[Upper abdominal complications after coronary artery bypass operations using right gastroepiploic artery].

We discussed upper abdominal complications after 161 coronary artery bypass graftings using right gastroepiploic artery (RGEA). We had one case of bleeding from RGEA branches (1), one case of pyrolus stenosis because of anterior stomach pressed by RGEA (2), three cases of gastric ulcer (3) (three cases with a past history of gastric ulcer, two case with IABP) and one case of gastric perforation (4) as early postoperative complications. As long-term postoperative complications, we had two cases of abdominal hernia (5) and two cases of gastric cancer (6). The cases of number 1, 2 and 3 are solvable because these are technical problem. As concerns gastric mucosal ischemic complications (3.4), long-term antiulcerative medication was required for the patients with gastric ulcer as a past history or with IAPB, RGEA should not be used in case of bad general condition. And as far as gastric cancer is concerned, frequent gastric fiberscopic examinations are necessary after CABG using RGEA for the purpose of finding out early gastric cancer until we can perform culative gastrectomy without cutting RGEA.

Relationships between biochemical abnormalities and anthropometric indices of overweight, adiposity and body fat distribution in Japanese elementary school children.

OBJECTIVE: To determine the anthropometric indices linked to the biochemical risk factors for atherosclerosis in Japanese obese elementary school children, ages ranging from 6 to 12 years. DESIGN: Cross-sectional study of obese children based on fasting blood samples. SETTING: Outpatient clinic of University Hospital. SUBJECTS: 65 consecutive patients with simple obesity (38 boys and 27 girls), and age-matched controls, 184 boys and 205 girls. MAIN OUTCOME MEASURES: Percent obesity and body mass index as indices of being overweight; percent body fat and the sum of four skinfold thicknesses as indices of adiposity; waist-to-hip circumference ratio and waist-to-thigh circumference ratio as indices of body fat distribution. The anthropometric indices were standardized by calculating standard deviation scores based on data from control children. RESULTS: In the obese boys, all six anthropometric indices studied correlated closely with serum biochemical indices, and strong correlations were observed among the indices of overweight, adiposity and body fat distribution. In contrast, only the indices of body fat distribution, not those of overweight or of adiposity, were correlated with serum biochemical indices in the obese girls. No relationship was found between the indices of body fat distribution and the other anthropometric indices in the obese girls. Thus, the profile of the obese girls differed from that of their male counterparts. CONCLUSION: The results suggest that body fat distribution is related to certain biochemical complications of childhood obesity, and that androgyny in fat patterns induces metabolic derangements in children.

[Complications of endocardial biopsy in heart transplant patients].

Despite the increasing use of alternative techniques, endomyocardial biopsy remains the primary method for diagnosing cardiac allograft rejection. Between March 1986 and May 1994, 2,894 endomyocardial biopsies performed on 183 heart transplant patients were reviewed. A total of 53 (1.8%) complications occurred. 33 (1.1%) complications were associated with the introduction, including carotid puncture (0.9%), neurological reaction (0.1%), and pneumothorax (0.1%). Complications during biopsy included arrhythmias (0.4%) and ventricular perforation (0.2%). In addition, we observed three episodes of allergic reaction to a reusable biotome, three episodes of liver biopsy, and one case of pacemaker dislodgement. All complications were without significant long-term sequelae. In contrast to the cardiomyopathy population, no severe ventricular perforations or deaths occurred. Thus although endomyocardial biopsy has some risk, it continues to be a safe and effective way of monitoring rejection.

[Application of microdialysis for pharmacokinetic study in rabbit anterior chamber].

We evaluated a microdialysis technique for analyzing the pharmacokinetics of instilled or orally administered ofloxacin in the anterior chamber of pigmented rabbits. A microdialysis probe was inserted into the anterior chamber and was perfused (2 microliters/min) with Ringer solution using a microinjection pump. Two hours later, 20 microliters of 0.15 and 0.3% ofloxacin was instilled into an eye, or 20 mg/kg of the drug was administered into the stomach through an intubated catheter. Dialysate was then collected every 15 or 20 min for 6 (instillation) or 8 (oral administration) hours. Ofloxacin concentration in dialysates was determined with a HPLC-spectrofluorometry system. Ofloxacin levels in dialysates increased after the instillation in a dose-related manner, reached a maximum at 30 and 45 min, and then decreased gradually with t1/2 of 136 and 114 min after the 0.15% and 0.3% instillation, respectively. After oral administration, ofloxacin levels in dialysates reached a maximum at 120 min and decreased with t1/2 of 175 min. These data suggest the usefulness of microdialysis for pharmacokinetic studies in the anterior chamber, since continuous and stable data can be obtained from each animal.

[Coronary revascularization with arterial graft alone by normothermic cardiopulmonary bypass and warm blood cardioplegia].

We studied the effects of Calafiore technique that is intermittent (every 15 minutes) antegrade warm blood cardioplegia oxygenated and included high potassium solution with normothermic cardiopulmonary bypass. From September 1994 to February 1995, 20 patients having continuously elective coronary artery bypass grafting with arterial conduits alone were randomized to traditional intermittent cold crystalloid cardioplegia and slight hypothermic cardiopulmonary bypass (cold group) or warm heart surgery by Calafiore technique (warm group). Preoperative variables were similar to both groups, as were the intraoperative variables of number of coronary grafts, aortic cross-clamp time, and cardiopulmonary bypass time. Warm group had more rapidly spontaneous defibrillation at cross-clamp removal than cold group. CK on one postoperative day was lower than cold group. Warm group had good results as concern postoperative blood loss volume, intubated time, and change of base excess. Events of PMI (cold, 2 and warm, 0) and need of an intraaortic balloon pump (cold, 2 and warm, 0) were significantly lower in the warm group. We suspected the possibility of vasospasm was lower in the warm group. We concluded warm heart surgery (Calafiore technique) was a best technique for CABG with arterial conduits.

The concomitant augmentation of urokinase-type plasminogen activator and collagenase-like proteinase activities in X-ray irradiated cells of a human metastatic carcinomatous line.

The enzymatic activities of uPA, and a collagenase-like proteinase in the post-nuclear fraction of cell homogenates of a metastatic carcinomatous cell line following X-ray irradiation were examined by the use of chromogenic substrates and by casein- or gelatin-containing zymographies and electrophoretic gel stained with avidin-conjugated peroxidase. Enhanced activities were observed in these cells, while those of 5'nucleotidase and Na(+)-K(+)-ATPase were attenuated. A partial purification and characterization of the collagenase showed that it was able to hydrolyze the heat-denatured type-I collagen more efficiently than the native one. The activation of both uPA and collagenase enables an efficient degradation of matrix barrier proteins. These findings suggest that following a certain dose range of X-ray irradiation, tumor cells may increase their ability to migrate and invade through the enhancement of uPA and collagenase activities.

Rapid and sustained hematopoietic recovery in lethally irradiated mice transplanted with purified Thy-1.1lo Lin-Sca-1+ hematopoietic stem cells.

Hematopoietic stem cells (HSCs) are believed to play a critical role in the sustained repopulation of all blood cells after bone marrow transplantation (BMT). However, understanding the role of HSCs versus other hematopoietic cells in the quantitative reconstitution of various blood cell types has awaited methods to isolate HSCs. A candidate population of mouse HSCs, Thy-1.1lo Lin-Sca-1+ cells, was isolated several years ago and, recently, this population has been shown to be the only population of BM cells that contains HSCs in C57BL/Ka-Thy-1.1 mice. As few as 100 of these cells can radioprotect 95% to 100% of irradiated mice, resulting long-term multilineage reconstitution. In this study, we examined the reconstitution potential of irradiated mice transplanted with purified Thy-1.1lo Lin-Sca-1+ BM cells. Donor-derived peripheral blood (PB) white blood cells were detected as early as day 9 or 10 when 100 to 1,000 Thy-1.1lo Lin-Sca-1+ cells were used, with minor dose-dependent differences. The reappearance of platelets by day 14 and thereafter was also seen at all HSC doses (100 to 1,000 cells), with a slight dose-dependence. All studied HSC doses also allowed RBC levels to recover, although at the 100 cell dose a delay in hematocrit recovery was observed at day 14. When irradiated mice were transplanted with 500 Thy-1.1lo Lin-Sca-1+ cells compared with 1 x 10(6) BM cells (the equivalent amount of cells that contain 500 Thy-1.1lo Lin-Sca-1+ cells as well as progenitor and mature cells), very little difference in the kinetics of recovery of PB, white blood cells, platelets, and hematocrit was observed. Surprisingly, even when 200 Thy1.1lo Lin-Sca-1+ cells were mixed with 4 x 10(5) Sca-1- BM cells in a competitive repopulation assay, most of the early (days 11 and 14) PB myeloid cells were derived from the HSC genotype, indicating the superiority of the Thy-1.1lo Lin-Sca-1+ cells over Sca-1- cells even in the early phases of myeloid reconstitution. Within the Thy-1.1lo Lin-Sca-1+ population, the Rhodamine 123 (Rh123)hi subset dominates in PB myeloid reconstitution at 10 to 14 days, only to be overtaken by the Rh123lo subset at 3 weeks and thereafter. These findings indicate that HSCs can account for the early phase of hematopoietic recovery, as well as sustained hematopoiesis, and raise questions about the role of non-HSC BM populations in the setting of BMT.

Mouse alpha N-catenin: two isoforms, specific expression in the nervous system, and chromosomal localization of the gene.

We isolated cDNAs encoding mouse homologues of chicken alpha N-catenin, a protein associated with the cadherin cell adhesion molecules, and identified two isoforms of this protein. One isoform (alpha N-catenin I) was identical to the chicken alpha N-catenin that had previously been identified, and the other (alpha N-catenin II) differed in having a 48-amino acid insertion in its C-terminal region. The ratio of the two isoforms changed during development; the isoform II was more abundant than the other in earlier embryonic stages, whereas isoform I was predominant in the adult stage. Immunostaining and in situ hybridization analyses revealed that the mouse alpha N-catenin was expressed almost exclusively in the nervous system. During embryogenesis, alpha N-catenin was first detected in nerve fibers of cranial and dorsal root ganglia and also in early neurons in the neural tube, including motor neurons. Thereafter, the expression of this protein occurred in various regions of the nervous system. Neurons, in general, strongly expressed alpha N-catenin, especially in their axonal fibers. On the other hand, the expression in glial cells varied with the region. For example, the ependymal layers of the neural tube generally expressed low levels of alpha N-catenin except at the inner limiting membrane facing the central canal, whereas the floor and roof plate exhibited strong expression of this protein at various portions of the central nervous system. The choroid plexus was devoid of alpha N-catenin. In the alpha N-catenin-negative regions, another subtype of alpha-catenin, alpha E-catenin, was expressed. Concerning nonneural tissues, alpha N-catenin was expressed only in some local mesenchymal cell clusters and the lens fibers. These results suggest that alpha N-catenin plays specific roles in neural cell-cell interactions. We also localized the mouse alpha N-catenin gene to chromosome 6.

Direct dissolution of gallstones with methyl tert-butyl ether (MTBE) via endoscopic transpapillary catheterization in the gallbladder (ETCG).

In a pilot study of direct dissolution therapy of gallstones with methyl tert-butyl ether (MTBE), endoscopic transpapillary catheterization in the gallbladder (ETCG) was performed. Complete dissolution was seen in 8 out of 12 (66%) patients and partial dissolution was seen in 2 (16%) patients. In one of the 8 complete dissolution patients, combined extracorporeal shock wave lithotripsy (ESWL) and dissolution therapy was carried out successfully. These 8 patients were followed up for 12-20 months with regular ultrasonography. During this period, 1 patient underwent laparoscopic cholecystectomy due to stone recurrence. Thickening of the gallbladder wall was seen in 2 patients, but there were no other complications. Using Tsuchiya's classification based on ultrasound, complete dissolution was seen in type Ia stones. This pilot study suggests that the direct dissolution of gallstones with MTBE via ETCG might be a useful and safe non-invasive treatment in patients with cholesterol stones in preserved gallbladders.

Serum antioxidant status in streptozotocin-induced diabetic rat.

To determine whether decrease in serum antioxidants contributes to the increased oxidative stress, we measured the antioxidant activity (AOA), total peroxyl radical-trapping antioxidant parameter (TRAP), and their component individual antioxidants in the serum of diabetic rats. AOA was assayed as the ability of serum to inhibit lipid autoxidation in brain homogenates. TRAP was assayed as the ability to delay lipid peroxidation induced by an azo initiator. Antioxidants measured were ceruloplasmin, unsaturated iron binding capacity (UIBC) and albumin as components of AOA; and uric acid, protein sulfhydryl and alpha-tocopherol as components of TRAP. AOA was decreased markedly in the diabetics due to the decrease in ceruloplasmin, UIBC and albumin. On the other hand, the directly measured TRAP in the diabetics was unaltered. Uric acid and alpha-tocopherol were increased in the diabetics. However, decrease in unidentified scavengers offset the increase brought about by those antioxidants These results suggest that the decreased metal binding capacity contributes to the increased oxidative stress in the diabetic rats.

Experimental approaches for the reconstitution of hair in vitro.

When plucked follicles were cultured together with isolated dermal papillae in a collagen gel matrix, outer root sheath cells (ORSCs) grew more rapidly and actively than without the papillae. Cultured dermal papilla cells also activated the colony growth of ORSCs in vitro. These results may suggest the existence of some papilla-derived factor(s) activating the growth of ORSCs. In cultures of excised whole follicles whose dermal papilla had been removed, epithelial cells of bulb matrix origin grew out from the bulbous portion, and formed spikes. When a dermal papilla was implanted close to the follicle, the spikes elongated toward the papilla, and finally reached and surrounded it. This finding suggests that dermal papillae may produce some factor(s) attracting epithelial cells of hair bulb origin. In cultures of excised whole follicles, when the dermal papilla remained originally positioned in contact with the hair bulb matrix, the hair and follicle elongated for more than 1 week. But when the dermal papilla was detached from the bulb matrix, the matrix cells proliferated into the gel and formed a hair-follicle-like structure (folliculoid). In cultures of excised whole follicles whose papilla-matrix junction had been damaged by dispase, elongation of the hair and follicle was almost completely suppressed. Thus, the attachment of the dermal papilla to the bulb matrix appears to be necessary for normal hair and follicle growth.

Novel antitumor sesquiterpenoids in Achillea millefolium.

Three new antitumor sesquiterpenoids, achimillic acids A, B and C, were isolated as methyl esters from Achillea millefolium and their structures were determined spectroscopically. The compounds were found to be active against mouse P-388 leukemia cells in vivo.

Thoracic aortic aneurysmectomy with a sutureless intraluminal ringed graft.

Forty-two patients underwent replacement of the thoracic aorta with a sutureless intraluminal graft. Early and long-term results were evaluated. The operative mortality rate was 7.1%. There were two postoperative complications related to a ringed graft. One complication, the formation of a pseudoaneurysm, was caused by insufficient fixation of the graft; the other, a cerebral infarction, was related to the location of the proximal anastomosis with respect to the origin of the left subclavian artery. The 3- and 5-year actuarial survival rates were 86 and 69.5%, respectively. These were similar to survival rates of age-matched controls in the general Japanese population. Replacement of the thoracic aorta using a sutureless intraluminal graft can be performed with acceptable operative mortality, with good long-term results.

[The effect of preclotting and collagen coating on endothelializing rate and thrombogenesity of Dacron grafts in the canine thoracic aorta].

Although high porosity knitted Dacron is generally recognized to have superior healing characteristics over woven Dacron, its porosity must be controlled at the clinical operation. This can be achieved with several materials, including geratin, insoluble collagen, albumin, and fibrin. We made atherocollagen coated graft using EX-313 as a new crosslinking agent. The purpose of this study is to compare the endothelializing rate and thrombogenesity of Dacron grafts coated by atherocollagen in the canine thoracic aorta with preclotting grafts with blood or albumin. Five groups were studied: Control group (n = 10), without preclotting; A-P group (n = 8), preclotting with albumin; B-P group (n = 5), preclotting with blood; W-C group (n = 5), atherocollagen coating with low cross-linkage; S-C group (n = 7), atherocollage coating with high cross-linkage. Thoracic aorta was replaced with 8 mm graft in length of 5.0 to 5.5 cm using temporary bypass with anthron tube. Grafts were harvested 3 months following implantation, and the endothelized surface ratio was calculated by microscopic line sampling method. Endothelized surface ratio of Control group, A-P group B-P group, W-C group and S-C group were 85%, 55%, 67%, 93% and 85%, respectively. Endothelized surface ratio of W-C group and S-C group were higher (p < 0.05) than those of A-P group, B-P group. There were thrombus in non-epithelized area. We conclude that atherocollagen coated graft had superior antithrombogenesity compared to albumin or blood preclotting graft.

Expression of genes encoding connective tissue proteins in androgen-dependent SC115 tumors after androgen removal.

BACKGROUND: Shionogi carcinoma 115 (SC115) is an androgen-dependent medullary carcinoma with a compact cell pattern. When SC115 tumors grow in androgen-depleted hosts, spindle-shaped and round cells with abundant cytoplasm develop. These cells originate from the SC115 cells (Kitamura et al., Cancer Res 1979;39:4717; Terada et al., Lab Invest 1987;57:186). EXPERIMENTAL DESIGN: We investigated whether these spindle-shaped and round cells expressed mRNAs of noncollagenous connective tissue proteins, which are expressed by normal spindle-shaped cells and normal chondrocytes in developing bones. The expression and localization of osteonectin (OSN), osteopontin (OSP), matrix Gla protein (MGP), and osteocalcin (OSC) were determined by Northern blotting and in situ hybridization. RESULTS: No mRNA signals of these proteins were detectable in SC115 medullary carcinoma cells growing in DS mice that had been castrated but received injections of testosterone propionate. However, when the injection of TP was stopped, spindle-shaped and round cells with abundant cytoplasm developed. The former expressed OSN and OSP signals, and the latter OSN, OSP, and MGP signals. CONCLUSIONS: Transcripts of OSN, OSP, and MGP were expressed by some SC115-derived cells during the differentiation events that occurred after androgen removal. These results provide molecular biologic evidence that a tumor of epithelial origin can progress along the connective tissue differentiation pathway.

[The surgical treatments of ischemic heart disease: the prospective view based on the experiences at University of Bordeaux].

The indication of myocardial revascularization has been extended in this decade. Patients who are older, more debilitated, and in unstable condition are now considered acceptable surgical candidates. 355 cases of ischemic heart disease, which the author experienced at Haut-Lèvêque hospital (University of Bordeaux, France), were reported in this article. 349 cases underwent coronary artery bypass grafting, and 6 underwent other surgical procedures containing heart transplantation (4 patients). Multiple revascularization and associated procedures are performed for scheduled patients, while resuscitative interventions for emergency cases with life-threatening complications secondary to acute myocardial infarction. Today, heart transplantation is performed with well-established techniques in the world, but not yet in Japan. Actually, however, we have many patients with deteriorated ventricular function secondary to ischemic heart disease, who have no other solutions but heart transplantation, and the number of the patients is increasing year after year. Therefore, the resumption of heart transplantation should contribute to survival in the majority of patients now suffering from severely compromised cardiac function.

[Early and long-term results of 233 heart transplant patients in University Bordeaux in France: quality of life following heart transplantations].

Between March 1986 and December 1993 we had 233 heart transplant patients who were 218 males and 15 females and had a mean age of 50.9 years (range, 2 to 65 years). We analyzed the actuarial survival for these patients and investigated the status of rehabilitation and return-to-work from the view point of quality of life after heart transplant. Actuarial survival (Kaplan-Meier) was 81.7% at 1 year, 76.3% at 3 years, and 72.2% at 5 years. In 57 dead patients 24 patients (42%) died in 1 month after heart transplant. In 176 living patients 165 patients (53%) returned to life. In 129 patients except 76 retired patients only 69 patients (53%) returned to work. In 60 patients, who didn't return to work, 38 patients (63%) were physically able to work.
