A solid-phase radioimmunoassay for the detection of nRNP immune complexes.

We developed a solid-phase radioimmunoassay for complement (C)-fixing nuclear ribonucleoprotein (nRNP):anti-nRNP immune complexes (nRNP ICs). The assay was based on the ability of the C-fixing nRNP ICs to bind strongly to immobilized F(ab')2 anti-C3. The extent of binding was quantified by incubating the C-fixing nRNP ICs bound to anti-C3 with 125I-labeled anti-nRNP-specific IgG. The interaction between anti-C3 and C-fixing nRNP ICs was rapid, time- and concentration-dependent and sensitive over a broad range of nRNP IC concentrations in an antigen-antibody ratio of 8 : 1 (9.8-5000 ng of human aggregated IgG equivalent per ml). We found that the assay also detected an immunoreactive U1-RNP antigen in Sm : anti-Sm immune complexes but did not detect SSA : anti-SSA immune complexes. The assay was preliminary applied for serum samples obtained from patients with mixed connective tissue disease (MCTD), and elevated concentrations of nRNP immune complexes were found in 3 out of 5 patients with MCTD. This assay appears to be applicable to the detection and quantification of circulating nRNP ICs in patients with MCTD.

Direct quantitation of activated C3 in human plasma with monoclonal anti-iC3b-C3d-neoantigen.

We developed a microtiter solid-phase radioimmunoassay for quantitating C3 breakdown products (iC3b, C3dg, C3d) in human plasma with a unique monoclonal antibody specific for a neoantigen present on iC3b and C3d (MoAb 130). This monoclonal antibody reacts with a neoantigen which appears when C3b is converted to iC3b. The neoantigen is also present on the C3dg and C3d fragments derived from iC3b. The concentration of the neoantigen is elevated in the plasma of most patients with rheumatoid arthritis and systemic lupus erythematosus as compared to normal volunteers. Some patients with glomerulonephritis also had elevated concentration of the neoantigen in their plasma.

Differential diagnosis of fever in systemic lupus erythematosus using discriminant analysis.

In an attempt to find a reliable method to assess fever in patients with systemic lupus erythematosus (SLE), a multifactorial analysis was applied to the routine laboratory examinations, including white blood cell count (WBC) and serum globulin fraction concentrations. During 74 febrile episodes, 49 SLE patients showed increased disease activity and the remaining 25 febrile episodes were due to intercurrent infection. The two different groups of fever episodes were clearly separated by a principal component analysis using five variables from the routine laboratory tests, including WBC, serum alpha-1, alpha-2, beta, and gamma globulins. Discriminant analysis showed that 95% of 74 febrile episodes could be correctly classified as to the cause of fever when a combination of WBC and alpha-2 globulin level was used as variables. A simple discriminant formula which we calculated was considered to be of practical use for the differentiation of the two clinical entities.

[A randomized control study of the antiemetic efficacy of betamethasone versus methylprednisolone].

A randomized control study of the antiemetic activity of betamethasone (B) vs. methylprednisolone (MP) was carried out. Fifty-six patients receiving CDDP (60 mg/m2-80 mg/m2) were entered. B (8 mg/body on day 1, 4 mg/body on days 2 and 3) was administered intravenously in 18 patients, and MP (1,000 mg/body on day 1, 500 mg/body on days 2 and 3) was administered intravenously in 19 patients. Severe vomiting occurred in 5 of the 19 (26.3%) with MP, 10 of the 18 (55.6%) with B, and 11 of 19 (57.9%) controls. Severe nausea occurred in 3 of the 19 (15.8%) with MP, 6 of the 18 (33.3%) with B, and 5 of the 19 (26.3%) controls. Methylprednisolone was thus considered effective (P less than 0.05) for CDDP-induced emesis.

Serum lactate dehydrogenase and its isozymes in lupus nephritis.

The relationship between the renal pathologic activity of systemic lupus erythematosus (SLE) and serum lactate dehydrogenase (LDH) was examined in 28 patients with active SLE involving only the kidney. Serum levels of total LDH, LDH1, and LDH2 were significantly higher in the patients with diffuse proliferative lupus nephritis (World Health Organization class IV) than in those with milder renal disease (classes I through III and V). Total LDH levels showed good correlations with the activity index and the total pathologic score of the renal pathologic scoring system, and with the glomerular hypercellularity and overall deposits. The elevated level of LDH was mainly due to elevated levels of its isozymes LDH1 and LDH2. These results suggest that the elevation of serum LDH levels in patients with SLE reflects the renal pathologic changes due to lupus nephritis.

Uptake of intravenously administered soluble immune complexes by type I alveolar cells and macrophages in mice.

Mice (C57BL/6, n = 5) were given a single intravenous injection of soluble bovine serum albumin (BSA)-rabbit anti-BSA immune-complexes (IC) prepared at five-fold antigen excess. The mice were sacrificed 15 and 30 min, and 1, 2, 4 and 12 h after injection. Granular immunofluorescence for BSA and rabbit IgG, in a pattern consistent with IC aggregation, was observed in the alveolar walls and in the cytoplasm of alveolar cells. Moderate immunofluorescence was observed 15 min after IC injection and maximal immunofluorescence was observed at 30-60 min which decreased rapidly 2 h after IC administration. Only a trace amount of immunofluorescence was observed at 4 h and none was seen at 12 h. Light microscopic immunoperoxidase staining showed localization of IC in the interstitium of the alveolar septa and in type I alveolar cells and alveolar macrophages. By immunoelectronmicroscopy, the uptake of IC in the cytoplasm of type I alveolar cells and alveolar macrophages was confirmed.

Gene expression of endothelin receptors in aortic cells from cyclosporine-induced hypertensive rats.

1. We examined preproendothelin-1, ETA and ETB receptor mRNA levels in aortic endothelial and smooth muscle cells from cyclosporine (CyA)-induced hypertensive rats using the reverse transcription polymerase chain reaction method. 2. Aortic endothelial preproendothelin-1 mRNA expression was about 1.5-fold higher, while that of ETB receptor mRNA was markedly decreased in CyA-treated rats compared with those in controls. 3. The expression of ETA receptor mRNA in smooth muscle cells from CyA-induced hypertensive rats was increased about two-fold over that in cells from control animals. 4. Thus, increased endothelial preproendothelin-1, and ETA receptor mRNA levels in smooth muscle cells, which are concomitant with the decrease in ETB receptor mRNA levels in endothelium, may contribute to CyA-induced hypertension in rats.

Long-term observation of avascular necrosis of the femoral head in systemic lupus erythematosus: an MRI study.

OBJECTIVES: To assess long-term prognosis of clinically silent, early-stage avascular necrosis of the femoral head (ANFH) in patients with systemic lupus erythematosus (SLE). METHODS: Twenty-four hips that showed ANFH by magnetic resonance imaging (MRI) in 13 patients with SLE were studied. All hips were radiographically normal and clinically asymptomatic. The percentage volume of necrotic bone was calculated at each study by dividing the sum of the necrotic areas by the sum of the femoral head areas from all MRI slices. Hips were also classified into three categories by the relation of the necrotic area to the weight bearing portion according to the system of the Japanese Investigation Committee for avascular necrosis of the femoral head, with modifications: Type A (medial lesions): 8 hips, Type B (central lesions): 4 hips, and Type C (lateral lesions): 12 hips. Patients were followed up with MRI for 12-95 (mean 51) months. RESULTS: Fifteen hips improved (more than 15% reduction in the volume of necrosis), 5 did not change and 4 worsened during the observation period. All hips with a volume of necrotic area less than 25% showed improvement. All but one Type A hip and one Type B hip improved, while the mean volume of necrosis did not change in Type C. The volume of the necrotic area was smaller in Type A & B than in Type C hips (p < 0.001). CONCLUSIONS: Long-term prognosis of early-stage ANFH was favorable in patients with SLE when the necrotic area was small (less than 25%).
