The problem of stage Ia (FIGO, 1985) carcinoma of the uterine cervix.

The FIGO definition of stage Ia (microinvasive) carcinoma of the uterine cervix had been vague and caused continued confusion. In 1985 FIGO revised the definition of stage Ia by including measurements in the definition and it also subdivided stage Ia into stages Ia1 and Ia2. One hundred and eighteen of eight hundred and sixty patients with stage 0 to IIb carcinoma, who retrospectively satisfied the new FIGO definition of stage Ia, were reviewed with respect to depth of invasion, horizontal spread, number of invasive foci, vascular space involvement, pattern of invasion, incidence of lymph node metastasis, and clinical outcome. There were three potentially high-risk patients: one with pelvic lymph node metastases, one with parametrial metastasis, and the third with recurrence 23 months after initial treatment. It was pointed out that the border between stages Ia1 and Ia2 is very vague in the 1985 FIGO definition. It was proposed that a lesion with 3-mm or less stromal invasion and without vascular space involvement and confluency be defined as stage Ia1 and be safely treated with simple means.

Carcinoma of the uterine cervix with variegated histological patterns and calcitonin production.

This is a report of a calcitonin-producing cervical cancer with variegated histological patterns. The levels of calcitonin correlated with the clinical course. Multiple lung metastases were completely controlled by cisplatin/Adriamycin/cyclophasphamide combination chemotherapy, although only for a short period. Microscopically, the tumor showed dominant small cell carcinoma; however, it did show differentiation into large cell nonkeratinizing carcinoma, keratinizing carcinoma, adenocarcinoma, and possibly carcinoid. Ultrastructural study revealed a few dense core secretory granules. Multiple immunohistochemical studies were performed. It is presumed that the tumor originated from the totipotent, immature precursor cells in the cervix and differentiated into endocrine carcinoma, squamous cell cancer, and adenocarcinoma.

Preoperative CT study of lymph nodes in cervical cancer--its correlation with histological findings.

A study was conducted to retrospectively evaluate the accuracy of abdominopelvic computed tomography (CT) in the diagnosis of paraaortic and pelvic lymph node metastases from carcinoma of the uterine cervix. Seventy patients with a diagnosis of invasive carcinoma of the cervix had preoperative CT of abdomen and pelvis and subsequently underwent a radical hysterectomy with pelvic lymph node dissection and paraaortic lymph node biopsy or an exploratory laparotomy with paraaortic lymph node biopsy. Five of six patients with metastatic paraaortic lymph nodes larger than 15 mm in diameter on the histologic slides were diagnosed by CT scan to have enlarged nodes. CT diagnosis was true-positive in five of seven patients with paraaortic lymph node metastases (71.4%). Two patients with false-positive paraaortic lymph nodes had clusters of small lymph nodes less than 10 mm in diameter on the histologic slides. In contrast, only a small number of the metastatic pelvic nodes were diagnosed by CT as enlarged nodes. CT diagnosis was true-positive in 5 of 11 sites with pelvic lymph node metastases (45.5%).

Pulmonary metastasis from carcinoma of the uterine cervix.

Of 817 patients with carcinoma of the uterine cervix that were treated and followed-up, 50 (6.1%) developed pulmonary metastases. The incidence of pulmonary metastasis was 3.2% in stage I, 5.0% in stage II, 9.4% in stage III, and 20.9% in stage IV disease. The incidence of pulmonary metastasis in patients with adenocarcinoma and undifferentiated carcinoma was higher. Of the patients in whom lung metastases were detected, 41.7% had no symptoms; 96% was diagnosed within 2 years from the initiation of treatment. All patients had abnormal shadows in chest X rays. We recommend that chest X rays be obtained every 2 months within the 8 months after treatment and every 6 months thereafter. Eighty-one percent of the patients had local recurrence or other distant metastatic lesions. The main treatment for these patients was chemotherapy, and CAP was effective for the patients with adenocarcinoma. Surgical resection of the pulmonary lesion may be an effective treatment for the patients who have no lesions in other sites.

Bone metastasis from cervix cancer.

Among 713 patients followed after treatment for cancer of the uterine cervix, 48 (6.6%) had a metastasis to the bone. The rates of metastasis in each of the four clinical stages were 11/273 (4.0%) in stage I, 16/243 (6.6%) in stage II, 13/162 (8.0%) in stage III, and 8/35 (22.9%) in stage IV, respectively. The most frequent site of metastasis was the vertebral column, particularly the lumbar spine (23 cases, 48%) followed by the pelvic bones. Pulmonary metastasis also frequently occurred. For 32 (67%), lesions of the bone were detected within 1 year after completion of the initial treatment, and 36 (75%) died within 1 year after detection of the metastasis. Those patients who survived over 2 years were those for whom the lesions of the bone had been treated by radiotherapy followed by chemotherapy.

Toxicity of cadmium oxide instilled into the rat lung. I. Metabolism of cadmium oxide in the lung and its effects on essential elements.

The metabolism of cadmium oxide (CdO, insoluble form) and cadmium chloride (CdCl2, soluble form) instilled intratracheally into the rat lung was investigated. CdO might be solubilized rapidly in the lung and consequently pulmonary clearance rate of CdO was not so different from that of CdCl2. At a dose of 5 micrograms Cd/rat about 20% of the dose was translocated to the liver within 12 h, whereas gradual and consistent accumulation of Cd was observed in the kidney up to 7 days. Both pulmonary clearance and translocation of Cd to the liver were accelerated with the dose of instilled CdO, however, Cd accumulated in the kidney was proportional to the dose. Lung weight was increased by the instillation of CdO. Lung essential elements such as S, P, Mg, Zn and Mn were not affected in the inflammatory-reparative proliferative process, but Cu content of unit lung weight was slightly decreased.

Toxicity of cadmium oxide instilled into the rat lung. II. Inflammatory responses in broncho-alveolar lavage fluid.

Biochemical and cytological responses in the broncho-alveolar lavage fluid were investigated after instillation of cadmium oxide (CdO) or cadmium chloride (CdCl2) into the rat lung. Although biochemical responses of the lung to CdO were similar to the CdCl2-exposed lung, cytological response was more sensitive to CdO than CdCl2. Increases of lactate dehydrogenase, protein content and number of cells in the lavage fluid were proportional to the dose over the range of 0.5-10 micrograms Cd/rat. beta-Glucuronidase activity in the fluid increased with dose at low doses of Cd, but the activity did not continue to increase above 2 micrograms Cd/rat. A dose-response profile of phosphorus content in the lavage fluid, which might indicate amount of surfactant produced by Type II cells was similar to that observed for beta-glucuronidase in CdO-treated rats. Thus, tolerable level of instilled CdO for the rat lung was about 2 micrograms Cd/rat.

Urinary prostaglandins and renal function in obstructive jaundice.

Changes in urinary prostaglandin E2 (PGE2), 6-keto PGF1 alpha, and thromboxane (TXB2) excretion in 12 patients with obstructive jaundice were observed in relation to renal function and the renin-angiotensin (R-A) system. In obstructive jaundice before percutaneous biliary drainage the creatinine clearance (CCr) was significantly lower (p less than 0.001) and the PGE2 and plasma angiotensin II (AII) concentrations were significantly higher (p less than 0.005 and p less than 0.005, respectively) than those in normal subjects. Both 6-keto PGF1 alpha and TXB2 were widely distributed. When CCr returned to normal after drainage, PGE2 and plasma AII also returned to normal, but when CCr decreased after drainage, PGE2 and plasma AII increased. Before drainage, PGE2 correlated negatively with CCr (r = -0.72, p less than 0.01) and positively with plasma AII(r = 0.69, p less than 0.02). 6-Keto PGF1 alpha correlated positively with serum total bilirubin (r = 0.66, p less than 0.02). The percentage change in PGE2 after drainage correlated negatively with that in CCr (r = -0.95, p less than 0.005). The percentage chang in plasma AII correlated positively with that in urine PGE2 (r = 0.94, p less than 0.005) and negatively with that in CCr (r = -0.85, p less than 0.02). These results suggest that PGE2 is closely related to the R-A system and might assist in the maintenance of renal circulation in obstructive jaundice.

[Studies on thermal stimulation accompanying visible-light irradiation].

Visible-light-cured composite resin is one of the most frequently used materials in clinical practice. The use of this material inevitably involves dental pulp stimulation, i.e., thermal stimulation due to visible-light irradiation, during the restoration procedure. To determine the variations of temperature involved in visible-light irradiation, the author examines differences in temperature generated by various types of visible-light curing units. Changes in the temperature under varying conditions of irradiation were also examined. The results were as follows: 1) Among the visible-light curing units used in the present study, the highest temperature was obtained from Translux CL (a rise of 17 degrees C) whereas Hiliomat showed the lowest value (a rise of 7 degrees C). 2) The temperature increased with prolongation of irradiation time, reaching a maximum level (a rise of 15 degrees C) at 60s. 3) The temperature increased with shortening of irradiation distance, reaching a maximum level (a rise of 20 degrees C) at 3mm.

[Pernicious anemia with indicanuria].

A 67-year-old female was diagnosed as having classical pernicious anemia. Laboratory data included low serum vitamin B12 concentrations, abnormal deoxyuridine suppression test, methylmalonic aciduria, atrophic gastritis, positive anti-intrinsic factor antibody and Schilling test results typical of pernicious anemia. During hospitalization it was incidentally noted that her urine was green colored. Jaffe' reaction and Obermayer reaction for indicanuria were both positive. Dark purple crystalline material was obtained by centrifugation of her urine. The crystalline substance was soluble in methanol and its absorbance curve was identical to that of authentic indoxylsulphate potassium salt. Daily output of this substance was nearly 50 times normal. There was no increase in urinary excretion of monoamino-monocarboxyl amino acides. The exact reason for her indicanuria was not clear, although abnormal bacterial growth in the intestine remained as a possibility.

Molecular genetic approach to the analysis of clonal proliferation in hematologic disorders.

The clonal composition of each cell population was determined from the characteristic methylation pattern of DNA and the restriction fragment length polymorphism (RFLP) of the hypoxanthine phosphoribosyltransferase (HPRT) and phosphoglycerate kinase (PGK) genes, both located on the X chromosome. About 71% of Japanese females are heterozygous in terms of the RFLP of either HPRT or PGK genes, which was demonstrated by using 5' genomic DNA or cDNA probes for these genes. All 3 cases of chronic myeloproliferative disorders showed monoclonal patterns. AML or ALL cases demonstrated either monoclonal or polyclonal patterns depending upon the percentage of blastic cells. Monoclonal patterns were seen in 3 of 4 cases of myelodysplastic syndromes and both PNH cases.

[Strengthening for dental porcelain surface].

UNLABELLED: Although the metal crown is aesthetically improved when porcelain is fused to it, a large number of different factors often cause the porcelain to break. Here we report a method to prevent this from happening. As a hardening treatment for the porcelain surface, the Na-ions in the porcelain, which have a short radius, were replaced by K-ions having a long radius to strength the surface against compressive forces. RESULTS: 1. The bending test showed that the strength of the porcelain had increased from 20 to 38%. 2. The hardness test indicated that the strength had increased from 23 to 24%. 3. The breaking test showed that although the strength of the porcelain had not increased to any great extent, the strength of the veneer shell had increase by more than 30%. 4. The EPMA measurement showed that ion replacement had penetrated to as much as 50 to 80 microns. 5. Clinical tests indicate that even after 19 months, the surface of the porcelain remains the same and that the condition is quite stable.

[Temporary restoration using light-cured resin].

In spite of various problems involved in chemically cured resin (self-curing resin), e.g., irritability, and heat and shrinkage at polymerization, it has traditionally been the material of choice in the production of temporary restorations. However, light-cured resin, which does not have the disadvantages of chemically cured resin, has recently been developed and applied to clinical treatment. The present study was conducted to examine the clinical application of light-cured resin to temporary crowns. Two types of light-cured resin and one type of chemically cured resin were examined for their mechanical properties. The properties involved in handling and setting, and the methods of producing crowns were also studied. The results were as follows: 1) In the tensile strength test, there was no significant difference between the two types of light-cured resin, i.e., Triad and Unifast LC. The tensile strength of Unifast, a chemically cured resin, was slightly greater than the others, the difference being significant. 2) The Vickers hardness test showed no significant differences between the three types of resin. 3) The mechanical properties of Triad were not influenced by differences in the curing unit (the unit for general oral use or exclusive use) used for polymerization. 4) The test of setting properties revealed that Unifast LC needed longer than Unifast from the 2nd through the 4th clinical stage. In particular, Unifast LC required about 2.5 times the duration needed by Unifast at the 2nd stage in the tray method. 5) The heat generation test showed that the exothermic temperature in Unifast was significantly higher than in the two types of light-cured resin. 6) Heat generation associated with light curing was noted during the use of a light-curing unit for oral use.

[Staging laparotomy in gynecologic cancer].

Gynecologic cancers have been routinely staged by clinical means and treatment plans have been decided according to the results of clinical staging. However, it is a well known fact that there is a substantial disparity between clinical and surgical disease extent, which has been reported between 20% and 50%. The purpose of staging laparotomy seems to be; (1) to determine precise histological extent of the disease before treatment, and (2) to individualize treatment on the basis of specific patterns of disease spread. Staging laparotomy consists of adequate midline incision, meticulous exploration of the abdominal cavity and organs with biopsies if necessary, peritoneal cytology, para-aortic lymph node (PAN) biopsy, and pelvic lymph node biopsy or dissection. PAN biopsy was performed 419 patients with cervical cancer. Twenty-three (5.5%) of 419 showed positive PAN. PAN(+) rate according to clinical stage was; Ib 1.4%, IIa 2.0%, IIb 7%, III 18.6%, and IV 25%. Scalene lymph node (SLN) biopsy was also performed on 47 cervical cancer patients. When PAN was positive, 17.6% of SLN was also positive. There was no positive SLN when PAN was negative. Of 125 patients who had peritoneal cytology studied, 14 (11.2%) had positive cytology. About 30% of patients with positive PAN were expected to survive 5 years, however, all patients with positive SLN died within 2 years. Peritoneal cytology was also thought to be an important prognostic factor. Ninety-eight patients with endometrial cancer underwent pelvic lymphadenectomy and 16 (16.3%) had positive nodes. Positive rate according clinical stage was; I 5%; II 33.3%, III 25%, IV 100%.(ABSTRACT TRUNCATED AT 250 WORDS)

[An evaluation of scalene lymph node metastasis in patients with gynecologic malignancies].

From 1981, through 1985, 90 patients with primary or recurrent gynecologic malignancies underwent a scalene lymph nodes (SLN) biopsy. Pelvic lymph nodes (PN) and paraaortic lymph nodes (PAN) also were examined for metastasis with CT, lymphography, and palpation or were biopsied after a laparotomy. Twenty-three of these 90 patients (25.6%) were found to have positive SLN. In 5 of these 23 (21.7%), their SLN had not been palpable on physical examination. All signs of PN, PAN and SLN were examined in 42 patients. Twenty-one of these 42 (50%) had positive PN, 14 (66.7%) had positive PAN, and 4 (28.6%) had positive SLN. Seven patients with negative PAN had no SLN metastasis. Of twenty-one patients with negative PN, 2 had positive PAN and none had an SLN metastasis. Thus, it has been concluded that an evaluation of SLN is important for the management of a gynecologic malignancy in patients with positive PAN.

[Diagnosis and treatment of cervical dysplasia].

Between 1981 and 1986, 219 patients with cervical dysplasia were treated at the Kyushu University Hospital. All patients were diagnosed by cervical smear, colposcopy, and cervical biopsy and/or endo cervical curettage. They have been followed for more than one year (average 37 months). Of the 219 patients, 63 had mild dysplasia, 99 had moderate dysplasia, and 57 had severe dysplasia. One hundred and forty patients were treated with carbon dioxide laser. The remission rate was 95.0% after one treatment and 98.6% after two treatments. The laser therapy is effective, but a certain rate of recurrence is to be expected. Recently, 23 patients were treated with the cryotherapy and the remission rate was 83%.

[Study on the treatment of malignant vulvar diseases].

During the past 15 years, 8 squamous cell carcinoma in situ and 21 invasive squamous cell carcinoma were treated in our department. Squamous cell carcinoma in situ was adequately treated by either wide local excision or CO2 laser vaporization. Radical vulvectomy was the main treatment modality in our department. Older patients tolerated well the surgery and had better outcome than those treated with radiotherapy alone. Histologically proven groin node metastasis was the risk factor to be overcome.
