BACKGROUND: Sleep disturbance is one of the major and unsolved problems in older people. Most of the previous sleep studies rely on self-reported documents, and memory disturbance in older people might bias sleep complaints and health status. OBJECTIVE: Sleep disturbances were studied as a mortality risk. METHODS: In 272 patients who were aged, infirmed and chronically institutionalized in a skilled-care geriatric hospital, the presence or absence of sleep disturbances were examined by hourly observations of patients over 2 weeks at baseline, and they were prospectively followed up for 2 years to assess mortality. RESULTS: Mortality after 2 years was significantly higher in the nighttime insomnia, daytime sleepiness, and sleep-onset delay groups. Further, adjusted for age, gender and activities of daily living status, the presence of nighttime insomnia and sleep-onset delay remained associated with a higher risk of mortality. CONCLUSION: Sleep disturbance may be one of the symptoms indicating poor health or functional deficits, and be an independent risk factor for survival.
Gastric acid outputs were measured and related to plasma oestradiol concentrations in women. An inverse relationship between the acid output and the oestradiol concentrations was detected regardless of the stage of the menstrual cycle. It was also noted that the acid output was reduced in a dose-dependent manner after oral administration of conjugated oestrogen and that the response was partially blocked by atropine sulphate. These findings suggest that oestrogen participates in the control of gastric acid secretion in women and that a cholinergic action is involved in the mode of action.

The relationship between the sperm motility and the adenylate cyclase activity in spermatozoa from patients with male infertility was investigated. Cyclic AMP contents were measured from 34 semen specimens and adenylate cyclase activity were assayed from 50 specimens. All semen specimens were collected from infertile men at our clinic by masturbation after 5 days sexual abstinence. Spermatozoa were washed and concentrated according to the method described by Harrison. The cAMP contents and the adenylate cyclase activity of spermatozoa were determined by radioimmunoassay. Positive correlations were found among the cAMP contents, the adenylate cyclase activity in human spermatozoa and the sperm motility. This result suggests that the sperm motility is regulated by the adenylate cyclase activity via cAMP and that poor sperm motility observed in infertile men is partially caused by the impairment of adenylate cyclase system.
