We report three cases of propranolol overdose in two patients, both of whom suffered cardiovascular collapse after receiving an emetic compound in a hospital's casualty department. Such a collapse may be caused by vagal activity in the presence of sympathetic beta-blockade. We recommend the administration of atropine, which may increase vagal tone, before a patient receives an emetic compound, or undergoes gastric lavage or endotracheal intubation.
The ability of a new monoclonal antibody against thymidine (MoAb 20B7) to detect chemotherapy induced single stranded DNA damage is described. HL-60 cells were used for these experiments. Damage to DNA was caused by incubation of cells with alkylating agents. Portions of DNA which is damaged are cleaved by cellular endonucleases, thus exposing thymidine on the opposite DNA strand. Processing of these samples by MoAb 20B7 showed that such damaged segments could be detected consistently. Furthermore, this method was combined with autoradiographic detection of unscheduled DNA synthesis thereby allowing for assessment of DNA repair simultaneously from the same cell.
Percutaneous tracheostomy has become a commonly used technique in the management of the critically ill. It is a relatively simple procedure to perform at the bedside but it does have, as with any practical procedure, potential pitfalls and complications. Two commonly used methods are described.
