[Treatment of "difficult" duodenal stump].

The Plenk method in the author's modification was successfully applied in resection of the stomach "for exclusion" in 22 patients. When there was a threatening incompetence of the duodenal stump sutures, to prevent generalization of peritonitis the stump in 24 patients was covered with a "hood" of the greater omentum and external drainage of this space was installed; the procedure was an alternative of duodenostomy. Incompetence occurred in 3 cases with subsequent recovery. In resection of the stomach for recurrent ulcer after previous vagotomy combined with gastroduodenostomy after Jaboulay, the defects in the duodenal wall were closed with a demucosed antral-pyloric gastric segment after the author's method. The operation was conducted on 4 patients.

[Morpho-functional analysis of median-sector approach in abdominal surgery].

The article presents comparative analysis of original median-link access in abdominal surgery under various surgical interventions on the abdominal cavity organs. On the vast experimental material including 60 rabbits and data of clinical observation of 426 patients with different surgical diseases the authors showed the advantages of median-link access.

[A rare complication of splenic torsion in a child].

A 13-year-old girl with a clinical picture of severe peritonitis was subjected to emergency operation upon 18 h since the symptoms onset. Laparotomy which detected a 360 degree torsion of the spleen with necrosis and perforation in the fundus of the stomach was followed by splenectomy and suture of the perforation. The patient's death occurred 17 h after the surgical treatment in progressive cardiovascular, renal and hepatic insufficiency and associated DIC syndrome. Specific blood supply to the organs is considered the underlying cause of gastric necrosis in relevant case of splenic torsion.

[Choice of the surgical intervention method in peptic ulcer combined with diverticula of the stomach and duodenum].

Diverticula of the stomach and duodenum were revealed in 21 (2.3%) of the examined patients with ulcer disease. Resection with radical interventions on the diverticulum were fulfilled in 8 patients, with by-passed duodenum in 7 patients, with selective proximal vagotomy associated with invagination of the diverticulum in 6 patients, among them 1 patient with a subpyloric Y-shaped bypass of the duodenum. There were no lethal outcomes.
