Congenital retinal macrovessel.

Congenital retinal macrovessel is a rare condition in which anomalous retinal vessels cross the macula. Visual impairment can result if the anomalous vessels pass across the foveola, if foveolar cysts form, or if hemorrhage occurs. A now 17-year-old African-American female has an anomalous retinal vein that crossed the macular region beyond the horizontal raphe in her left eye. This patient exhibited the classic characteristics of congenital macrovessels. Her visual acuity remained unaffected after a 5-year follow-up and therefore treatment was not required.

Implementation of the problem-oriented system in an optometric teaching clinic.

Problem-oriented records, performance audit, and a clinical index were implemented in the eye-care service of the Dorchester House Multi-Service Center, a neighborhood health center and teaching affiliate of the New England College of Optometry. We found that problem-oriented records facilitate communication and encourage logical thinking, that audit helps assure the quality of care and teaching, and that a clinical index provides a perspective on the overall performance of the eye-care service.

Pupillary examination.

Examination of the pupils is an essential part of the evaluation of patients in optometric practice. In this article, the normal pupil and pupillary reflexes are reviewed, along with a discussion of anisocoria and specific anomalies of the pupils. Explanation of step by step office examination techniques is presented, as well as a discussion of the pathophysiology of abnormal pupils.
