[Cancer of the intra-pancreatic common bile duct observed 9 years after choledochojejunostomy].

A 54-year-old female was admitted in June 1982, complaining of jaundice and fever of several months' duration. She had undergone cholecystectomy and choledocho-jejunostomy, 22 years and 9 years before, respectively. On admission, PTCD was carried out immediately. It showed marked dilatation, of the common bile duct and stricture of the anastomosed portion. On August 2, she died of jaundice and sepsis. On autopsy, a cystic tumor was found at the common bile duct remnant in the head of the pancreas. It was a tubular adenocarcinoma and invaded the ostium of the choledochojejunostomy. The occurrence of adenocarcinoma may be one of the late complications of choledochojejunostomy.

[A case with Bartter's syndrome associated with type II hyperlipidemia, increased fluidity of plasma VLDL and abnormal configuration of peripheral erythrocytes (author's transl)].

A 34-year-old female complaining of numbness and weakness of the extremities was examined. Consanguineous marriage was contracted between mother and father. She was of short stature (149 cm), and her blood pressure was normal (118/60 mmHg). Her serum potassium concentration had decreased to a level between 2.5 and 3.2 mEq/L, and hypokalemic alkalosis was present. Potassium clearance had increased and urinary concentrating capacity was impaired. Plasma renin activity was high at 25 ng/ml/hr but plasma aldosterone concentration was normal. Hypertensive response to angiotensin II (50 ng/kg/min) was weak but improved to nearly the normal value after the administration of indomethacin for 17 days at a dose of 50 mg/day. A slight elevation in blood pressure was observed during the infusion of norepinephrine (250 ng/kg/min). A decrease in blood pressure was observed during the infusion of 1-sarcosine, 8-isoleucine angiotensin II (600 ng/kg/min) with concomitant increase of plasma renin activity. Twenty-four hour urinary excretion of prostaglandin E decreased somewhat (225 approximately 252 ng/day), and hyperplasia of the juxtaglomerular cells and increased JG index were demonstrated in the biopsy specimens of the right kidney. From the findings, the present case were diagnosed as Bartter's syndrome. Although mild enlargement of the sella turcica was found in skull x-ray films, no abnormalities in pituitary function were demonstrated. Other unusual complications, i.e. hyperlipidemia (type II, beta-dominant) and abnormal configuration of peripheral erythrocytes, were demonstrated. Phospholipid composition of the erythrocyte membrane was normal. The fluidity of plasma VLDL examined by electron spin resonance was increased. Hypokalemia and hyperreninemia were improved through the administration of indomethacin. However, because of headache as an adverse effect, further administration could not be accepted. The patient's complaints were resolved by the rectal application of indomethacin with oral administrations of spironolactone and triamterene. Changes in serum lipid levels did not occur with the above mentioned treatment. alpha-tocopheryl nicotinate lowered the levels of serum lipids and normalized the configuration of peripheral erythrocytes. But increased fluidity of plasma VLDL remained, and phospholipid composition of erythrocyte membrane was also unchanged. The relationship between the rare complications mentioned above and the pathophysiology of Bartter's syndrome is still obscure.

Characterization of a temperature-sensitive defect of enterovirus type 70.

The mechanism of the failure of enterovirus type 70 to replicate at a nonpermissive temperature (39 degrees C) was investigated, and the following results were obtained. (i) Viral RNA synthesis was not observed at 39 degrees C in LLC-MK2 cells, in accordance with our previous findings with primary monkey kidney cells (Miyamura et al., Intervirology 9:206-213, 1978). (ii) Shutoff of host cell macromolecular synthesis by virus infection was as efficient at 39 degrees C as at a permissive temperature (33 degrees C). This inhibitory effect similarly occurred even in the presence of guanidine hydrochloride. (iii) Viral protein synthesis proceeded in vivo at the nonpermissive temperature, and the rate of the protein synthesis was higher than that at the permissive temperature under the conditions in which sufficient viral mRNA had been accumulated. This was also confirmed by analyzing the intracellular proteins synthesized at the nonpermissive temperature by sodium dodecyl sulfate-polyacrylamide gel electrophoresis, which identified them as virus-specific proteins. (iv) When infected cells were incubated at 39 degrees C and then transferred to 33 degrees C, viral RNA synthesis took place even in the presence of cycloheximide. (v) Furthermore, in experiments performed with an in vitro cell-free assay system, viral polymerase activity was found in the membrane-bound preparation extracted from infected cells which had been incubated at 39 degrees C in the presence or absence of guanidine hydrochloride. These results indicate that early translation of mRNA proceeds normally at the nonpermissive temperature and that the temperature-sensitive defect resides in the transcriptional stage.

Dynamics of contrast enhancement in delayed computed tomography of brain tumors: tissue-blood ratio and differential diagnosis.

Thirty-one patients with brain tumors were studied by delayed computed tomographic (CT) scanning performed one and two hours after intravenous administration of contrast medium. Dynamics of contrast enhancement in the lesion were analyzed quantitatively and qualitatively by calculating the tissue-blood ratio (TBR) at each scan, and are expressed as relative TBR (R-TBR). The R-TBRs obtained two hours after the first contrast-enhanced scan were found to be most useful in diagnosis, and were classified into three groups: less than 1.5 (Class I), 1.5 to 3.0 (Class II), and more than 3.0 (Class III). In the glioma group, seven of eight anaplastic gliomas were Class III and all of three anaplastic astrocytomas were Class II. All of seven meningiomas were Class I. Four of five pituitary adenomas were Class II. Three of four neurinomas were Class III. This method is potentially useful in differential diagnosis of some brain tumors.

[Effects of zotepine, a psychotropic agent, on the serum uric acid level of inpatients at a psychiatric hospital].

UNLABELLED: Serum uric acid levels were examined and compared to diagnostic groups of inpatients of a mental hospital. An action lowering serum uric acid level of zotepine was further evaluated in those patients. RESULTS: (1) In patients receiving some kind of psychotropic drugs, except for zotepine, mean serum uric acid level of patients with epilepsy in both sexes was significantly lowered in comparison with that of schizophrenia. Mean serum uric acid levels of male and female patients with alcoholism and organic brain disease were higher than that of schizophrenia without any statistical significance. (2) Mean serum uric acid level of patients treated with various psychotropic drugs, except for zotepine, was higher than that of patients receiving zotepine. (3) Zotepine decreased serum uric acid level parallel with its dosage (r=0.70, p less than 0.001) with a mean reduced rate of 54.1% (p less than 0.001), when serum uric acid levels of 32 patients were examined 3 times over 3 months before and during zotepine administration, respectively. (4) Maximum reduction of serum uric acid level occurred in 3-6 days following zotepine application. Controversely, it takes about 4 weeks to recover baseline level of serum uric acid after stopping zotepine administration. (5) Zotepine had no significant effect not only on the results of creatinine clearance test, but also on BUN level. However, it significantly increased level of uric acid clearance, and rate between uric acid clearance and creatinine clearance. The rate of uric acid clearance vs creatinine clearance correlated with the dosage of zotepine given. Zotepine is thought to decrease serum uric acid level via selective blocking of tubular reabsorption of uric acid in the kidney.

Seroepidemiologic studies of acute hemorrhagic conjunctivitis virus (enterovirus type 70) in West Africa. II. Studies with human sera collected in West African countries other than Ghana.

Human sera were collected in Senegal, Nigeria, Ivory Coast, Dahomey, Liberia, Gabon and Togo during the pre-epidemic period of acute hemorrhagic conjunctivitis (AHC) from 1965 to 1969, and tested for virus neutralizing (VN) antibody to enterovirus type 70 (EV70). Of these, 1109 (91%) were antibody negative (less than equal to 1:4), 116 (9%) neutralized at a dilution of 1:8 or over, and 45 (4%) at dilutions of at least 1:16. The distribution pattern is not significantly different from that of sera collected from Kenya in 1967 or from army recruits in the United States, Argentina, Brazil and Colombia in the 1960s. Sera collected during the post-epidemic period (1970 to 1977) in Senegal, Sierra Leone, Mali, Upper Volta, Chad, Niger and Gabon were also examined; 1573 (68%) were VN antibody negative (less than or equal to 1:4), while 733 (32%) and 433 (19%) had titers of 1:8 or greater and 1:16 or over, respectively. There is a significant difference in distribution between pre- and post-epidemic antibody titers (p less than 0.001), although the incidence of AHC was lower in these countries than in Ghana and Southeast Asia. The prevalence of VN antibodies tends to be lower in the dry, hot inland areas and thus humid coastal monsoonal climates and dense populations seem to favor the spread of AHC.

[Hormonal pituitary functions of women with increasing age (author's transl)].

Pituitary functioning was observed endocrinologically in women in mature, climacteric and senile periods, and also patients after ovariectomy of both sides. Serum FSH and LH levels, especially in the former, showed a tendency towards increasing from the climacterium (45-49 years) up to the age of 60-64, and thereafter remained constant in a defined level. Serum PRL, ACTH and GH levels had a tendency to decrease after menopause. Serum TSH levels decreased from the age of 60-64 years. The change of responses for LH-RH tests with advancing age was found to be similar to that of serum gonadotropin levels. On the other hand, in the changes of response of PRL and TSH to TRH test with increasing age, there was a reverse tendency to that of gonadotropin to LH-RH. In ovariectomized women who still had a menstrual cycle, there was a great increase in serum FSH and LH after the operation, as against those who had spent 3 years and 10 months, or 4 years and 4 months after menopause, a slight or little increase respectively. Meanwhile, the LH response to LH-RH was higher in the premenopausal ovariectomized women than that of the post-menopausal unovariectomized women.

Presence of Forssman-like and Hanganutziu Deicher-like antibodies in Japanese sera.

Absorption studies were performed to analyse the properties of heterophile antibodies in Japanese sera. The conventional absorption test using ox erythrocytes and guinea pig kidney tissues classified 28 of 3570 pathologic sera and 12 of 19 IM-like syndrome sera into 2 categories of Forssman type and Hanganutziu Deicher type. No sera contained the antibody of Paul-Bunnell type. Absorptions of the representative sera with cells of various species disclosed the presence of Hanganutziu Deicher-like and Forssman-like antibodies in Japanese IM-like syndrome sera. The activity of both antibodies were removed partly or non-significantly with rat cells. The antibody properties in serum sickness serum between Japanese and a Caucasian were compared. Cells of ox, rabbit, guinea peg and mouse removed the antibody in Japanese sera, however, rabbit cells did not absorb it in a Caucasian case. It could be proposed that various cells other than ox and guinea pig should be employed in the absorption study of the heterophile antibodies.
