[Free dermatoplasty in traumatic amputations of the fingers].

Twenty-two patients with 27 traumatic amputations of the fingers comprise the study group. The fingers of the right hand were affected in 19 cases and of the left hand in 8. The traumatic injuries of the right hand affected mostly the II. and the III. finger, whereas the left hand injuries affected almost with equal frequency the II. to the V. finger. Primary free skin plasty was performed after sparing surgical processing without shortening of the bone. In 3 cases this was done late after the inflammatory changes in the conquassated tissues had disappeared. Primary cure after postoperative hospital stay from 7 to 15 days was observed. Early rehabilitation was attempted. The results, both cosmetic and functional were satisfactory. Fine sensitivity was impaired during the early period after the operation, but was improved after the sixth month. Of the 11 tested patients who reported for control 6 months to 5 years after the operation, 10 were able to distinguish two points of pressure at 6 mm distance from one another and could fulfil their professional work without difficulties. Taking into account that of all cases of traumatic injuries to the hand only 4 per cent report to specialized hospital departments, the author considers free skin plasty a method of choice for the general surgical units over all other plastic methods of non-free skin plasty which are also presented and illustrated in the paper.

[The surgical treatment of hemorrhoids. Their suturing ligation without excision].

Analysis is made of the results of application of a modified method offered by the author "suturing ligature without excision" in hemorrhoids. It consists of suturing ligature of the vascular bundle, including ligation of the hemorrhoid node itself without excision. For the period 1981-1988 the method was applied on 32 patients. The results were compared with those in a control group of 105 patients operated by the method of Milligan, Morgan. The mean postoperative hospital treatment lasted 3.2 days, vs. 11.4 days in the control group. It was moreover characterized by minimal pain, good state of well-being and hygiene, normal intestinal passage from the first day with flatulence and defecation. Pain lasting more than 24 hours had only 2 patients, vs. 97 in the control group. There was no constrained retention of bowel movement, which in the control group led to difficult flatulence in 101 patients and exacerbation of colitic complaints. The method is pathogenetically substantiated. It provides ligation of the terminal branchings of a. rectalis sup. averts relapses and postoperative bleeding, preserves a sufficient mucosal surface to ensure the physiologic act of flatulence.

[Surgical treatment of stress ulcer].

Results are reported of the operative treatment of stress ulcers by a refined method of selective proximal vagotomy (SPV), selective vagotomy (SV) with distal resection (DR) completely preserving the blood supply to the stomach along the minor curvature. For the period 1981-1988 a total of 82 patients with severe hemorrhage from stress ulcers have been treated. Thirty two of them were operated. In 2 patients SPV was performed by a modification suggested by the author and in another 6--SV also by modified technique with antrectomy B I. All patients recovered, none of them died. The results were compared with those in classical resection (17 operations, 1 reoperation, 12 repeated hemorrhagic episodes, 14 lethal cases) and with ulcer suturing (7 operations, 2 reoperations, 2 repeated hemorrhagic episodes, 7 lethal cases). The methods of SPV and SV with DR are recommended by the author as methods of choice, as they are pathogenetically grounded: satisfactory anacidity is achieved and the perfusion capacity of the stomach is preserved.
