[Histogenesis of the clear cytoplasm in the tumor cells of smooth muscle origin].

Perinuclear clear cytoplasm observed in the light microscopic specimens of the tumor cells of smooth muscle origin is in general, understood as the artefact caused by the formalin fixation. However, the precise mechanisms of the histogenesis of clear cytoplasm are still not clear. We observed the clear cytoplasm directly by mean of the electron microscopy of materials detached from light microscopic specimen. Furthermore, we observed the light microscopic specimens made by varying types of methods, examining whether the clear cytoplasm was present or not. The electron microscopy of materials detached from light microscopic specimens revealed the band-like defects of cytoplasm along the long axes of tumor cells. These defects were thought to result from the falling off of cytoplasm. The 1 mu section of the epon embedded block derived from the paraffin embedded block for light microscopic specimen presented no clear cytoplasm, suggesting that the cytoplasm falls off at the procedure of deparaffinization and staining. Although the specimen of conventional frozen section showed no clear cytoplasm, the specimen made by the frozen sectioning after formalin fixation revealed clear cytoplasm. Consequently, it is thought that the fixation of the tissue before the sectioning makes the cytoplasm fragile, thereafter, the cytoplasm falls off at the procedure of deparaffinization and staining.

Intraventricular neuroblastoma. A light and electron microscopic study and review of the literature.

The neuroblastoma can generally be diagnosed from the histologic findings, but electron microscopic evidence is often required. A neuroblastoma occurring in the lateral ventricle of a 17-year-old female is described in this report. The tumor cells, which were strongly stained with hematoxylin, contained a small round or oval nucleus with scant cytoplasm and an obscure cell border as observed by light microscopy. No calcification, mitoses and typical ganglion cells were detected. An electron microscopic examination disclosed dense-cored vesicles, cell processes, clear vesicles, dumbbell-shaped dense bodies, microtubules and zonulae adherentes in the tumor cells. These findings suggest differentiation of the tumor cells to neuronal cells.

Primary extranodal non-Hodgkin's lymphoma of the oral region.

Seventy cases (47M, 23F) of primary extranodal non-Hodgkin's lymphoma of the oral region were studied to determine tumor characteristics. The most frequent disease sites were the palate (21 cases), gingiva (17 cases) and parotid gland (13 cases). Each lymphoma was classified according to the criteria of the Working Formulation for Clinical Usage. Only 5.7% of cases were follicular lymphomas while diffuse lymphomas had a high incidence. Histologic subtypes included small lymphocytic (1%), small cleaved cell (7%), mixed small and large cell (20%), large cell (43%), large cell, immunoblastic (17%), lymphoblastic (9%) and small non-cleaved cell lymphoma (3%). Immunologic study utilizing the avidin-biotinylated horseradish peroxidase complex (ABC) technique demonstrated the presence of intracytoplasmic monoclonal immunoglobulin in 24 (34%) of the suggested B-cell lymphoma cases; 20 tumors (28%) were classified as T-cell lymphoma based on a positive reaction for mouse monoclonal antibody (UCHL-1) to T-cell related membrane antigen; 11 tumors (16%) contained intracytoplasmic alpha 1-antitrypsin, suggesting true histiocytic lymphoma; 15 tumors (22%) did not contain immunoglobulin, UCHL-1 or alpha 1-antitrypsin positive cells and showed no definite characteristics.

Extracorporeal immunoadsorption with IM-PH or IM-TR column.

In order to selectively remove pathogenic macromolecular reactants, a biological affinity type adsorbent (a DNA colloidin charcoal column or protein A sepharose CL4B = Prosorba) has been developed and used for the treatment of immune disorders, alloimmunization and cancer. However, because physiologically active materials are required in this procedure, it is difficult to ensure an adequate supply of raw materials (and their handling, sterilization and preservation as an immunoadsorbent. To overcome the above-mentioned problems, we developed physicochemical type immunoadsorbents IM-TR and IM-PH, which consist of polyvinyl alcohol gel where tryptophan, in the former, and phenylalanine, in the latter, is used as a ligand. IM-PH has a better selectivity than IM-TR, however, IM-TR is a more efficient adsorbent of anti-acethylcholine receptor antibody than IM-PH. IM-PH plasma perfusion has been successfully used with patients with rheumatoid arthritis, systemic lupus erythematosus (SLE), and multiple sclerosis (MS).

[Effect of isometric hand-grip exercise on left ventricular diastolic filling in patients with effort angina: a pulsed Doppler echocardiographic study].

To detect myocardial ischemia and to estimate cardiac reserve in patients with effort angina pectoris without history of myocardial infarction, left ventricular diastolic filling was measured using Doppler echocardiography during isometric handgrip exercise. Nineteen patients with effort angina pectoris undergoing coronary angiography and 16 normal subjects were studied. The angina patients were categorized in two groups: 12 with single vessel disease (SVD) and seven with multiple vessel disease (MVD). Fifty percent maximum voluntary contraction isometric handgrip exercise was performed for two minutes. 1. The resting A/R in the angina group was significantly greater than that of the normal subjects (SVD: 1.20 +/- 0.24, MVD: 1.21 +/- 0.27, normal 0.85 +/- 0.10) (p less than 0.001). However, the values of many cases in these three groups overlapped. 2. In SVD, the A/R increased significantly during isometric handgrip exercise (1.20 +/- 0.24 to 1.96 +/- 0.66: p less than 0.001). The delta A/R (0.76 +/- 0.15) was significantly greater than that of patients in other groups (MVD: 0.10 +/- 0.13, normal: 0.09 +/- 0.01) (p less than 0.001). Consequently, the A/R after exercise clearly distinguished the SVD from the normal subjects. 3. In MVD, the A/R did not change significantly during exercise (1.21 +/- 0.27 to 1.31 +/- 0.41), and there were no significant differences in delta A/R as compared to the normal subjects (p less than 0.01). The A/R decreased during exercise in three of the seven patients, and this was markedly different from that of the normal subjects. These findings suggest that assessment of changes in left ventricular diastolic filling during isometric handgrip exercise is useful in detecting myocardial ischemia and in estimating cardiac reserve in patients with effort angina pectoris.

[Magnetic resonance imaging for cardiovascular masses].

We examined 10 patients with cardiovascular masses using 0.15 tesla permanent magnetic resonance imaging (MRI) incorporating an ECG-gated spin echo technique, and investigated the usefulness of MRI in the noninvasive diagnosis of cardiovascular masses. Patients were one with rhabdomyoma, one with leiomyosarcoma, one with malignant mesothelioma (diffuse type), three with pulmonary arterial invasions by lung carcinoma, one with left atrial invasion by lung carcinoma, and three with the superior vena cava (SVC) syndrome. 1. Each image obtained with TE of 40 msec, and TR of one R-R interval clearly defined the mass by its intermediate signal intensity. 2. In the case of rhabdomyoma occupying the right ventricle, leiomyosarcoma growing from the inferior vena cava into the right atrium and malignant mesothelioma diffusely invading the mediastinum, the morphology and the extent of each mass in relation to the surrounding structures were distinctly identified in the coronal, sagittal or oblique sections for situation, in addition to the routine transaxial section. 3. In the cases of pulmonary arterial invasion by lung carcinoma and the SVC syndrome, the masses invading the pulmonary artery or narrowing large vessels were clearly separated from adjacent structures on the sections through each of the large vessels. 4. In the case without left atrial invasion, which was proved by autopsy, a high intensity line due to mediastinal fat demarcated the mass distinctly. This finding is a useful sign for evaluating the extension of a tumor. 5. The image obtained with TE of 80 msec, and TR of double R-R interval (long SE image) showed a different contrast from the image obtained with TE of 40 msec and TR of one R-R interval. Leiomyosarcoma had a more heterogenous intensity on the long SE image which may result from a different T2 value caused by necrotic, edematous or fibrotic changes within the tumor. These results indicate that MRI also provides some information about the differentiation between benign and malignant masses.

[A case of primary hypothalamic malignant lymphoma with diabetes insipidus].

A case of primary malignant lymphoma involving the hypothalamus is reported. A 31-year-old man was admitted with progressive disorientation. A computed tomography (CT) scan demonstrated a normodense, homogenously enhanced, and nodular shaped mass lesion involving the suprasellar region and extending to the third ventricle. In view of marked cerebral edema, dexamethasone was intravenously administered, at a dose of 4 mg/4 hr, and gradually tapered off. CT scans obtained 10 days after the admission revealed dramatic regression of the lesion, which corresponded with the clinical remission. However, on the 4th day after the discontinuance of corticosteroid, a CT scan demonstrated the recurrence of the mass lesion. Because of the rapid deterioration of consciousness and the progressive enlargement of the mass and dilatation of the ventricles, partial removal of tumor and ventricular drainage were carried out on the 24th day after admission. Dexamethasone was intravenously administered again at a dose of 4 mg/4 hr. A CT scan obtained five days after the operation showed complete disappearance of the lesion, and the patient had became almost alert. CT scans obtained on the 18th day and the 35th day after the operation showed complete disappearance of the mass. However, Magnetic Resonance Imaging revealed a localized area of abnormal signal intensity in the hypothalamus. Diabetes insipidus was continuously observed from the time of admission. A concomitant change of beta-2-microglobulin (beta-2-MG) and immunoglobulin (IgA, IgG, IgM) concentrations in serum and cerebrospinal fluid (CSF) with clinical profiles and CT findings were observed.(ABSTRACT TRUNCATED AT 250 WORDS)

Papillary cystic tumor of the pancreas: an immunohistochemical and ultrastructural study of 14 patients.

We report herein the clinicopathological, immunohistochemical and ultrastructural studies on 13 female patients and one male patient with papillary cystic tumors of the pancreas. Their ages ranged from 12 to 60 (mean 25) years. Most patients complained of abdominal mass or abdominal pain. Following complete resection of the tumor, all have remained well for between 3 months and 19 years (maen 5 years). In one patient the tumor was malignant and, 10 years after the initial partial resection, there was a recurrence with involvement of the colon, metastasis to the lymph nodes and venous invasion. Immunohistochemically, most tumor cells were positive for neuron specific enolase, synaptophysin, alpha-1-antitrypsin and vimentin and sometimes for the estrogen receptor related antigen, ER-D5, and the oncogen product of c-Ha-ras, c-Ha-ras P21. Ultrastructurally there were zymogen-like intracytoplasmic granules, intercellular junctions and intercellular spaces. These results support the hypothesis that the tumor originates from undifferentiated cells capable of differentiation toward acinar, endocrine or ductular cells. Estrogen and the c-Ha-ras oncogene presumably are linked to the development.

[A pleomorphic lipoma of the palm--comparison to spindle cell lipoma].

A pleomorphic lipoma usually occurs in the neck, the shoulder, and/or on the backs of men over the age of 50. This tumor displays a mixture of fat cells, pleomorphic cells, floret cells, and bundles of mature collagen fiber. Herein we report on a pleomorphic lipoma that occurred on the palm of a 56-year-old woman. An immunohistochemical study, performed to compare the characteristics of a pleomorphic lipoma with those of a spindle cell lipoma, has suggested that the pleomorphic and floret cells of a pleomorphic lipoma are similar to the spindle cells of a spindle cell lipoma and not to fat cells. Our result seems to indicate that these two tumors are on the same spectrum and only the quantity of the pleomorphism keeps them distinct.

Transplant-induced recovery from 6-OHDA lesions of the nigrostriatal dopamineneurones in mice.

Attempts to reconstruct the damaged nigrostriatal pathway in experimental models of Parkinson's disease have thus far been carried out in animals with neurotoxically induced dopamine deficiency. Our study established that unilateral 6-hydroxydopamine (6-OHDA) lesions of the nigrostriatal dopamine (DA) neurons produced a well-characterized functional asymmetry in the behaviour of the C57BL/6 (H-2b) mice. The intraperitoneal administration of methamphetamine induced ipsilateral rotation at 7-20 turns/min 1 x 10(6) syngenic DA-rich cells of embryonic ventral mesencephalon were stereotaxically transplanted in the caudate-putamen. A complete recovery of methamphetamine-induced rotational response was produced around the 60th day after the syngenic cell suspension graft. And a complete compensation of the rotational response was also brought about with the DA-rich cells from embryonic ventral mesencephalon (crown-rump length; 10-13 mm) of allogenic C3H/HeN (H-2k) mice. The FACS IV analysis revealed no H-2 (Kk and Iak) antigens before transplantation of these embryonic cells. Immunohistochemistry showed that the dopaminergic fibers had grown predominantly into the ipsilateral caudate-putamen. These results provide evidence of integration of syngenic and allogenic grafts and host tissue. And the immunological response in the transplanted brain are under investigation.

Effects of phenytoin on cell-mediated immunity.

The effects of phenytoin on cellular immunity were examined in murine models. Fresh splenocytes were obtained from mice which had received 1 mg/day of phenytoin i.p. for 28 days. The serum concentration of phenytoin in these animals was 10-20 micrograms/ml. The proliferative response of splenocytes to mitogens was assessed by 3H-thymidine incorporation. The cytotoxic activities of cells such as natural killer (NK) cells, cytotoxic T lymphocytes (CTL), and lymphokine-activated killer (LAK) cells were estimated by a 4-h 51Cr release assay. The 3H-thymidine incorporation of splenocytes was reduced significantly (P less than 0.01) in phenytoin-treated mice. The NK and CTL activities of splenocytes from phenytoin-treated mice were significantly suppressed. However, the LAK activity of phenytoin-treated mice was equal to that of control mice.

An adoptive immunotherapy of patients with medulloblastoma by lymphokine-activated killer cells (LAK).

An adoptive immunotherapy of 6 patients with medulloblastoma by lymphokine-activated killer (LAK) cells is described. They were from 2 to 9 years in age and had cerebrospinal fluid (CSF) dissemination of the tumours. All patients underwent the whole-neuraxis irradiation and chemotherapy. After the usual treatments, they were submitted to an adoptive transfer of one-haplotype identical LAK cells. The LAK cells were induced from peripheral blood lymphocytes (PBL) of their relatives with human recombinant interleukin-2 (rIL-2). 3 - 15 x 10(9) LAK cells were transferred intrathecally in 2-3 months. In 3 of 6 patients, neurological signs were improved and malignant cells had never been detected on CSF cytology after the adoptive immunotherapy. One among these 3 patients showed complete response in 20 months. Thus, this is an attractive approach for the treatment of medulloblastoma with CSF dissemination of the tumour which current therapeutic intervention can not cure.

Difference of immunohistochemical reactions in epithelial cells of adenolymphoma.

An immunoperoxidase staining technique was used for detecting alpha one-antichymotrypsin (alpha 1-ACT), alpha one-antitrypsin (alpha 1-AT), lactoferrin and transferrin in routine histological paraffin sections of 30 adenolymphomas, as well as in normal salivary gland tissue. Microscopically, the epithelial, component of adenolymphomas consisted of tall columnar luminal cells and irregularly shaped basal cells. alpha 1-ACT was detected in the luminal layer of epithelium in 27 (90%) of 30 adenolymphomas, while the basal layer was positive in 4 cases (13%). Lactoferrin could be observed in the columnar cells of 21 cases (70%) and was positive in the basal cells of 2 cases (7%). In normal salivary gland tissue, alpha 1-ACT and lactoferrin were observed in the intercalated duct and serous acinar cells. The staining pattern of alpha 1-AT in adenolymphoma was similar to those of alpha 1-ACT and lactoferrin, however, the number of positive cases for alpha 1-AT was fewer than in the alpha 1-ACT and lactoferrin. alpha 1-AT was not found in the normal salivary gland. On the contrary, the localization of transferrin in the epithelial component of adenolymphomas was exclusively different from those of alpha 1-ACT, alpha 1-AT and lactoferrin. Transferrin was found more often in the basal cells than in the tall columnar apical cells. The staining pattern of transferrin in the normal salivary gland was different from alpha 1-ACT and lactoferrin, and transferrin was positive in the cytoplasm of intercalated ducts, serous acinar and myoepithelial cells.(ABSTRACT TRUNCATED AT 250 WORDS)

Subretinal precipitates of retinal detachments associated with intraocular tumors.

Yellowish white precipitates are sometimes observed clinically in the subretinal area of the retinal detachments. We investigated retinal detachments observed in a 47-year-old man associated with choroidal malignant melanoma and in a 1-year-old girl with retinoblastoma. Yellowish white precipitates were foam cells located around degenerated outer and inner segments of photoreceptor cells. They were more abundantly present associated with malignant melanoma than with retinoblastoma. They exhibited abundant lipid droplets, abundant lysosomes with phagocytosed materials including inner segments, fibrin, melanin and lipofuscin, abundant glycogen particles, indented nuclei, and cytoplasmic processes without intercellular junctional apparatus. They appeared to be macrophages.

[A clinicopathological study of mediastinal tumors].

One hundred and fifty-eight cases of mediastinal tumors have been studied clinicopathologically, based on patients of Nagasaki University Hospital from 1961 to 1984. Of these tumors, 29.7% were thymic tumors, 23.4% neurogenic tumors, and 21.5% were germ cell tumors, the frequencies of which reflected the same tendency as other such reports in Japan. Thirty-three cases were malignant tumors. Among them, thymomas were the most common (48.5%) as Wada et al. have reported. Extrathoracic metastases have observed in 2 thymomas. Thymomas associated with myasthenia gravis were seen in 6 cases in which 5 patients are still alive.
