[Potentials of a noninvasive method for studying volumetric cerebral blood flow in mitral stenosis and following its surgical correction].

The volumetric parameters of the cerebral blood flow were studied in 15 patients who had undergone comissurotomy, using the comprehensive rheographic method including tetrapolar rheoencephalography and transthoracic rheography. The non-invasive method makes it possible to determine the minute's volume of the brain circulation as well as its ratio to cardiac output. The results obtained point to a positive time-course of the brain blood flow following the surgical correction of mitral stenosis. The rheographic findings were verified by the results of computer-aided angioscintigraphy of the brain which showed the validity of the technique.

[Enhanced atrial ECG and its characteristics in atrial fibrillation].

A method of recording intensified atrial ECG (IAECG) is described. A total of 150 subjects were examined by this technique including 60 with the normal sinus rhythm and 90 with atrial fibrillation. A certain homogeneity between P atrial waves and the intensified waves of atrial fibrillation was observed over time in the same patients in the presence of prominent heterogeneity of individual types of atrial fibrillation. Characteristic features of the atrial complexes revealed in patients with atrial fibrillation may be of importance for the differential diagnosis, as well as for the assessment and prediction of therapy efficacy. They also appear to be essential in solving a number of theoretical questions concerned with the genesis of both atrial fibrillation and other heart rhythm irregularities.

[Integral ECG and ventricular gradients, differential ECG, their diagnostic potentials].

Synchronous recording of the integral ECG (IECG), ECG and is first derivative (DECG), made according to the original method, shows that the measurement of the amplitude of IECG elements can substitute the difficult calculations of the area of ECG waves, required for the determination of AQRS, AT and the ventricular gradient (G). On this basis the normal dimensions and directions of the AQRS, AT, G vectors were calculated in the frontal and horizontal planes, the typical changes were recorded in the blocks of the His' bundle, ischaemic heart disease and myocardial infarction. The offered methods of calculation make easily accessible the determinations of G in the right-angle system of coordinates, open new possibilities of the quantitative assessment of ST segment displacement. Recording of DECG permits one to detect P waves in those cases when they coincide with the QRS complex, with ST--T interval, which helps differential diagnosis of rhythm disorders of the heart.

[Pulmonary hypertension in chronic nonspecific lung diseases].

Pulmonary and intracardiac hemodynamic findings are reported. They were obtained by means of catheterization of right cardiac departments and the pulmonary artery with thermodilution in 57 patients with chronic nonspecific pulmonary diseases (CNPD), under conditions of resting, rationed bicycle-ergometric exercise, hypo- and hyperoxia, and nitroglycerin administration. Changes in pulmonary and intracardiac hemodynamics which reflect the ongoing exhaustion of compensatory mechanisms in the external respiration apparatus, the pulmonary vascular system and the heart, are shown to precede the formation of right ventricular hypertrophy in obstructive CNPD A classification of secondary pulmonary hypertension, based on clinical and instrumental signs, is offered.

[Metabolic disorders in the myocardium in noncoronarogenic diseases].

Metabolic disorders develop in the myocardium in its noncoronarogenic diseases as a consequence of changes in the myocardiocyte electrolyte balance, disturbed energy potential of the muscle cell, changes in the properties of the myoplasm protein, and the direct damaging action on the cell organnelles. The authors studied the metabolic processes in the myocardium in myocardial dystrophy, myocarditis, and cardiomyopathy (55 cases). Subendomyocardial biopsy was conducted in 18 patients, post-mortem material was studied in 42 cases. Electron microscopy and histochemical studies were carried out. It was established that the morphological manifestations of metabolic disorders in the myocardium in its noncoronarogenic diseases are very diverse: these are disorders of oxidative processes and ATP synthesis, protein metabolism, glycogen metabolism, electrolyte balance, etc. A conclusion is drawn that it is necessary to record these changes in the treatment of noncoronarogenic myocardial diseases.

[Noninvasive method of determining the cerebral blood flow circulation rate and its correlations with cardiac minute volume].

A method is suggested for noninvasive determination of the cerebral circulation rate and its correlations with the cardiac output. It is based on the metrological possibilities of tetrapolar rheoencephalography in correlation with chest (transthoracic) rheography. The percentage of cerebral circulation rate in relation to the cardiac output measured by this method (Ic/o) characterizes the direction and value of compensatory blood redistribution in various hemodynamic situations, while the absolute values of cerebral circulation rate (Cm, in millilitres per minute) and their relation to the proper values (Cm. in % of proper values) make it possible to appraise the afficacy of the compensatory shifts in each individual case. The method may gain wide use in the clinic for indirect appraisal of the condition of the cerebral circulation.
