Investigation of brachial plexus injuries by intraoperative cortical somatosensory evoked potentials.

With the intention of estimating the extent and site of damage in brachial plexus injuries which involve close to the root outlet and also the distal portion, cortical somatosensory evoked potentials (SEP) were recorded in 21 patients by directly stimulating the exposed brachial plexus. In 38 avulsed nerve roots which showed positive sensory action potentials (SNAP)/nerve action potentials (NAP) in their peripheral part, 15 roots (nine patients) were apparently in continuity and confirmed as root avulsion injury by the absence of cortical SEP. However, 11 roots (ten patients) which showed neither SNAP nor SEP would suggest either extensive lesions involving root and more distal segment or a combination of root avulsion and postganglionic injury. Fourteen patients who showed a positive Tinel's sign had at least one root with a postganglionic type lesion, but cortical SEP evoked by stimulation of the most proximal root zone sometimes revealed a reduced amplitude and prolonged latency. This would indicate the retrograde extension of damage. To confirm the extent and degree of the nerve lesions in brachial plexus injuries, an intraoperative SEP and NAP recording is very useful, practical, and also indispensable.

Vascular anatomy of flexor tendons. I. Vincular system and blood supply of the profundus tendon in the digital sheath.

The detailed blood supply of the flexor tendons in the digital canal was studied in 35 fresh human hands by means of an India ink-latex vascular injection technique. The specimens were examined by first exposing the pulley system, followed by examination of the intact tendon which had been rendered transparent by immersion in a solution of tributyl and tricresyl phosphate. Five annular and three cruciform pulleys were shown. There were five types of long vincula to the profundus (VLP) and three types of long vincula to the superficialis (VLS). The vincular systems of index and little fingers were symmetrical. Occasionally, neither the VLP nor the VLS was found in either the long or the ring fingers. The vincula received blood supply from four transverse branches of the digital arteries. A volar avascular area of the profundus tendon was seen in the cleared cross-sections, and the cross-over zone of its intrinsic vessels was found to be at the midproximal phalanx.

Mastopathy-mimicking ultrasonographic appearance in a case with Burkitt's lymphoma.

We report a case of Burkitt's lymphoma, which was suspected to involve both breasts. US showed a replacement of the normal breast tissues by a diffusely mixed or mottled pattern of echo texture, associated by hypoechoic cyst-like masses with slight internal echogenicity. This pattern was indistinguishable from the sonographic changes produced by a mastopathy. These findings disappeared after successful chemotherapy for the Burkitt's lymphoma, suggesting that the ultrasonographic pattern of mastopathy was caused by the infiltration of the breast tissues by the lymphomatous cells.

MR imaging of Pellegrini-Stieda disease.

OBJECTIVE: To describe the MR imaging of Pellegrini-Stieda disease, a post-traumatic ossification of the medial collateral ligament with typical radiographic findings. METHODS: Coronal and/or axial T2*-weighted field echo and T1-weighted conventional spin echo MR images were obtained with 1.5-Tesla or 0.5-Tesla units. Four patients with radiographic and clinical diagnoses of Pellegrini-Stieda disease were examined. Surgery was performed on one patient, and histopathologic correlation was obtained. RESULTS AND CONCLUSION: Delineating it as a signal void, T2*-weighted MR imaging indicated the precise extent of the osseous fragment in relation to the medial collateral ligament. T1-weighted imaging showed the presence of fatty marrow as a high signal region within the fragment, indicating maturity of the fragment. These findings may help to determine preoperative planning for resection of massive fragments around the ligament. Pellegrini-Stieda disease cannot be considered rare, since it is often encountered in daily MRI examinations.

Relationship between the patellar height and the disorder of the knee extensor mechanism in immature athletes.

SUMMARY: The purposes of this study were to evaluate the outcome of longitudinal research on the patellar height of immature athletes, and to clarify the relationship with disorders of the knee extensor mechanism. The patellar height of 40 young male soccer players (80 knees) was observed with radiographs of the bilateral knee joint for 2 years. The patellar height was measured using the epiphyseal line method. No subject showed patella alta before the occurrence of the disorder. Of the 12 knees in which the disorder occurred during the observation period, 10 knees (83%) had higher patella after the onset of symptoms than before. The patellar height in the painful stage was considered significant compared with the normal group. Although the patellar position in the predisorder stage was relatively high, this state was considered within the normal scope of physiologic variation, such as the growth spurt.
