[Age-related changes in the size and shape of the prostate observed by transrectal ultrasonography].

To elucidate the change with aging in prostatic morphology, 447 cases without acute diseases and prostate cancer of the first visit from January 1988 to December 1990 were examined by transrectal ultrasonography. Total weight of the prostate, area of maximum section, lateral and anteroposterior diameter on the maximum section and supero-inferior diameter were evaluated. The total weight of the prostate was not found to be correlated with aging. The total weight of the prostate showed a high variation with aging. Prostate growth with more than 30 g was greater in the fifties and increased gradually thereafter. Similar findings were observed for antero-posterior diameter on the maximum section and supero-inferior diameter. With aging, the total weight of the prostate in 20-30% of the patients showed an increase while in 30-40% of the patients, no increase. The total weight of the prostate increased in the fifties with subsequent gradual increase in the sixties. According to prostate shape, antero-posterior diameter on the maximum section and supero-inferior diameter was related to the weight of the prostate.

Relationship between steatocystoma multiplex and eruptive vellus hair cysts.

Steatocystoma multiplex (SM) and eruptive vellus hair cysts (EVHC) share a clinical resemblance, but have distinctive histologic features. We describe a patient who had both SM and EVHC lesions and discuss the relationship between the two diseases. We believe that SM and EVHC are within the spectrum of the same disease process and could both be termed multiple pilosebaceous cysts.

Resistance to a new catecholic cephem, BO-1341, in Pseudomonas aeruginosa PAO.

Two distinct types of mutant resistant to the catecholic cephem BO-1341 were isolated spontaneously from Pseudomonas aeruginosa PAO2146: (i) a mutant (CCB5-type) which produced beta-lactamase constitutively, and which appeared to result from a mutation in the regulatory gene (blaI at 41 min) controlling the structural gene (blaP at 25 min); (ii) a mutant (CCB7-type) with specific resistance to BO-1341. Induction of beta-lactamase production in strain CCB7 by BO-1341 was diminished markedly when compared with that in the parent strain. This was not the case for ceftazidime or cefoxitin, suggesting impaired uptake of BO-1341 as an explanation for the resistance. Electrophoretic analysis demonstrated increased production of a 55 kDa protein in the periplasmic space and a 84 kDa outer membrane protein. These data suggested that specific resistance to BO-1341 might involve quantitative alterations in the composition of the outer membrane and periplasmic proteins.

[Prostatic specific antigen (PA) in mass screening for prostate cancer].

The usefulness of prostatic specific antigen (PA) was compared with that of prostatic acid phosphatase (PAP). PA was determined in the serum of 2,183 patient examined by the mass screening for prostate cancer from 1987 to 1990. The serum samples of these patients were obtained from our serum bank. PA was measured by the E test "TOSOH" II (PA). The relationship of PA and PAP to prostate size estimated by digital rectal examination (DRE) and ultrasound tomography (US), and age was investigated. PA and PAP correlated with aging and prostate size estimated by DRE. However PA was more apparently related with these things. The correlation between PA and prostatic size estimated by US was relatively high (r = 0.53), but the correlation between PAP and prostate size estimated by US was low (r = 0.20). When the upper limit of normal range was set at 6.0 ng/ml, the sensitivity, specificity and efficiency was 64%, 97% and 62%, respectively. PA was more sensitive than PAP and could be more useful since none of the patients with prostate cancer was PAP positive and PA negative. We conclude that PA should be a reliable tumor marker in our mass screening system.

A case of cutaneous squamous cell carcinoma associated with sporadic porphyria cutanea tarda due to liver disorder after Schistosoma japonicum infection.

A 56-year-old man suffered from cutaneous squamous cell carcinoma (cutaneous SCC) which occurred on a cutaneous lesion of sporadic porphyria cutanea tarda (sporadic PCT). His liver function decreased from the time he was infected with Schistosoma japonicum at the age of 10. He drank a little alcohol. Erythematous maculae with blisters or erosions occurred on sun-exposed areas of his skin when he was 52. His urine continued to be red. After detailed examinations including liver biopsy, he was diagnosed as having sporadic PCT due to liver disorder after infection with Schistosoma japonicum. At the age of 56, a small red papule arose on his right earlobe at the site of a sporadic PCT lesion. The papule rapidly enlarged with ulceration; this completely destroyed his right earlobe, which was covered with odoriferous yellow-white necrotic tissue. The tumor then extended to his right preauricular area with ulceration. A skin biopsy confirmed well-differentiated cutaneous SCC. The association of cutaneous SCC with sporadic PCT has not been previously reported; we think that the association is significant. Such an occurrence may have been induced by either direct or indirect effects of ultraviolet light or a scar formed by the sporadic PCT.

Postoperative sequelae of augmentation mammaplasty by injection method in Japan.

Postoperative sequelae of 696 cases following augmentation mammaplasty by the injection method have been studied. The most frequently used foreign material for injection was silicone. Paraffin had been previously used somewhat less frequently. Of the 696 cases studied, the most common complications were hardening, skin change, transformation, and pain. Of the total number of cases studied for removal of the foreign material, 244 were seen in university hospitals and 416 in private clinics. Reaugmentation was performed in many cases using both a flap technique and bag prostheses with no significant degree of incidence of complications following reaugmentation.

In vitro and in vivo antibacterial activities of BO-1341, a new antipseudomonal cephalosporin.

BO-1341, a new antipseudomonal semisynthetic cephalosporin, was evaluated for in vitro and in vivo antibacterial activities in comparison with ceftazidime, cefotaxime, and cefoperazone. The in vitro activity of BO-1341 was generally superior or comparable to the activities of the reference antibiotics against clinical isolates of the family Enterobacteriaceae. BO-1341 was highly active against Pseudomonas aeruginosa (MIC for 90% of the strains tested, 1.56 micrograms/ml), Pseudomonas maltophilia (MIC for 50% of the strains tested, 1.56 micrograms/ml), and Acinetobacter calcoaceticus (MIC for 90% of the strains tested, 3.13 micrograms/ml). Furthermore, BO-1341 was highly active against P. aeruginosa isolates resistant to the other antibiotics. Of 199 P. aeruginosa isolates tested, only 2 were resistant to BO-1341. These two strains were also resistant to ceftazidime, cefotaxime, and cefoperazone. Haemophilus influenzae, Branhamella catarrhalis, and nonenteric streptococci were also susceptible to BO-1341, but Staphylococcus aureus, Streptococcus faecalis, and Bacteroides fragilis were not susceptible to the compound. The protective efficacy against experimental infections in mice caused by nine strains of gram-negative bacteria, including P. aeruginosa, reflected the potent in vitro activity.

Immunocytochemical study of the intracellular localization of M protein of vesicular stomatitis virus.

The purpose of this paper is to describe the immunocytochemical localization of M protein of vesicular stomatitis virus (VSV) in infected cells. Vero cells, MDBK cells, Swiss 3T3 cells, and BHK cells were examined at various times after infection. For immunofluorescent staining, the cells were fixed with PLP fixative and then treated with 0.05% Triton X-100 before incubation with antibodies. Three hours after infection, M protein exhibited diffuse immunostaining throughout the cytoplasm and later accumulated along the cell membrane. The localization of M protein differed from the granular localization of the nucleocapsid N protein of VSV in the cytoplasm. For electron microscopy, the cells were fixed first in a mixture of 2% paraformaldehyde and 0.05% glutaraldehyde and then with PLP fixative, this being followed by treatment with 0.05% saponin. They were then immunostained using the immunoperoxidase method. The M protein was found to be distributed throughout the cytoplasm and later under the cell membrane, especially at virus budding sites. We also used postembedding immunostaining and freeze-fracture immunostaining to avoid the translocation of M protein caused by the detergent treatment. These techniques confirmed our previous results. Our findings are consistent with the view that the M protein of VSV is synthesized on free ribosomes and is then associated with the cell membrane where viral assembly may occur.

Attachment of cytoskeletons to cell membranes in human blood platelets as revealed by the quick-freezing and deep-etching replica method.

This study evaluated ultrastructures of cell membrane-cytoskeletal interactions in resting and activated human blood platelets. Some blood platelets were fixed with paraformaldehyde to inhibit activation and the others were activated with thrombin treatment. The replicas of inside-out cell membranes, cell membrane surfaces, and Triton shells were made by the quick-freezing and deep-etching replica method. A geometrical pattern consisting of main hexagons and partial pentagons was revealed to be formed with microfilaments which were attached laterally to cytoplasmic sides of cell membranes in resting platelets. After the activation, cytoskeletal meshworks in the cytoplasm were densely associated with cell membranes, and parallel microfilamentous bundles in filopodia were connected with small polygonal meshworks lying under the cell membrane. The cell membrane-cytoskeletal interactions are essential to keeping a discoid shape at the resting stage, and to changing the shape quickly after the activation.
