Clinical significance of the free vascularized bone grafts in fractures with large bone defects and non-union.

With the rapid progress of microscopic techniques in surgery, the free vascularized bone graft has been widely applied in fractures with large bone defects, non-union, congenital pseudoarthrosis, bone tumors, osteomyelitis, aseptic necrosis of the bone and osteogenesis imperfecta. We obtained excellent results with the free vascularized bone graft in three cases. One was a case of non-union with large bone defects of the right femur that had not improved despite many attempts at treatment over nine years after an open fracture of the right femur. The other two cases were open fractures with massive bone defects of the right legs. These results are reported here with an outline of the free vascularized bone graft.

[The optimum dose of iohexol 399 in excretory urography].

A randomized clinical trial was conducted to determine the optimum does of iohexol (iodine concentration 300 mgI/ml) as a contrast agent for excretory urography in screening outpatients. Cases were divided into two groups, 30 cases in Group A receiving 0.4 ml/kg of iohexol and 29 cases in group B receiving 0.8 ml/kg. Urography was performed at intervals of 5 minutes and 10 minutes following the injection. The diagnostic effectiveness of various X-ray images including nephrograms, the pyelocaliceal system, ureter and bladder were assessed. There was a significant disparity between the two groups during pyelocaliceal urography after 5 minutes, with a fair diagnosis attainable for 73.3% of group A versus 89.6% for group B (P less than 0.05), but after 10 minutes the rates for the two groups had evened out to 93.4% for group A and 96.6% for group B. During urography of the ureter after 5 minutes, only 16.6% of the cases in group A could be easily diagnosed against 51.7% of the cases in group B (P less than 0.05). However, after 10 minutes the rate for group A was up to 56.7% while the rate for group B was 82.8% (P less than 0.1). In the ureter after 5 minutes, non-diagnosable images constituted 26.7% of group A and 10.4% of group B, while after 10 minutes this had dropped to 16.6% for group A and 0% for group B. A comprehensive study of these results suggests that for diagnostic screenings which use iohexol as a contrast agent in excretory urography, optimal results may be obtained using iohexol in amounts of between 0.4 ml/kg and 0.8 ml/kg.

[Treatment of chlamydial urethritis--studies on clinical effects of ofloxacin].

Clinical effects of ofloxacin (OFLX) in the treatment of the patients with chlamydial urethritis was studied. OFLX was administered at a dose of 200 mg, three times daily (600 mg) for 14 days. In all of the 26 patients with chlamydial urethritis, C. trachomatis was eliminated in 7 to 14 days after the start of administration. The subjective clinical symptoms such as pain on urination and abnormal urethral feeling was disappeared in all cases within 7 days after the administration. The objective clinical symptoms, urethral discharge, polymorphonuclear leukocyte (PMNL) in urethral discharge and PMNL in first urine were improved in 94, 68, 91% respectively on 14 days after the administration. Overall clinical efficacy rate of OFLX on 7 days, 14 days and 21 days after the administration in this study was 63, 82, 91% respectively. Whereas subjective symptoms of side effects were noted in 2 patients (7.6%), any additional medical care was needed in none of them. Because of the marked improvement of clinical symptoms and the safety administration, OFLX could be the first regimen to be chosen for the treatment of the patients with chlamydial urethritis.

The hypoglycemic effects of tryptamine in mice: mediation by 5-HT receptors.

The effects of tryptamine on blood glucose levels were studied. Tryptamine induced significant hypoglycemia in mice. The hypoglycemia elicited by tryptamine was strongly antagonized by methysergide, an antagonist of both 5-HT1 and 5-HT2 receptors. A 5-HT2 receptor antagonist, ketanserin, partially inhibited the tryptamine-induced hypoglycemia. These results suggest that tryptamine-induced hypoglycemia is mediated by 5-HT1 and 5-HT2 receptors.

Prevalence of binge-eating and bulimia among adolescent women in Japan.

This study examined by questionnaire the prevalence of binge-eating and bulimia among 220 women at a school of nursing and 236 women at a junior college, all from 18 to 21 years old, in two different areas of Japan. Binge-eating more than once a week was found in 14 (6.5%) of the nursing school students and 21 (9.1%) of the college women. Self-induced vomiting and use of purgatives were found in 19 (8.7%) and 12 (5.5%) of the nursing school students, and 19 (8.1%) and 9 (3.8%) of the college women, respectively. The difference between these two groups was not significant. The prevalence of binge-eating more than once a week, together with self-induced vomiting or purgative use, was 3.6% in the nursing school students, 2.1% in the college women, and 2.9% in the total sample. These women appeared to meet both DSM-III criteria for bulimia and Russell's criteria for bulimia nervosa.

Monoclonal antibodies recognizing different epitopes of the 27-kDa gap junction protein from rat liver.

Monoclonal antibodies (2-3E2, 6-3G11, and 7-3H6) against gap junction plaques purified from rat liver were prepared and characterized. Immunoblot analysis of liver gap junctions revealed that all three antibodies reacted with the 27-kDa protein, but not with the 22-kDa one. The 2-3E2 and 6-3G11 antibodies both reacted with the 27-kDa protein in gap junctions purified from livers of the rat, mouse, rabbit, and guinea pig; the 7-3H6 antibody, however, failed to react with the 27-kDa protein from guinea pig liver. The 7-3H6 antibody reacted strongly with the 24- to 26-kDa degradation products of the 27-kDa protein. Indirect immunofluorescence showed that the 6-3G11 and 7-3H6 antibodies both gave the same specific fluorescence labeling on rat liver cryosections, suggesting that these two antibodies recognized the cytoplasmic sites of the 27-kDa protein. Immunoblot analysis of protease-digested fragments from the 27-kDa protein revealed that the 7-3H6 antibody reacted with the 24- and 17-kDa fragments (including portions of the carboxyl-terminal domain of the 27-kDa protein) produced with endoproteinases Arg-C and Lys-C, respectively. Immunoblot analysis of CNBr fragments of the 27-kDa protein revealed that all three antibodies reacted with the 10-kDa fragment, which is thought to be the carboxyl-terminal domain of the 27-kDa protein. These results demonstrate that three monoclonal antibodies recognize different epitopes of the cytoplasmic sites (probably the carboxyl-terminal domain) of the 27-kDa liver gap junction protein.

Small-scale DNA preparation for rapid genetic identification of Campylobacter species without radioisotope.

A simplified and rapid genetic identification method for Campylobacter species without radioisotope was established. Three different amounts of DNA (200, 50, and 12.5 ng) extracted from each type strain of Campylobacter species with standard Marmur's procedure were spotted on a nitrocellulose filter. DNA obtained from one ml bacterial suspension at a concentration of McFarland standard turbidity No. 1 of Campylobacter fetus, C. jejuni, C. coli, and C. pylori isolates were sufficiently labeled with photo-biotin within 15 min and clearly hybridized with the type strain of the corresponding species within four to six hours. Hybridized spots were visualized with alkaline-phosphatase-conjugated streptavidin color-detection method. The reaction was usually stopped within 30 min. Atypical clinical isolates such as a nitrate-negative C. jejuni, two nalidixic acid-resistant C. jejuni, and two strains of C. fetus able to grow at 42 C, which were tentatively identified as such, were definitely identified by the simplified DNA hybridization method presented here. This method will be applicable routinely for the definite identification of atypical strains of Campylobacter species and other gram-negative bacteria difficult to identify biochemically.

Simple genetic method to identify viridans group streptococci by colorimetric dot hybridization and fluorometric hybridization in microdilution wells.

Simple dot hybridization and fluorometric hybridization methods in microdilution wells were designed and established for rapid and routine genetic identification of viridans group streptococci. Reference DNA extracted from each strain of 24 reference Streptococcus species was fixed both on a nitrocellulose filter and in a microdilution well. A 1-ml portion of the bacterial suspension which matched the turbidity of McFarland no. 2 standard was prepared when a streptococcal strain was isolated. It was lysed with achromopeptidase, and the DNA was quickly labeled with photobiotin under a sunlamp for 15 min. Dot hybridization and fluorometric hybridization were then carried out between the labeled DNA of the unknown organism and 24 unlabeled reference DNAs. Hybridized fragments on a nitrocellulose filter were detected by using alkaline-phosphatase-conjugated streptavidin and analyzed with a color graphic analyzer. Hybridized fragments in microdilution wells were quantitatively detected by using an enzyme, streptavidin-conjugated beta-D-galactosidase, and a fluorogenic substrate, 4-methylumbelliferyl-beta-D-galactoside. Strains belonging to each genetically distinct species could be identified by this dot blot hybridization test. However, some clinical strains cross-hybridized with two or more reference species, and then they were difficult to differentiate by dot blot hybridization. In such a case, fluorometric identification provided reliable results because the fluorometric method was more quantitative than dot blot identification. By these methods, it was possible to determine species assignment within the viridans group.

CT appearance of acute radiation-induced injury in the lung.

To determine how soon radiation-induced lung injury is detectable, to compare the CT findings with those on chest radiographs, and to observe the appearance of the abnormality during the acute phase, we performed 83 CT studies in 17 radiotherapy patients at relatively short intervals. All 17 patients received fractionated radiotherapy to the thorax with a large irradiated lung volume. The CT findings were variable; pulmonary infiltrates were homogeneous, patchy, or discrete. CT abnormalities were evident in 15 of 17 cases within 16 weeks after radiotherapy; in 13 of these it was detected within 4 weeks. In three of these 15 cases, no abnormality was detected on chest radiographs, and in three other cases, the change was observed much later on chest radiographs than on CT scans. In the other nine cases, abnormalities were detected simultaneously on CT scans and chest radiographs. In four cases, extensive radiation pneumonitis was observed on CT, but in two of these, the change was misdiagnosed on the chest radiograph. We conclude that CT is useful in the detection of acute radiation-induced pulmonary disease.

[A twin type heat flow microcalorimeter for radioactivity measurement].

A microcalorimeter is described which was developed and tested for measuring the activity of pure beta particle emitters. Two identically made calorimeter cells are used, each 46.5 mm i.d. and 54.0 mm deep and each fitted with thermomodules generating an emf of about 29 mV degrees C-1. The thermal energy from the cells flows into a surrounding mass of aluminium (15 kg) embedded in 30 mm thick styrofoam. The entire assembly is then immersed into an open thermostat bath controlled by electric heaters an arrangement which facilitates access for sample changing. The outputs of the modules (sensors) are connected in opposing polarities, the net output being amplified and this voltage signal is used to estimate the radioactivity in the samples. In order to demonstrate usefulness of this calorimeter, approximately 3.7 GBq(100 mCi) of tritiated water was measured. The result was compared with that of liquid scintilation counting method and found to be agreed within 5%.
