Use of albumin in neonatal resuscitation.

The use of albumin plasma has become popular during resuscitation of the term baby with very low Apgar scores (< or = 2 at 1 min). There is no evidence of benefit from this practice which may actually be damaging to babies with severe asphyxia causing myocardial damage.

Acquisition of cytomegalovirus infection by premature neonates.

In order to evaluate the risk of premature infants acquiring cytomegalovirus (CMV) infection, a prospective study was made of infants of 32 weeks gestation or less admitted to a neonatal unit. Infants were assessed at birth to exclude congenital CMV infection and followed to the age of 1 month post-term. A total of 16/131 (12.2%) infants acquired CMV infection; 6 of CMV antibody-negative and 10 of CMV antibody-positive mothers. Six of 65 (9.23%) infants of CMV antibody-negative mothers who had been transfused with blood acquired CMV infection compared with 0 of 18 infants of CMV antibody-negative mothers, and who had not been transfused with blood. Acquired CMV infection was also more common in infants of CMV antibody-positive mothers and who had been transfused with blood, i.e. 9 of 34 (26.47%) compared with 1 of 14 (7.14%). The relative risk was 3.7 (95% confidence interval 1.53-5.87). This study suggests that all premature infants requiring blood transfusion should be transfused with CMV antibody-negative blood regardless of the CMV immune status of their mothers.

Intussusception in preterm infants.

Two cases of intussusception in infants born at 26 and 30 weeks' gestation are described. The two infants presented in the neonatal period with abdominal distension, intolerance of feeds, and rectal bleeding. An initial diagnosis of necrotising enterocolitis was made and the infants were treated medically. This led to a delay in the diagnosis of the intussusception. Published work on neonatal intussusception is reviewed and attention is drawn to the fact that the presenting signs and symptoms can be similar to those of necrotising enterocolitis. A diagnosis of intussusception should therefore be considered in any preterm infant with suspected necrotising enterocolitis.

Fatal acquired cytomegalovirus infection in a neonate with maternal antibody.

An extremely premature infant was born to a mother who had antibodies to cytomegalovirus (CMV). Despite having acquired transplacental antibodies, the baby developed an episode of presumed CMV hepatitis at the age of 10 weeks after blood transfusions from seropositive donors. Although the hepatitis settled, the baby developed chronic lung disease from which he died at the age of 5 months. Post mortem, widespread infection with CMV was demonstrated. The fact that serious CMV infection can develop in an infant of a seropositive mother further justifies the routine use of blood from CMV-negative donors for all transfusions given to infants of low birth weight.
