[Radiotherapy of esophageal carcinoma. Results checked clinically by radiology, endoscopy, biopsy and autopsy].

Forty-four patients with histologically confirmed esophageal cancer were irradiated with 60 Gy, checked endoscopically and by biopsy, and then followed clinically or radiologically until their death. The findings confirm the local destructibility of esophageal cancer with loosely ionized radiation and a favorable effect on quality of life. Three quarters of the patients were able to take normal food at the end of the radiotherapy, while the remainder were at least able to swallow soft foods. Radiotherapy can be offered as an effective alternative to patients, especially when operation is associated with a high risk. This is also demonstrated by the fact that 23 of 32 patients who had endoscopy and biopsy again at the end of the radiotherapy had no histologically demonstrable tumor.

Adenocarcinoma↗

[Local recurrences of carcinoma of the breast (author's transl)].

Distant metastases were sought for in 90 patients with local recurrence of breast carcinoma. Only 11 patients had demonstrable spread of the disease at the time of referral. Two-thirds of all local recurrences presented within two years of the primary operation. In 66 out of 90 patients the recurrence was in the chest wall, in 11 cases it was in the chest wall and in the lymphatic system and in only 13 cases was it primarily in the regional lymph nodes. The recurrence rate depended mainly on the previous treatment of the primary tumour: 42 recurrences were found in those cases treated only by surgery, 46 in cases treated surgically and with low-dose post-operative irradiation and only two in patients treated with over 4500 rad to the area of subsequent recurrence.

Breast Neoplasms↗

[Radiotherapy of esophageal neoplasms. Review of the 1st post-therapeutic year].

The first post-therapeutic year in 33 patients with esophageal cancer treated by radiotherapy alone is analyzed. Effectiveness of treatment was measured by clinical symptoms, radiological findings, and autopsy results. Out of a group of 33 patients, 36.4% showed no subjective or objective symptoms 1 year after commencement of therapy. Based on our own experience and the literature, the concept of therapy in esophageal cancer is discussed.

Adrenal Gland Neoplasms↗

[Radiological aspects of esophageal intramural diverticulosis].

The 9th observation of esophageal intramural diverticulosis is reported. The findings are compared with those of former publications. Clinically dysphagia is a leading symptom. Suction biopsy proved diverticulosis being the correct interpretation for radiological findings. The etiology is discussed. The change between symptom-free intervals with diverticulosis only and episods of dysphagia by secondary inflammation (reflux esophagitis, moniliasis, stenosis) stresses the importance of such secondary complications. These inflammations can hide the real diagnosis for a long time. The radiological findings are the key for diagnosis, however, they can be very small for years. During this period a diagnosis can only be achieved by knowledge of the disease and the skilful search for early symptoms.

[Radiological features of the rectal manifestations of myelofibrosis(author's transl)].

In addition to the well known changes in bone structure, myelofibrosis can result in extramedullary tumour formation; in the gastr-intestinal tract, proliferation of reticulum can lead to fibrotic changes. This results in appearances similar to linitis plastica or Crohn's disease. In the discussion, the authors refer to tumours in other organs which have been described in the literature and which can be demonstrated radiologically.

Autopsy↗

Gallstone in the lung.

A gallstone was found in the lung in a 70-year-old woman. The authors suggest that it perforated the gallbladder, subsequently penetrating the diaphragm and eroding into a right-lower-lobe bronchus.

[Value and limits of different method in diagnosis and differential diagnosis of retroperitoneal tumors within the lymphatic systems (author's transl)].

The demonstration of retroperitoneal intralymphatic primary or secondary tumors requires a progressive radiological investigation beginning with simple methods and advancing to more complicated ones. Radiological lymphography is a method which is superior due to its ability to demonstrate small structures in lymphatic vessels and lymphnodes. However, the usefulness of lymphography is limited partially to the retroperitoneal lymphatic system and to gross changes. To improve diagnostic accuracy further diagnostic measures are needed, namely intravenous urography, venocavography, lumbar venography of catheter, sonotomography of the upper abdomen and lymphatic scintigraphy. Exstirpation of lymphnodes- possibly by explorative laparotomy- and histopathological examination enables an extact assessment of lymphnodes of microscopical dimension as determination of the tumor type. The value and limits of complementary methods and the criteria for the diagnosis and differential diagnosis of metastatic lymphnodes and lymphoreticular systemic diseases are discussed. The significance of lymphography for evaluation of topographic expansion of pathological lymphnode changes, the further course of the disease and the effect of therapy according to the authors' experience are presented. It is stressed that the value of lymphography in diagnosing the type of disease is limited.
