The authors studied salivary and serum testosterone (T) levels in basal conditions and at 6 and 12 months after orchidopexy in 36 pubertal boys with unilateral or bilateral cryptorchidism. Boys were divided into two groups according to Tanner classification. Salivary and serum T levels were significantly lower in the group with scanty or absent sexual development than those obtained in the group with normal development. Both groups exhibited similar saliva and serum testosterone values at 12 months of the follow-up study. The data shown indicate that salivary T seems a good index for evaluating the androgenic status of these patients.

Fifty-seven patients with T1 transitional cell carcinoma of the bladder, 57 patients with T2-3 and 10 patients with T4 were treated by radical irradiation +/- salvage cystectomy. Thirty-five patients with T1 were treated by transurethral resection (TUR) plus local chemotherapy +/- salvage cystectomy. Eleven patients with T2-3 were treated with 5 X 4 Gy in one week followed by cystectomy while four patients with T2-3 were treated with 40 Gy in 4 weeks also followed by cystectomy. Five patients with T4 were treated with 40 Gy in 4 weeks plus cystectomy. Seven year survival rates are: 69% for T1 treated by radical irradiation, 43% for T1 treated by TUR (p less than 0.02), 36% for T2-3 treated either by radical irradiation or 5 X 4 Gy plus cystectomy, and 18% for T4 treated by radical irradiation. All the survival rates indicated are free of disease and include the cases in which salvage cystectomy was successfully performed. Sixty per cent of the cured patients conserved very good functional bladders. We believe that external beam irradiation plus salvage cystectomy is indicated in the treatment of T1 and T2-3 tumours of the bladder.
We report 5 cases of renal allograft rupture in which diagnosis was established early through clinical data, laboratory tests and echography. Immediate surgery confirmed the suspicion in all cases. Acute rejection was present in 4 patients and in 1 the previous surgical puncture from the perirenal collection demonstrated serohematic fluid with a biological character similar to that of lymph. This latter case seemed to confirm the suspicion that any process associated with edema in a kidney with obstructed lymphatic tracts (meticulous lymphatic ligatures during donor extraction) is capable of causing a rupture. Conventional surgical treatment is dangerous and insecure on an edematous and friable kidney, resulting in a nephrectomy rate of 55.7 per cent and a postoperative death rate of 8 per cent. Hematoma evacuation, hemostasis by local compression and tridimensional containment of the ruptured areas should be the principles of this operation. By means of renal corsetage with lyophilized human dura these principles can be achieved. This surgical technique, which is simple and secure, its variations and future possibilities are described. In 3 of our 5 patients corsetage with lyophilized human dura was applied. All 5 grafts have taken. Renal function is good in 3 cases and acceptable in 2 at followup between 2 and 15 months.
Severe hypertension and a decrease of renal function, with or without oliguria, suggest renal artery stenosis in the transplanted kidney. 6 renal artery stenoses were observed in 100 transplanted kidneys followed up for more than 3 months. In 4 patients, percutaneous transluminal angioplasty was performed. 1 patient required a new percutaneous transluminal angioplasty 3 months later and a 2nd patient was submitted to surgery after 14 months. Surgery was performed in 2 more cases, with failure in 1. It seems that the endoarteric lesion during cold perfusion could be the main etiopathological factor, when associated with rejection episodes. Percutaneous transluminal angioplasty is the treatment of choice in the management of renal artery stenosis in transplanted kidneys. Surgery must be reserved when it fails.
A histological study, by means of bilateral biopsy, on 44 cryptorchid patients (26 of whom are monolateral and 18 bilateral), comparing them with the response obtained for the HCG gonadal stimulation test, is presented. A direct, although not always exact, relation is established between the testicular histology and the testosterone levels after the administration of 4,500 U of HCG.
Here are the results of bilateral biopsies on 52 cases of unilateral cryptorchidia. In 12 patients the scrotal testicle showed equal lesions or more important lesions in the undescended testicle. It points out the importance of systematizing the histological testicular study, keeping in mind: (1) spermatogenic index; (2) variation in spermatogonias; (3) tubular diameter, and (4) interstitial tissue. Stress is made on the innocuousness of the bilateral testicular biopsy and its indispensability since it is the only way to recognize a dysgenetic cryptorchidia and evaluate the future fertility of the patient.
A study is made of 158 pregnant patients, between 3 and 9 months pregnant, in order to determine, by means of urine culture and ACB, the index of subclinical infections during pregnancy involving the renal parenchyma. The conclusion is reached that these infections occur in 4.42% of cases and out of those cases 57.14% were later shown to have an organic pathology of the excretory tracts.
The urinary sediment of 56 patients with active urinary infection were studied through a direct immunofluorescence technique in order to detect the presence of antibody-coated bacteria. In 35-36 pyelonephritis cases the results were positive. However, in 11 cases with lower urinary infection there was no sign of antibody coasted bacteria. A pathological intravenous pyelogram (IVP) was obtained from 33 pyelonephritic cases. In the flurorescent antibody (FA) test was positive, one was negative. In the remaining three pyelonephritic cases the IVP was normal the the FA test positive. In these three cases, cystography revealed the presence of ureterovesical backflow. A good statistical correlation was obtained by comparing the positive results of the test with the titer of serum antibody against the bacteria causing the infection. We have introduced some variation to the original method which permits simpler administration of this test to ambulatory patients and increases the diagnostic possibility of those cases of bacteriuria less than 100,000 bacteria/ml.

We have introduced some variations to the original method which permits simpler administration of this test to ambulatory patients and increases the diagnostic possibility of those cases of bacteriuria less than 100,000 bacteria/ml.
In this study, 32 cases of cancer of the genito-urinary tract are discussed from the viewpoint of immunology. In eight cases treated surgically, there has been no evidence of recurrence over a period of 1-4 years. Those with a good immunological response have a satisfactory course. Patients with a good response have been treated by radical surgery depending on the stage of the tumour, whilst those with a poor response have been treated less radically by reduction of the tumour mass in the hope that a better response may develop. The authors of this report feel that the 'inhibition of the lymphocyte migration test' is a very important factor to evaluate in the study and immunological evolution of the patient. Also, and concerning cases with good immunological response, the poor results after surgery makes us consider the importance and value of blocking factors of the serum closely related to the B type lymphocytes.
Investigation in a patient aged 46 years with decompensated heart failure and severe renal insufficiency demonstrated a small, poorly functioning right kidney and severe stenosis of the left renal artery. Cardiac decompensation was corrected and the left kidney revascularised by autotransplantation. Renal function recovered considerably (FG 75/min) and the severe hypertension was reduced. In hypertension patients by main renal artery stenosis, renal autotransplantation is recommended, since it is a safe method without technical difficulty and has given good results. The mutual dependence of hypertension and renal insufficiency is reviewed. When renal function is poor, revascularisation of the stenosed kidney will lead to recovery. The hypertension will usually improve but will always become more responsive to drug therapy.
