Desorption chemical ionization mass spectrometry of anthracyclines and of trisaccharides related to aclacinomycin A and marcellomycin.

Desorption chemical ionization mass spectrometry with ammonia as the reagent gas was used to obtain positive ion spectra of twelve alkyl-anthraquinones, seven anthracyclinones, eleven anthracycline antibiotics and nine oligosaccharides (di- and trisaccharides). Ions corresponding to the intact molecules (M) were present in all spectra as pseudo-molecular ions [M + H]+ and/or [M + NH]+. However, the most significant results were obtained in the case of anthracyclines and oligosaccharides. With anthracyclines, in addition to the preceding ions, spectra exhibited characteristic fragment ions due to the aglycon or to the sugar moiety while ions corresponding to the sequential loss of monosaccharide units were observed with oligosaccharides. All these fragment ions can be readily explained by an initial site of protonation located at the oxygen atom of the glycosidic linkages.

[Post-traumatic arthritis of the wrist caused by an intra-articular foreign body].

The authors report the case of a 12 year-old child presenting with post-traumatic arthritis of the wrist with intra-articular metallic foreign bodies. The trauma was initially considered to be benign, not operated on and forgotten by the child and its family. Excision of a synovial granuloma was followed by a favorable outcome.

Use of chemical ionization in gas chromatographic/mass spectrometric screening of human urine for disaccharides containing inositol.

Gas chromatography/mass spectrometry (GC/MS) in the positive chemical ionization (CI) mode was used to screen normal urine for inositol-containing disaccharides in the form of permethylated derivatives, after borodeuteride reduction of the reducing saccharides. Ammonia was the reactant gas. The results revealed the existence of deoxyhexosyl-inositol and hexosyl-inositol disaccharides, and of a new compound, N-acetylhexosaminyl-inositol disaccharide. Up to four isomers of deoxyhexosyl-inositol could be present in the same sample even though only one of them has so far been fully characterized in man. As regards the hexosyl-inositols, one to three isomers were present in the same sample and probably corresponded to the three isomers of galactosyl-inositol recently described in man. N-Acetylhexosaminyl-inositol (identified elsewhere by us as N-acetylgalactosaminyl-alpha (1-1)-myo-inositol) was seen in only a few samples. No relationship can be found between the excretion of all these inositol-containing disaccharides on the one hand, ABO(H) blood group and 'Secretor' status (Se or sese) of the donors on the other. The gas chromatographic CI mass spectrometric technique used here with various ammonia pressures can be applied to the screening of other biological fluids or tissues for inositol glycosides.

Isolation and characterization of 1-O-alpha-2-acetamido-2-deoxy-D-galactopyranosyl-myo-inositol from pregnancy urine.

A new neutral glycoside of myo-inositol was isolated from the pregnancy urine of a single donor. Its structure was investigated by 1H-NMR spectroscopy and mass spectroscopy. It was identified as 1-O-alpha-2-acetamido-2-deoxy-D-galactopyranosyl-myo-inositol. No such structure or sequence has previously been reported in either myo-inositol or glucose glycosides.

[Delayed cortical hyperostosis in children].

A case of recurrent infantile cortical hyperostosis in a 3 1/2 year-old girl is reported. Nine other late cases have been previously described. When compared with Caffey's disease, this delayed form of the illness constitutes a distinct homogeneous syndrome of unknown etiology.

Isolation and identification of major metabolites of tixocortol pivalate in human urine.

The metabolism of tixocortol pivalate (PIVALONE), an anti-inflammatory steroid without systemic glucocorticoid effects, has been investigated in man. The analysis was conducted using urine samples collected from two volunteers who had received a 2-g oral dose of 14C-tixocortol pivalate as an oral suspension. Metabolites were purified and isolated from urine by normal phase HPLC, and structural identification was achieved by desorption chemical ionization/NH3 and electron impact/direct line introduction mass spectrometry. Unchanged tixocortol pivalate was not detected in urine; all metabolites were sulfo- and glucurono-conjugates. Metabolites were identified in the neutral steroid fraction obtained after hydrolysis of conjugates. Metabolic transformations in common with cortisol were reduction of the 3-keto, delta 4 system, reduction of the C-20 carbonyl group, oxidation of the C-11 alcohol, and cleavage of the side chain at C-17. Specific metabolic pathways involving the C-21 thiol ester function were transformations into methylthio, methylsulfinyl and methylsulfonyl derivatives, and a reductive cleavage of the C-21-S bond leading to 21-methyl structures. Since none of these metabolites had binding affinity for glucocorticoid receptors in vitro, fast and extensive transformation of tixocortol pivalate into inactive metabolites provides an explanation for the large dissociation between the topical and systemic activities of this drug.

[X-ray computed tomographic aspects of lumbar articular synovial cysts with intraspinal development].

CT scan imaging findings are described in 22 patients with lumbar spine joint synovial cysts, of intraspinal development, provoking sciatica or lumbosciatica from nerve compression in spinal canal. Diagnosis was suggested by a mass at the posterior joint level, of variable density, sometimes with peripheral calcification, presenting a vacuum appearance on occasions, and with enhanced image with contrast. Differential diagnosis is from excluded hernia and postoperative fibrosis. Posterior intraarticular arthrography can confirm diagnosis and allow treatment with prolonged action corticoid infiltrations.

[Posterior lumbar vertebral arthrography. Normal anatomic study].

Historical aspects and anatomophysiologic features of posterior lumbar vertebral joints are summarized and results of arthrography of 55 joints presented, together with a simple, original method for performing this examination. Normal anatomic appearances condition understanding of principal lesions encountered, particularly with respect to compression of adjacent nerve structures.

[Posterior lumbar vertebral arthrography. Pathologic aspects].

Findings on arthrography of posterior lumbar spine joints in 55 patients were compared with those reported in the literature. Two major degenerative lesions of these joints, diverticula and cysts, can provoke back pain and symptomatic nerve compression. Whereas diagnosis of a cyst is dependent mainly on computed tomography imaging, that of a diverticulum is ensured by arthrography, an investigation which also has a therapeutic aim since it allows infiltrations to be performed.

[Preoperative embolization in spinal pathology. Apropos of 21 cases].

Problems encountered during pre-operative embolization in 21 patients with spinal affections could be differentiated as a function of the 3 main regions affected: cervical, dorsolumbar, and lumbosacral. The distinction between the totally different problems raised was based on histologic and angiographic studies of the vascularization of the spinal cord and vertebral column. Whenever possible, and particularly when it can be complete, embolization markedly facilitates the surgical procedure.
