Serum sialyl-Tn antigen levels in patients with digestive cancers.

Serum levels of sialyl-Tn antigen (STN) were measured using a one-step radioimmunoassay kit in 257 patients with digestive cancers, 121 patients with benign digestive diseases, and in 64 healthy controls. With 45 U/ml regarded as the cutoff value, the positive rates of serum STN in digestive cancers were as follows: pancreas 40.0%, stomach 28.1, colon and rectum 27.8, biliary tract 25.0, liver 7.1, and esophagus 0%. In benign digestive diseases, the positive rate of STN was low (4.1%). In gastric and colorectal cancers, simultaneous measurements of STN and carcinoembryonic antigen (CEA) revealed that the positive rates of STN, CEA, and STN and/or CEA were 28, 42, and 55%, respectively. There was no significant correlation between STN and CEA in these cancers. STN may be a useful serum marker for digestive cancers, especially gastric and colorectal cancers.

Expression of placental alkaline phosphatase in gastric and colorectal cancers. An immunohistochemical study using the prepared monoclonal antibody.

The authors developed monoclonal antibodies (MoAb) against human placental alkaline phosphatase (PLAP). Four specific MoAb reacting only with PLAP and two nonspecific MoAb reacting equally with isozymes of alkaline phosphatase (hepatic, intestinal, and placental) were obtained. Immunohistochemical staining with the specific MoAb showed that the cell membrane and cytoplasm of cancer cells were stained in gastric and colorectal carcinoma. The incidence of PLAP positivity was 23% (25 of 107) of all gastric carcinomas. Among gastric carcinomas, the 42% (13 of 31) positivity of highly differentiated carcinoma (papillary adenocarcinoma and well-differentiated tubular adenocarcinoma) was a significantly higher rate than that found in poorly differentiated carcinoma (poorly differentiated adenocarcinoma and signet-ring cell carcinoma, five of 41, 12%). The incidence of PLAP positivity was 11% (four of 35) in colorectal carcinoma. In contrast, gastric adenoma, intestinal metaplasia, and noncancerous tissue adjacent to cancer did not show staining. These results indicated that expression of PLAP was apt to occur in more highly differentiated gastric carcinoma and was highly specific for carcinoma in the gastrointestinal tract, although its incidence was not high.

Characterization of gamma-glutamyltranspeptidase from human pancreatic cancer.

To elucidate the specific changes of pancreatic gamma-glutamyl-transpeptidase (gamma-GTP) associated with malignant transformation, some properties of gamma-GTP purified from pancreatic cancer were compared with those of gamma-GTPs from normal pancreas and other tissues. Four of five pancreatic cancer gamma-GTPs showed distinctly slower electrophoretic mobility than normal pancreatic enzymes. Isoelectric points of pancreatic cancer gamma-GTPs varied in each case, but they were all higher than those of normal pancreatic enzymes. This difference in isoelectric points of gamma-GTPs between cancerous tissue and normal tissue was reduced by neuraminidase treatment. Lectin affinity chromatography revealed two of five pancreatic cancer gamma-GTPs with a greater affinity to concanavalin A (Con A) than normal pancreas gamma-GTPs. Four of five pancreatic cancer gamma-GTPs had a greater affinity to Lens culinaris agglutinin (LCA) than normal pancreas gamma-GTPs. Normal pancreas gamma-GTPs had little affinity to Phaseolus vulgaris erythroagglutinating (E-PHA), but two of five pancreatic cancer gamma-GTPs had an apparent affinity to E-PHA and one of them had a slight affinity to E-PHA. These results indicate that the transformational changes of pancreatic cancer gamma-GTP are mainly induced in the sugar chains of the enzyme molecule, resulting in lower content of sialic acid and higher content of fucose and bisecting GlcNAc residue (the beta-N-acetylglucosamine residue linked at the C-4 of the beta-mannosyl residue of the trimannosyl core of the asparagine-linked sugar chain) as compared with the normal pancreatic enzyme.

Serum levels of tumor-associated glycoprotein (TAG-72) in digestive cancers.

Serum levels of tumor-associated glycoprotein (TAG-72) were measured using a two-step sandwich radioimmunoassay kit in 281 patients with digestive cancers and 135 patients with benign digestive diseases. The positive rates of TAG-72 with the cut-off values of 2.2 and 4.0 U/ml were high in pancreatobiliary (56%, 38%), gastric (49%, 37%) and colorectal (62%, 29%) cancers, while the false-positive rate in benign diseases was 11%, 0.7% respectively. Very high levels were found in patients with advanced cancer. TAG-72 was positive in 17 (greater than 2.2 U/ml) and 7 (greater than 4.0 U/ml) out of 60 CEA-negative patients, and in 7 (greater than 2.2 U/ml) and 3 (greater than 4.0 U/ml) out of 17 CA19-9-negative patients. TAG-72 might be a useful serum marker for digestive cancers, especially gastric and colorectal cancers.

[An operated case of multiple well-differentiated hepatocellular carcinomas and adenomatous hyperplasia].

A 66-year-old male with non-A, non-B liver cirrhosis underwent an operation for a partial resection of the S8 area. Intraoperative ultrasound (US) examination of the entire liver revealed 10 space-occupying lesions (SOLs), and a biopsy of 5 out of the 10 SOLs showed 3 hepatocellular carcinomas (HCCs), adenomatous hyperplasia (AH), and an atypical AH (AAH). In a resected specimen that including 4 SOLs that had been detected by operative US, 6 early HCCs, an HCC of the Edmondson grade I + II, and 2 AAHs were found. All tumors were less than 2 cm and were not encapsulated.

Idiopathic esophageal varices with longitudinal erosion.

A 60-year-old man with idiopathic esophageal varices is presented. Endoscopy and endoscopic ultrasonography clearly demonstrated a longitudinal erosion on one of two straight varices. There was no evidence of portal hypertension, and no abnormality in the superior vena cava system. Peritoneoscopy and liver biopsy were normal. These varices with longitudinal erosion represent unique endoscopic findings as compared with previously reported cases. There has been no variceal bleeding during a 6-year follow-up.

[Clinical evaluation of serum sialyl SSEA-1 (SLX) in diagnosis of cancers].

In order to evaluate the usefulness of sialyl SSEA-1 (SLX) as a tumor marker of digestive cancers, serum levels of the antigen were determined in 334 patients with malignancies and 196 patients with benign diseases. The results indicated that positivity of the antigen in sera from malignant patients was highest in pancreatic cancer (58%) and biliary tract cancer (56%). False positive incidence of SLX in sera from benign diseases was as low as 6%, revealing low false positivity. Comparison with other tumor markers such as CA19-9, CA-50, CEA and ST-439 showed that positivity of SLX was as high as that of CEA, whereas it was lower than that of CA19-9 or CA-50. On the other hand, false positivity of SLX as well as ST-439 was lowest, and accuracy of SLX was no less high than that of CA19-9 or CA-50. In sera of pancreatic and biliary tract cancer, positive incidences of CA19-9, CEA and ST-439 were 80%, 64% and 53%, respectively, and the diagnostic efficiency increased by combined assay of SLX with CA19-9 (88%), CEA (81%) and ST-439 (71%). SLX appears to be no less useful than the other recently developed carbohydrate antigens or CEA as serum tumor marker for pancreatico-biliary cancer.

[Clinical evaluation on an enzyme immunoassay kit for basic fetoprotein (BFP). (1) Clinical usefulness of BFP].

Clinical usefulness of a sandwich enzyme immunoassay kit for basic fetoprotein (BFP), which has been developed with two kinds of monoclonal anti-BFP by Nippon Kayaku Co., LTD., was evaluated on detection of BFP in the sera. Based on the consideration of BFP-positive rate under 5% in healthy donors and diagnostic efficiency for cancer, a BFP-concentration of 75ng/ml was used as the cut off value. BFP in the sera was positive in 3.5% of 368 healthy donors, 18.2% of 549 patients with benign diseases and 45.3% of 870 patients with cancer. High positive rates of 50 approximately 60% for the serum BFP were found in cancer of the liver, bile duct, pancreas, kidney, prostate, testis and ovary. In early cancer of the bile duct and urogenital organs, high BFP-positive rates beyond 46% were recognized. BFP values in 7 of 9 BFP-positive patients with urogenital cancer fell down to negative serum level after the radical operation. These results indicate that BFP is a tumor marker with widespread representation in cancer and contributes to serological diagnosis for cancer of digestive and urogenital systems, particularly for early cancer of these systems. Furthermore, it is suggested that BFP is useful for monitoring of cancer patients after surgery.

[Clinical evaluation on an enzyme immunoassay kit for basic fetoprotein (BFP). (2) Comparison and combination of BFP with other tumor markers].

Comparison of basic fetoprotein (BFP) with 10 other tumor markers was made with sera from 549 patients with benign diseases and 870 patients with cancers, using BFP-EIA kit and commercial kits for others. BFP-positive rates higher than CEA or CA19-9 were found in various cancers except CEA in cancer of the colon, pancreas and lung, or CA19-9 in cancer of pancreas and bile duct. Furthermore, BFP showed higher positive rates in comparison with AFP in cancer of liver and testis, SLX(sialyl SSEA-1) or SCC in lung cancer and CA125 in uterine cancer. The correlation coefficient of BFP with other tumor markers except for SCC in lung cancer were low (below 0.262) in cancer and benign diseases. The combined assay of BFP with some other makers such as CEA in cancer of the digestive organ, lung, markers ovary and uterus, CA19-9 in cancer of the bile duct and lung, CA125 in ovarian cancer, AFP in cancer of the liver and testis, and PAP in prostatic cancer, showed an elevation of diagnostic efficiency compared with single assay. These results indicate that BFP is superior to other tumor markers for serological diagnosis of various cancer and also available for the combined assays.

[Serodiagnosis: tumor marker].

The researches of tumor markers started at the discovery of Bence Jones protein in 1848. Now, alpha-fetoprotein and carcinoembryonic antigen are popular tumor markers and recently several tumor markers using the methods of monoclonal antibodies have been invented. Usually we measure the tumor markers in sera, so the early diagnosis of cancer by the tumor markers are sometimes difficult. For the early diagnosis by tumor markers, inventions of new tumor markers, improvement of assay systems and materials and the combination assays of several tumor markers have been tried. The researches of tumor markers are progressing with those of cancer. We hope we will be able to diagnose early cancer or precancerous state by tumor markers.

[Cancer-associated carbohydrate antigens available for serum diagnosis].

Many of the recently developed tumor antigens detected by monoclonal antibodies are sugar chains and frequently associated with blood group substance. By immunohistological studies, we evaluated mainly the clinical usefulness and significance of the serum assay of CA-50 classified as a type 1 sugar chain, sialyl SSEA-1 classified as a type 2 sugar chain, and ST-439 with an undetermined structure, as well as their clinicopathological significance. In addition, the value of measurement of CA 19-9 and ST-439 in pancreatic juice was studied. The incidence of serum CA-50 was highest in pancreatic cancer (86%) and biliary tract cancer (66%), but relatively low in benign diseases. Moreover, the positivity and specificity of CA-50 as a tumor marker were no less useful than those of CA 19-9. However, comparison of serum levels of CA-50 with those of CA 19-9 in the same samples revealed a highly positive correlation in malignant diseases. Thus, improvement of the diagnostic rate through a combination assay of CA-50 and CA 19-9 seems unlikely. Both serodiagnostic and immunohistological studies showed that sialyl SSEA-1 and ST-439 were highly specific for tumor, whereas their appearances in serum or tumor were lower than CA 19-9 or CA-50 carrying the type 1 sugar chain. In addition, an appreciable number of sialyl SSEA-1 or ST-439 positive patients were found among those negative for CA 19-9 or CA-50. These results indicate that assay of sialyl SSEA-1 or ST-439 improves the diagnostic rate in combination with CA 19-9 or CA-50. Although the concentration of CA 19-9 in pancreatic juice from pancreatic cancer patients was highest, it was also significantly higher in patients with chronic pancreatitis than in controls. Furthermore, the overlap between the values in patients with cancer and those with chronic pancreatitis was great. On the other hand, the concentration of ST-439 in pancreatic juice was significantly higher only in patients with pancreatic cancer. These results indicate that the measurement of ST-439 in pancreatic juice is more useful as a tumor marker than measurement of CA 19-9, and that moreover, the assay of CA 19-9 in pancreatic juice could be used as a sensitive marker for nonspecific pancreatic injury.

[Clinical study of the NCC-ST-439 EIA kit using serum from patients with various types of cancer and benign disease (1)].

The amounts of antigens in the serum of patients with various types of cancer and benign diseases were measured in order to clinically evaluate the NCC-ST-439 EIA kit. From the results of measurements in 583 healthy persons, cut-off values of 4.5 U/ml for males and females 50 years or older and 7.0 U/ml for females 49 or younger were set. The false-positive rate for healthy persons at this time was 5.0%. The positive rates for malignant tumors at these cut-off values were 58.9% for pancreatic carcinoma, 53.1% for cholangiocarcinoma, 41.9% for mammary carcinoma, 36.6% for rectal and colonic carcinoma, 33.3% for hepatocellular carcinoma and 35.3% for carcinoma as a whole. The false-positive rates for benign diseases were 14.0% for pancreatic disease, 9.7% for liver disease, 8.0% for biliary duct disease and 8.3% for benign diseases as a whole. These rates were low when compared with those for other tumor markers. These results showed that NCC-ST-439 is a tumor marker with a high degree of specificity.

[Clinical study of the NCC-ST-439 EIA kit using serum from patients with various types of cancer and benign disease (2)].

The positive rates for NCC-ST-439 in cases where other tumor markers were negative were 16.8% for CA19-9-negative cancer cases and 23.5% for CEA-negative cancer cases. The positive rates obtained using a combination of these three markers were 86% for pancreatic and cholangiocarcinomas, and at least 65% for hepatocellular, rectal and colonic and mammary carcinomas. The following combinations of tumor markers were the best from the standpoint of diagnostic efficiency: NCC-ST-439 + CA19-9 for pancreatic carcinoma, NCC-ST-439 + AFP for hepatocellular carcinoma, and NCC-ST-439 + CEA for rectal and colonic carcinoma. In cases which were positive preoperatively, 83.3% became negative after curative surgery, which reflected the clinical course very well. These results indicate that NCC-ST-439 improves the diagnostic rate in combination with other tumor markers because of its high specificity, and it was evaluated as useful for the monitoring of therapeutic effects.

[Clinical investigation of carbohydrate antigen CA-50].

The CA-50 enzyme immunoassay kit (EIA kit) that has been developed with the use of C-50 monoclonal antibody prepared by L. Lindholm et al. was evaluated for diagnosis of human cancer. The levels of CA-50 in the sera were determined using this kit supplied from Mitsui Pharmaceuticals, Inc. Co. in 759 healthy donors, 728 patients with benign disease and 1,263 untreated patients with cancer. A CA-50 concentration of 40 U/ml of serum was used as the cut-off value. Patients with pancreatic cancer and patients with bile duct cancer had high positive incidence of 75% and 68%, respectively, compared with a low positive incidence of under 40% in patients with other cancers. On the other hand, positive rates in patients with benign disease were as low as 13%. Comparison of the serum levels of CA-50 with CA19-9 in the same samples did not exhibit complete positive correlation in patients with pancreatic cancer, patients with bile duct cancer and patients with liver cancer. These findings indicated that C-50 antibody reacted with two epitopes of CA19-9 and sialosyllactotetraose. From the above results, the usefulness of CA-50 as a tumor marker for pancreatic cancer and bile duct cancer was recognized with this EIA kit.
