[Analysis of factors related to early mortality in digestive hemorrhage caused by portal hypertension].

In order to determine immediate criteria of prognosis for patients with portal hypertension hospitalized for digestive hemorrhage, in an intensive care unit, 18 variables were recorded during the 24 hours following admission in 65 patients. Data related to death were age, ascites, hepatic encephalopathy, shock, active hemorrhage, acute pneumonia, decrease in prothrombin time, use of esophageal balloon tamponade, use of mechanical ventilation, number of red blood cell units transfused. Discriminant analysis yielded a linear combination of 4 variables which best separated survivors from non survivors with the following equation: F = 0.330 X hepatic encephalopathy + 0.433 X shock + 0.226 X active hemorrhage + 0.0097 X age - 0.396. The threshold decision of the hemorrhage prognosis index (HPI) was F = 0.57; 80 p 100 of all patients were correctly classified. In order to be validated, HPI was compared with a general (SAPS) and specific (Pugh's classification) scoring system, in a prospective study of 57 episodes of digestive hemorrhage. In this study, sensitivity was better with HPI than with SAPS (0.70 versus 0.45), specificity was higher with HPI than with Pugh's classification (0.86 versus 0.70). Percentage of correctly classified patients was higher using HPI (81 p. 100) than SAPS (77 p. 100) and Pugh's classification (68 p. 100). We suggest that the HPI, determined with 4 easily defined and recorded variables should be used prospectively to compare efficacy of different treatments.

[Sclerodermiform porphyria].

We give a retrospective survey on the clinical, histological, biochemical, and pathogenetical aspects of sclerodermiform changes rarely accompanying porphyria cutanea tarda (PCT). Sclerodermiform changes were seen in 12 patients (2% of all our PCT cases). In these patients, no correlation was found between the severity of the dermatological signs and symptoms and the degree of disturbance in the porphyrin metabolism. Biochemical remission was not accompanied by improvement of the sclerodermiform changes. The proportion of porphyrins with 4 or 5 COOH-groups was higher than that of PCT patients without sclerosis. The findings are consistent with the view that the development of sclerodermiform changes cannot be merely explained by phototoxic reactions, but the "dark-effect" of the porphyrins may also play an important role in the pathogenesis.

[Lupus vulgaris (tuberculosis cutis luposa). Clinical aspects and therapy past and present].

In the developed industrial countries, the frequency of tuberculotic diseases of the skin including lupus has decreased to a very low level because of the significant decline of morbidity and mortality, the elimination of Bovine infections, the introduction of BCG vaccination, as well as effective anti-tuberculotic drugs employed for about 30 years. In spite of the very favorable therapeutic results, we have to reckon with the continuance of the present lupus incidence as long as the number of tuberculotic affections in the world is about 20 million, and even in developed industrial countries, the yearly rate of tuberculotic patients amounts to 43 in 100,000 inhabitants. We know that the disease still occurs very frequently in the developing countries of the Far East.

The functions of polymorphonuclear leukocytes emigrating into the skin.

The functions of polymorphonuclear leukocytes (PMNs) emigrating into the skin and of others separated simultaneously from the circulation were determined and compared in healthy individuals. The PMNs emigrating into the skin were separated with a skin chamber technique. The C3 rosette forming capacities of the skin migrating and the circulating PMNs were similar. The chemotactic responsiveness was evaluated using three different chemoattractants: zymosan-activated serum, casein and lymphocyte-derived chemotactic factor. The skin PMNs gave practically no chemotactic response in the Boyden chamber, irrespective of the chemoattractant used. On the other hand, the skin PMNs exhibited higher plastic surface adherence, nitroblue tetrazolium reduction and Candida albicans killing activities. The data indicate that some functional activities of PMNs undergo alteration during in vivo emigration into the skin. The alterations, overall, may be of importance in the physiological protective function of the skin.

The effect of mouse erythrocyte rosette forming lymphocytes on lymphokine production in T-cell cultures.

An enhancement effect of mouse erythrocyte rosette forming (MERF) cells on the production of migration inhibitory factor, chemotactic factor for neutrophils and skin reactive factor in T-lymphocyte cultures stimulated with the purified protein derivative of tuberculin was observed. We consider it likely that the MERF cells, possessing the appropriate cell surface constituents to construct an immunogenic moiety, present antigen on their surfaces to elicit lymphokine production.

[Nitroblue tetrazolium reduction activity of circulating and skin-migrating leukocytes in psoriasis].

The nitroblue tetrazolium (NBT) reductive activity of polymorphonuclear leucocytes separated from the circulation as well as that of leucocytes migrating into the skin collected by skin chamber technique was studied with a quantitative method in patients with psoriasis vulgaris and in healthy persons. The NBT reductive activity of the leucocytes migrated into the skin was significantly higher in the healthy persons and slightly but not significantly elevated in psoriasis when compared with the cells separated from the blood. The dye reduction by the circulating cells was enhanced in psoriasis. On the other hand, in psoriasis the NBT reduction by the migrating cells did not differ from that of the leucocytes in healthy persons. The most likely explanation for the lack of the enhancement of activation during migration into the skin in psoriasis is that the polymorphonuclear leucocytes are already present in the circulation in hyperactive state.

[Premature aging syndromes with special reference to lipodystrophy (a case report)].

The authors survey the modern classification of progeria and report on a 12-year old girl suffering from lipodystrophia totalis and diabetes mellitus (Lawrence-Seip-syndrome). The girl developed sclerodermiform changes at the age of one year, at her 10th year acanthosis nigricans, xanthoma tuberosum Fredrickson hyperlipoproteinaemie IV, later II/B type and finally at an age of 11 1/2 year diabetes mellitus.
