CIMDD, but also additionally featured less dense subepithelial deposits superimposed on the CIMDD to produce an appearance simulating membranous transformation.(ABSTRACT TRUNCATED AT 250 WORDS)
