Transbronchial needle aspiration in the diagnosis of sarcoidosis.

A total of 258 patients with suspected sarcoidosis underwent transbronchial needle aspiration lymph node biopsies and forceps bronchial biopsies during rigid bronchoscopy. Transbronchial needle aspiration biopsies revealed noncaseating granulomas in 66.3 percent of patients. When transbronchial needle aspiration biopsies were combined with forceps bronchial biopsies, the positive rate increased to 77.7 percent. No major complication occurred when using transbronchial needle aspiration. Rigid bronchoscopy with transbronchial needle aspiration biopsies appears to offer a sensitive and specific alternative to more invasive techniques used in the diagnosis of sarcoidosis.

[Cylindrical cancer of the lung. Analysis of a series of 191 cases].

The authors analyse 191 cases of cylindrical carcinoma of the lung. Study of the clinical and radiological characteristics, as well as the course, revealed the following: the reduced male predominance of this type of lesion, the often distal site and the primary pulmonary nature. The prognosis is identical to that for squamous forms: 31% survivors at 5 years.

Postpneumonectomy empyema does not improve survival in bronchogenic carcinoma.

There is much disagreement in the literature about the beneficial effect of postpneumonectomy empyema on survival following operation for bronchogenic carcinoma. We had the opportunity to gather data on 407 patients with this serious complication. The survival data for a group of patients with postpneumonectomy empyema and fistula were compared with those for another group without such complications. Our statistical analysis confirms that postpneumonectomy empyema does not improve life expectancy.

[5-year survival of resected bronchial cancer. Multifactorial analysis of its prognosis].

Prognosis in patients having undergone surgery for a bronchial carcinoma (oat cell. carcinoma excluded) was studied by comparing, with two statistical methods, a group of 178 patients surviving five years and more (group I) and a group of 178 patients who died within five years (group II). The two groups had undergone treatment during the same period. Comparison of the two groups suggested that the following factors were indicators of a good prognosis: ratio weight-height within normal limits without recent weight loss, fortuitous detection of the carcinoma; normal aspect on bronchoscopy; T1 N0 type without vascular extension, limited exeresis. Recurrences of the carcinoma were generally observed in group II but could be present in group I. The differential prognosis between pulmonary metastases and a "new" bronchial carcinoma was difficult. Survival was finally determined by the frequency of recurrences and metastases. By a correspondences factorial analysis it was possible to separate and to define the main characteristics of the patients with a good prognosis and a long survival and of those with a poor prognosis and a short survival. However this proved to be exact in only 30% of the individual cases because occurrence of metastases was generally unforeseable. From these data it appears that surgery alone is indicated in only one kind of tumours; T1 N0 carcinomas detected by routine examinations.

HLA antigens and primary bronchial carcinoma.

A comparative study of HLA, A, and B antigens frequency was undertaken on patients with primary bronchial carcinoma. A prospective study was carried out on 103 patients admitted for the first time to the hospital, and a retrospective study was done on 53 patients who survived at least five years. Forty-seven patients died early while on the prospective study. The results show an increased frequency of B12 antigen both in patients of the prospective study and the retrospective study. This would suggest that the increase is associated with susceptibility to the disease rather than resistance to it. There was no finding in favor of an association between a HLA antigen and survival.

Regional immunotherapy in resectable squamous cell lung carcinoma. Analysis of a randomized study.

Patients with resectable squamous cell carcinoma were randomly allocated after surgery to receive either no further treatment (57 patients) or a single intrapleural injection of BCG (61 patients). No significant improvement in survival was observed in patients treated with BCG, even when their disease was staged as N0. There was a slight trend for the recurrence rate to be lower in patients classed as N0, but this was not significant.

[The postoperative course in 82 cases of bronchiectasis (author's transl)].

After a description of their clinical and radiological characteristics, the authors analyse the postoperative course in 82 cases of bronchiectasis. Whilst the severity of postoperative complications tends to decrease, a large number remains. Long term results were good in 82% of cases but were related to the age of the patients, and the extent of the lesions and the excision. As a result, success is dependent upon the decision concerning the indication for surgery, but also the quality of postoperative surveillance which must be rigorous.

[Transbronchial lymph node biopsy in the diagnosis of sarcoidosis (author's transl)].

Two hundred and fifty eight transbronchial lymph node biopsies using a Menghini type needle 1.5 mm in diameter were performed in 193 patients with a radiological and clinical picture suggestive of sarcoidosis. In 73 cases, the histological argument for the diagnosis was provided solely by this technique. The combination of two methods of obtaining material (multiple biopsies and transbronchial puncture) during rigid bronchoscopy led to a histological diagnosis in 77.7% of patients. The authors emphasize the relative safety of the method, as well as its value when material obtained by fibroscopy has proven to be unsatisfactory.

["Active" rosettes and skin tests to recall antigens and phytohemagglutinin in 48 patients with non metastatic squamous cell lung carcinoma and before treatment (author's transl)].

Looking for an early immune defect assay in non metastatic (MO) squamous cell lung carcinoma, 48 untreated patients were tested at the time of diagnosis for peripheral blood "active" rosette-forming T lymphocytes, delayed cutaneous hypersensitivity to recall antigens and skin reactivity to phytohemagglutinin (PHA). 32 patients had a resectable tumour (group 1) and 16 patients had a loco-regional extension which made the tumour inoperable (group 2). No significant difference (p greater than 0.05) was observed between any of the two groups of patients and normal controls nor between group 1 and 2 for "active" rosettes and skin tests to recall antigens. However, skin reaction to PHA was significantly impaired in patients of both groups compared to normal age-matched controls whatever the dose of PHA (4 microgram and 10 microgram). The number of unreactive patients in group 2 was higher than in group 1, but the difference was not significant (p greater than 0.05). It is concluded that skin reactivity to PHA is an useful and easy assay for cell-mediated immunity deficiency in early squamous cell lung carcinoma, whereas "active" rosettes are not altered.

[Clinical and physiological post-operative course of 45 bullous emphysema (author's transl)].

45 patients exhibiting a diffuse emphysema associated with voluminous emphysematous bullae underwent surgical resection of the bullae. They were then regularly followed-up. Pulmonary function investigations were performed preoperatively in 38 patients at 6 months, 1 year and 3-7 years after resection. Postoperative subjective improvement was experienced by all patients, but diminished progressively with years. An associated chronic bronchitis was a predictor of bad prognosis. Vital capacity increased whereas plethysmographic residual volume and TLC decreased post-operatively, due to the recovery of available pulmonary parenchyma. The improvement of bronchial obstruction was probably related to the increased elastic recoil pressure. Hypoxemia also improved in most cases. In patients having undergone a second resection (controlateral lung), functional improvement progressively disappeared as the emphysematous disease progressed.

[Clinical, pathologica and genetic analysis of 53 cases of broncho-pulmonary cancer, still surviving after 5 years (author's transl)].

The authors take up again the clinical, pathological and genetic analysis of 53 bronchial cancers with a survival exceeding 5 years. The operation samples were reexamined; a H.L.A. grouping was done in every cases. On the whole, it concerned patients under 60 in good general condition; the extension of their lesions only required a limited exeresis (lobectomy). The epidermoid form predominates, the lymph node invasion is rare. Seventy four per cent of the patients carry at least one of the following antigens: W 19, A 10, B 5, BW 35. But the study did not prove the existence of a genetic factor conditioning the survival.
