A 60 year old woman with computed tomographic-verified bilateral thalamic infarction is described. She demonstrated downward gaze paralysis, amnesia and hypersomnolence. On the functional level such tasks as walking and eating were disconcerting. Although there have been only a few reports of the complete triad it may be more common than realized and represents a distinct syndrome that can be identified by the bedside.

The case of a 15 year old girl with a rapidly progressing right sided hemiparesis is reported. The history and clinical manifestations were suggestive of either herpes simplex encephalitis or occlusion of the left middle cerebral artery. At necropsy an occlusive thrombosis of the left middle cerebral artery due to a segmental necrotizing granulomatous panarteritis was found. The cause was obscure. A plausible incriminating factor was the contraceptive pill.
In a study of 29 Sri Lankan patients with primary liver cancer, it was found that the highest incidence was in the over 55 year age group and the male:female ratio was 5:1. The ratio of hepatocellular carcinoma to cholangiocarcinoma was 13:1. Seventy-five percent of the males in this study had consumed alcohol. This may account for the higher incidence of cirrhosis and hepatocellular carcinoma among males and the lower incidence in females in this country in this country. The role of additives in local alcoholic beverages, the various alkaloids in decoctions and aflatoxin in articles of diet, await systematic investigation.

Three patients with aplastic anaemia following chloroquine therapy are described. In two, chloroquine had been administered in large doses over a long period. One of them subsequently developed acute myeloblastic leukaemia. The third received only a small dose and pancytopenia with aplasia followed three weeks after ingestion of the drug. The mechanism(s) of chloroquine-induced marrow injury is not known. A parallel to chloramphenicol-induced blood dyscrasias is drawn, but remains unproved.

This paper analyses the clinical features of 8 patients with post-thrombotic obstruction of the subclavian and axillary veins demonstrated by phlebography. The classic physical signs of swelling of the arm with dilated superficial collateral veins were seen. The causes of thrombotic obstruction are discussed and the precipitating cause in this study was possibly filariasis.
