[Prevention of supraventricular tachysystole with anti-arrhythmia agents].

The paper substantiates the necessity of using a set of antiarrhythmic drugs for the prevention of supraventricular tachysystoles. In the development of supraventricular tachysystoles an important role is attributed to the disorders in automatic regulation of the sinus node. Cases are described in which the development of paroxysmal cardiac fibrillation with approximately equal intervals of time was caused by an inhibition of the sinus node activity. The set of antiarrhythmic drugs included beta-adrenergic blockers, isocholinic-type drugs, cardiac glycosides, Rauwolfia serpentina preparations, parasympatholitic drugs.

[First and second derivatives of central pulse curves in bloodless methods of study of the force and rate of myocardial contractions].

An original technique was developed for the analysis of volume sphygmograms of the central pulse and of their derivatives that permit an indirect judgement of the maximum speed of aortic pressure growth and maximum acceleration. The technique of the examination, the methods of calculations, their substantiation and normal limits are presented. Examples are given of the employment of the technique in cases of functional and pharmacological tests. The method may be widely used under clinical conditions for the study of the functional state of the myocardium and of the individual effect of drugs (cardiac glycosides, alpha-and beta-adrenergic receptors blockers, etc.).

[Effect of antikinin agents on the course of experimental myocarditis].

The therapeutic action of 5 preparations: specific antikinin substances -- anginin and contrical, and nonspecific -- epsilon-aminocapronic acid, aspirin and indometacine, was tested in 71 rabbits with allergic myocarditis. It has been concluded that in the allergic distruction of the myocardium plasma kinins, which are formed and accumulated in large quantities, take part in the pathogenesis of the disease serving as mediators of the tissue distruction, therefore specific antagonists of kinins and inhibitors of the kinin system should be included as pathogenetic agents into the complex of allergic myocarditis treatment.

[Changes in the kinin system components and structure of the myocardium under the influence of antikinin preparations in allergic myocarditis in rabbits].

Experimental allergic myocarditis was induced in rabbits; a study was made of the effect on the kininogen content and on the activity of kininase in the myocardium, and on its morphological structure produced by anginine, contrical, aspirin, indometacine and epsilon-aminocapronic acid. The former two preparations were much more potent (in comparison with the others) in inhibiting the activation of the myocardial kinin system and promoted restoration of the morphological structure of the myocardium. Development of an allergic inflammation in the myocardium could be arrested or decreased by inhibition of the kinin system of the plasma and myocardium by specific agents.

Hypersensitivity of delayed type manifested during the development of allergic lesions of the myocardium.

Changes in humoral and cellular responses were studied during the development of allergic lesions of the myocardium in rabbits. These responses,manifestations of hypersensitivity of delayed type, are connected with transformation of cells of the active mesenchyme, responsible for the process of immuogenesis, into lymhocytes and plasma cells. In allergic lesions of the myocardium hypersensitivity of delayed type is manifested as lympho-marcrophagal reaction in the intermuscular stroma of the heart, the thymus, and lymph glands and also as blast transformation of the lymphocytes. A complex series of disturbances of immunologic homeostasis arises, in which the primary components are micronaglopahies, blast-transformation of the lymphoctes and activation of iysosomal enzymes.

[Determination of the maximum speed of increase of pressure, the index and reserve of myocardial contractility as factors in hemodynamic studies of hypertension].

The authors' original non-invasive technique of determining the speed of pressure, elevation in the aorta, the index and reserve of myocardial contractility that reflects its contractile function was used in 80 persons (among them 25 normal individuals and 55 hypertonics). The main types of haemodynamic shifts noted in hypertension and its variants were described on the basis of simultaneous measurements performed by the above technique and by determinations of the cardiac output, speed of a pulse wave spreading and partial peripheral resistance. The suggested indices appeared to be the most sensitive ones permitting to detect the hypertensive type of the circulation of the initial stages of hypertensive disease.

[Myocardial damage syndrome in allergic conditions].

Experimental and clinical studies were conducted that permitted to suggest a possible direct damaging effect of allergy upon the myocardium. Distinct types of allergic reactions of the myocardium were singled out in cases of the states being studied, namely infectious-allergic myocarditis and bronchial asthma. The reactions observed in infectious-allergic myocarditis pertain to hypersensitivity of a delayed type which morphological manifestation is embodied in cellular lymphohistocytic, monocytic perivascular infiltration and myocardial stroma damage. In bronchial asthma haemodynamic disorders are noted in the vessels of the microcirculatory bed, as well as oedema of the stroma and muscle fibers destruction.
