Anatomical observations on the accessory obturator nerve (based on 1000 specimens).

In a series of 500 adult human embalmed cadavers the accessory obturator nerve has been looked for, bilaterally, and its formation and distribution studied. This nerve was formed by roots from the anterior primary divisions of L3 and L4 (63.6%) or L2, L3 and L4 (10.6%), or L2 and L3 (7.6%), or L3 (6.1%) or from the trunk of the obturator nerve (12.1%). The frequency of this nerve was found to be 13.2 percent (13.3 % in males and 12.9% in females) of the 1,000 specimens examined, with a predominance on the left side of the body (15.2 : 11.2). Some questions with regard to the variability of the frequency of this nerve referred to in the literature are analyzed and discussed.

A rare case of spontaneous rupture of the urinary bladder.

An extremely rare case of spontaneous intraperitoneal rupture of the urinary bladder, caused by a combination of a tumor of the vesical wall and an impacted urethral stone, is reported. The patient's symptoms suggested a perforated hollow viscus. Under general anesthesia the urethral stone was removed, while at laparotomy a diffuse peritonitis with blood-stained purulent fluid emerging from the ruptured bladder was found. The vesical wall was repaired and the peritoneal cavity was drained. In spite of the patient's advanced age and debilitated condition and the generalized peritonitis, the postoperative course was uneventful and the patient recovered promptly. Although spontaneous rupture of this kind is rare, one needs to consider unsuspected bladder rupture in any acute abdomen, especially if the patient is in the prostatic age group or has voiding problems.

Urine porphyrins and their precursors in homozygous beta-thalassaemia.

In 68 patients with homozygous beta-thalassaemia the excretion of porphyrins and porphyrin precursors in urine has been assayed. Though a definite excretion pattern has not been established, the majority of the thalassaemics excrete an increased amount of coproporphyrin and porphobilinogen. The results are compared to those published by other authors.

Renal extramedullary hematopoietic tumor diagnosed by fine needle aspiration: a case report.

UNLABELLED: We report a case of a 58 year-old-male with a history of sickle cell anemia, who presented with a left kidney mass. Guided fine needle aspiration of the mass revealed extramedullary hematopoiesis and enabled avoiding an unnecessary surgical procedure. INTRODUCTION: Extramedullary hematopoiesis (EMH) generally occurs in patients with deficient bone marrow hematopoiesis secondary to either peripheral red cell destruction or marrow replacement. EMH is most commonly seen in the liver and spleen as a diffuse lesion. Rarely EMH presents as a solitary mass, posing a diagnostic dilemma. In asymptomatic patients without obvious evidence of hemato-pathology, the differential diagnosis is even more complex. Despite several reports describing the radiographic findings in EMH, fewer promote the use of fine needle aspiration (FNA) in making this diagnosis.

A phase II evaluation of bexarotene (Targretin) capsules in patients with metastatic melanoma.

A phase II study was undertaken to determine the efficacy of Bexarotene in melanoma. Between November 1997 and April 1998, 19 patients were given Bexarotene in single daily oral doses of 450 mg/m2 in capsule form continuously. Nineteen patients, four with choroidal metastatic melanoma, were treated. No responses were seen. Five patients had stable disease, two of the four with choroidal melanoma, had tumor progression. Myelosuppression was mild. Grade 3 myalgia, asthenia, diarrhea, cold hands/feet, and mood changes were seen in one patient each. Changes in serum triglyceride and thyroxine levels were common. Bexarotene, as used in this study, is not effective against melanoma.

Immunophenotypic profile and FISH analysis in a case of Ewing's sarcoma.

Ewing's sarcoma arising in the right fibula was diagnosed in a 12 years old female, considering clinical presentation, radiological findings and histological features. Our work points towards a possible neuroectodermal origin of Ewing's sarcoma, since MIC2/12E7 marker was found to be positive. Translocation t (11;22) (q24;q12) was found as the main cytogenetic change in this tumor, by means of FISH analysis.

Does neoplastic gastrin expression remodel the embryonal pattern of the protein? A study in human pancreas.

BACKGROUND/AIMS: To determine the immunoreactivity of gastrin during the development of the human fetal pancreas and ductal pancreatic adenocarcinoma, given that, gastrin positive cells were demonstrated either into its embryonic anlage or into pancreatic cancer. METHODOLOGY: Tissue sections from 15 pancreatic fetal specimens, and an equal number of ductal adenocarcinoma specimens, were assessed using immunohistochemical methods for gastrin. RESULTS: The density of positive cells in the primitive exocrine ductal walls and outgrowing buds was significantly higher than the relevant density in the neoplastic pancreatic tissue of mixed (ductal endocrine) and pure ductal type (p1=0.017, p2<0.0001, p3<0.0001 and p4=0.019, respectively). The above values were estimated from 20-22 weeks of gestation. There was no significant difference in the density of positive cells in the islet cell epithelium from 25-30 weeks, and the neoplastic tissue of mixed (p5=0.24) and pure ductal type (p6=0.55). CONCLUSIONS: The immunostaining for gastrin identifies a subgroup of pancreatic ductal adenocarcinomas with a neuroendocrine component (initially considered as pure ductal tumors), and mixed ductal-endocrine tumors. This pattern of expression in neoplasms recapitulates the normal pattern during the embryonal development of the organ, and may be important for the development of new therapeutic approaches with eventual clinical utility.

Correlation of immunohistochemically detected HER-2/neu (c-erbB-2) with histological stage and perineural invasion in pancreatic cancer.

BACKGROUND/AIMS: HER-2/neu (c-erbB-2) oncoprotein is a transmembrane glycoprotein and may function as a growth factor being involved in the regulation of cell growth and cell transformation. c-erbB-2 was correlated with established outcome factors in pancreatic cancer. METHODOLOGY: We performed an analysis of 100 patients with pancreatic cancer using an immunoperoxidase technique on formalin-fixed, paraffin-embedded tissue samples in order to determine HER-2/neu oncoprotein expression along with stage and perineural involvement. RESULTS: HER-2/neu oncoprotein was expressed in the tumors of 21 patients (21%). CONCLUSIONS: Histologic stage, and perineural invasion did not correlate with HER-2/neu oncoprotein.

Diffuse large B-cell lymphoma arising from a multicentric mixed variant of Castleman's disease.

This case report describes a patient with multicentric mixed type Castleman's disease and concomitant non-Hodgkin's lymphoma of diffuse large B cell type in the neck. Multicentric CD is a systemic illness with disseminated lymphadenopathy; its aggressive and usually fatal course is associated with infectious complications and risk for malignant tumors, such as lymphoma or Kaposi sarcoma.

Inflammatory pseudotumor of the spleen.

We report a case of a patient with inflammatory pseudotumor (IPT) of the spleen. IPTs can appear at many sites and represent rear lesions of uncertain etiopathogenesis. Usually they present as mass lesions, so the clinical and radiologic features often suggest malignancy. However, the microscopic findings are quite characteristic, and the diagnosis can be made readily by identifying the reactive nature of the cells. Diagnostic problems can arise when these lesions occur in lymphoid organs or the spleen.

Expression of HLA-DR antigen and characterization of the lymphocytic infiltrate in normal mucosa, tubulo-villous adenoma and invasive carcinoma of the colon.

Prognosis of colonic carcinoma is poor. The two most important factors having the greatest effect on survival are pathologic stage of disease and histologic grade of the tumor. Our study points towards the value of HLA-DR antigen in the prognosis of colonic carcinoma. We studied 31 cases of normal colonic mucosa, 12 cases of tubulo-villous adenoma, and 39 cases of invasive carcinoma for the detection of HLA-DR monoclonal antigen. Yet, we investigated the association of HLA-DR and DQ genes and adenoma and carcinoma by PCR. We also studied the T helper (TH) marker (CD4) in the lamina propria of the relevant cases, given that the dependence of immune responsiveness on the class II antigens reflects the central role of these molecules in presenting antigen to TH cells. HLA-DR was expressed in 20 of 31 normal colonic mucosa (64.5%), 4 of 12 adenomas (33.3%), and in 10 of 39 invasive carcinomas (25.6%). No significant correlation between HLA-DR and DQ genes and adenoma or cancer of the colon was found. CD4 was expressed in 9 of 31 normal colonic mucosa (29%), 5 of 12 adenomas (42%), and in 26 of 39 invasive carcinomas (67%). The results showed decreased expression of HLA-DR and increased expression of CD4 as the lesion progressed to malignancy. HLA-DR and DQ genes do not contribute to a susceptibility to adenoma or carcinoma. The immune attract mechanism by low HLA-DR signaling seems to be of minor importance in the malignant and metastatic potential of the colonic carcinoma.

The role of basic-fibroblast growth factor (b-FGF) in cyclosporine-induced nephrotoxicity.

BACKGROUND: The effect of the b-fibroblast growth factor (b-FGF) on cyclosporine A (CsA)-induced nephrotoxicity in the rat kidney was investigated. MATERIALS AND METHODS: The rats were divided into six groups: A (control), B (b-FGF-treated), C, D: (CsA-treated and sacrificed on days 14 or 21), E, F (Cs A- and b-FGF- treated and sacrificed on days 14 or 21). The antibody mouse anti-rat CD31 was used to evaluate the kidney vessels present in histological preparations. RESULTS: The kidney vessels in group B were increased in comparison with the control group (p<0.05). Reduction of kidney vessels in groups C and D (p<0.05) in comparison with the controls was observed, while in groups E and F they were increased when compared to group C (p<0.05) and D (p<0.05), respectively. CONCLUSION: The angiogenic role of b-FGF was confirmed in normal rats and a possible "protective" role of b-FGF was shown in rat kidney with CsA-induced nephrotoxicity.

Reduction of susceptibility to NK lysis in cisplatin-resistant K562 cell lines.

The development of resistance to cisplatin (CDDP) occurs both in vitro and in vivo. We cultured K562 cells with continuous exposure to gradually increasing doses of CDDP and produced CDDP resistant K562 sublines. CDDP-resistant sublines were found to be cross-sensitive to Mit-C and cross-resistant to DXR and cyclophosphamide. These sublines were tested for their susceptibility to NK-mediated lysis using NK cells isolated from 10 healthy donors and 12 patients with breast cancer. K562/B6 and K562/C9 CDDP-resistant sublines were more resistant (22 +/- 5% and 19 +/- 3% lysis, respectively for Ca patients, and 34 +/- 6 and 31 +/- 5% for healthy donors) than the maternal K562 cells (36 +/- 3% for Ca patients and 55 +/- 5% for healthy individuals). The correlation of cisplatin resistance, with a diminished susceptibility by NK lysis, should permit the selection of patients suitable for cisplatin-based chemotherapy.
