The computed tomographic appearance of subcutaneous nodules occurring in metastatic neuroblastoma.

The authors report a case of subcutaneous nodules in a 2-month-old girl with metastatic neuroblastoma; the appearance of the nodules in computed tomography scans is described. Such nodules are sometimes overlooked by clinicians and radiologists, even though they almost always occur in disseminated disease. In patients with neuroblastoma and other neoplasms subcutaneous nodules may be an easily accessible source of samples for histopathologic examination.

An outbreak of penicillin-resistant Neisseria gonorrhoea in southern Israel.

Since the early 1980s the incidence of gonorrhea has been declining worldwide, as well as in Israel. This decline has been attributed to the fear of AIDS. We report an outbreak of penicillin-resistant Neisseria gonorrhoea that occurred in the last quarter of 1988 and throughout 1989 in southern Israel. During 1987 and the first three-quarters of 1988 only 13 new cases were diagnosed, all penicillin sensitive. During the epidemic, 94 new cases were diagnosed, 41% of them penicillin resistant. In March 1989, ceftriaxone was substituted for penicillin for the treatment of gonococcal urethritis. The number of cases of penicillin-resistant gonorrhea dropped dramatically. It is concluded that despite the AIDS panic, outbreaks of gonorrhea can still be encountered, as demonstrated in this study.

[Primary iliopsoas abscess in children].

Primary iliopsoas abscess is a relatively rare disease occurring mainly in children. Since it simulates more common conditions, delay in diagnosis is almost the rule. In 2 children it simulated acute appendicitis, but in both cases a normal appendix was removed. In a third child it led to the diagnosis of right sacroiliitis, and antibiotics were given for several weeks. Ultrasound examinations in all 3 failed to reveal the diagnosis, which was reached by CT scan. The latter is the method of choice for diagnosis and is of aid in deciding on the best route for drainage.

Mercury exposure and effects at a thermometer factory.

Mercury levels in air, urine, blood, and hair were studied for workers (14 women, 2 men, ages 18-44 a) employed in a small thermometer factory (metallic mercury vapor exposures) with generally inadequate engineering and hygienic arrangements. At exposure levels of 50-99 microgram/m3 (5 workers), 100-149 microgram/m3 (3 workers) and 150-200 microgram/m3 (7 workers), respectively, the mean mercury levels were 299, 449 and 628 nmol/l in urine; 105, 122 and 143 nmol/l in blood, and 4.2, 8.5 and 12.2 mumol/kg in hair (1 cm from base). Headache, irritability-hair loss, and wrist and ankle pain were the most frequently occurring symptoms. The correlation coefficients between the measured mercury levels in air for individual workers and mercury in urine, blood, and hair (1 cm from base) were r = 0.92, r = 0.79, and r = 0.42, respectively. Mercury determinations for segmental hair cuts indicated peaks and drops in absorption/deposition consistent with data on the vacations of the individual mercury levels of 150-770 nmol/l may be helpful when current thresholds of exposure/absorption are reconsidered.

In vitro activity of novel fluoroquinolones against Streptococcus pneumoniae isolated from children with acute otitis media.

BACKGROUND: In recent years, novel fluoroquinolones with improved activity against gram-positive organisms have been introduced into clinical practice. These drugs may be of potential benefit for the treatment of pneumococcal otitis media, including infections caused by organisms resistant to conventional drugs. METHODS: In vitro activity of 6 fluoroquinolones against 77 pneumococcal isolates from middle-ear fluid was determined by the E test. RESULTS: Resistance to penicillin, co-trimoxazole, erythromycin, clindamycin, and tetracycline was present in 59 (76.6%), 47 (61.0%), 19 (24.7%), 11 (14.3%), and 17 (22.1%) isolates, respectively. Fluoroquinolone MIC(50) and MIC(90) (in microg/ml) were as follows: ciprofloxacin: 1.0 and 3.0, levofloxacin: 0.75 and 1.0, sparfloxacin: 0.25 and 0.38, grepafloxacin: 0.25 and 0.38, trovafloxacin: 0.094 and 0.125, and moxifloxacin: 0.19 and 0.25, respectively. CONCLUSIONS: Novel fluoroquinolones and especially trovafloxacin and moxifloxacin appear to be of potential value for the treatment of acute otitis media caused by pneumococci resistant to traditional antibiotics.
