Gastroesophageal reflux and posterior laryngitis.

Esophageal acid exposure was assessed by 23-hour ambulatory pH monitoring and compared with a biopsy of the posterior larynx and proximal esophagus in 97 patients with hoarseness, burning pharyngeal discomfort, or globus sensation. Patient results were compared with normal acid exposure times obtained in 54 control subjects. In 24 patients there were laryngeal abnormalities but both esophageal biopsy results and acid exposure times were normal. Laryngeal disease was found in association with prolonged acid exposure time or esophagitis in only 17 of the 97 patients (17.5%) studied. Recent reports appear to have overestimated the importance of acid reflux as a cause of posterior laryngitis.

Fine needle aspiration biopsy and the otolaryngologist.

A survey of the diagnostic value of fine needle aspiration cytology in ENT practice was carried out over a 3-year period. The 119 aspirates were taken from a variety of head and neck sites and examined by an experienced cytopathologist. Cytology was accurate in 111 (93 per cent), with two false negative aspirates (1.7 per cent) and no false positive results. There were no complications and the technique was found to be a rapid, safe and cost-effective diagnostic tool.

Cholesteatoma following stapedectomy.

Among the possible complications of stapedectomy for otosclerosis, cholesteatoma is extremely rare. We report a case which was secondary to implantation of keratinising squamous epithelium inadvertantly included in the fat graft taken from the ear lobe to seal the footplate.

Rigid endoscopy in ENT practice. Appraisal of the diagnostic yield in a district general hospital.

A 10-year retrospective survey of 204 elective rigid oesophagoscopic examinations in the ENT unit of a District General Hospital is reported. One Hundred and Forty Five (71 per cent) were normal and there was marked preponderance of normal endoscopy in patients under 60 years old. Clinical examination was usually unhelpful but barium studies were found to complement oesophagoscopy in reaching a diagnosis. There were six complications of oesophagoscopy, two following normal examinations.

Toxic shock syndrome after nasal surgery.

Toxic syndrome was described as a complication of nasal surgery in 1982 (Thomas et al.) and eight cases have since been reported. To our knowledge this is the first case described outwith the United States. The diagnosis should be considered in any case of unexplained collapse following nasal surgery.

Estimation of oestrogen and progesterone receptors in chronic rhinitis.

The turbinates of 38 patients with chronic rhinitis were examined biochemically for oestrogen and progesterone receptors. Low levels of oestrogen-receptor-like activity (1-20 fmol/mg protein) were found in 50% of patients of both sexes. Progesterone receptor activity was also weak (1-16 fmol/mg protein) but was present only in 5 female patients. Immunocytochemical assay failed to demonstrate focal areas of oestrogen receptor activity. One juvenile nasopharyngeal angiofibroma was negative for both oestrogen and androgen receptors. Other possible mechanisms of hormonal action are considered.

Juvenile stapedectomy.

Stapedectomy in children has long been regarded as taboo and published accounts of this surgery in this age group are few. This paper presents a small series of 29 stapedectomies in patients aged 21 or less; 19 for otosclerosis, 10 for other pathologies associated with stapedial fixation. The results on the otosclerotic group (17 [89%] success, 2 no change, 0 sensorineural loss) were comparable with stapedectomy in adults. The results in the non-otosclerotic group (5 better, 2 no change, 3 sensorineural losses) were not satisfactory, and stapedectomy for stapedial fixation caused by conditions other than otosclerosis cannot be recommended in this age group.

Submucosal diathermy of the inferior turbinate and the congested nose.

Surgical correction of the congested nose is frequently attempted. In the absence of a septal deformity, engorgement of the inferior turbinate is usually held to be the culprit, and various procedures have been advocated to counter this problem. Submucosal diathermy to the inferior turbinates is widely practised, but relatively little has been published about it. 204 patients who underwent submucosal diathermy of the inferior turbinates between 1977 and 1983 have been studied. 60 patients have been followed up by questionnaire, for periods ranging from 1 to 5 years since surgery. Surgery was of benefit in up to 72% of cases. Surgical technique and the problems of patient selection and assessment are discussed.

Cerebrospinal fluid rhinorrhoea and otorrhoea: extracranial repair.

Cerebrospinal fluid otorhinorrhoea is a well recognized problem. The otolaryngologist has a major role to play in the diagnosis and management of this condition. Five cases are described which illustrate the problems involved; four presented as leaks and one as pneumoencephalus and meningitis. Of the various methods of diagnosis and localization non-invasive techniques, i.e. tomography, CT and flexible endscopy are preferred. The extracranial extradural approach for anterior, middle and posterior fossa leaks allows a direct assault on the fistula. It may be usefully augmented by temporary lumbar drainage.
