[Possibilities for correcting the neck profile in pterygium colli].

Pterygium colli is characterized by bilateral webbing of the neck extending from the mastoid to the acromion. There is a broad variation in aetiology, very often chromosomal and congenital diseases are included, and most of the known cases present in association with Turner's syndrome. In one case we applied a posterior approach for correction of the webbed neck defect, which involved wide anterolateral skin undermining and skin resections from the nape of the neck. The redundant tissue and underlying defect are corrected and the resultant scar is hidden in the scalp. In two other cases a Z-plasty procedure was used to reduce the hypertrophic tissue, leaving the hairline in the original course. On the long-term follow-up all cases presented with improved aesthetic results, but showed hypertrophic scars.

[Analysis of the facial profile in art and medicine].

The different schools of proportion theory throughout the various epochs of art history are reviewed. Our basic knowledge of proportion theory is attributed, to Vitruvius. Also to be mentioned are Leonardo da Vinci's and Dürer's works, which played a prominent role in the development of proportion theory from the Middle Ages until the late Renaissance. Measurement of the face in medicine, which adopts anthropological nomenclature, is comprised of angular measurements using circle sectors and planar measurements. Today, the medial section, first introduced in 1944 as a concept in facial surgery, is a guideline in the planning and assessment of a face. It should be said, however, that these methods of measurement should only be used as guidelines in conjunction with consideration of the particular circumstances, the surgical possibilities, and retention of the patient's individuality.

[Gunshot injuries of the mandible].

43 patients with gunshot wounds of the mandible were treated in Tübingen and Basel between 1968 and 1995. Among them were 31 patients ( = 72.09%) with bone loss and defect fractures. Over that time period the treatment principles of gunshot wounds have not changed. They are based on an early soft tissue closure and an approach from the inside to the outside. Besides that modern techniques in maxillofacial reconstructive surgery, which include the application of biocompatible plates and screws and the introduction of microvascular reconstructive techniques, have enlarged the therapeutical options. Every patient needs an individual approach to his special medical condition, which is illustrated in three exemplary cases. The combination of the above mentioned modern reconstructive techniques prevents long and multiple hospital stays and invalidity. Furthermore it is the basis for social reintegration as well as functional and esthetic rehabilitation.

A new implant site in iliac crest bone graft for retention of orbital epistheses: a preliminary report.

Osseointegrated percutaneous implants were placed at different sites of the facial skeleton to improve the retention of facial prostheses. Based on four cases a new implant position in autogenous bone grafts for retention of orbital restorations is described. As a first step an autogenous iliac bone graft was transplanted into the orbit. After a minimum interval of 3 months endosseous implants were placed into the grafted bone in axial direction in the middle of the orbit. A third operation several months later uncovered the implants. Using single free-standing rare earth magnetic attachments the orbital episthesis or the eye prosthesis was reliably stabilized. In all cases the bone grafts were integrated at the host site. Nine implants with a follow-up period of 4-66 months have been placed of which one was lost. The patients experienced subjective and objective improvement. Indications and advantages of the method are discussed.

Perioperative antibiotic prophylaxis with cefuroxime in oral-maxillofacial surgical procedures.

A study was carried out to investigate the suitability of cefuroxime for perioperative antibiotic prophylaxis in maxillofacial surgical procedures. Serum and tissue samples were taken, to determine the intraoperative cefuroxime concentration, from 40 patients who had been given 1.5 g cefuroxime (Zinacef) i.v. during maxillofacial surgery. The time between i.v. administration and taking the blood and tissue samples varied between 10 and 260 min. The serum tissue kinetics were determined using an HPLC method in the jaw area, particular emphasis being given to the concentrations measured in bone. It was demonstrated that concentrations of cefuroxime were reached which are above the MIC values for many of the pathogens in the maxillofacial area and therefore adequate protection during intraoperative bacterial contamination is guaranteed. Maximum serum levels averaging 80 mg/l were measured within 30 min of administration. An average of 15 mg/l could still be demonstrated after 4 h. The bone samples gave maximum levels of 8-9 mg/kg 90 min after administration. Average levels of 1-3 mg/kg were still measurable after 4 h. It is only possible to describe trends due to the wide variation in the values, particularly of the bone samples. No postoperative wound infection was seen under prophylaxis with cefuroxime. Cefuroxime is suitable for perioperative prophylaxis during maxillofacial surgery procedures because of its favourable kinetics and broad spectrum of action.

Vasoconstrictors during pregnancy--in vitro trial on pregnant and nonpregnant mouse uterus.

In an in vitro trial on 80 pregnant and nonpregnant mice, the sensitivity of the uterus myometrium to the vasoconstrictors vasopressin, ornipressin, epinephrine, and norepinephrine was examined in comparison with oxytocin as a standardized stimulative drug. The pregnant uterus showed a significantly increased sensitivity to ornipressin, vasopressin, and norepinephrine. Epinephrine showed no uterus-stimulating effect, and an increase of sensitivity caused by pregnancy was not detected.

[Bone transplantation in the orbits as a prerequisite for prosthetic management].

We report on a method of fixing orbital prostheses and artificial eyes: First the orbit is filled by cortico-spongious bone graft. Afterwards two implants are centrally located in the graft, so that the prosthesis can be kept in place by magnetic retention. The advantage of this procedure is that the implants are more stable than those located in the orbital rim.

Fibrin sealing and histometrical changes in conventionally sutured microvascular anastomoses.

Reports from the literature demonstrated an early occlusive thrombosis rate of microvascular anastomoses of up to 25%. In order to reduce free flap failure due to kinking and pressure in the region of the anastomosis, fibrin sealing of microvascular anastomosis was recommended in previous studies. However, it is well accepted that haemostasis is activated by vascular wall injuries (lesions of the endothelial layer). For that reason, a fast re-endothelialisation of the inner surface of the anastomosis is thought to reduce early occlusive thrombosis and, subsequent, free flap failure. To clarify whether application of fibrin adhesives exert any effect on microvascular anastomoses, we constructed 84 anastomoses in rat arteries applying sealant or non-sealant in randomized order. At certain time intervals, arteries operated on were removed and histologically analyzed. Early complete endothelial regeneration, 4 days after surgery, was observed in the unsealed anastomosis group, whereas sealed anastomosis showed a complete re-endothelialisation only after 7 days. These observations may be explained by a reduced multiplication rate and migration speed (0.3 mm/day) of endothelial cells during the first 3 days (unsealed anastomosis: 0.63 mm/day). In addition, in sealed anastomoses a higher incidence of media necrosis was found (60.7% vs 49.3% in the unsealed group). These histological changes were confirmed by scanning and transmission electron microscopy.

Malignant lymphoma of the mucosa-associated lymphoid tissue (MALT)--consecutive unusual manifestation in the rectum and gingiva.

Non-Hodgkin's lymphomas (NHL) of the mucosa-associated lymphoid tissue (MALT) are characterized by a preferential mucosal manifestation. Each organ system may be involved. Exclusively local growth is usually treated with surgical tumour reduction, combined with radiotherapy. In cases of tumour dissemination, chemotherapy is warranted. Follow-up should be performed closely. This case report highlights an unexpected recurrence of NHL in the oral MALT, four years after primary manifestation in the rectum.

[Oral leukoplakias--a histopathological and retrospective study].

In a retrospective study 85 patients of our department with clinical oral leukoplakia occurring between 1980 and 1985 were checked histologically and reclassified. Compared to other studies and unusual high malignant transformation rate (14.1%) was found which seemed to be depended on the high rate of radiated patients. Four of twelve patients were healed due to early diagnosis and radical surgery performed consecutively. Using these data as a basis the different courses and therapies are demonstrated.

[The melanotic neuroectodermal tumor of childhood. Tumor identification with immunohistochemical methods. A case report].

The authors report a case of a melanotic neuroectodermal tumor of infancy occurring in the maxilla of a 6-month-old female infant. The tumor displayed a biphasic cellular pattern with small neuroblast-like cells and large melanocytic cells showing a highly characteristic immunophenotype. Both tumor cell types immunostained for neuron specific enolase (NSE) and vimentin. The melanocytoid cells reacted additionally with the anti-keratin antibody Kl 1 and with HMB 45, an antibody specific for melanocytic antigens. The melanotic neuroectodermal tumor of infancy usually runs a benign course, however, local recurrence and metastasis may occur. The course of the disease cannot be predicted from morphological findings.
